보기약(補氣藥)의 부량반응(不良反應)에 관한 문헌적 고찰 (A philological study on adverse effect of restoratives for invigorating qi(補氣藥))

  • Objectives & Method:We investigated adverse symptoms, toxicity, treatment and prevention against adverse effects of restoratives for invigorating qi(補氣藥) in order to use herbal medicines accurately. Result:Ginseng Radix(人參), Codonopsis Pilosulae Radix(黨參), Panacis Quinquefolii Radix(西洋參), Astragali Radix(黃芪), Atractylodis Rhizoma Alba(白朮), Dioscoreae Rhizoma(山藥), Dolichoris Semen(白扁豆), Glycyrrhizae Radix(甘草), Jujubae Fructus(大棗) and Mel(蜂蜜) may give rise to some side effects, allergic reaction or toxic symptoms in restoratives for invigorating qi(補氣藥). The representative methods of poisoning treatment in western medicines are stopping medication, washing out the stomach, promotion of vomiting, causing diarrhea, supplies of grape sugar and symptomatic treatment, etc. The representative methods of poisoning treatment in oriental medicine take advantage of various herbs. And Oriental medical doctor should meet symptoms as patients call for attention. In order to prevent against poisoning of restoratives for invigorating qi(補氣藥), the patients should keep usage, dosage and notes. Conclusion:We should pay attention to clinical using of Ginseng Radix(人參), Codonopsis Pilosulae Radix(黨參), Panacis Quinquefolii Radix(西洋參), Astragali Radix(黃芪), Atractylodis Rhizoma Alba(白朮), Dioscoreae Rhizoma (山藥), Dolichoris Semen(白扁豆), Glycyrrhizae Radix(甘草), Jujubae Fructus(大棗) and Mel(蜂蜜) in restoratives for invigorating qi(補氣藥).

Status Epilepticus Caused by Nefopam

  • Nefopam, a centrally acting analgesic, has been used to control postoperative pain. Reported adverse effects are anticholinergic, cardiovascular or neuropsychiatric. Neurologic adverse reactions to nefopam are confusion, hallucinations, delirium and convulsions. There are several reports about fatal convulsive seizures, presumably related to nefopam. A 71-year-old man was admitted for surgery for a lumbar spinal stenosis. He was administered intravenous analgesics : ketorolac, tramadol, orphenadrine citrate and nefopam HCl. His back pain was so severe that he hardly slept for several days; he even needed morphine and pethidine. At 4 days of administration of intravenous analgesics, the patient suddenly started generalized tonic-clonic seizures for 15 seconds, and subsequently, status epilepticus; these were not responsive to phenytoin and midazolam. After 3 days of barbiturate coma therapy the seizures were controlled. Convulsive seizures related to nefopam appear as focal, generalized, myoclonic types, or status epilepticus, and are not dose-related manifestations. In our case, the possibility of convulsions caused by other drugs or the misuse of drugs was considered. However, we first identified the introduced drugs and excluded the possibility of an accidental misuse of other drugs. Physicians should be aware of the possible occurrence of unpredictable and serious convulsions when using nefopam.

약물 유해 반응에 대한 간호사의 모니터링 실천에 관한 연구 (Nurses' Monitoring Practice for Adverse Drug Reactions)

  • Purpose: The aim of this study was to identify the relationship between nurses' knowledge of adverse drug reactions (ADR), attitudes, and monitoring practices and to establish an effective ADR reporting system and provide baseline data for its activation. Methods: The participants in this study were chosen by convenience sampling and included 182 nurses working at major general hospitals that operate a Regional Drug Safety Center. Data were collected from June 1 to 12, 2015 and analyzed using Cronbach's ${\alpha}$, descriptive statistics, independent t-test, one way ANOVA, Pearson correlation coefficient and stepwise regression with the SPSS program. Results: The nurses' average score for knowledge was 7.62 points, for attitude, 41.04 points and for monitoring practices, 34.22 points. ADR monitoring practices positively correlated with knowledge (r=.19, p=.011), attitude (r=.41, p<.001), drug performance competency (r=.54, p<.001), and drug education satisfaction (r=.54, p<.001). Drug performance competency, drug education satisfaction, and attitudes explained 42.0% of the ADR monitoring practices (Adj $R^2=.42$, F=43.95, p<.001). Conclusion: In order to facilitate and encourage nurses' voluntary monitoring practice of ADR, efforts must be made to create positive attitudes toward ADR, and to increase drug performance competency and drug education satisfaction.

