Background: Smoking and smokeless tobacco use are almost always initiated and established during adolescence. More than 80% of adult smokers begin smoking before 18 years of age. The main objective of the present study is to assess the feasibility of preventing adolescent tobacco use with the help of a specially designed tobacco control program. Materials and Methods: A cross sectional survey on tobacco use and related health effects was conducted using a structured questionnaire in 13 randomly selected schools in Kannur district of Kerala. These students were followed for a period of one academic year with multiple spaced interventions such as anti-tobacco awareness classes, formation of anti-tobacco task forces, inter-school competitions, supplying IEC (information, education and communication) materials and providing a handbook on tobacco control for school personnel. Final evaluation was at the end of one year. Results: There were 4,144 school children who participated in the first phase of the study. The prevalence of tobacco smoking and chewing habits were 9.85% and 2.24% respectively. Ninety-one percent had parental advice against tobacco use and only 3.79% expressed desire for future tobacco use. The final evaluation witnessed a sharp decline in the current tobacco use as 4.68%. We observed a statistically significant difference towards the future use of tobacco (p<0.001) and awareness about the ill effects of passive smoking (p<0.001) among boys and girls. Further a significantly increased knowledge was observed among boys compared to girls about tobacco and oral cancer (p=0.046). Conclusions: The comprehensive school based tobacco control project significantly reduced the tobacco use pattern in the target population. School tobacco projects incorporating frequent follow ups and multiple interventions appear more effective than projects with single intervention.
Objectives: The aim of this study was to verify relationships between light- and heavy alcohol drinking and health-risk behaviors such as smoking, inappropriate weight control, physical inactivity, and sexual activity among adolescents. Methods: The data of 68,043 adolescents in middle- and high school collected from the Korea Youth Risk Behavior Web-based Survey in 2015 were analyzed. For statistical analysis, logistic regression model was used. Results: The rate of heavy alcohol drinking increased with increasing age up to 21.5% among boys and 13.9% among girls. Among boys, heavy alcohol drinkers were at a higher risk of smoking (adjusted odds ratios [OR]=3.3, 95% confidence interval [CI]=2.8-3.8), inappropriate weight control (OR=1.7; 95% CI=1.4-2.1), having sexual intercourse (OR=3.6; 95% CI=3.0-4.2) than light drinkers. Multiple health-risk behaviors of smoking, inappropriate weight control and sexual intercourse also increased in accordance with the severity of drinking alcohol beverage compared to non-drinker. Physical inactivity was decreased among heavy drinkers compared to light drinkers (OR=0.8; 95% CI=0.7-0.9 for inactivity of moderate exercise). Similar results were observed among girls. Conclusions: Alcohol drinking especially heavy alcohol drinking among adolescents is related to other risky behaviors including smoking, inappropriate weight control, and sexual activity. To prevent health problems in adolescents, it is necessary to adopt a combined approach regarding heavy drinking and health-risk behaviors.
The purpose of this study was to test the revised Health Promotion Model of Pender and to determine the factors to promote health behavior for adolescents' smoking behavior. The subjects of the study was 783 boys of 4 high school students. among 39. schools locating in Daejeon metropolitan city. From above results school health nurse has to emphasize on health responsibility for health promotion of adolescent. But they couldn't intervene for parent pattern. socioeconomic status. family structure of individual characteristics and experience domain. it could be possible for school health nurse to promote health of adolescents through improving perceived barrier. also develop program to increase self-efficacy and through parent health class for fathers. Above results point to the importance of including parents in smoking prevention effort targeting adolescents. Because increasing control also promotes health of adolescents. it should be studied further about the specific measure. To verify the variables for increasing the fitness of health promoting model. it needs further replication of the research.
Background: The use of heated tobacco products (HTP) causes many adverse health effects. Although the use of HTP by adolescents is prevalent worldwide, research related to it is very limited. There is a considerable lack of research related to the current HTP use rather than ever-HTP use. In particular, research related to predictive factors for current HTP use in adolescents is scarce. Objectives: The purpose of this study was to analyze related characteristics according to the current use of HTP among South Korean adolescents. Methods: This was a cross-sectional study that used data from the Seventeenth Korean Youth Risk Behavior Web-based Survey (KYRBWS). A total of 54,848 students in 2021 were included in this study. Chi-square-test, multiple logistic regression analysis, and chi-square test for trend were used for analyzing related characteristics according to use of HTP. Results: Overall, 715 (1.3%) students responded as having used HTP during the last 30 days among the 54,848 students. It was found that residence type, subjective body type recognition, subjective health recognition, alcohol use, habitual drug experience, close friend current smoking, and conventional cigarette smoking were significantly associated characteristics with the current use of heated tobacco products. Comparing 'very thin recognition' with 'very fat recognition', the adjusted odds ratio (OR) was 1.93 (95% confidence interval [CI]: 1.29~2.87) for current use of HTP. Additionally, comparing 'very unhealthy recognition' with 'very healthy recognition', the aOR was 3.82 (95% CI: 2.40~6.07) for current use of HTP. Conclusions: Based on these results, residence type, subjective body type recognition, subjective health recognition, alcohol use, habitual drug experience, close friend current smoking, and conventional cigarette smoking were associated with significantly increased odds of current HTP use. Therefore, the results of this study can provide useful evidence about adolescent behaviors in predicting current HTP use.
