성인 당뇨병 환자의 혈당, 혈압 및 지질 조절률과 그 영향요인 (Glucose, Blood Pressure, and Lipid Control in Korean Adults with Diagnosed Diabetes)

  • Purpose: The purposes of this secondary data analysis study were first to identify the number of Korean adults achieving goals set by the American Diabetes Association for hemoglobin A1c (HbA1c), blood pressure (BP), and low-density lipoprotein cholesterol (LDL-C), and secondly to identify the characteristics associated with lack of goals attainment. Methods: The sample was 413 Koreans with diagnosed diabetes aged thirty years or older who participated in the Fourth Korea National Health and Nutrition Examination Survey. Goals attainment for HbA1c, BP, and LDL-C were presented in percentages. Logistic regressions were used to examine associations between participants' characteristics and lack of goals attainment. Results: About 48% had HbA1c<7%, 48.2% had BP<130/80mmHg, and 34.1% had LDL-C<100mg/dL. Only 8.7% of the sample achieved all three parameters. In multivariate analysis, younger age, longer diabetes duration, insulin use, and abdominal obesity were associated with not meeting HbAlC goal. Smoking and use of antihypertensive medication were associated with BP${\geq}$130/80 mmHg. No use of lipid lowering agents was associated with LDL-C${\geq}$100 mg/dL. Conclusion: Many Koreans with diabetes were not at goals for HbA1c, BP, and LDL-C. For optimal control, appropriateness of therapy and poor lifestyle habits should be assessed periodically and managed accordingly.

Dietary Intake of Korean Adults Is Not Significantly Affected by Sleep Duration

  • This study investigated the effect of sleep duration on food and nutrient intake among adult Koreans. The effects of sleep duration on dietary patterns was studied in 7,370 Korean adults, aged 20 and older, who participated in the 1998 Korean National Health and Nutrition Survey in which the 24 hour recall method was used to estimate dietary intake. Sleep duration data from individual subject were collected by interview as a part of a behavior survey. Analysis of variance and Duncan's multiple range test were used to test the differences in food and nutrition intake and sleep duration. Food intakes of the male subjects were not significantly different among three groups. However, there were significant differences observed in intakes of beverage and alcohol among female subjects among the three groups. There were no significant differences observed for nutrient intake by sleep duration for either gender. Unlike the observation from European and American studies, food and nutrient intake of Korean subjects, in general, were not greatly affected by duration of sleep. This result may suggest that dietary habits of Koreans, as affected by sleep, are quite different from those of Europeans & Americans.

한국인의 요중 크레아티닌 농도에 관한 연구 -국민건강영양조사 4기 자료 이용- (Urinary Creatinine Concentration in the Korean Population in KNHANES IV, 2009)

  • Objectives: Biomonitoring is used to assess human environmental exposures. Urinary biomonitoring data are typically adjusted to a constant creatinine concentration to correct for variable dilutions among spot samples. Our objective was to investigate urinary creatinine concentrations by region, gender, age and lifestyle. Methods: We studied urinary creatinine concentrations in 6,286 Koreans aged 20 years old and older who participated the fourth Korea National Health and Nutrition Survey (KNHANES IV) in 2009. Urinary creatinine concentration analysis used the Jaffe method. Results: The average urinary creatinine concentration of 6,286 Koreans was 158.99 mg/dl. The urinary creatinine concentration was significantly higher among men (184.97 mg/dl) than women (130.02 mg/dl). In both men and women, urinary creatinine concentrations were significantly different according to age group (p < 0.01). Whereas the urinary creatinine concentration of men significantly differed with smoking in adjusted comparison analysis, that of women significantly differed by education level. Ninety-two percent of urinary creatinine concentration was included within WHO guidelines effective range(30 mg/dl ${\leq}$ urinary creatinine concentration ${\leq}$ 300 mg/dl). In multiple regression analysis, creatinine was influenced by gender, age and body mass index (BMI). Conclusions: The urinary creatinine value obtained from representative samples of adult Koreans aged 20 year and older in KNHANES IV 2009 could be used as a reference value for other nationally studied surveys, such as abandoned metal mine surveys and surveys for industrial complexes.

