부산지역 일부 성인들의 신체활동, 식습관 및 영양소섭취상태에 관한 연구 (A Study on the Physical Activity, Food Habit and Nutrient Intakes of Adults in Pusan)

  • This study was performed to assess the physical activity, food habit and nutrient intakes by gender and age groups in 193 adults aged 20-59 years (84 men and 109 women) in Pusan. Data for physical activity and dietary survey was assessed by a questionnaire and 24hr recall method. The mean BMIs of men and women were 24.0 and 22.2 respectively and BMI of women in the 20-29 years group (20.9) was significantly lower than that of women in the 30-49 and 50-59 years group (22.7, 23.2) (p < 0.01). 56.0% for men and 44.0% for women exercised regularly. The mean exercise duration per once of men (69.7 minutes) was significantly higher than that of women (52.4minutes) (p < 0.01). The mean exercise duration per day was 36.0 minutes for men and 29.9 minutes for women. 67.9% for men and 78.0% for women often skipped meals and 68.4% for men and 69.4% for women skipped breakfast in the main. The mean energy intake of men was 2067.2 kcal and that of women was 1783.1 kcal comprised of 87.2% and 92.1% of the Estimated Energy Requirements (EER). The mineral intakes of men and women were over Recommended Intake (RI) and Adequate Intake (AI) except calcium and potassium. The mean calcium intake was 88.3% for men and 84.0% for women of RI. The mean potassium intake was 63.3% for men and 59.2% for women of AI. The mean vitamin intakes of men and women were over RI and AI except vitamin C and folic acid. The mean vitamin C intake was 92.5% for men and 85.6% for women of RI. The mean folic acid intake was 76.6% for men and 70.0% for women of RI. The mean energy, protein, sodium and zinc intakes of men were significantly higher than those of women (p < 0.01, p < 0.05, p < 0.05, p < 0.05). The mean vitamin $B_6$, vitamin C and folic acid intakes of men in the 20-29 years group were significantly lower than those of men in the 30-49 and 50-59 years group (p < 0.01, p < 0.05, p < 0.05). For energy, proportions of subjects with intake levels less than 90% EER were 64.3% for men and 56.0% for women. For calcium, proportions of subjects with intake levels less than Estimated Average Requirement (EAR) were 52.4% for men and 59.6% for women. For folic acid, proportions of subjects with intake levels less than EAR were 78.6% for men and 83.5% for women. For iron and phosphorus, proportions of women (36.7%, 14.7%)with intake levels less than EAR were significantly higher than those of men (6.0%, 1.2%) (p < 0.01, p < 0.001). For men, age was positively correlated with intakes of potassium, vitamin $B_6$, vitamin C and folic acid (p < 0.05, p < 0.05, p < 0.05, p < 0.01). For men, weight showed significantly negative correlations with intakes of carbohydrate, phosphorus, potassium, zinc, vitamin $B_6$ and folic acid (p < 0.01, p < 0.05, p < 0.05, p < 0.05, p < 0.01) and BMI showed significantly negative correlations with protein, lipid, phosphorus, potassium, zinc, vitamin E and folic acid (p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05) For men, exercise duration per once showed significantly positive correlations with intakes of calcium, phosphorus, potassium, zinc, vitamin $B_1$, vitamin $B_2$, niacin, vitamin C and folic acid (p < 0.01, p < 0.05, p < 0.01, p < 0.05, p < 0.05, p < 0.01, p < 0.01, p < 0.01, p < 0.05). Therefore, nutritional education for adult health management is needed by gender and age groups.