청소년기의 흡연은 음주, 약물남용 등의 다른 건강위험행태로의 관문역할을 할 뿐 아니라 성인기의 생활습관으로 이어져 일평생의 건강을 위협 할 잠재적 요소를 내포하고 있다. 본 연구는 우리나라 중 고등학고 남녀학생을 대상으로 흡연 경험과 인터넷 중독과의 관계를 알아보고자 한다. 2009년 우리나라 중학생 38,409명, 고등학생 36,657명 총 75,066명을 대상으로 조사되었다. 흡연경험 여부와 인터넷 중독 여부는 빈도분석과 카이제곱 검정을 하였고 흡연경험과 인터넷 중독과의 관계는 로지스틱회귀분석을 실시하였다. 통계적 유의성은 0.05 미만으로 하였다. 대상자의 흡연경험은 남학생이 34.2%, 여학생은 19.7%로 나타났고 인터넷 중독은 남학생이 17.6%, 여학생이 11.4%로 조사되었으며, 흡연경험 있는 남학생이 흡연경험 없는 학생의 1.06배 여학생은 흡연경험 있는 학생이 없는 학생보다 1.62배 유의하게 높았다. 본 연구 결과는 부정적인 건강생활습관인 흡연경험이 인터넷 중독 경향에 유의하게 관련되어 있으며, 흡연경험이 비흡연경험에 비해 높은 스트레스를 갖고 있고, 이러한 스트레스가 인터넷 중독으로 발전해 가는 경향을 보이는 것으로 나타났다. 그러므로 청소년기의 흡연행위와 컴퓨터 중독은 청소년기의 위험행동으로 서로 영향을 미치고 있으므로 각각의 행위에 대한 개별적인 예방과 치료를 하기보다는 흡연과 스트레스, 인터넷 중독을 복합적인 건강위험행위로 보아 대책을 종합적으로 강구해야 할 것이라 사료된다.
In recent years, adolescent issues including smoking, drinking, drug abuse, juvenile delinquency, deviant sexual behavior, mental health problems, high suicide rate, misconducts and absence without due notice, etc are emerging as serious social problems and the debate on these controversial issues is heating up. Accordingly, the purpose of this study is to analyze factors which are changeable and fixable among the factors influencing the adolescent's health behavior and misconducts and, eventually influencing factors which can be used as the basis to establish health policies and health promotion program to reduce the health risk behavior and misconducts of adolescents. The smoking rates are 34.1% for male students of prep schools and 13.8% for females students of the same school and 55.7% for males, 31.8% for females of the vocational schools and 58.3% for males and 48.8% for females of social institutional schools, which showed the great diffence among the different types of schools and between sex. In particular, male students of social institutional school showed 1.7 times higher smoking rate than those of prep schools and in case of female students, 3.5 times higher rate. The time of initial smoking was most frequently during the middle school days for both males and females. In case of drug abuse, 5.4% of males and 2.7% females of general schools were using adhesives and inhalants which was 2.5 times higher for males and 6.3 times higher for females of social institutional schools. 41.8% of males and 30.3% of females of prep schools, 41.8% of males and 59.4% of females of vocational schools and 55.1% of males and 36.6% of females of social institutional schools have experienced kissing. Regarding the health promoting behavior and misconducts, female students practiced the health promoting behavior more than male students while male students showed higher rate of health risk behavior and misconducts than female students, which was statistically significant. The group of students who have not attended the health education class, in comparison to those who have attended, were more likely to practice health risk behavior and misconducts. Those with higher academic achievement was more likely to practice the health promoting behavior while those with poor academic achievement were more likely to practice health risk behavior and misconducts. As the perceived health status was higher and as students experienced less illness, the health promoting behavior was higher.
Objectives : To evaluate the internal burden and hazardous effects associated with smoking in middle and high school students. Methods : We analysed urinary cotinine(U-cotinine) concentrations and the frequency of Sister Chromatid Exchanges (SCE). A comparison was done of U-cotinine concentrations and the frequency of SCE in peripheral lymphocytes across school levels (middle vs. high) and smoking types (direct: daily & occasional smoking, indirect; usual indirect & non-smoking), in 122 males. Results : The middle school student group comprised 6.8% daily smokers, 15.9% occasional smokers, 40.9% daily indirect smokers, and 35.4% nonsmokers, while the high school student group comprised 18.0%, 20.5%, 35.7%, and 21.8%, respectively. The U-cotinine concentration and the frequency of SCE among the middle school students were $79.11{\mu}g/l$ and 2.0 per cell, respectively, which were significantly lower than the $146.85{\mu}g/l$ (p=0.078) and 2.6 per cell (p=0.005) of the high school students. Among the 40 direct smokers, these two biomarkers were $236.66{\mu}g/l$ and 2.59 per cell, significantly higher than the $67.33{\mu}g/l$ (p=0.0001) and 2.1 per cell (p=0.003) among indirect smoking groups. The variation in individual U-cotinine concentration ranged widely in both the indirect and direct smoking groups. Conclusion : Urinary cotinine concentrations and the frequency of Sister Chromatid Exchange seem to objectively and effectively evaluate student exposure whether it was direct or indirect smoking. Consequently, these biomarkers may be useful in monitoring the objective efficacy of anti-smoking programs in adolescent populations.
