Social Networks and hypertension in Some rural residents Aged 60-64 (일부 60~64세 농촌 인구에서 사회조직망과 고혈압)

  • Face-to-face interviews were carried out to investigate the relationship between social networks and hypertension in 958 rural residents(males=440, females=518) aged 60-64 of a community-dwelling sample of Dalsung County from April to September in 1996. Eight elements of social network were measured : marital status, regular religious attendance, membership in groups, number of friends, relatives, siblings, children, grandchildren. Hypertensives were defined as meeting at least one of following criteria : hypertension history, systolic blood pressure more than 160 mmHg, diastolic blood pressure more than 95 mmHg. In univariate logistic regression for males, having 1-4 friends vs. none showed odds ratio 0.43 (95% Confidence interval CI 0.19-0.96) and having 2-3, 4 and more than 5 children had reduced prevalence of hypertension with odds ratios 0.21 (95% CI 0.06-0.72), 0.14 (95% CI 0.04-0.49), 0.24 (95% CI 0.07-0.82), respectively when compared with persons without children. In females, there was no elements of social network statistically significant. Having 5-9 grandchildren vs. none showed a marginally significant odds ratio 0.42. In multivariate logistic regression models for males with adjustment for age, education, body mass index, smoking and drinking, number of friends and children showed increased odds ratios and number of close relatives gained a statistically significant odds ratios (0.44-0.50). In females, the adjustment yielded little changes of odds ratios except number of grandchildren which gained a statistically significance. These results suggest that only a certain elements of social network may be associated with reduced risk of hypertension and they may be different between genders in rural resident aged 60-64.

  • This study was conducted to find out the factors which have an effect on the rural aged's natural history by disability in Activities of Daily Living(ADL) and ADL decline with 475 old people which were chosen except the dead 90 and unconfirmable 56 old people by performing follow-up survey for 4 years since 1993 on the basis of presurveyed data about 621 home old people over 60 years residing in 13 villages at Kangdong-myon, Kyongju-city, Kyongsangbuk-do Province, Korea. Such activities of daily living as bathing, dressing, going to toilet, transfer, feeding and continence were examined. 1. In the follow-up survey for 4 years, the ADL distribution of the dead was remarkably lower than the subject group. 2. It was also shown in the 4-year follow-up survey that 82.1% of high ADL group maintained high ADL while 77.8% of low ADL group died within 4 years. 3. The occurrence percentage of disabilities of each group by ADL item for 4 years appeared high in such an order as bathing, continence, dressing, going to toilet, transfer and feeding. 6.7%(8.1% for female and 4.6% for male) of the old people who were the high ADL group at the time of the first survey had disabilities that occurred after 4 years. 4. In the change of ADL according to general characteristics used to analyse the factors which have an effect on ADL decline, there was a significant difference in age and job, that is, the jobless old people had the higher degree of ADL decline. In the change of ADL according to behavioral pattern, for the male old people there was a significant difference in support of living expenses and subjective health condition. In this case, the degree of ADL decline was higher if they depended on their sons & daughters or spouses and felt that they were not healthy.

The Level of Diabetes Management of Agriculture, Forestry, and Fishery Workers (농림어업인의 당뇨병 관리 수준)