어린이들의 식습관이 비만도와 혈청 지질 수준에 미치는 영향 (Eating Behavior, Obesity and Serum Lipid Levels in Children)

  • Interactive effects among eating behavior, obesity and serum lipid levels were studied in 117, 4~12 year old children residing suburban Seoul. Fasting blood samples are obtained and analyzed for serum triglycerides (TG), total cholesterol(TC), high density lipoprotein-cholesterol(HDL-C) and hematochrit. Obesity was determined by weight for length index(WLI)and the information on eating behavior including food habits and dietary intakes was obtained by questionaire using food record method for 2-consecutive days. Over 40% of children was classified overweight or obese by WIL and children's physical parameters were closely related to those of parents implying genetic influence on obesity. Although it did not reach the statistical significance, there was a tendency of higher TG, TC and low density lipoprotein-cholesterol(LDL-C)levels among girls compared to boys. Blood lipid levels of obese children were similar to those of other groups except TG, which was significantly higher(p<0.05) in obese group. Nutrient intakes seemed adequate in all subjects except iron, calcium and total calorie which were lower than RDAs. Lacking significant relationship between individual nutrient intake and obesity, there was significant correlation between food intake and blood lipid level especially in 10-12 year old group. Vegetable intake was negatively related to TG, LPH(LDL-C/HDL-C) and atherogenic index(AI), and positively to HDL-C. Skipping breakfast and frequent eating out appeared to cause imbalances in nutrient intake. These findings clearly revealed the influence of eating behavior on childhood obesity along with blood lipid profile. To ensure the proper growth and health of these children, devising method and developing media for nutrition education suited to our society should be accomplished first. With well-planned nutrition surveys and thorough intention, childhool obesity could be prevented from progress into adulthood obesity.

서울 일부지역 남자 고등학생의 주중과 주말의 식품 및 영양소 섭취에 관한 연구 (A Study on Food and Nutrient Intakes of Weekday and Weekend Among High School Boys in Seoul)

  • This study was conducted to examine food and nutrient intakes of weekday and weekend with 329 high school boys residing in Seoul using the 3-day food record. Frequency for breakfast skipping was the highest as compared with lunch and dinner skipping. Frequencies for breakfast and lunch skipping were significantly higher in weekend than weekday (p<0.05). The daily average food, vegetable food and animal food intakes were 1,327.6 g, 800.5 g, and 425.8 g, respectively. Total food and vegetable food intakes of weekday were significantly higher than weekend(p<0.05). The dietary variety score(DVS) was 20.5 in daily average, 23.3 in weekday and 15.1 in weekend, which showed significantly higher in weekday than weekend(p<0.05). The daily averages of energy, protein, fat and carbohydrate intake were 2244.9 kcal, 89 g, 72.6 g, and 311.2 g, respectively. Fat intake was significantly lower and carbohydrate, fiber, phosphate, iron, sodium, potassium, vitamin A, niacin, folate, and vitamin C intakes were significantly higher in weekday than weekend (p<0.05). The percentages of energy intake from carbohydrate, protein, and fat were 55.4%, 15.8%, 28.8% in daily average, 56.8%, 15.8%, 27.4% in weekday and 53.6%, 15.8%, 30.7% in weekend, respectively. The percentages of energy intake from carbohydrate in weekday and weekend were below 60%, and that from fat was above 27% in weekday and weekend. Carbohydrate intake was significantly higher and fat intake was significantly lower in weekday than weekend(p<0.05). Energy intakes of daily average, weekday and weekend were above 83% as compared with estimated energy requirement(EER). Intakes of dietary fiber, calcium, potassium, vitamin C, riboflavin and folate were below 75% as compared with adequate intake(AI) or recommended intake(RI). Mean adequacy ratios(MAR), an index of overall dietary quality were 0.78 in daily average, 0.80 in weekday and 0.75 in weekend. MAR of weekend showed significantly lower than weekday(p<0.05). This study revealed that the overall nutrient intake status was worse in weekend than weekday among high school boys.

