12주간의 음악 줄넘기 운동이 비만 남자 중학생의 혈청 Adiponectin 농도와 복부지방에 미치는 영향 (The Effect of 12-week Music Rope Skipping Exercise on Serum Adiponectin Concentration and Abdominal Fat in Obese Boys Student in Present Middle School)

  • 청소년층의 비만 증가 추세에 심각성을 고려하여 학생 체육의 활성화를 높이기 위해 음악 줄넘기 운동 프로그램을 실시하여 비만지표로 활용하는 adiponectin과 복부 지방에 어떠한 영향을 주는지 살펴보았다. 남자 중학생 14세~16세 중 건강체력평가에서 기초 체력이 부족한 체력 미달로 판정받고 BMI $25kg/m^2$이상인(실험군 11명, 통제군 12명) 23명을 대상으로 12주간 음악 줄넘기 운동 프로그램을 실시하였다. 집단별 음악줄넘기 운동 프로그램 실시 전과 12주후 adiponectin과 복부 지방을 측정하여 개선정도를 비교하였다. 프로그램 실시 후 체조성의 변화로 BMI가 감소하였으며 음의 상관으로 adiponectin은 유의한 증가를 보였다. 또한 복부 내장지방, 복부 피하지방, 복부 총지방에서도 통계적으로 유의하게 감소하는 것으로 나타났다. 비만 청소년에서 음악줄넘기 운동 프로그램 실시는 체질량지수 감소가 동반될 때 adiponectin 증가의 개선을 보이고 복부 지방 또한 긍정적인 영향을 주는 것으로 나타났다.

Abdominal Subcutaneous Fat Thickness Measured by Ultrasonography Correlates with Hyperlipidemia and Steatohepatitis in Obese Children

  • Purpose: The aim of this study is to evaluate the relationship between abdominal subcutaneous fat thickness measured by ultrasonography (US) and serum lipid profile and liver transaminases in obese children. Methods: One hundred and sixty-six children diagnosed with obesity from May 2001 to December 2013 were included in this study. Data on serum lipid profile and liver transaminases were collected from clinical records. Abdominal subcutaneous fat thickness and grade of hepatic steatosis were evaluated by US. Results: Of the 166 children, 107 were diagnosed with hepatic steatosis by US, 46 with grade I, 56 with grade II, and five children with grade III. According to the grade of hepatic steasosis, the average values of midline abdominal subcutaneous fat thickness and right flank abdominal subcutaneous fat thickness measured $2.9{\pm}0.8cm$ and $1.9{\pm}0.7cm$ in the normal group, $3.3{\pm}0.8cm$ and $2.0{\pm}0.7cm$ in grade I, $3.8{\pm}0.8cm$ and $2.3{\pm}0.8cm$ in grade II, and $4.1{\pm}0.8cm$ and $2.8{\pm}1.4cm$ in grade III, respectively. Abdominal subcutaneous fat thickness correlated with grade of hepatic steatosis (p<0.01). In addition, abdominal subcutaneous fat thickness correlated with concentration of serum lipids and liver transaminases in the age group of 12-14 years (p<0.01). Conclusion: Abdominal subcutaneous fat thickness measured by US can be used as a reliable predictor of possible hyperlipidemia and steatohepatitis in children, especially during the adolescent stage.

Clinical characteristics of obese boys and girls in a high school: focused on abdominal fat indices, fatty liver and carotid intima-media thickness

  • Purpose: Our study aimed to evaluated sex differences in clinical features of obese high school students. Methods: One hundred three obese high school students (body mass index [BMI]${\geq}$85th percentile) and 51 control students (BMI<85th percentile) were enrolled in this study. Anthropometric measurements were performed. Fasting serum glucose, insulin, aspartate aminotransferase, alanine aminotransferase, total cholesterol, triglyceride, low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and high-sensitive C-reactive protein were measured. Abdominal fat thickness, degree of fatty liver, and carotid intima-media thickness were measured by ultrasound. Results: In control and obese groups, waist circumference was significantly longer in boys but body fat mass was Significantly higher in girls. In the control group, total cholesterol and LDL-C were higher in girls. In the obese group, however, aspartate aminotransferase, alanine aminotransferase and triglyceride were higher and HDL-C was lower in boys. Preperitoneal fat thickness was significantly higher in obese girls. In obese group, the degree of fatty liver was significantly higher in boys. Carotid intima-media thickness was not significantly different between boys and girls. Conclusion: Obese adolescents had distinguishable sex differences in body measurements, metabolic abnormalities, abdominal fat thickness and fatty liver. We can infer that these characteristics may extend into adult obesity.