  • Objectives: The purpose of this study was to compare the diabetic management indicators between agriculture, forestry, and fishery workers (AFF) and other occupational adults (non-AFF) in community-dwelling diabetes. Methods: The study population consisted of 22,127 diabetic population ${\geq}19years$ who participated in the 2015 Community Health Survey. Chi-square test and logistic regression analysis was used to compare the diabetic management indicators between AFF and non-AFF. Socioeconomic characteristics such as age, gender, education level, monthly household income, National Basic Livelihood Security status, and marital status was sequentially adjusted. Results: Among total diabetic population, 3,712 people (16.8%) was AFF and 18,415 people (83.2%) was non-AFF. The fully-adjusted odds ratio [OR] (95% confidence interval [CI]) of current non-medical treatment (0.72, 0.66-0.79), measurement of hemoglobin A1c (0.61, 0.55-0.67), screening for diabetic retinopathy (0.76, 0.70-0.83), screening for diabetic nephropathy (0.75, 0.70-0.81), non-alcoholic or moderate drinking (0.70, 0.64-0.78), nutrition label reading (0.83, 0.71-0.98), low salt preference (0.85, 0.78-0.93), dental examination (0.60, 0.54-0.66), scaling experience (0.84, 0.77-0.93), regular toothbrushing (0.66, 0.58-0.76), and diabetes management education (0.84, 0.77-0.92) was significantly lower in AFF compared to non-AFF. In contrast, the fully-adjusted OR (95% CI) of AFF's low stress level (1.39, 1.26-1.52) and adequate sleep duration (1.22, 1.13-1.32) was significantly higher than non-AFF, which are better indicators of diabetic management in AFF. Conclusions: Overall, the level of diabetes management of AFF was not as good as that of non-AFF. In order to improve the level of diabetes management of AFF, a delicate diabetes intervention strategy considering the occupational characteristics of AFF will be needed.

STANDARDIZATION STUDY FOR THE KOREAN VERSION OF THE LURIA-NEBRASKA NEUROPSYCHOLOGICAL BATTERY FOR CHILDREN II : EVALUATION OF THE VALIDITY & CLINICAL UTILITY OF THE KOREAN VERSION OF LNNB-C (한국판 아동용 Luria-Nebraska 신경심리 검사의 표준화 연구 II : 타당도 및 임상적 유용성 검증)

  • Present study was to evaluate the validity and the clinical utility of the Korean version of Luria-Nebraska Neuropsychological Battery for Children(LNNB-C) in various groups including normal, brain damaged attention deficit hyperactivity disordered(ADHD), and psychiatrically disordered. The Korean version of LNNB-C and BGT were administered to clinical groups consisted of 51 patients(19 brain damaged, 16 ADHD. and 16 psychiatric controls), and to normal group composed of 147 children between the age of 8 and It Also KEDI-WISC was administered D clinical groups as a part of comprehensive psychological assessment There were significant differences between the brain damaged and the normals on all scales of LNNB-C, and between the normals and the ADHD on 11 clinical scales and 3 summary scales, which indicate the clinical validity for the scales of the Korean version of LNNB-C. The significant differences between the ADHD and the brain damaged on 3 summary scales were found, suggesting that the summary scales might play an important role id discriminating between two groups. Multiple discriminant analysis showed that the Korean version of LNNB-C significantly discriminates 3 groups - normals, ADHD, and brain damaged. Percentages of correct classification were ranged from 62.5% in the ADHD to 98.6Ta in the normals. For further evaluating the discriminant validity of the LNNB-C, the discriminant power of each items were calculated, and 131 of the 147 items discriminated significantly between the brain damaged and the normals. The scales of LNNB-C significantly correlated with the error scores of BGT and the most of scales of KEDI-WISC. These results put together : strongly support the concurrent and the discriminant validity of the Korean version of LNNB-C in diagnosing brain damage. The limitations of present study and several issues for the luther study were discussed.

A DOUBLE BLIND CROSS-OVER COMPARISON OF ANTIDEPRESSANT AND ANTIANXIETY EFFECTS OF PAROXETINE AND PLACEBO IN CHILD-ADOLESCENT AND ADULT DEPRESSIVE NEUROSIS (소아 ${\cdot}$ 청소년 및 성인 우울신경증 환자에 대한 Paroxetine의 항우울 및 항불안 효과 검증 : 위약과의 이중맹 교차 비교실험)

