The Dopamine $D_2$ Receptor Locus as a Modifying Gene in Korean Schizophrenia, Alcoholism and Drug Addiction (정신분열증, 알코올중독, 약물중독에서 도파민 $D_2$ 수용체 유전자의 조절유전자(modifying gene)로서의 역할 - 충동적.강박적.탐닉적 행동을 나타내는 정신질환들에서 도파민 $D_2$ 수용체의 조절유전자로서의 역할 -)

  • The authors attempted to examine the allelic association between the A1 allele of Dopamine $D_2$ receptor and schizophrenia, alcoholism, drug addiction in Koreans. Schizophrenic patients(n=31), alcoholism(n=65), drug addiction(n=18) and controls(n=52) were examined by case-control study for distribution of the TaqI polymorphism of the dopamine $D_2$ receptor gene in Korean population to minimize the effect of racial differencies in gene frequencies. In schizophrenics, the numbers of schizophrenics with A1A1, A1A2, A2A2 were 9(29.0%), 15(48.4%) and 7(22.6%) respectively and in alcoholics with A1A1, A1A2, A2A2 were 14(21.5%), 36(55.4%) and 15(23.1%) respectively and in drug addiction with A1A1, A1A2, A2A2 were 2(11.1%), 10(55.6%) and 6(33.3%) respectively and in controls with A1A1, A1A2, A2A2 were 4(7.6%), 24(46.2%) and 24(46.2%) respectively. The prevalence of the A1 allele in schizophrenics, alcoholics, drug addiction and controls were 77%, 76.9%, 67% and 53.8% respectively. And the frequency of the A1 allele in schizophrenics, alcoholics, drug addiction and controls were 0.53, 0.49 0.39 and 0.31 respectively. There was significant difference in the frequency of the A1 allele between schizophrenics, alcoholics and controls. We also classified our alcoholic population. For classification by severity, we used the median MAST score 30 in our samples. There was also significant difference in the frequency of the A1 allele between less severe group(0.42) and more severe group(0.57). This data suggest that the A1 allele is associated with schizophrenia and alcoholism in Koreans. Furthermore the prevalence of the A1 allele increassed in more severely affected alcoholics. The authors conclude that our data support an allelic association between the A1 allele at dopamine $D_2$ receptor and schizophrenia, alcoholism. These results suggest the A1 allele of the $DRD_2$ gene is associated with a number of behavior disorders in which it may act as a modifying gene rather than as the primary etiological agent.

Effects of the Cheonggansoyo-san added Injinwiryeong-tang on Recovery from Damaged-liver and Mental-faculty Improvement in Alcoholism (청간소요산합인진위령탕의 Alcoholism에서의 간손상회복 및 학습능력향상 효과)

  • Effects of the Cheonggansoyo-san added Injinwiryeong-tang on recovery from damaged-liver and mental-faculty improvement in alcoholism were studied using male Sprague-Dawley rats, The rats were assigned into 4 groups; normal, control and CIX group. Control group administered ethanol(25 v/v %) at a dose 3 g/kg, while CIX group administered the water extract of Cheonggansoyo-san added Injinwiryeong-tang (CIX) 30 min before treating same dose of ethanol as control group for 10 days, orally. The GOT and GPT activities of rats were checked by Reitman & Frankel method, and all groups were subjected to trials of straight channel on the 1 st day and to those of multiple T-maze during the following 3 days. The GOT and GPT activities were increased in control group, but decreased in CIX group significantly. The time required in normal group for the straight channel of the 2nd and 3rd trials was significantly shorter than that of the 1 st, while the control group showed no significance. In the time required for the multiple T-maze trials, the control group showed no significance. But in the straight or T-maze trials, the CIX group showed significant decrease in the time required against the control group.

A Study on the Lifestyle of Korean Medical Students : Drinking Patterns and Eating Attitudes (한의대생의 생활양식 연구 - 음주양태와 섭식태도를 중심으로)

  • Objectives : The purpose of this study was to examine the lifestyle of korean medicine students based on their drinking patterns and eating attitudes. Methods : The survey was conducted on 101 korean medicine students to investigate their psychological characteristics. They were asked to complete Self-Administered Alcoholism Screening Test, Eating Attitude Test, and Minnesota Multiphasic Personality Inventory-2. Results : A total of 101 Korean medical students participated in the study. Non-drinker 19, drinker were 82. Unlikely to, possible and potential alcoholism 79(78.2%), 17(16.8%) and 5(5%) respectively. The mean and standard deviation of SAAST in the subjects were $3.88{\pm}3.33$ and the EAT score was $8.09{\pm}8.693$. There was a significant difference in the MMPI-2 items between non-drinker and drinker. The SAAST had an effect on the EAT score(${\beta}=0.319$). Conclusions : As a result of MMPI-2 test, in the potential alcoholism group, scale D, Mf, Si, INTR, TPA, SOD, Mt and GF were shown higher, and AAS and GM were shown lower than the normal group. In the bad eating attitude group, scale Hy, RC4, DISC and AA were shown higher than the good eating attitude group.