우리나라 일부 초등학생과 중학생의 영양강화식품 섭취 실태 및 영양강화식품을 통한 비타민과 무기질 섭취량 조사 (Survey on the Patterns of Fortified Food Consumption and Intake of Vitamins and Minerals in Fortified Foods by Elementary School and Middle-School Students in Korea)

  • This study was performed to investigate patterns of fortified food (FF) consumption and intake of vitamins and minerals from FFs among 577 Korean children (12.4 years of age) who attended elementary or middle school. FFs eaten by children as a snack were surveyed using the food record method during 3 days, including 2 week days and one weekend. As a result, 114 FF items were eaten by the children, and several kinds of nutrients such as vitamin A, D, E, B complex, C, calcium (Ca), iron (Fe), and zinc (Zn) were fortified in these foods. Ca-FFs (65.8%) were most frequently consumed, followed by vitamin C-FFs (33.4%) and vitamin D-FFs (33.3%). The number of FF items in each food group was the most in the milk group (n=24, 21.0%), followed by the beverage group (n=19, 16.7%), and the cookie/bread/cake group (n=17, 14.9%). Fortified nutrients in FFs were in various combinations, but the major combination patterns were Ca, Ca plus vitamins, Ca plus vitamins plus other minerals, and Ca plus other minerals. Daily mean intakes of vitamins and minerals from the FFs were 66-300% more than those of the recommended nutrient intake (RNI ) or adequate intake (AI) for most vitamins and minerals. Daily maximum intakes (95th percentile) of vitamins and minerals from FFs were 1-15 times the RNI or AI for most vitamins and minerals. Vitamin and mineral consumption ratios from each FF group were different according to the kind of fortified nutrient. For example, vitamin C was mostly eaten in fortified beverages (46-54%), and Fe was mostly eaten in fortified cookie/breads/cakes (87%). The above results show that FF consumption varied widely among the children, and that most of the children's foods were fortified with several vitamins and minerals without a common rule; thus, subjects risked over consuming vitamins and minerals by eating FFs. Therefore, practical guideline on FF use for children's optimal nutrition and health should be provided through nutrition education.

한국 성인의 칼륨 섭취 현황 : 2007~2010년 국민건강영양조사 자료 이용 (Potassium intake of Korean adults: Based on 2007~2010 Korea National Health and Nutrition Examination Survey)

  • 본 연구에서는 2007년부터 2010까지의 국민건강영양조사 자료를 이용하여 한국 성인의 칼륨 섭취량, 나트륨과 칼륨의 섭취 몰 비율 (molar ratio)을 비교분석하고 칼륨의 급원 식품군과 식품, 건강지표들과 칼륨의 섭취 수준과의 관계를 알아보았다. 모든 통계분석은 변수에 국민건강영양조사 자료에서 제공하는 가중치를 적용하였으며, 복합표본설계 일반선형모형을 이용하였다. 칼륨의 1일 평균 섭취량은 남성은 충분섭취량에 근접하고 있었으나 여성에서는 충분섭취량을 크게 밑돌고 있었으며 남녀 모두 20~64세 연령층에 비하여 65~74세 연령층에서 높았다. 나트륨과 칼륨 섭취량의 몰 비율 (molar ratio)은 남성이 2.89~3.23로 여성 (2.62~2.95)에 비해 높았고, 남녀 모두 50세 이후 연령층의 Na/K 몰 비율 (molar ratio)이 낮고 20~49세 연령층에서 높게 나타났다. 한국인이 섭취하는 칼륨의 급원식품 중 기여도가 높은 식품군은 조사 시기, 성별, 연령층에 무관하게 채소군이었으며, 그 기여도는 전체의 28~33%로 전체 칼륨 섭취량의 1/3을 차지하였다. 그 다음으로 기여도가 큰 식품군으로 곡류, 과일류, 육류 등이었다. 기여도가 높은 식품은 백미와 배추김치였고, 감자-참외-고구마-미역-무-사과-검정콩 등이 그 다음 순으로 조사되었다. 칼륨 섭취수준을 사분위로 나누어 건강지표들과 관련성을 보았을 때 칼륨 섭취 수준이 증가할수록 50~64세 여성수축기 혈압이 유의하게 감소하였고 (p < 0.01), 1사분위군 (1일 평균 1,851.2 mg 이하 섭취자)의 수축기 혈압은 4사분위군 (1일 평균 3,602.2 mg 이상 섭취자)의 수축기 혈압보다 3.6 mmHg 높았다. 또한 동일 연령대의 여성에서 혈중 중성지방의 농도가 1사분위군에서 가장 높았고, HDL-콜레스테롤의 농도는 칼륨섭취량이 많을수록 증가하였다 (p < 0.05). 본 연구결과, 칼륨섭취량은 식사섭취량에 따라 변화하므로 특히 성별열량 섭취량에 따라 영양섭취기준을 달리 설정하는 것도 고려해 볼 여지가 있으며 칼륨 섭취를 높이고 나트륨의 섭취를 줄일 수 있는 한국식 DASH diet의 개발이 필요할 수 있음을 시사한다.