  • Paroxetine is a potent and selective serotoin re-uptake inhibitor. It is well known as an effective and safe antidepressant and increasingly used for neurotic or non-psychotic depression with anxiety symptoms. The present study assessed antidepressant and antianxiety efficacy and tolerability of paroxetine against placebo in child-adolescent and adult depressive neurosis patients. 232 subjects aged 8-55 years and meeting DSM-III-R criteria for depressive neurosis or dysthymia were divided into 8 subgroups according to their sex and age(8-11 yeard old, 12-17 years old, 18-35 years old and 36-55 years old subgroup in each male and female group). In each subgroup, the randomly assigned half of the patients were treated with paroxetine(10-30mg/day) and the others with placebo for the first 2 weeks in double blind fashion. After 1 week of drug-washout period, paroxetine and placebo groups were crossed over. The depression and anxiety symptoms were assessed with Hamilton Depression Scale(HDS) and Hamilton Anxiety Scale(HAS) at baseline and every 1 week during the trial periods. The levels of reduction in HDS and HAS scores from baseline after 2-week trial were compared between paroxetine- and placebo- treated periods by paired t-test. In all the 8 subgroups, statistically significant differences between paroxetine and placebo were found on the antidepressant efficacy after 2-week treatment. The antidepressant efficacy of paroxetine compared to placebo was most prominent in child and adolescent female groups. On anxiety symptoms, paroxetine was also significantly more effective than placebo. The antianxiety efficacy of paroxetine compared to placebo was most prominent in male and female child groups and young adult female group aged 18-35 years. As for the adverse effects of paroxetine, 3 out of 232 subjects reported mild indigestion and abdominal pain. however, in all the 3 cases, the symptoms improved without reduction of dosage or discontinuation of the drug. In conclusion, paroxetine showed significantly higher antidepressant and antianxiety efficacy compared to placebo in child-adolescent and adult depressive neurosis patients after 2-week treatment. Further trials of paroxetine in depressive neurosis are warranted to elucidate the long-term antidepressant and antianxiety efficacy of paroxetine.

Plant Community Structure of Abies holophylla Community from Sinseongam to Jungdaesa in Odaesan National Park (오대산국립공원 신성암~중대사 전나무림 식물군집구조 특성)

  • This study was carried out to the structure of plant community from Sinseongam to Jungdaesa in Odaesan National Park, furthermore, it seeks to curate the basic data for planning of the Abies holophylla's forest management in Odaesan National Park. In order to identify the current ecological environment, this study explored the actual vegetation as primary research and set to twenty plots(i.e. $400m^2$) for analysing detailed structure of plant communities. The research methodology was qualitative analysis, therefore it used TWINSPAN and DCA analysis tools. Especially, TWINSPAN performed well in several comparisons of classification techniques, DCA is one of the ordination technique showed that the plant communities. The plant community was analysed classification and ordination by TWINSPAN and DCA, moreover it was analysed the structure of plant community such as importance percentage of woody species, DBH class distribution, the index of diversity and rate of sample tree growth. The main vegetation was A. holophylla-Quercus mongolica forest and Deciduous broad-leaved forest in the communities where located in low altitude and valley, whereas main vegetation where located in high altitude and slope was Q. mongolica forest. The research site's plant communities were classified four groups. In all of communities, A. holophylla was dominant species in main canopy layer, furthermore, the three communities (community I, II, III) are growing up next generation of A. holophylla excluding community IV. The communities (community I, II, III) can be sustained current status which dominates the A. holophylla communities, simultaneously, there might be expanded the Deciduous broad-leaved communities by Carpinus cordata, Betula schmidtii and so on. While, it showed that the community IV tended to be weaken the forces of A. holophylla, therefore the community IV can be transferred to C. cordata-Deciduous broad-leaved communities in the future. The age of sample trees was 79~128(i.e. A. holophylla), 75~87(i.e. Pinus koraiensis) and 190 years(i.e. Ulmus davidiana var. japonica). The index of Shannon's Species diversity (H') were ranged from 0.3889 to 1.3332 in the communities.

A Study for Plant Community Structure and Management Plan of Pinus densiflora Forest in Byeonsanbando National Park (변산반도국립공원 소나무림 식물군집구조 및 관리방안 연구)

