Purpose: The most common cuase of transfusion for trauma victims in an emergency department is hypovolemic shock due to injury. After an injury to an internal organ of the chest or the abdomen, transfusion is needed to supply blood products and to compensate tissue oxygen transport and bleeding. From the 1990's, there have been some reports that transfusion is one of the major factors causing multiple-organ failure. Thus, as much as possible, tranfusion has been minimized in the clinical setting. This study aims to analyze the prognostic factors for mortality among trauma victims transfused with blood products in an emergency department. Methods: We conducted this study for the year of 2010 retrospectively. The study group included adult trauma victims tranfused with blood products in our ED. The exclusion criteria were discharge against medical advice, and missing follow-up due to transfer to another facility. During the study period, 34 adult trauma victims were enrolled. We compared the clinical variables between survivors and non-survivors. Results: the mean age of the 34 victims was 58.06 years, and males account for 58.5% of the study group. The most-frequently used form transportation was ambulance(119, 55.9%), and the most common injury mechanism was mobile vehicle accidents(67.6%). The mean revised trauma score (RTS) was 5.9, and the mean injury severity score (ISS) was 47.76. The mortality rate in the ED was 58.5%, Comparison of survivors with non-survivors showed statistical differences in injury mechanism, initial SBP, DBP, RTS, ISS, and some laboratory data such as AST, ALT, pH, PO2, HCO3, glucose (p<0.05). Regression analyses showed that mortality among adult trauma victims transfused in the ED correlated with RTS. Conclusion: When an adult trauma victim is transported to the ED and needs a tranfusion, the emergency physician carefully assess the victim by using physiologic data.

This research is conducted to investigate musculoskeletal diseases' that can be developed in paramedics body whose are in charge of emergency situations before they arrive at the hospital. The data were collected from 216 of paramedics from July 2, 2012 to July 31, 2012. As a result, 57.9% of participants have already felt the subjective symptoms of the muscular skeletal diseases, and 47.2% of them felt the pain on their waists and hips. These pains are highly related to their working situations. In the aspects of the muscuoskeletal diseases' symptoms, general characteristics, gender (t=16.579, p=.000), age (t=102.344, p=.000), education (t=5.363, p=.027), drinking (t=6.999, p=.030), and smoking (t=6.266, p=.009), have significance differences. Professional characteristics, career (t=67.684, p=.000), passengers of the ambulance (t=7.717, p=.004), qualification or license (t=25.480, p=.000), and position (t=74.615, p=.000), have also significance differences.
Purpose: The purpose of this study was to provide descriptive data about the characteristics of pediatric patients and nursing interventions in Regional Emergency Medical Centers (REMC). Methods: A retrospective design was used to examine the medical records of 4,310 children. The clinical data and nursing terminologies of REMC were analyzed using the Nursing Intervention Classification (NIC). Results: Male toddlers dominated the sample. The mean age of the children was 3.51 yr. In more than half of the visits, patients arrived between from 7 a.m. to 3 p.m., on a weekday. There were only 189 ambulance transports to REMC, (4.4% of visits). The most frequent injuries were due to falls (28.0%) and contusions (27.3%), but the most common reasons for visits were non-injury (73.4%): fever, cough/shortness of breath, seizures, and abdominal pain. Of the 4,310 visits, 27.8% spent 6-24 hours in the REMC, while 33% resulted in hospital admission and 2.1% in transfer to another hospital. Of the 17,929 nursing interventions, 17,909 elements (99.9%) were classified under NIC. All the listed NIC interventions, however, were not reflected in the level of practice demonstrated by REMC nurses. Conclusion: These results can enhance the understanding of pediatric emergency nursing interventions and can make NIC more applicable.

Purpose: Abdominal pain is the most common symptom of patients visiting the emergency department (ED). Abdominal pain is caused by a variety of causes, so it is difficult for a triage nurse to determine the urgency of a patient, but it is still a must. The purpose of this study was to identify the related factors to the urgent diseases of patients with abdominal pain visiting ED. Methods: This study was a retrospective descriptive study. The study setting was an ED in a tertiary hospital in Korea. Data were collected from September 1, 2017 to October 15, 2017. During the study period, of a total of 6,181 patients visiting the ED, 731 complained of abdominal pain. Patients with obvious cause of pain and patients who could not express detailed symptoms were excluded. The 573 patients were included in the final analysis. We collected demographics, clinical characteristics, and final diagnosis. We divided final diagnoses into urgent diseases which were more likely to be life-threatening without treatment and non-urgent diseases. We identified the related factors to the urgent diseases of patients with abdominal pain using the logistic regression. Results: 173 (30.2%) patients had urgent diseases. Age (OR=1.02, 95% CI=1.00~1.03), referral from other clinics (OR=2.92, 95% CI=1.86~4.60), ambulance utilization (OR=2.00, 95% CI=1.27~3.15), diarrhea (OR=0.44, 95% CI=0.25~0.76), and tachycardia (OR=2.27, 95% CI=1.44~3.58) were related to urgent diseases. Conclusion: Triage nurse should take into account the patient's age, mode of visiting, and route of visiting ED; and check the symptom of tachycardia or diarrhea.

