병원 행정직원의 조직효과성 수준 (A Study on Organizational Effectiveness of Administrative Personnel in Hospitals)

  • The hospital is characterized by it's remarkable labor industry and human resources input by unit. Recently, the administrative personnel are recognized as an important staff to provide a hospital guidance to consumer and also easiness for consumer's visit to hospital. The purpose of this study is to find the organizational effectiveness of the administrative personnel in hospitals. The survey data involved in the study was derived from 229 personnels working in 3 medium and small sized hospitals and 1 university hospital in Inchon area. The major finding are as follows; 1. The organizational efficiency in accordance with the general characteristics of subjects in order of over 31 years of age, university graduates, long-term tenure and high position is higher, whereas, in as much as wage and well trained personnels in are higher, general hospital's organizational efficiency is higher in comparison with those of university hospital. 2. The organizational efficiency in accordance with satisfaction and the psychological motive contributional factors is higher as much as the high satisfactory level in every hospitals in general. 3. The organizational efficiency in accordance with the factors of job characteristics is higher in as much as difficulty of the jobs is lesser, however there was not statically significance. In as much as job standard level is higher and the more job responsibility the higher organizational efficiency. It was obvious that the higher professional expertise as well as the training and application level are improving the organizational efficiency. 4. The organizational efficiency in accordance with the factors of structural characteristics was higher in as much as the intercommunication was smooth and the structural formalization level are higher, however there was not statically significance between the participation level of decision making and the organizational efficiency. 5. In as much as older age, management of organization and the job level are satisfied, the higher structural formalization level, the smooth intercommunication have affected as major influence factors of organizational efficiency. 6. In the university hospitals is satisfied in the management and job level of hospital organization as there are no difficult jobs while the level of hospital's organizational formality is high and the intercommunication is smooth, which are improving the organizational efficiency. In the general hospitals is also satisfied the management and job level of hospital organization and psychological motive contributional factors is higher, it was apparent that the organizational efficiency is higher in as much as the level of job standardization is high and the intercommunication is smooth. As a result of this study, in order for improving the organizational efficiency of administrative personnel in hospitals, the management and job level as well as personal relation are preferably satisfied, whereas formalization of organization, intercommunication and etc. should be satisfied, and, therefore, it is advisable to buildup discriminated organizational management and environment for different division on the basis above factors. Since this study is carried on four hospitals in Inchon area, there is a certain limit to generalize its result to all domestic hospitals, nevertheless the gallop poll was made by developing the questionnaires with reasonability and reliability. Especially. as the study was carried by analyzing the comparison of influence factors' difference of organizational efficiency in accordance with the divisional characteristics of the university and general hospitals.

대학부속 한방병원의 수익성 영향요인 연구 (The Factors Affecting the Profitability of Oriental Medicine Hospital of University in Korea)

  • 본 연구는 대학부속 한방병원의 재무지표인 유동성, 안정성, 활동성 및 비용 관련 요인이 수익성 지표와 어떤 관계를 맺고 있는지를 밝히고, 이로부터 경영개선 방안을 제시하는 것이다. 분석 결과, 대학부속 한방병원의 의료수익의료이익률, 총자본의료이익률, 의료수익순이익률과 총자본순이익률은 각각 1.17%, 3.76%, 2.37%, -1.89%로서 우리나라 전체 한방병원의 동일 지표 값인 8.9%, 8.7%, 2.6%, 2.5%와 비교해보면 의료수익의료이익률과 총자본의료이익률, 그리고 총자본순이익률이 낮음을 알 수 있었다. 그러나 안정성 지표인 유동비율과 당좌비율, 고정비율은 각각 256.76%와 231.17%, 121.02%로서 전체 한방병원 평균과 비슷하였으며, 성장성 지표인 총자본증가율과 의료수익증가율은 -2.21%와 1.89%이었고 활동성 지표인 총자본회전률은 135.69%로서 전체 한방병원과 유사한 수준이었다. 한편, 의료원가인 인건비, 재료비, 관리비 비율은 각각 53.39%, 16.62%, 28.58%로서 전체 한방병원의 35.3%, 10.7%, 45.1%와는 차이가 있었다. 그리고 2011년 우리나라 전체 한방병원의 원가율은 91.1%였으나, 대학부속 한방병원의 원가율은 98.59%로서 7.49%가 높았다. 상관관계 분석에서 대학부속 한방병원의 경우, 의료수익의료이익률은 의료수익증가율과 더불어 증가하고, 의료수익순이익률과 총자본순이익률은 총자본증가율과 더불어 하는 증가하는 상관관계가 있었다. 관리비 비율은 모든 수익성 지표와 부의 상관을 보였는데 이것은 한방병원의 수익성 개선을 위해서는 관리비 절감에 집중해야 함을 의미한다. 다중회귀분석에서 수익성 지표에 영향을 미치는 독립변수는 성장성지표인 총자본증가율과 활동성 지표인 총자본회전률이었으며 비용지표 중에서는 관리비 비율 값이 가장 크게 나타났다. 따라서 병원의 수익성이 개선되기 위해서는 병원의 총자본을 증가시키고 적극적인 진료활동을 통해서 총자본회전률을 높이며, 관리비를 절감하는 노력이 필요하다는 결론을 얻을 수 있었다.