알레르기를 포함한 의약품 부작용에 대한 환자의 인식도 조사 (Study on the Patients' Perception on Adverse Drug Reactions Including Drug Allergy)

  • Adverse drug reactions (ADR) including allergy are more preventable if patients recognize. This study was to investigate ADR recognition by patients who visited one university hospital located in Seoul, by face-to-face or telephone interviews using questionnaires. Recognitions, understandings, and managements on ADR in 225 adult patients enrolled in this study, were compared between ADR experienced group (n=89) and no-experienced group (n=137). Common knowledges and direct experiences on ADR were attributable to high perceptions on ADR, and lacking of active communications with clinical professionals to manage ADR was shown. In general, there were no significant differences in ADR perceptions between ADR experienced and no-experienced groups in almost items. This study findings would be useful to discuss clinical solutions for preventing ADR including drug allergy from patient individual level, and strategies including public education, guidebook on drug allergy, patient medication history record, and proactive efforts by professionals to improve ADR perception levels would be suggested.

The Clinical Implications of Poly Implant Proth$\grave{e}$se Breast Implants: An Overview

  • Mammary implants marketed by Poly Implant Proth$\grave{e}$se (PIP) were found to contain industrial grade silicone and this caused heightened anxiety and extensive publicity regarding their safety in humans. These implants were used in a large number of patients worldwide for augmentation or breast reconstruction. We reviewed articles identified by searches of Medline, PubMed, Embase, and Google Scholar databases up to May 2014 using the terms: "PIP", "Poly Implant Proth$\grave{e}$se", "breast implants" and "augmentation mammoplasty" "siloxanes" or "silicone". In addition the websites of regulating bodies in Europe, USA, and Australia were searched for reports related to PIP mammary implants. PIP mammary implants are more likely to rupture than other implants and can cause adverse effects in the short to the medium term related to the symptoms of rupture such as pain, lumps in the breast and axilla and anxiety. Based on peer-reviewed published studies we have calculated an overall rupture rate of 14.5% (383/2,635) for PIP implants. However, there is no evidence that PIP implant rupture causes long-term adverse health effects in humans so far. Silicone lymphadenopathy represents a foreign body reaction and should be treated conservatively. The long-term adverse effects usually arise from inappropriate extensive surgery, such as axillary lymph node dissection or extensive resection of breast tissue due to silicone leakage.

소음인(少陰人).소양인(少陽人) 처방에 대한 사상인(四象人)의 반응에 대한 임상보고 (A Clinical Report on the Adverse Reactions of Sasangin by the Prescriptions of Soeumin.Soyangin)

  • 1. Objective The aim of this study was to survey the adverse reactions by the prescriptions of Soeumin.Soyangin and to compare the tendency of their symptoms according to Sasang constitutional type. 2. Methods The clinical data for this study were based on the medical records of total 91 cases collected from an oriental clinic during 1996-2004. Their constitutional types were diagnosed by a specialist and then confirmed by drug reactions. To evaluate reliability of the analysis, we only analysed the cases above grade C. The target prescriptions were Palmulgunja-tang, Bojungikgi-tang, Hyangsayangwi-tang, Ijung-tang, Hyeongbangsabaek-san, Jeoryeongchajeonja-tang, Yanggyeoksanhwa-tang, Yangdokbaekho-tang and the sorts of Jihwang-tang. 3. Results and Conclusion The prescriptions which were not suitable for their Sasang constitutional type induced indigestion, stomachache and evacuation troubles basically. Besides, the prescriptions of Soeumin caused some fever of the upper body and skin on Taeeumin and Soyangin. And the prescriptions of Soyangin lead to more severe digestive and evacuation troubles on Taeeumin and Soeumin. 4. Conclusion: This study need to be compensated by additional clinical studies which are more systematic and continuous. This way we can make the firm evidence for approving the difference of drug susceptibility according to the Sasang constitutional type.