The Relationship between Risk Factors for Cardiovascular Disease and Levels of Plasma Total Homocysteine, Folate and Vitamin {TEX}$B_{12}${/TEX} in Koreans

  • The elevation of total plasmahomocysteine is now an established risk factor for cardiovascular disease. Plasma folate and vitamin {TEX}$B_{12}${/TEX} influence Hcy metabolism as cofactors. In this study, we studied the relationship of major risk factors for cardovascular disease, including advanced age, male gender, obesity, hypertension, hyperglycemia, and dislipidemia and plasma homocyteine, folate and vitamin {TEX}$B_{12}${/TEX} levels in Koreans. A total of 195 adult Koreans participated. The subjects were divided into three groups according to how many major conventional risk factors of cardiovascular disease they had: no risk, low risk (1~3 risk factors) and high risk (>3 risk factors) groups. As the number of risk factors increased, the plasma homocysteine levels significantly increase, while the plasma folate levels significantly decreased. The plasma homocysteine levels re higher in males than in females. The subjects with hyperglycemia had higher plasma homocysteine levels than the subjects without the risk factor. Also the subjects with dislipidemia had higher plasma homocysteine levels than the subjects without the risk factor. The plasma folate and vitamin {TEX}$B_{12}${/TEX} levels were significantly lower in males tan females. However, there were no significant differences in plasma folate and vitamin {TEX}$B_{12}${/TEX} levels between the subjects with or without other risk factors. These results indicate that plasma homocysteine levels were positively related with risk factors for cardiovascular disease and plasma folate levels were negatively related with the risk factors for cardiovascular disease. Also, we conclude that plasmahomocysteine levels might be related to the combination of risk factors, rather than an individual risk factor.

Zinc Intake and Status of the Selected Korean Adults

  • Zinc intake and status of South Koreans from rural, urban and metropolitan areas were investigated. The dietary habits of 760 healthy male and female adult subjects with a mean age of 54 were assessed using a food frequency questionnaire and were verified using 24 h dietary recall. Daily Zn intakes for men and women were 7.4$\pm$5.4 mg and 7.0$\pm$5.4 mg, respectively, which were 62% and 70% of the Korean RDA. The phytate : zinc and phytate ${\times}$ calcium : zinc molar ratios were 38 and 398, respectively. Both the low intake of zinc and the high extremely phytate and phytate ${\times}$ calcium ratios with Zinc suggest that South Koreans may be at risk of zinc deficiency. Plasma zinc (86$\pm$61 $\mu\textrm{g}$/dL), urinary zinc (33$\pm$27 $\mu\textrm{g}$/dL) and plasma alkaline phosphatase (102$\pm$52 mU/mL) levels within the normal range did not however suggest marked Zinc deficiency in these subjects. However, conventional zinc biomarkers aye known to be unreliable for assessment of marginal zinc deficiency. Based on zinc intake alone, it is likely that at least a proportion of these subjects were marginally zinc deficient and the wider consumption of zinc rich, phytate deficient foods, particularly in rural areas, would be beneficial.

Age-Specific Thyroid Internal Dose Estimation for Koreans

  • Background: The International Commission on Radiological Protection is preparing to provide reference dose coefficients for environmental radioiodine intake based on newly developed age-specific biokinetic models. However, the biokinetics of iodine has been reported to be strongly dependent on the dietary intake of stable iodine; for example, the thyroidal uptake of iodine may be substantially lower in iodine-rich regions than in iodine-deficient regions. Therefore, this study attempted to establish a system of age-specific thyroid dose estimation for South Koreans, whose daily iodine intakes are significantly higher than that of the world population. Materials and Methods: Korean age-specific biokinetic parameters and thyroid masses were derived based on the previously developed Korean adult model and the Korean anatomical reference data for adults, respectively. This study complied with the principles used in the development of age-specific biokinetic models for world population and used the ratios of baseline values for each age group relative to the value for adults to derive age-specific values. Results and Discussion: Biokinetic model predictions based on the Korean age-specific parameters showed significant differences in iodine behaviors in the body compared to those predicted using the model for the world population. In particular, the Korean age-specific thyroid dose coefficients for 129I and 131I were considerably lower than those calculated for the world population (25%-76% of the values for the world population). Conclusion: These differences stress the need for Korean-specific internal dose assessments for infants and children, which can be achieved by using the data calculated in this study.