우리나라 사회봉사명령제도의 효과적 도입, 운영 및 정착에 관한 연구 (A Study on Community Service Order : It's Effective Introduction, Operation, and Establishment in Korea)

  • 사회봉사명령은 우리나라 보호관찰프로그램 가운데 가장 큰 몫을 담당하고 있는 만큼 사회적 기대 또한 매우 높다. 그러나 일반적인 자원봉사활동과 혼동될 정도로 그 프로그램 운영에 문제가 제기되고 있기 때문에 이 연구에서는 사회봉사명령 프로그램의 '교정'효과를 극대화시킬 수 있는 효과적인 운영 및 정착방안을 모색하고자 한다. 이러한 목적을 위해서 미국 미네소타주 교정국에서 지난 15년간 실시해 오고 있으며 매우 성공적으로 평가되고 있는 'Sentencing to Service' 프로그램을 방문하여 관련 행정, 실무책임자, 사회봉사그룹책임자, 사회봉사대상자들과의 면접 및 설문조사를 실시하였다. 표집방법으로는 체계적 표집(systematic sampling)을 사용하였다. 연구결과 사회봉사명령 프로그램은 그 교정효과 면에서 매우 고무적으로 나타났으며, 이를 위해서는 사회봉사대상자들의 자발적 참여동기를 극대화하고 지역사회로부터 적극적인 협조가 필수적이다. 특히 사회봉사그룹책임자의 자질이 매우 중요한 것으로 나타났다. 이와 같은 연구결과를 토대로 사회봉사명령의 관련자들을 위한 지침사항(guidelines)이 필요하다는 것과, 사회봉사그룹 책임자를 위한 교육 및 훈련이 중요하고 요구되는 자격요건으로 보아 사회복지사가 사회봉사그룹책임자로 채용된다면 프로그램의 효과를 극대화시킬 수 있다는 것, 정부가 사회봉사를 위탁받은 기관에 일정한 재정을 지원하는 것은 합리적이고 당연하다는 것, 마지막으로 사회봉사명령 프로그램이 구금형 대신에 부과됨으로써 법망의 확대(widening the net)효과를 초래하지 말아야 한다는 시사점이 있음을 발견할 수 있다.

처방전달체계에 대한 사용자 만족도에 영향을 미치는 요인 (Factors Affecting Users' Satisfaction with Order Communicating System)

  • Objectives : To identify the factors affecting users' satisfaction with the Order Communicating System(OCS) and to highlight the factors important for the successful establishment of OCS. Methods : A Users Satisfaction survey was sent to 4,513 people, consisting of 1,503 doctors, 2,379 nurses, 255 pharmacists and 370 administrative workers in 16 hospitals which had introduced OCS. The response rate was 63.9%. Measurement of users' satisfaction was peformed with the instrument which was used in Doll's study. Some aspects of Doll's instrument were adjusted according to the aims of this study. The classifying sections of this survey included age, job and status classification, computer experience, OCS education, duration of daily OCS use, type of order entering, number of personnel in the Hospital Information System's department, cost of OCS, problem frequency, proportion of work managed by hand, OCS type, and Hospital establishment type. Results : There was a positive correlation between satisfaction level and managerial status throughout all job classifications. Irrespective of the importance of OCS education as a factor relating to users' satisfaction, the additional work load caused by OCS lowered users' satisfaction. Different factors affected users' satisfaction according to job and status classification. The composition of factors affecting the pharmacist and administrative worker satisfaction levels was simpler than that of the doctor and nurse levels. There were no statistically significant differences between the actual computer experience duration of daily OCS use and users' satisfaction with OCS. Conclusions : There was an understandable relationship between users' attitude to OCS and factors affecting users' satisfaction. The results of this study could be used as a basis for the successful expansion of the operation of OCS. But more detailed studies on users' satisfaction and further improvements of methodologies are required for the successful establishment of OCS.