폐경기 안면홍조 증상에 대한 한약처방 이선탕의 투여경과 및 이상반응 보고 : 증례보고 및 문헌고찰 (Administration Progression and Adverse Event Report of Er-Xian Decoction for Menopausal Hot Flushes: Case Report and Review of Literature)

  • Objective: We report on the administration progression and adverse event of Er-Xian decoction for menopausal hot flushes. A hot flush is a rapid and intense heat-dissipation reaction that involves considerable sweating, enlargement of the peripheral blood vessels, and internal body heat. Er-Xian decoction is a prescription for treating menopausal hot flush symptoms in postmenopausal women. We report administration progression and adverse event of Er-Xian decoction for menopausal hot flush. Methods: We used the Menopause Rating Scale and Menopause-Specific Quality of Life questionnaire to evaluate the progression of hot flushes, and we conducted a literature review to determine the effective dosage of Er-Xian decoction. Result: In all cases, Facial hot flushes and evaluation variables were improved by the administration of EXD. However, in Case 1, a rise of liver function indexes which may be related to EXD administration was observed. Conclusion: Considering that menopausal flushing usually lasts for several years, The resluts are meaningful that the short-term administration of EXD led to improvement of symptoms. In order to further use EXD in the future, it will be necessary to conduct follow-up studies on the subject of safety verification, such as repeated dose toxicity studies.

  • Stevens-Johnson syndrome (SJS) is a rare but life-threatening skin reaction disease and carbamazepine is one of its most common causes. We report a case of SJS secondary to carbamazepine in a patient with previous pruritus due to carbamazepine which was given for treatment of trigeminal neuralgia. We would like to caution all providers that carbamazepine readministration should be avoided in the patient with a previous history of SJS or adverse skin reaction. In addition, we strongly recommend gradual titration when initiating treatment with carbamazepine.

Adverse drug reaction monitoring during antimicrobial therapy for septicemia patients at a university hospital in New Delhi

  • Background/Aims: Adverse drug reaction (ADR) is an appreciably harmful or unpleasant reaction, resulting from an intervention related to the use of a medicinal product. The present study was conducted in order to monitor the frequency and severity of ADR during antimicrobial therapy of septicemia. Methods: A prospective, observational, and noncomparative study was conducted over a period of 6 months on patients of septicemia admitted at a university hospital. Naranjo algorithm scale was used for causality assessment. Severity assessment was done by Hartwig severity scale. Results: ADRs in selected hospitalized patients of septicemia was found to be in 26.5% of the study population. During the study period, 12 ADRs were confirmed occurring in 9, out of 34 admitted patients. Pediatric patients experienced maximum ADRs, 44.4%. Females experienced a significantly higher incidence of ADRs, 66.7%. According to Naranjo's probability scale, 8.3% of ADRs were found to be definite, 58.3% as probable, and 33.3% as possible. A higher proportion of these ADRs, 66.7% were preventable in nature. Severity assessment showed that more than half of ADRs were moderate. Teicoplanin was found to be the commonest antimicrobial agent associated with ADRs, followed by gemifloxacin and ofloxacin. Conclusions: The incidence and severity of ADRs observed in the present study was substantially high indicating the need of extra vigilant during the antimicrobial therapy of septicemia.

알레르기 비염 질환자를 대상으로 한 더덕(Codonopsis lanceolata (S. et Z.) Trautv.) 추출물의 과민면역반응 개선에 대한 유효성 및 안전성 평가 (Efficacy and Safety of Codonopsis lanceolata (S. et Z.) Trautv. Extract on the Improvement of the Hypersensitivity Reaction in Allergic Rhinitis Patients)

  • Objectives : This clinical trial is performed to investigate the effect of improving the hypersensitivity reaction and the safety of powdered ethanol extract Codonopsis lanceolata (S. et Z.) Trautv. for allergic patients. Methods : This study was randomized, double blinded, placebo-controlled clinical trial. 60 allergic rhinitis subjects were randomly assigned to Treatment group(n=30) and Control group(n=30). They were received the test substance or placebo in their $1^{st}$ and $2^{nd}$ visit, and took it once a day, 1 powder bag per 1 time, for 8 weeks. The freeze dried powder of Codonopsis lanceolata (S. et Z.) Trautv. was mixed with 30% ethanol and the filtrate was collected thrice with constant stirring of the mixture. The placebo was prepared in the same form as the test substance by replacing with corn starch. Total Nasal Symptom Score(TNSS) questionnaires and laboratory tests were performed at the screening and last visit to evaluate the efficacy. And to assess safety, vital signs and adverse events were confirmed at every visit. Statistical analysis about the result was performed using SAS 9.4. Results : In treatment group, there was a significant decrease in B cell after taking the test substance(p<0.005). And there was a statistically significant difference between groups(p<0.005). Changes in nasal symptoms were not statistically significant between groups, and both groups showed statistically significant differences before and after the test(p<0.0001). Also there were no adverse events associated with the clinical trial product. Conclusions : This clinical trial showed that Codonopsis lanceolata (S. et Z.) Trautv. extract was helpful in improving the hypersensitivity reaction in allergic patients without significant side effects.