국내 수면 질 관련 양적 연구동향 및 측정도구 분석 (Research Trends and Instrument for Assessment in Sleep Quality)

  • Purpose: The purpose of this study was to identify the research trends and instruments for assessment of sleep quality in Korea. Methods: 66 quantitative research reports published from 2000 to March 2009 in Korea were selected from 5 databases: RISS4U, KISS, KISTI, DBpia, and the National Assembly Library and were analyzed according to criteria such as publication years, type of journal, participants, study fields, research design, participants, key words and instruments for assessment. Results: Research in sleep quality has been rapidly increasing in recent years. More than half of the research analyzed was conducted in nursing. The majority of research methods were survey and correlational research. Patients with chronic illness and the elderly were the major subjects in those researches. Most research was performed based on hospitals and communities. The types of variables tested for their relation and influence on sleep quality were quite limited. With respect to the instrument used for assessing sleep quality, most studies did not consider whether or not the validity of their instrument had been established. Conclusion: It is recommended to conduct clinically applicable interventional research with an experimental design and to develop a reliability and validity established sleep quality instrument for Koreans.

성인의 한방의료서비스 이용 추세와 관련요인 (A Study on the Sociodemographic Characteristics of Adult Users of Korean Traditional Medicine)

  • Purpose: Patterns of traditional medicine (TM) utilization vary depending on culture, historical significance, and regulations. We seek to understand how socio-demographic factors are related to TM use in Korea. Methods: The longitudinal cohort survey data of the Korea Health Panel (2008-2012) were used for the analysis. Information on TM included questions on 4 types of TM therapies used in South Korea (e.g., acupuncture, moxibustion and cupping, herbal therapies, and massage using hand). To adjust clustering across multiple observations per individual, individual random effects were used in the multivariate analysis. All analyses were performed using Stata 11.0. Results: A majority of Koreans (60%) used WM only during a year, and few people used TM only. Among TM users, most (93%) used it along with WM and 8% used TM only. Our regression analysis showed that female, age 34-64, low-educated, and sicker people were more likely to use TM. Conclusion: This study provides evidence on the relationship between TM utilization and socio-demographic factors.

Thrown in a Different World: The Later Lives of Korean Elderly in an American Nursing Home

  • Purpose: Increasing numbers of Koreans have immigrated to the United States since the late 1960s. The first generation of Korean immigrants or their parents become old and institutionalized in American nursing home setting. Although the Korean elders would experience many cultural differences in the nursing home, no study to date has investigated their everyday lives on how they live through their later lives within a different cultural environment from their own. Methods: Using ethnographic methodology, the purpose of this paper was to illustrate Korean residents' experiences and daily lives in a nursing home located in an east coastal city in the U.S. Participant observation, filed notes, semi-structured interviews were utilized by means of data collection. Eighteen Korean residents were observed, and five of them and two nurses participated in informal qualitative interviews. Results: The overriding theme from the findings is "thrown in a different world." Three sub-themes include "constant struggles in making themselves understood", "dealing with culturally inappropriate nursing care," and "maintaining their own ways of life". Conclusions: The discovered themes reflect culturally isolated lives of the participants and open a venue for designing a culturally congruent nursing care for Korean elders living in the U.S. nursing homes.

정상 성인 말속도의 청지각적/음향학적 평가에 관한 기초 연구: 지역에 따른 말속도 차이를 중심으로 (Preliminary study of the perceptual and acoustic analysis on the speech rate of normal adult: Focusing the differences of the speech rate according to the area)

  • The purpose of this study is to investigate the differences of the speech rate according to the area in the perceptual and acoustic analysis. This study examines regional variation in overall speech rate and articulation rate across speaking situations (picture description, free conversation and story retelling) with 14 normal adult (7 in Gyeongnam and 7 in Honam area). The result of an experimental investigation shows that the perceptual speech rate differs significantly between two regional varieties of Koreans with a picture description examined here. A group of Honam speakers spoke significantly faster than a group of Gyeongnam speakers. However, the result of the acoustic analysis shows that the speech rate of the two groups did not differ. And there were significant regional differences in the overall speech rate and articulation rate on the other two speaking situation, free conversation and story retelling. It suggest that we have to study perceptual evaluation with regard to the free conversation and story retelling in future research, and based on the results of this study, a variety of researches on the speech rate will be needed on the various conditions, including various area and SLPs who have wider background and experiences. It is necessary for SLPs to train and experience more to assess patients properly and reliably.