Association of Genetic Polymorphisms of Aldehyde Dehydrogenase II and CYP2E1 and Clinical Characteristics of Patients with Alcohol Dependence (알코올 의존 환자에서의 Aldehyde Dehydrogenase II와 CYP2E1 유전자 다형성과 임상적 특성간의 연관성)

  • Objective:This study was to explore the relation of genetic polymorphisms of ALDH2 and CYP2E1 to clinical characteristics of alcoholic patients and alcohol induced liver damage. Methods:The genotype and allele frequencies of 128 male hospitalized patients who met DSM-IV criteria for alcohol dependence were compared with 128 healthy male control subjects. The genetic informations of ALDH2 and CYP2E1 were identified with the technique of polymerase chain reaction and restriction fragment length polymorphism. The clinical characteristics of the alcoholic patients were assessed and analyzed in relation to the family history of alcoholism. For the relation of CYP2E1 genetic polymorphism to the liver damage, the blood levels of various liver function indicators such as ALT, AST, and protein were checked out. Results:1) The alcoholic patients with the family history of alcoholism had the earlier onset of age (p=0.001), the longer duration of illness(p=0.045), and higher NCA scores(p=0.018) than those without the family history of alcoholism. 2) Most alcoholic patients were homozygous for $ALDH2^*1$, compared to control subjects.(p=0.000) 3) There was no difference of CYP2E1 distribution between alcoholic patients and control subjects. However, alcoholic patients having mutant c2 allele showed higher alcoholism severity scores(p=0.004) and more hospitalizations(p=0.014) than those having c1 allele. 4) There was no relationship between CYP2E1 genotype and the functional abnormalities of the liver. Conclusion:This study suggests that $ALDH2^*1$ is highly related with alcohol dependence. Also mutant c2 allele of CYP2E1 is correlated with the severity of alcoholism and the number of hospitalization. But genetic polymorphim of CYP2E1 seems to have no relation to liver damages.

Impact of Adult ADHD Tendency on Self-Esteem and Alcohol Addiction (성인 ADHD 성향이 자아존중감과 알코올 중독에 미치는 영향)

  • The purpose of this study is to investigate the effect of adult ADHD tendency on self-esteem and alcohol addiction. For this purpose, 433 adults in Daegu and Gyeongsangbuk-do were studied. The results were as follows. First, there was a significant difference between self-esteem and alcoholism according to adult ADHD tendency. There was a significant difference in both self-esteem and sub-factors of alcoholism. The higher adult ADHD, the lower score of high class in self-esteem, and the scores of the upper group were higher in alcohol addiction. Second, the correlation among adult ADHD tendency and self-esteem, and alcohol addiction was found. As a result, it showed a high negative correlation with self-esteem, low correlation with alcoholism, and sub-factors. Third, As a result of examining the effect of adult adhd tendency on self-esteem and alcoholism, There were differences according to sub-factors. Therefore, it was found that ADHD tendency affects self-esteem and secondary problem of alcohol addiction. In other words, low self-esteem and alcoholism can be causes of ADHD. And it suggests that multifaceted and integrated-prevention treatment programs need to be developed.

A Comparative Study on the Degree of Alcoholism Recovery, Quality of Life, Empowerment and Self-esteem Based on Different Abstinence Periods of Alcohol use Disorder (알코올 사용장애자의 단주기간에 따른 알코올의존 회복, 삶의 질, 임파워먼트, 자아존중감 비교 연구)

  • This study investigated the differences in sociodemographic variables, alcoholism recovery, quality of life, empowerment and self-esteem based on different abstinence periods of alcohol use disorder. The study population from seven different regions in nationwide were 207 individuals with alcohol use disorder who had been abstinent. According to previous studies, 207 individuals were classified into different abstinence periods: less than one year, more than one to five years, and more than five years. As a result, the sociodemographic variables affecting differences in groups were gender, age, marital status, and economic activity. In addition, the group that maintained abstinence from alcohol for more than one year showed significant differences in alcoholism recovery, quality of life, empowerment, and self-esteem than those that maintained abstinence from alcohol for less than one year. Regardless of abstinence periods, alcoholism recovery, quality of life, empowerment, and self-esteem showed significant positive correlations. Therefore, the results of this study identified the factors influencing long-term abstinence and effects and suggested the need for alcohol-dependent recovery programs or therapeutic interventions for individuals who abstained from alcohol use.

The Experiences of Nursing Student on Alcoholism Care (간호대학생의 알코올중독 환자간호 실습경험에 관한 연구)

  • Purpose: The aim of this study was to describe the experiences of student nurses during their clinical practice periods in an alcoholic ward. Methods: A descriptive phenomenological study design was employed. Individual interviews were conducted and audio-taped. Colaizzi's method of data analysis was applied to inductively determine themes and formulated meanings. Data saturation was achieved and methodological rigor was established. Results: Four themes were identified and these themes included anxiety of nursing practice in unfamiliar setting, sympathy with patients, learning about the inpatient alcoholics care, and self-reflection. Conclusion: Nursing students had positive and negative experiences in their clinical practice. The results of this study had important implications regarding clinical practice and research for nursing clinical education.

A case of Couple Therapy in Oriental Medical Psychotherapy (한방정신요법을 이용한 부부치료 증례보고 -기능성 두통 부인과 알콜중독 남편의 부부치료를 통한 관계회복-)

  • In oriental medicine, Psychotherapy has been regarded as a part of physical therapy that seemed inseparable from psychological therapy from old times. Oriental Medical Psychotherapies are like following: Ii-Gyeung-Byun-Qi therapy(移精變氣療法), Oh- Ji-Sang-Seung therapy(五志相勝療法), Kyeongja- pyeongji therapy(驚者平之療法), Jieongoron therapy(至言高論療法) and so on. The special feature of these Psychotherapies is the unification of body and mind. For them, various oriental therapies have been under progress. Especially, Many Oriental Medical Psychotherapist insisted that pain of mind evoke pain of body, or vice versa. We observed Oriental Medical Psychotherapies to treat married couple and had a good result. Husband had alcoholism and wife had functional headache. Especially, wife wanted divorce. The problems of divorce and illness in married couple were resolved from Oriental Medical Psychotherapies.
