  • Objectives : The purpose of this study is to report the effectiveness of Korean medical treatment on 222 patients with alopecia areata. Methods : The subjects of this study were 222 patients who had received Korean medical treatment among the patients who visited the Balmer's Korean Medicine Clinic from January 1, 2016 to December 31, 2017. The extent of the alopecia areata was measured every month to determine the improvement. We analyzed the relationship between the types of alopecia areata, the extent of hair loss, duration of illness, age at first visit, and the treatment of alopecia areata. Results : 1. There were 100 males(45.0%) and 122 females(55.0%). The mean age of patients was 35.3 years old. 2. Of the 222 patients, 142(64%) were "cured". 31(14%) showed "good", 21(9.5%) showed "fair", 28(12.6%) showed "poor" improvement. 3. According to clinical records, of the 64 patients with alopecia areata(AA), 48(75.0%) were cured. Of the 100 patients with alopecia areata multiplex(AAM), 68 patients (68.0%), 12(50.0%) out of 24 patients with ophiasis, 5(55.6%) out of 9 with alopecia semi-totalis(AST), 3(50.0%) out of 6 patients with alopecia totalis(AT), 6(31.6%) out of 19 patients with alopecia universalis(AU) were cured. 4. Patients with the wider hair loss extent and with more severe type of alopecia areata required longer treatment period to cure, and the cure rate was lowered(p<0.05). 5. The shorter duration of the disease was related to the higher cure rate(p<0.05). If the treatment was started after six months of the duration, the cure rate was lowered. Therefore, beginning treatment within six months of the onset is recommended. 6. The period until terminal hair showed was 1.5 months for AA, 1.7 months for AAM, 1.6 months for ophiasis, 1.9 months for AST, 2.3 months for AT, 3.2 months for AU. 7. The treatment period to cure was 4.9 months for AA, 7.3 months for AAM, 7.7 months for ophiasis, 6.8 months for AST, 7.7 months for AT, 16.3 months for AU. 8. The major prognostic factors were extent and type of alopecia, duration of the disease and relapse. Conclusions : We analyzed the effectiveness of Korean medical treatment through cure rate according to the prognostic factors. This study will provide useful data for the treatment of alopecia areata.

  • Objectives : Alopecia areata has been known to be closely related to stress. The purpose of this study is to examine the relationship between stress and alopecia areata investigating how life events(which alopecia areata patients experience), amount of stress, coping strategy, and their personality type affect the oneset and aggravation of the disease. Methods : The subjects were 43 alopecia areata patients who visited the department of dermatology outpatients clinic, and 36 fungal infection diseases patients as the control group. They all completed Scale of Life Events, Coping Style Cheklist, Eysenck A-type Personality Inventory. The analysis of covariance with the age as covariate was carried out. Results : Compared to the control group, alopecia areata patients had significantly more stressful life events, and higher amount of stress, but there was no significant difference in coping strategy. Type A Personality is also resulted to have no significant relationship to alopecia areata. Conclusions : Alopecia areata patients had significantly higher amount of stress than the control group, and this result is consistent with the previous studies which suggest that stress contributes to the oneset of alopecia areata. The causual relationship between stress and alopecia areata should be further examined in order to be applied in clinical setting.

A 14-year-old woman was diagnosed as having MG. Seven months before diagnosed with MG, her hair and eyebrows had began to disappear and alopecia areata was diagnosed. We report this patient as a rare case of MG associated with alopecia areata without thymoma.

  • Treatments for alopecia areata include topical corticosteroid treatment, corticosteroid intralesional injection, systemic corticosteroid treatment, PUVA(psoralen-UVA) and topical immunotherapy. The therapeutic effects are variable. Alopecia totalis is hard to treat completely. Topical immunotherapy with dinitrochlorobenzene (DNCB), squaric acid dibutyl ester (SADBE) or diphenylcyclopropenone (diphencyprone, DPCP) represents the most accepted therapeutic modality for the treatment of extensive alopecia areata. We report two cases of alopecia totalis treated with DPCP. After DPCP treatment, total scalp hair was completely recovered.

  • Alopecia areata (AA) is an autoimmune disease that can affect any hair-bearing area. AA is known to be caused by immunological disorder but still, the pathogenesis is not fully understood. Many therapeutic modalities have been used to treat alopecia areata, with variable efficacy and safety profiles. Unfortunately, none of these agents is definitely curative or preventive alone. We studied hair growth efficacy and safety of Korean red ginseng (KRG) in AA comparing corticosteroid intra-lesional injection (ILI) alone patient group with ILI with KRG taking patient group using Folliscope 2.5 for 12 wk. Herein, we would like to report the efficacy of KRG in the treatment of AA and recommend KRG as a useful complimentary food for gaining efficacy of treatment for AA.

  • Objectives : We have studied female patients with either alopecia areata or androgenetic alopecia to evaluate psychological aspects, such as anxiety, depression, alexithymia, and characteristic personalities. In addition, we tried to examine the differences in psychological characteristics between these two types of alopecia, where the alopecia areata has been cotroversial on the role of stress in its etiology and the androgenetic alopecia seems to be more influeced by genetic and biological factors. Methods : All participated patients were females with alopecia for more than 1 you. Among them, 52 were with alopecia areata and 33 were with androgenetic alopecia. They were compared with 54 normal healthy controls by using MMPI, BDI, STAI-S, STAI-T, and TAS-20K. Results The average scores of F, Hs, D, Pd, Pa, Sc, Si in MMPI of alopecia groups were significantly higher than that of normal controls, and the androgenetic alopecia group had highest Hy and Pt scores. The average scores of BDI, STAI-S, and STAI-T in alopecia groups were higher than the normal controls. 94.2% of alopecia areata patients and 97.0% of androgenetic alopecia patients had severe depression, who scored higher than 23 in BDI. In TAS-20K, the average total scores of alopecia groups were higher than the normal control group, and the average Factor 3 score in androgentic alopecia was higher than the other groups. The alopecia groups scored higher than normal control group in STAI-S and STAI-T. Conclusion : Females with chronic alopecia were more depressed, had higher levels of anxiety, and more alexithymic than normal healthy females. In spite of arguments about etiological role of stress to alopecia, psychiatric interventions are needed for depression, and considerations for personality and psychological defense mechanism were needed in both types of alopecia.