  • This study is intended to provide basic date for the efficient management of Pinus densiflora community by analyzing ecological characteristics of Pinus densiflora inhabiting Byeonsanbando National Park. According to investigations, P. densiflora community and P. densiflora-Quercus community are widely distributed, occupying 40.3% of the total area. 21 sites ($400m^2$per site) are selected for TWINSPAN analysis, and the result indicates that the whole community of P. densiflora and P. densiflora-Quercus can be classified into 8 types, and the age of P. densiflora is 40-50 years, which is similar to that of deciduous broad-leaved trees. The 8 community types are: P. densiflora community which competes with Quercus variabilis; P. densiflora community in which Fraxinus sieboldiana and Quercus serrata grow in the understory layer; P. densifloa community in which Q. serrata grow in the understory layer and Smilax china var. microphylla in the shrub layer respectively; P. densifloa community in which P. densiflora and F. sieboldiana grow in the understory layer; P. densiflora community which competes with Q. serrata and Carpinus tschonoskii; P. densiflora community which competes with Q. variabilis and Q. serrata; P. densiflora community in which Prunus sargentii grow; P. densiflora community in which Abies holophylla grow. P. densiflora community which competes with Q. variablis and C. tschonoskii seems to be in a stage of succession to deciduous broad-leaved community. The analysis indicates that Shannon diversity index is 0.2756-1.3879. It also indicates that there is a negative correlation between P. densiflora and Q. variabilis and C. tschonoskii; there is a positive correlation between P. densiflora and F. sieboldiana and Rhododendron schlippenbachii. These investigations show that the transformation of vegetation is already under way. There is a possibility that ecological succession can take place in 30.4% of the total area from P. densiflora to Quercus and deciduous broad - leaved trees. Therefore, it is recommended that the preservation and maintenance of P. densiflora be implemented by taking control of competing species which undermine the stability of P. densiflora forest community.

  • This study is aimed at investigating the compensatory education which was already implemented or is being implemented in the U.S.A. and Japan; and at studying the types of programs and their characteristics; and at sounding out the possibilities of the application of such programs in family and social conditions is Korea. In order to achieve the above mentioned objectives, the established items for the study are as follows: (1) Various types of early children's education (2) Programs of compensatory education for the disadvantaged Children (3) Head Start Program, Early Training Project and Montessori School (4) Integrated Preschool Programs (5) Day-Care Center for employed mothers We investigated the various compensatory education programs for the preschool children who are in economically, socially, culturally disadvantaged conditions. Head Start Programs were federally supported programs for preschool children and opened as summer programs in 1965 for the first time. The purpose of Head Start has been to give preschool children the kinds of experiences they need in preparation for school. The Head Start children were found to be significantly better prepared for school than the normal children. However, after six to eight months, their initial advantages had virtually. disappeared and then the simple problem with Head Start and other such programs was that little long-term good could be evidenced unless the high quality educational environment was maintained. Therefore, to solve this problem, three other programs were funded as part of the overall Head Start. These three programs are the Parent-Child Center, Home Start, and the Child and Family Resources Program. The Early Training Project for disadvantaged children was implemented by Klaus and Gray of Peabody College in 1962. The program was a field research study concerned with the development and testing over time of procedures for improving the educability of young children from low income homes. Its major concern was to study whether it was possible to offset the progressive retardation observed in the public schooling careers of children, living in deprived circumstances. Children, who were trained through the Early Training Project were superior to control groups in the test of IQ and vocabulary as well as linguistic abilities, and preparation for reading. This project showed the possibilities which could prevent preschool children from being disadvantaged socially, culturally and mentally. In 1907, Montessori School was established by Maria Montessori in Italy and her school program has been introduced at present to several countries in the world as one compensatory educations. She first began her experimental methods with retarded children, followed by disadvantaged children from the tenements of Rome. The Montessori approach futures a prepared environment and carefully designed, self-correcting materials. The Montessori curriculum presents tastes that feature sequence, order, and regularity, in addition to those that develop motor and sensory skills. She was interested in children's intellectual development and in developing good work habits. One of the latest developed programs for disadvantaged children is "Integrated Preschool Program" which has successfully integrated handicapped and nonhandicapped children. Several studies have showed that handicapped children in integrated school environments are accepted by and interact with their nonhandicapped peers. In fact, this program provides a number of potential, and perhaps opportunities for nonhandicapped children to serve as valuable resources in fostering the development of their handicapped peers. Next we turn to Japanese programs which are divided into two different types. One is Day-Care Center which was established by Child Welfare Law and the other is kindergarten organized by School Education Law. The kindergarten opened in 1876 and it has been part of school systems since 1947 by the implementation of education law, and the Day-Care Center which started in 1890 for the employed mothers. was changed into Day-Nursery by the enactment of child welfare law in 1947. The laws and operational regulations for the Day-Nursery were set up and were put in effect by the establishment standard acts of children welfare facilities, and the Day-Nursery has been operated in various types by the increasing demand, chiefly because of the socio-economical changes of family structures in both urban and suburban areas. Nursery education for physically and mentally disadvantaged children is for those who are blind, deaf and dumb, mentally retarded; physically disadvantaged by accidents or diseases. Montessori education in Japan was started in 1968 and many research groups for studying Montessori were organized. In 1977, Montessori remedial education society was also organized in which they started a number of studies; a study for developing materials; in-service training for the remedial education; and seminars and lectures, etc It is strongly suggested that we study the early educations that are being implemented in Japan and a variety of compensatory educations that were already implemented in the U.S.A. and modify them for the organization of our own model and properly accommodate them to our social needs.