부산지역 일부 대학생의 식습관 및 영양소 섭취상태에 관한 연구 (The Comparison in Daily Intake of Nutrients and Dietary Habits of College Students in Busan)

  • The purpose of this study was to investigate the dietary habits and daily intake of nutrients in college students. This survey was conducted using a self-administered questionaire. The average heights and weights were 173.5 cm and 72.3 kg of male students and 161.8 cm and 57.2 kg of female students. The average of BMI was $24.2kg/m^2$ of male and $21.9kg/m^2$ of female, and the value of male students was higher than the value of female students. The response to the daily meals was 54.6% for '$2{\sim}3$ times/week'. The regularity of mealtime was 41.7% for irregular and the frequency eating after nine was 45.7% for '5-6 times/week', respectively. The repast was 72.2% for 'overeating and little eating' and was a significant difference of male and female students (p<0.05). The eating rate was higher '$10{\sim}20min$'. As for breakfast food eaten, skipping breakfast was 23.6% for 'no/week' and female students were higher than male students (p<0.05). The frequency of snacks was 36.0% for 'nothing' of males students and 34.8% for '3-4 times/week' of female students (p<0.05). The type of snack was a significant difference of males and females students (p<0.01), and was the highest 75.0% for carbonated drinks of males and 37.5% for snacks of females. The eating due to stress solution was a significant difference of male and female students (p<0.01), and was the highest 23.0% for 'frequency' of males and 44.7% for 'sometime' of females. As for food intake of male and female students, the meat intake was 66.7% for 'everything of male and female students. The fish intake was 68.1 % for '1-2times/week'. The milk, milk products, eggs and beans were each 40.3%, 58.3%, 56.9%, 47.2% for '1-2 times/week' (p<0.05). The fat intake was 55.6% for '$1{\sim}2$ times/week'. The average consumption of energy was 58% of male and 67% of female of estimated energy requirement (EER). Their mean ratio of carbohydrate: protein: fat was 57 : 15 : 28 of all subjects. The mean intakes of vitamin C and folic acid were 70% and 51% of males and 62% and 52% of females of recommended intake (RI). The mean intakes of Ca, P, Fe and Na were 71%, 140%, 146% of males and 72%, 122%, 76% of female of RI and 273% of males and 233% of females of adequate intake (AI). Therefore, nutritional education is necessary for college students to establish physicall and mentall optimal health conditions though nutritional intervention.

시설노인의 수분 섭취 강화 프로그램의 효과분석 (The Effect of Fluid Intake Enhancing Program for Institutionalized Elderly)

