Ameloblastoma of the maxilla is an unusual epithelial tumor of odontogenic origin. According to many authors and reports, ameloblastoma account for approximately 1% of all tumors of the jaws, but when pseudotumors and cysts are excluded, the ratio rises to 11%. Of these tumors,80% originate in the mandible, while 20% originate in the maxilla. Although it is considered benign histopathologically, it can behave in a slowly growing infiltrative fashion, with multiple recurrences and eventual intracranial, or even distant, spread. We clinically analyzed common site in maxilla, radiographic findings, recurrence rate, duration between treatment and recurrence, the presence and site of distant metastasis in 15 patients who were diagnosed as ameloblastoma of the maxilla and took treatments from 1985 to 1999 in Dept. of Oral and Maxillofacial Surgery, Dental Hospital, Seoul National University. In this paper, treatment outcomes and our clinical experiences of maxillary ameloblastoma are reported with review of literatures.

Marsupialization is widely used as a primary treatment modality for reducing size of large cysts. However, there is no recommendation for specific duration of marsupialization. In addition, Carnoy's solution usually is applied at the time of enucleation as a fixative agent. In this report, we present an appropriate marsupialization duration of ameloblastoma involving two unerupted teeth. In this present study, marsupialization using a Foley catheter was performed in two cases of ameloblastoma of the mandible involving two adjacent impacted teeth. Carnoy's solution was applied for 3-5 minutes after enucleation in both patients. Periodically during marsupialization, the size of the radiolucency was measured in panoramic view, and clinical examination was performed. No remarkable paresthesia or soft tissue injury was observed after application of Carnoy's solution or during follow-up. We recommend 12 to 16 weeks as an adequate marsupialization duration for a large ameloblastoma involving two impacted teeth based on increased radiopacity along the margins of the lesions. Poor oral hygiene was an issue after 12 weeks of marsupialization in one case. There were no remarkable complications with Carnoy's solution in either case. The Foley tube has a two-way system that is more effective for irrigating the cavity than is the conventional one-way system.
Ameloblastic carcinoma is a rare odontogenic malignant tumor with the histologic features of both ameloblastoma and carcinoma. It occurs more frequently in the mandible than in the maxilla and it may appear de novo or develop from a preexisting ameloblastoma or odontogenic cyst. Rapidly progressing, painful swelling is the most common symptom, and radiographically, it shows significant bone resorption and cortical perforation. This report described a case of ameloblastic carcinoma in a 45-year-old man who presented with swelling in the left mandible. The lesion showed combined features of benign findings, such as an expansile cortex with a distinct border, and malignant findings, such as a large exophytic mass with frank bone resorption. Excisional biopsy was performed and a final diagnosis of ameloblastic carcinoma was made.

The unicystic ameloblastoma deserves separate consideration on the basis of its clinical, radiologic, and pathologic features and its response to treatment. It refers to those cystic lesions that show clinical, radiographic, or gross features of a jaw cyst, but on histologic examination show a typical ameloblastomatous epithelium lining part of the cyst cavity. The lesion is most commonly found on the mandible posterior area, and often asymptomatic, although large lesions may cause a painless swelling of the jaws. The lesion typically appears as a circumscribed radiolucency that surrounds the crown of an unerupted molar. These are usually considered to be a dentigerous, residual cyst on the relationship of the lesion to teeth in the area. Three histopathologic variants of unicystic ameloblastoma may be seen. 1) Luminal type, 2) Intraluminal type, 3) Mural type. In this case, these tumor was treated as cysts by enucleation with iliac bone graft, and the diagnosis of ameloblastoma is made after microscopic examination of the presumed cyst.
An ameloblastoma is one of the most common odontogenic tumors. Ameloblastoma is cytologically a benign tumor, but is clinically characterized by infiltrative growth and high recurrency. The treatment of ameloblastoma has been controversial. The aim of this paper is to consider effectiveness of curettage and enucleation after SSRO in the small-sized multilocular intraosseous ameloblastomas that have been treated more frequently by radical treatment. They were radiographically characterized by the cortical bone that was expanded or eroded locally and histopathologically by solid multilocular ameloblastomas. It is considered that curettage and enucleation after SSRO and long-term follow-up enable the small-sized multilocular intraosseous ameloblastomas that were characterized by almost destroyed cancellous bone and expanded cortical bone to treat minimizing facial disfigurement and masticatory dysfunction and sociopsychological impact produced by radical treatment. We recommend that the small-sized multilocular intraosseous ameloblastomas without involvement to the surrounding soft tissues be first treated by curettage and enucleation after SSRO.
Ameloblastoma is a rare epithelial tumor of the jaw, comprising approximately $1\%$ of all tumors and cysts of odontogenic origin. The tumor has been the subject of much controversy and cliscussion. Amelobiastoma is slow growing, locally invasive, and has a high rate of total recurrence, but rarely metastasizes. This neoplasm has been considered radioresistant, so it has been treated with surgery primarily except some cases. Recently it is suggested that ameloblastoma is radiosensitive, so radiation therapy (RT) would be used in addition to or, in certain cases, in place of surgery. We report these two cases which were referred to the Department of Therapeutic Radiology for deciding tile need of postoperative RT, because amelobiastoma is rare and has been known tot be radioresistant. Postoperative RT was given to one of these two Patients.