국민건강보험공단과 제약사 간 의약품 관련 협상 행위의 법적 성격에 관한 고찰 (A Study about the Legal Nature of Negotiations between NHIS and Pharmaceutical Company)

  • 공단과 제약사 간 협상은 제도 도입 이후 지속적으로 범위와 대상을 확대하여 왔고, 최근에는 품질관리에 관한 사항 등이 협상 대상으로 추가되었다. 그에 따라 복지부의 상한금액 고시가 아닌 공단과의 협상만으로도 제약사들의 법익이 침해될 우려가 제기되었고, 그로 인해 협상의 법적 성격과 법익 침해에 대한 구제방법 등의 검토 필요성이 대두되었다. 복지부장관이 공단 이사장에 대한 협상'명령'을 내리면, 공단이 제약사에 이를 '통보'하여 '협상'이 진행된다. '명령'은 행정권 내부의 행위로서 항고소송의 대상이 되는 처분성이 부인된다. 이에 반해 '통보' 내지 '협상'은 제약사를 상대방 당사자로 하는 행위로서, 협상 결렬에 대한 불이익이 존재한다면 그 자체를 '처분'으로 볼 여지도 있을 것이다. 협상은 '대상(보험급여목록에 등재되어 있는지 여부)'과 '목적(협상대상이 가격인지 조건 부여인지)'에 따라 4개 유형으로 세분할 수 있다. 조건 부여에 관한 협상은 협약을 통해 부담(부관)을 설정하도록 하는 합의가 된다. 신규 등재 또는 가격 인상 약제 협상의 경우에는 수익적·재량적 행정행위에 부담을 부착하는 경우로서 조건 부여 협상이 적법할 것이지만, 기등재 약제의 가격 조정은 대부분 가격을 인하하는 침익적 처분으로서 협상을 통해 부담을 부착할 경우 위법할 여지를 배제하기 어렵다. 따라서 부담의 설정이 필요하다면 인센티브 등의 부여를 통해 협상에 따른 고시가 수익적·재량적 행정행위가 될 수 있는 길을 열어놓을 필요가 있을 것이다.

수출입 공급망 안전 프로그램에 관한 연구 (A Study on the Supply Chain Security Program)

  • In this study, the scope of previous logistics security were focused only on port and ship. Because of it now extends to the overall (export and import) supply chain areas and in regards with supply security programs in the international level, it reviewed supply chain security programs categorized them into 'ships and port security system', 'container screening system', 'logistics chain authorization system' which are expanded to be adopted in the international level. The major features of those programs are summarized as in building risk management system, providing information ahead, selectivity test and benefits to AEO authorized companies in the customs administration level. The government and companies which are to ensure supply chain security and trade facilitation in order to cope actively with international customs administrative atmosphere need to do the followings : First, they need to build an intra-government integrated supply chain security and make efforts to conclude AEO MRA in order to increase trade competitiveness among major trading countries. Second, they need to build supply chain risk management system in order to enhance management performance through overseas market and company level strategy to obtain and maintain AEO authorization in the company level.

주거복지 현장의 전문인력 직무내용과 특성 분석 (Analysis of Roles and Responsibilities of Housing Welfare Professionals in Fields)

  • The study aimed to analyze roles and responsibilities of housing welfare professionals in fields in order to provide directions of the Housing Welfare Professional Certificate program operated by the Korean Housing Association. In October 2013, a series of interviews were performed to five housing welfare professionals in local governments, public enterprises and private organizations in order to explore field cases. Additionally, administrative works related to housing welfare were explored based on information from local government websites. It was found that roles and responsibilities of housing welfare professionals included a wide range of works such as provisions and management of public rental housing, housing counseling, investment and analysis of residential environment, exploring resources, education, and improvement of poor-condition housing and urban environment. In order for more efficient and effective provisions of housing welfare services, it was suggested to improve housing welfare service delivery systems and to make clear definition of job ranges, and knowledges and experiences required to be housing welfare professionals.