Purpose: This study surveyed how victims with acute traumatic hand injuries received pre-hospital care and how satisfied they were with that care. Method: A total of 100 adults were interviewed using a questionnaire at one microsurgical clinic in Korea. Results: Only 12 patients (12.0%) were transported by the ambulance and the others by private or company's vehicles. Only 9 patients (9.0 %) were able to get appropriate first response from the EMTs or industrial health providers at the scene of the accidents. The mean time required for transportation from the scene of the accident to the operation room of the microsurgical clinic was $372.65{\pm}719.17$ minutes. Most of the patients were satisfied with the pre-hospital care provided by the EMT or industrial health providers but dissatisfied with that provided by lay persons. Conclusion: This study demonstrates that there is a lack in the first response provided at the scene and the activation of EMS (Emergency Medical System) for acute hand injury. It is necessary to educate the public about the appropriate first response and rapid transportation to the appropriate microsurgical clinic.
As a result of analyzing problems in field practice and its effects, the following conclusions can be obtained. Man students were more satisfied with their choice of this department than woman students and woman students recognized the importance of hospital practice although they felt the need of fire fighting station practice and man students felt the necessity of hospital practice, but they answered the fire fighting station practice was important. Departments of field practice are emergency and nurse departments in hospitals and emergency aid section in fire fighting stations. And it is found that field practice was performed well in relation to major subject. Areas of practice included emergency room, operation room and extensive cure unit and the experience of ambulance was more in fire fighting station than in hospital. Clinical guidance of professor was preferred to once a week and the students answered they felt satisfaction with practice through conference. Answer that field practice was helpful in understanding class was very high as 96.8% and its connection with employment was low as 12.8%. It is found that a proper timing of field practice was winter vacation for hospital practice after completing the second semester of the first year and summer vacation for fire fighting station practice after the first semester of the second year. The most difficult department in field practice was nurse department in hospital because 'respondents were practice students' and administrative department in fire fighting stations because 'they lacked recognition of paramedic'. In making practice diary, 'describing measure results' was difficult most and the respondents wanted to keep the diary in their custody.
The purpose of this study was to survey a perception of frequency & importance level of job performance, from 249 paramedics who were working at fire station. The job related activities of 4 duties, 18 tasks and 145 task elements were checked by 4 rating scale. The data were analyzed using a SPSS program for descriptive statistics. The results of the study were as follows ; 1. Some of the most frequent tasks were medical tx, managing people & organization, trauma care, pt assessment, general coping skill 2. Some of the most frequent task elements were preparation of written reports, operation of pt lifting supplies, transportation of patients on stretchers, administration of cervical collar and utilization of scoop stretcher. 3. Some of the highest level of importance in task were resuscitation of circulation, surgical tx, safety & infection control, environmental emergency care, trauma care. 4. Some of the highest level of importance in task elements were administration of cervical collar, adult CPR, infant CPR, child CPR, and AED. 5. The highest level of task elements in perception of frequency & importance were administration of cervical collar, infection control after pt care, utilization of long back board, disinfection of ambulance after ride a long, care of chest pain pt, care of unconscious pt, tx of asthma. 6. A difference between frequent & importance score were due to lack of supplies(41%), structural problems(30%) and medical control system(16%), lack of skills(10%), Suggestion; 1. This paper would be more reliable and confirm through wilder range of survey. 2. It would be necessary of more depth survey through dacom study from paramedic. 3. Development of field oriented protocol & curriculum that based on task elements which have high score of both frequency & importance level is required.
This study aimed at evaluating and developing 119 emergency medical technicians' prehospital care for non-traumatic cardiac arrest. Total 322 EMT in Chungnam province and Daejeon city filled out the self-administered questionnaire. The data were analyzed by SPSS 18.0 for descriptive statistics. Among the 322 EMT, 309 (97%) and 169(53%) always or almost performed CPR and AED for nontraumatic cardiac arrest patient, respectively. Among the advanced EMT and nurse, IV were sometimes or not performed at 94.7% and medication including epinephrine which commonly used for survival of cardiac arrest were treated just at 9.3 % (14 person). The reason they did not perform each procedure for airway management, AED or IV was lack of manpower, limit of time or joggle of ambulance and legal restrictions. In conclusion, to increase survival rate of non-traumatic cardiac arrest in out-of-hospital, it is necessary to increase manpower, legal protection of EMS, establishment of standard operating procedure, practice for improvement technique and use of medication for ACLS.

High-quality customized services demand was growing due to the increase of aging, extremely nuclear family, disaster vulnerable of society. Ambulance service is required of fast and professional rescue and emergency service because of preventable death rate such as acute diseases, cerebral and cardiovascular diseases, suicides, etc. was higher than in developed countries. First aid will be available using patients information when emergency occurs, before arriving at the hospital. And emergency department is equipped with that patient care can be prepared in advance, increase the efficiency of emergency care. We received a variety of complex emergency call using high social awareness of 119 number and propose an efficient emergency medical service advancement strategy building an integrated response system with relevant organization.

Purpose: The purpose of this study is to analyze the clinical characteristics of anaphylaxis and anaphylactic shock caused by bee venom. Methods: We retrospectively collected the data of the patients who experienced anaphylaxis caused by natural bee sting or acupuncture using bee venom from January 1999 to December 2008. Seventy subjects were divided into the shock and non-shock groups. The clinical characteristics, sources of bee venom, treatments and outcomes were compared between the two groups. Results: The mean age of the subjects was $45.5{\pm}16.3$ years old and the number of males was 44 (62.9%). There were 25 patients in the shock group and 45 in the non-shock group. The age was older (p=0.001) and females (p=0.003) were more frequent in the shock group. Transportation to the hospital via ambulance was more frequent in the shock group (p<0.001). No difference was found in species of bee between the two groups. The cephalic area, including the face, was the most common area of bee venom in both groups. Anaphylaxis caused by bee sting commonly occurred between July and October. Cutaneous and respiratory symptoms were the most frequent symptoms related to anaphylaxis. Cardiovascular and neurologic symptoms were more frequent in the shock group. The amount of intravenously administered fluid and subcutaneous injection of epinephrine were much more in the shock group than that in the non-shock group. Conclusion: Older age was the factors related to anaphylactic shock caused by bee venom. Further validation is needed to evaluate the gender factor associated with shock.