Evidence-based customized nutritional intervention improves body composition and nutritional factors for highly-adherent children and adolescents with moderate to severe obesity

  • BACKGROUND/OBJECTIVES: Evidence-based customized nutritional interventions are required for effective treatment of moderate to severe obese children and adolescents. SUBJECTS/METHODS: Sixty six (64.1% of 103) of the eligible participants who joined the usual care or physical activity group in the clinic were involved in 16-week intervention. Customized nutritional intervention was implemented for each participant based on a nutrition care process (NCP) model. Sociodemographic assessment, anthropometrics data, health- and dietary-related behaviors, and dietary intake of the study subjects were assessed at baseline and follow-up. All participants engaged in 30-minute nutritional sessions on a monthly basis. RESULTS: After 16 weeks, there were significant improvements in body composition [BMI (-0.8 ± 0.9, P < 0.05), BMI z-score (-0.3 ± 0.2, P < 0.001), body fat (kg) (-1.3 ± 2.1, P < 0.05), and body fat (%)(-1.5 ± 1.9, P < 0.05)] as well as macronutrient intake [total energy intake (kcal) (-563.7 ± 656.8, P < 0.05), energy (%) (-26.5 ± 30.0, P < 0.05) and fat (g) (-28.3 ± 40.6, P < 0.05)] in the adherent group than the non-adherent group. The SOC was higher in both groups after the intervention (P < 0.001). CONCLUSIONS: Our results highlight the positive effects of an evidence-based approach as a multidisciplinary intervention for people-centered nutritional care and weight management.

정상체중 여고생들의 체중조절행위와 관련된 요인에 관한 연구 (Factors Associated with Weight Control Behaviors Among High School females with Normal Body Weight)

  • This descriptive study was designed to identify the factors associated with weight control behaviors among 273 high school girls with normal body weight. The objectives of the study were ⅰ) to evaluate subjective obesity and satisfaction with the body shape, ⅱ) to describe the weight control behaviors and the motivation, ⅲ) to reveal the relationships between the weight control behaviors and BMI (Body Mass Index), iv) to investigate the relationships between the weight control behaviors and subjective obesity and satisfaction with the body shape, and v) to investigate the relationships between the weight control behaviors and family support. The results showed that 62.6% of the adolescent girls with normal weight perceived that they were fatty and 9.9% thought they were obese. Seventy-five percent of them were not satisfied with their body shape. Eighty percent of the participants reported the main motivation of weight control was to have an attractive appearance. In this study, self-reported weight control methods included dieting (64.8%, skipping or reducing meals), exercise (36.6%), and special dieting (20.1%) such as eating an increased amount of juice or vegetables. It was shown that the subjects who were not satisfied with their body shape and perceived themselves as fatty or obese were active in exercise, diet, and other special diets. Subjects who were on diets and special diets had a higher level of BMI than who were not on diet and special diet. Subjects who exercise had a lower level of BMI than who did not exercised. Family support was significantly related to exercise behavior. The research suggested that there is a increasing responsibility for school nurse to instruct on the body shape and weight control behaviors through health education and consultation. Also, the results suggested that it is important to develop proper diet and exercise methods for adolescents girls to maintain their weight and health.

Dietary patterns and metabolic syndrome risk factors among adolescents

  • Purpose: Unbalanced diets and decreased physical activity have contributed to increased prevalence of obesity and metabolic syndrome in adolescents. We have performed a systematic review and data analysis to examine the association between dietary pattern and metabolic syndrome risk factors in adolescents. Methods: We searched the PubMed and BioMedLib databases for appropriate articles published during the past 10 years and selected 6 articles. The studies reviewed applied factor analysis or cluster analysis to extract dietary patterns. For data analysis, we examined the association between dietary patterns and the prevalence of metabolic syndrome risk factors using data of 3,168 adolescents (13 to 18 years) obtained from 4 consecutive Korean Nutrition Health and Nutrition Examination Surveys (1998, 2001, 2005, and 2007 to 2009). Results: Our systematic review confirmed that western dietary patterns are positively associated with metabolic syndrome risk factors such as obesity and elevated triglycerides, while traditional dietary patterns were negatively associated. Data analysis found that the number of adolescents aged 16 to 18 years who had "Rice & Kimchi" dietary pattern decreased, while the number having western dietary patterns increased during the 1998 to 2009 time frame. There were no changes in the dietary patterns in adolescents aged 13 to 15 years. The risk of elevated serum triglycerides and reduced serum high density lipoprotein cholesterol was high in the "Rice & Kimchi" dietary pattern compared to the other dietary pattern groups. Conclusion: Because adolescents' dietary patterns are changing continuously and have long-term effects, further studies on the dietary patterns of adolescents and their health effects into adulthood are necessary.

농촌 남녀 중학생의 영양소섭취, 식행동과 체형인식에 관한 비교 (Comparisons of Nutrient Intakes, Dietary Behavior and Perception about Body Image between Adolescent Body and Girls in Rural Area)

  • This study was undertaken to compare the obesity related dietary factors among rural middle school students living in Korea. Anthropometric data showed that mean height and weigh were 161.7$\pm$13.0 and 49.8$\pm$10.3kg in 226 male students and 158.3$\pm$6.1 and 48.2$\pm$7.5kg in 212 female students. Mean BMIs for boys and girls were 19.10$\pm$2.47 and 19.26$\pm$2.35, respectively. 71.68% of male students and 72% of female students were underweight by the BMI index. The BMI of older students was higher than young ones in both sexes. Most nutrient intakes of the subjects were below the RDA. In particular Ca and Fe intakes were extremely low in both sexes. 66.88% of the students skipped breakfast and it was found that the reasons for missing a meal for both sexes were significantly different. Compared to males, more female respondents felt guilty and depressed related to eating. Furthermore, females were more significantly concerned with body image, diet, and body weight(p〈0.001), and they were not satisfied with their weight. The sources of information on obesity and diet were from radio and TV in males while females got information from magazines and friends. In male students their ideal body weight was higher was higher than actual body weight. In contrasting, female students desired a thin body shape. A different approach for nutrition education for adolescents should be developed and implemented based on sex.