  • 본 연구는 시설노인을 위한 수분 섭취 강화 프로그램을 개발하고 적용하여 그 효과를 평가하고자 수행되었다. 자료 수집은 충청지역에 위치한 일개 노인요양시설에 거주하는 39명의 노인을 대상으로 하였으며 프로그램 적용 전, 4주후, 6주후의 연속 3일 측정한 수분 섭취량의 평균, 소변색, 요비중의 평균, 요비중이 탈수 내지 임박한 탈수의 범위에 속하는 대상자의 비율을 비교하였다. 수분 섭취 강화 프로그램 적용 전 1일 평균 수분 섭취량은 프로그램 적용 4주후, 6주후에 통계적으로 유의한 증가를 나타냈다. 1일 수분 충분섭취량인 1,100mL 미만을 섭취한 대상자의 비율은 프로그램 적용 전에 비해 4주, 6주후에 감소하였다. 요비중이 임박한 탈수 혹은 탈수의 범위에 속하는 대상자 비율은 중재 전 보다 중재 4주후에 감소하였으나 6주후 증가하였고 요비중의 평균은 프로그램 적용 4주후, 6주후에 통계적으로 유의한 차이를 나타내었다. 소변색은 중재 전, 후 모두 정상범위를 나타내었다. 본 연구의 결과는 시설노인 수분 섭취 강화프로그램은 노인의 수분 섭취를 증진하는데 기여할 수 있는 가능성을 시사하며 추후 임상에서의 적용을 위하여 해결해야 할 문제점들을 논의하였다.

우리나라 일부 심부전 환자의 영양소 섭취량 평가 연구 (The Assessment for Nutrient Intakes of Korean Patients with Heart Failure)

  • 본 연구에서는 우리나라 심부전 환자 78명을 대상으로 식이 섭취 빈도법을 통한 영양소 섭취량 조사를 시행하여 한국인 영양섭취기준과 비교하였으며 그 결과는 다음과 같았다. 1) 에너지와 단백질, 철분의 평균 섭취량은 한국인 영양섭취 기준에 비해 충분히 섭취하고 있었으나 나트륨 섭취가 과다하고, 칼륨 섭취가 부족한 불균형의 영양문제가 파악되었다. 2) 칼슘의 섭취가 부족한 환자들이 상당수 존재하여 심부전 환자의 골연화증 및 골다공증 위험이 높을 가능성이 있음을 확인하였다. 3) 비타민 $B_{12}$, 엽산의 섭취가 부족하며, 이외 리보플라빈, 나이아신 등의 비타민 섭취 부족의 가능성이 있음을 확인하였다. 4) 70세 이상의 고령 환자에서도 칼륨, 칼슘, 비타민 $B_{12}$, 엽산의 섭취가 부족한 것으로 나타났고 이러한 부족 현상은 고령 환자에서 더욱 심각한 것으로 파악되었다. 위의 결과로 보아 국내 심부전 환자의 경우 무기질, 비타민 등의 미량 영양소의 섭취 부족이라는 영양문제가 존재하고 있었다. 따라서 이러한 영양 결핍 문제가 지속된다면 심부전 환자의 장기적인 예후에 영향을 미칠 수 있을 것이므로 영양섭취 부족을 해결하기 위한 다각적 측면에서의 영양 섭취 평가 및 영양관리 대책과 지침이 마련되어야 할 것이다.

히알루론산 기반 비타민 D 함유 구강용해필름의 제조 및 특성평가 (Development and characterization of hyaluronic acid-based orally disintegrating film containing vitamin D)