Clinical Study of Heart Rate Variability on Patients with Idiopathic Parkinson's Disease according to Clinical Scale(Hoehn-Yahr stage, UPDRS) (특발성 파킨슨병 환자의 임상척도에 따른 심박변이도의 변화에 대한 연구)

  • Objective : This study was designed to assess the heart rate variablitv of patients with Parkinson's disease according to Hoehn-Yahr stage. Methods : Subjects were voluntarily recruited by newspapers and internet. All the subjects are confirmed as idiopathic parkinson's disease by a neurologist. The patients were grouped by Hoehn-Yahr(H-Y) stage. Heart rate variability was measured for 5 minutes after 5 minutes' bed rest. Subjects were comprised of 3 groups which were H-Y stage I (n=14), H-Y stage H(n=9), H-Y stage III(n=7). We compared the heart rate variability of each groups by one-way ANOVA test. We also studied the relationship between UPDRS score and Heat rate variability components (SDNN, RMS-SD and TP) by pearson correlation analysis and simple linear regressin analysis Results: Age, duration was matched among groups. SDNN, RMS-SD, TP were significantly decreased as H-Y stage increased(p<0.05). Especially there was significant difference between H-Y stage I and III group(p<0.05). But other components(Heart rate, LF, HF, LF/HF ratio) were not showed significant difference. SDNN, RMS-SD and TP were significantly decreased as UPDRS scores increase(p<0.05). Conclusion : This study suggests that the function of the autonomic nervous system decreases as Parkinson`s disease progresses.

  • Objective : The aim of this study was to rivew systemically literature and clinical trials in the treatment of urinary incontinence or lower urinary tract syndrome(LUTS). Methods : Computerized literature searches were carried out on two electronic database, and computerized searching on some korea oriental medicine journals in library of Kyung-Hee Medical center. Results : 1. Three reports of review study, six reports of experimental study and fourteen reports of clinical trials were collected and reviewed. Three reports of review study were all printed in the korea oriental medicine journal. From 2000, researches and studies have been increased in quantity and improved in quality. 2. Urinary disturbance include variable symptoms of lower urinary tract symptoms, urinary incontinence, in theaspect of Oriental medicine these symptoms are anurin, dysuria, urinary incontinence, nochumal enuresis, uracratia and so on. 3. Roughly physiological procedure of Acupuncture in Treatment of Urianry Disturbance may be that effect of acupuncture stimulation for parasympathetic nerve, sleep-arousal system in cerebrum, pontine/spinal urination center and pudendal/pelvic nerve affect bladder in expansion of bladder capacity, inhibition of urinary contraction and affection in periurethral muscle by continuous excitement of spinal annular circuit and synapse of neuron. 4. Clinical result for acupuncture treatment in urinary disturbance is summarized that acupuncture treatment in urianation disturbance of Neurogenic Bladder, Incontinence, Cycitis, Nocturnal Enuresis, Prostatitis/Pelvic Pain Syndrom and so on is significant clinical trials and technique. Conclusion : Hereafter, in the old age society these variable urinary disturbance patients are increased and desire of treatment may be also increased. So study of various and formal treatment and tecnnique is needed.