Objectives: The purpose of this retrospective study is to find the differentiating characteristics of cystic and cystic-appearing lesions that involve the impacted mandibular third molar by analyzing panoramic radiographs and computed tomography images, and to aid the preoperative diagnosis. Materials and Methods: Eighty-one patients who had a mandibular cystic or cystic-appearing lesion that involved impacted mandibular third molar and underwent cyst enucleation were included in the study. The preoperative panoramic radiograph and computed tomography findings were analyzed in accordance to the histopathologic type. Results: Most of the cystic lesions containing the mandibular third molar were diagnosed as a dentigerous cyst (77.8%). The occurrence of mesio-distal displacement of the third molar was more frequent in the odontogenic keratocyst (71.4%) and in the ameloblastoma (85.7%) than in the dentigerous cyst (19.1%). Downward displacement was primarily observed in each group. Odontogenic keratocyst and ameloblastoma showed more aggressive growth pattern with higher rate of bony discontinuity and cortical bone expansion than in dentigerous cyst. Conclusion: When evaluating mandibular cystic lesions involving the impacted mandibular third molar, dentigerous cyst should first be suspected. However, when the third molar displacement and cortical bone absorption are observed, then odontogenic keratocyst or ameloblastoma should be considered.

Purpose: Ameloblastomas are rare benign tumors of odontogenic origin, and compose about 1% of all oral and maxillomandibular cysts and tumors. Because this neoplasm has a high rate of local recurrence, segmental mandibulectomy with a 1~2 cm safety margin and immediate microsurgical reconstruction is an accepted treatment modality. The authors experienced four mandibular reconstruction cases that underwent secondary dental implantation. Here, the authors describe these cases and their long-term results. Methods: Four patients with ameloblastoma of the mandible underwent segmental mandibulectomy and reconstruction with a free fibula osseous flap from January 1999 to May 2005, followed by secondary dental implantation. Recurrence, bony union, implant osseointegration, and functional and aesthetic results were evaluated by radiologic imaging, by physical examination, and by using photographs. Results: All free flaps survived with no evidence of flap loss. To date, no recurrence has been noted clinically or radiologically. Imaging after mandibular reconstruction with a free fibular flap revealed satisfactory bony unions and mandibular contours. The patients achieved good aesthetic and functional results after the secondary implantation. Conclusion: Mandibular reconstruction using a fibular osseous flap and secondary dental implantation can produce good functional and aesthetic results after segmental mandibulectomy for ameloblastoma.

Introduction: Ameloblastoma is a benign but locally invasive tumor with a high recurrence rate. The aim of this study was to make an easier diagnosis and treatment planning of ameloblastoma. Materials and Methods: From January 1993 to October 2009, 139 cases from 123 patients, who had been diagnosed with ameloblastoma through radiologic and biopsy in the department of oral and maxillofacial surgery of Kyunpook National University, were selected as the subjects in this study. According to the medical charts, 9 factors (age, gender, location, chief complaints, duration, radiographic findings, size and recurrence) concerned in deciding the treatment method and the relevance between each factor and the treatment methods were examined.(Conservative treatments were marsu-pialization, enucleation, curettage and lateral decortication. Radical treatments included block excision, resection and hemisection) Results: In the patients under the age of 20, 77.14% had conservative treatments, whereas 22.86% underwent radical treatments. In the patients over the age of 20, 44.23% were treated conservatively treatments, and 55.77% underwent radical treatments. For unilocular types, 28.57% had conservative treatments, whereas 71.43% had radical treatments. For the multilocular types, 66.67% underwent conservative treatments, and 33.33% had radical treatments. For the primary cases, 58.68% were treated conservatively and 41.32% had radical treatments. For the recurrent cases, 16.67% and 83.33% underwent conservative and radical treatments, respectively. Conclusion: There was statistical significance in the factors affecting the treatment methods, such as age, radiographic findings and recurrence.
The Ameloblastoma is a true neoplasm of enamel organ-type tissue histopathologically. Ameloblastoma is recognized as a benign tumor, but its clinical behavior is locally invasive. Therefore treatment of choice is surgical resection rather than conservative enucleation. This is a case report about En-bloc excision of granular ameloblastoma arising from dentigerous cyst in the right mandibular angle area of 50-year male patient. He visited local clinic with chief complaint of facial swelling on the right mandibular angle area. Surgical removal was performed by cyst enucleation and En-bloc excision with preservation of inferior border of mandible. After 1 year, iliac bone graft was done and hyperbaric oxygen therapy was applied. We obtained a good result of bony regeneration without any sign of recurrence.