Purpose: This study aimed to understand the relationship between cigarette and electronic cigarette (e-cigarette) use and sleep health among Korean adolescents. Methods: Using the 14th Korea Youth Risk Behavior Survey, we included 52,928 adolescents who responded to sleep satisfaction and sleep duration. Participants were classified into four groups: non-users, cigarette-only users, e-cigarette-only users, and dual users of both products. To examine the associations between cigarette and e-cigarette use and sleep health, data were analyzed using complex samples cross tabulation and complex samples logistic regression. Results: Of the four groups, dual users reported the lowest level of sleep satisfaction and sleep duration; 57.0% and 86.9% of dual users were not satisfied with their sleep and have insufficient sleep duration, respectively. The proportion of students who were not satisfied with their sleep was higher among cigarette-only users compared to e-cigarette-only users (52.7% vs. 45.8%), but the two groups reported similar rates of insufficient sleep duration (84.2% vs. 84.3%). Compared to non-users, cigarette-only users, e-cigarette-only users, and dual users were more likely to not be satisfied with sleep (odds ratios [ORs] were 1.49, 1.36, and 1.75, respectively) and had significantly higher odds of experiencing insufficient sleep duration (ORs were 1.85, 2.06, and 2.34, respectively). Conclusion: E-cigarette-only use and dual use as well as cigarette-only use were associated with poor sleep health. Therefore, to improve adolescent sleep health, health professionals should provide sleep intervention strategies considering the association between smoking and sleep health.
Purpose: The objective of this study was to develop a predictive model for the sexual experiences of adolescents using the random forest method and to identify the "variable importance." Methods: The study utilized data from the 2019 to 2021 Korea Youth Risk Behavior Web-based Survey, which included 86,595 man and 80,504 woman participants. The number of independent variables stood at 44. SPSS was used to conduct Rao-Scott χ2 tests and complex sample t-tests. Modeling was performed using the random forest algorithm in Python. Performance evaluation of each model included assessments of precision, recall, F1-score, receiver operating characteristics curve, and area under the curve calculations derived from the confusion matrix. Results: The prevalence of sexual experiences initially decreased during the COVID-19 pandemic, but later increased. "Variable importance" for predicting sexual experiences, ranked in the top six, included week and weekday sedentary time and internet usage time, followed by ease of cigarette purchase, age at first alcohol consumption, smoking initiation, breakfast consumption, and difficulty purchasing alcohol. Conclusion: Education and support programs for promoting adolescent sexual health, based on the top-ranking important variables, should be integrated with health behavior intervention programs addressing internet usage, smoking, and alcohol consumption. We recommend active utilization of the random forest analysis method to develop high-performance predictive models for effective disease prevention, treatment, and nursing care.
Objectives: This study was performed to consider the association between Internet using time for non-educational purposes and adolescent health, and to examine how health status differs between Internet users and non-users. Methods: We analyzed 2009 data from the Korea Adolescent Risk Behavior Web-Based Survey, conducted on a nationally representative sample of students in grades 7 to 12. A total of 75 066 adolescents were categorized into four groups according to their Internet using time excluding using for educational purposes: non-Internet users (NIUs), occasional Internet users (OIUs) (<1 h/d), moderate Internet users (MIUs) (${\geq}1$ and <2 h/d), and heavy Internet users (HIUs) (${\geq}2$ h/d). Health factors included eight health risk behavior indices, four mental health indices and six physical health indices. Results: The distribution of Internet use was as follows: NIUs 17.4%, OIUs 68.1%, MIUs 12.7%, and HIUs 1.7%. In multivariate analysis, using OIUs as a reference, U- or J-shaped associations were observed for five health risk behavior indices (current smoking, current drinking, drug abuse, sexual intercourse, sedentary behavior on weekdays) and four mental health indices (stressed, depressed, suicidal ideation, attempted suicide) in both genders. After removing confounding effects, including age, region, school type, subjective school record, subjective economic status, presence of parents, living with family, and sedentary behavior, these associations were still observed. Conclusions: Health professionals should consider both Internet non-users (for non-educational purposes) and heavy users to be high-risk groups in terms of health status. Also, more well-designed studies are needed to clarify what factors are working in these nonlinear associations.