Serum alanine aminotransferase levels are closely associated with metabolic disturbances in apparently healthy young adolescents independent of obesity

  • Purpose: Liver metabolism plays a pivotal role in the development of metabolic disorders. We aimed to investigate the clinical and laboratory risk factors associated with alanine aminotransferase (ALT) levels in young adolescents from an urban population in Korea. Methods: A population of 120 apparently healthy adolescents aged 12-13 years was included in the cross-sectional design study; 58 were overweight or obese and 62 were of normal weight. We estimated anthropometric and laboratory measurements, including waist-to-height ratio, blood pressure, insulin sensitivity, aspartate aminotransferases (AST), ALT, and lipid profiles. Results: The mean ages of the overweight or obese and normal weight participants were $12.9{\pm}0.3$ and $13.0{\pm}0.3years$, respectively. Height, weight, body mass index, waist circumference, waist-to-height ratio, systolic and diastolic blood pressure, AST, ALT, total cholesterol, low-density lipoprotein-cholesterol, triglyceride, insulin, and the homeostatic model assessment of insulin resistance (HOMA-IR) score were significantly higher and the high-density lipoprotein-cholesterol and quantitative insulin-sensitivity check index were significantly lower in the overweight/obese participants in comparison to the normal-weight participants (all P<0.05). In multivariate linear regression analysis, waist-to-height ratio, systolic blood pressure, and HOMA-IR score were independently and positively associated with serum ALT levels. Conclusion: Screening for ALT levels in adolescents may help to differentiate those at risk of metabolic abnormalities and thus prevent disease progression at an early age.

  • Purpose: The purpose of the study was to investigate the changes in the prevalence of metabolic syndrome and its risk factors in Korean adolescents before and during the COVID-19 pandemic. Methods: The study employed a descriptive cross-sectional design. Data were obtained from the 2018 and 2020 Korea National Health and Nutrition Examination Surveys. Out of a total of 15,351 participants in the surveys, 971 adolescents aged 12 to 18 years were included. The data were analyzed using the rao-scott 𝝌2 test and t-test. Results: There was no significant difference in the prevalence of adolescent metabolic syndrome before and after the outbreak of COVID-19. As for general characteristics, changes in high blood pressure were confirmed from waist circumference, diastolic blood pressure, and metabolic syndrome risk factors. When analyzing the general characteristics according to gender, there were significant differences in waist circumference and diastolic blood pressure in boys. Concerning the risk factors for metabolic syndrome, there was a significant difference in high blood pressure in both boys and girls and in abdominal obesity in girls. Conclusion: Hypertension and abdominal obesity, which are risk factors for metabolic syndrome, should be managed by including them in the school health area. Communities and schools should take an active role in preparing the youth for healthy adulthood.

Double burden of malnutrition in ethnic minority school-aged children living in mountainous areas of Vietnam and its association with nutritional behavior

  • BACKGROUND/OBJECTIVES: Although children of ethnic minority groups are experiencing a transition from a predominance of undernutrition to overnutrition, there is little evidence of a dual-malnutrition burden. Therefore, this study examined the double burden of malnutrition among school-aged ethnic minority children living in mountainous areas and its association with their diets. SUBJECTS/METHODS: A cross-sectional study was conducted from September 2019 to March 2020 in 3 mountainous areas of Northern Vietnam among 1,556 ethnic minority school-aged children. The prevalence of under-nutrition (stunting and thinness) and over-nutrition (overweight and obesity) were measured using the WHO 2006 child growth standards (height-for-age and BMI-for-age Z-score). Nutritional practices were evaluated by the frequency of food consumption based on a 4-level scale. RESULTS: The percentage of children with stunting and thinness were 14.0% and 5.4%, respectively, while the figure for overweight/obesity was 9.4%. The factors positively associated with stunting were living in a family with more than 2 children or being Muong/other ethnicities compared to the Tay ethnicity. Children who consumed fish/shrimps/crabs or milk weekly/daily were less likely to be undernourished compared to those who never consumed these foods. By contrast, children who never consumed foods rich in vitamin A precursors and vitamin A and fruit or consumed daily snacks/junk food were more likely to be overweight/obese. CONCLUSIONS: Undernutrition remains a common issue among school-aged children and adolescents of ethnic minority groups, while over-nutrition exists simultaneously. Public health nutrition programs promoting adequate diets and positive lifestyle changes related to nutrition are essential to tackle the double burden of malnutrition among ethnic minority children.