내수면 어로어업의 동향과 재편과제에 관한 연구 (A Study on the Reorganization of the Management System in Korean Inland Fisheries)

  • This study analyses the management and utilization of Korean inland fisheries and suggests directions for reorganizing the fisheries in order to achieve qualitative growth. Inland fisheries developing into a unique fishery according to various aquatic systems in different areas have played various roles such as the efficient use of aquatic resources, development of local areas and economy vitalization in under-developed local areas. However, recently Korean inland fisheries are facing a lot of difficulties due to the circumstantial changes surrounding the fisheries. Even though it is difficult to expect the external expansion of the Korean inland fisheries, we could expect qualitative internal growth by achieving the sustainable management and aquatic ecosystem health. Therefore, this study suggests the followings in order to achieve the qualitative growth of the Korean inland fisheries : first, construction of integrated management system according the aquatic systems ; second, organization of fishermen's interest groups such as cooperatives or associations in order to increase the competitive advantage and spread voluntary fisheries management ; third, boosting stock enhancement through systematic stock assessment and stocking ; fourth, expansion and activation of administrative system and research organizations.

지방자치단체 건축행정 개선방안에 관한 연구 (A Study on the Improvement of Building Administration by Local Government)

  • The non-mandatory guidance and illogical ordinance of local government generate complicated and vague regulations and procedures. These cause inconvenience in building administration, and give rise to unnecessary social and administrative expenses. The purpose of this study is to suggest the improvement of building administration by local government from an institutional point of view through the literature review and the specialist survey focusing on the problems arising from the building ordinances and non-mandatory guidance. The results of this study are as follows; In the first place, the connection between law and ordinance should be improved in a systematic, definitude manner and high-degree of completion. Second, the monitoring management system should be arranged in order to excavate and improve the non-mandatory guidance. Third, the effort should go into improving ability and disposition of building official. Fourth, it is necessary to materialize and clarify the regulatory contents and adjust them to fit the legal system. Lastly, it is important to reinforce incentives and procedures of verification in order to manage building ordinances of legal government's own free will. In order to improve of building administration by local government, it is necessary to make the structure of sustainable improvement and discuss not only institutional aspect but also elements of building administration in later study.

보건소 근무 한의사의 법.제도적 지위와 역할에 관한 연구 (A Study on the Legal and Institutional Position and Role of Korean Medicine Doctors working at Public Health Center)

  • Objective : We proposed fundmental rules of prospective on legal and institutional position and role of Korean medicine doctors working at public health center. Methods : By the result of this research on the current situation, the grade and allowance given to the Korean medicine doctors working at public health center were different every self-governing body. Results : The reason the Korean Medicine Doctor can't serve as a regular order of 5th grade is that the 'The Enforcement Regulation about Administrative Organization and the Standard of Pixed Number of person of Self-Governing Body(지방자치단체의 행정기구와 정원기준등에 관한 규정 시행규칙)' prescribes the number of regular order of 5th grade is regulated within 7% among the number of regular order officials. But not appointing to office as the regular order of 5th grade infringes on the Constitution, the highest law. The reason the Korean Medicine Doctors can't be appointed to office as the regular order officials by the self-governing body is that 'The Enforcement Order of the Law of Preservation of good health of Local Area(지역보건법시행령)' prescribes the Korean Medicine Doctors are not indispensable to Public Health Center. But in fact, the Korean Medicine Doctors can execute many kinds of work such as medical examination or instructing house nursing. Conclusion : The Korean Medicine Doctors working at Public Health Center serve at low positions as daily use or common use, not receiving a regular order. All laws including the Constitution(헌법), the Medical Services Law(의료법), the Law of Preservation of good health of Local Area(지역보건법), the National Public Service Law(국가공무원법), the Local Public Service Law(지방공무원법) and the Law of Higher Education Law(고등교육법) describe that the Korean Medicine Doctors and the Western Medicine Doctors are equal to their position and right.