  • 칼슘과 함께 뼈 건강에 필수적이나, 그 섭취량이 부족하여 문제가 되고 있는 비타민 D를 쉽게 보충할 수 있는 새로운 제형으로 천연 고분자 물질인 히알루론산을 기반으로 한 비타민 D 함유 구강용해필름(orally disintegrating film, ODF)을 개발하고 비타민 D 함량에 따른 필름의 특성을 분석하였다. 첨가량은 2020 한국인 영양소섭취기준의 비타민 D 하루 충분섭취량(400IU: 10 ㎍)을 기준으로 4, 7배, 그리고 상한섭취량인 10배로 설정하였다. 제조한 필름의 두께는 기반물질의 농도가 가장 높은 대조군이 가장 두꺼웠고, 비타민 D 첨가량에 따른 유의적 차이는 없었다(p<0.05). 비타민 D 첨가군간 필름의 수분함량의 차이는 없었으며, 첨가량이 많아질수록 투습도는 다소 감소하는 경향을 보여 소수성 물질인 비타민 D가 영향을 미친 것으로 보인다. 비타민 D의 함량이 높아질수록 필름의 명도는 10AI만 유의적으로 높았으며, 적색도는 감소하고 황색도는 증가하였다(p<0.05). 이와 같은 색도특성은 첨가한 비타민 D 시료 자체 색의 영향을 받은 것으로 보인다. 대조군과 비교하였을 때 비타민 D의 증가는 불투명도를 유의적으로 증가시켰으며(p<0.05), 7AI와 10AI에서 가장 높은 불투명도를 보였다. 다른 친수성 고분자 필름과 달리 본 연구에서 제조된 히알루론산 기반 필름은 가소제의 첨가 없이도 타 연구의 필름에 비교하여 높은 인장강도(84.40-106.6 MPa)(p<0.05)와, 비슷한 수준의 연신율(4.71-9.43%)(p>0.05)을 갖는 질감 특성을 보였다. HPLC/MS 분석을 통해 필름 내의 실제 비타민 D 함량을 분석한 결과, 제조 과정 중 비타민 D의 손실이 발견되었으며 이로 인해 목표섭취량(2020 한국인 영양소섭취기준 비타민 D 충분섭취량)을 충족하기 위해서는 제조 기준의 1.5-2배를 섭취하는 것이 적절할 것으로 보인다. 본 연구에서는 생체물질인 히알루론산을 기반으로 비타민 D를 쉽게 섭취할 수 있는 새로운 제형을 개발하였으며, 비타민 D 첨가로 인한 필름의 특성이 향상되는 결과를 이용하여 구강용해용 뿐 아니라 가식성 포장재 등 다양한 활용이 가능할 것으로 제안하는 바이다.

The Supplementation of Yam Powder Products Can Give the Nutritional Benefits of the Antioxidant Mineral (Cu, Zn, Mn, Fe and Se) Intakes

  • Yam has been recognized having the beneficial effects for the prevention of various diseases, such as cancer, immunity, infection and obesity etc. There is increasing consideration to supplement the antioxidant nutrients to make up the lack of the antioxidant nutrient intakes. No study has been reported for the analysis of antioxidant mineral contents and comparison to dietary recommended intake for the sense of health promotion. In our study, we analyzed the contents of antioxidant trace elements (Zn, Mn, Fe, Cu and Se) and Cr contents in cultivated Korean yam powders for evaluation of nutrient intake aspects. We collected the commercial yam powders from six different cultivated areas in the South Korea and measured antioxidant minerals (Zn, Mn, Fe, Cu and Se) and Cr contents using trace element-free plasma spectrometer (ICP) or atomic absorption spectrometer (AAS) after dry-ashing and then wet-acid digestion. The accuracy of mineral analysis method was confirmed by the mineral analysis of standard reference material. Each analyzed element contents in yam were compared to dietary reference intakes of Koreans (KDRIs). The average levels of trace elements (Zn, Mn, Fe, Cu, Se and Cr) in yam powders were 18.3, 11.9, 36.0, 3.7, 1.9 and 1.27 ${\mu}g/g$ yam powder, respectively. The intakes of Zn, Fe, Cu and Se of which KDRIs is determined, are accounted as being up to 23.8%, 55.6%, 32.5% and 236% recommended intake (RI) of KDRIs, if daily yam supplementation (50 g) of commercial instruction would be considered. The intake of Mn is about 25% adequate intake (AI) of KDRIs with the daily supplementation of yam powder. Most of mineral intakes from daily yam supplementation were with the range of non-detectable to <10% upper limit (UL) level, which is very much safe. The study results show that daily supplementation of Korean yam power is beneficial to provide the supplemental nutrient intake and also is safe, if the suggested dosage would be considered.
