Helical Rim Advancement Flap with an Additional Postauricular Skin Flap: A Case Report

  • The auricle is a three-dimensional cartilaginous frame covered with thin overlying skin. Due to its complex features, reconstructing helical rim defects after the excision of an auricular mass is challenging. Shortage of subcutaneous tissue and the presence of a tightly bound epithelium further hamper the primary closure of lateral (anterior) auricular skin defects. We present herein a case in which we used a helical rim advancement flap along with an additional postauricular skin flap. We achieved a satisfactory esthetic result with minimal loss of helical diameter and a low risk of flap necrosis by preserving the vascular network of the flap. This technique is less traumatic and will facilitate faster healing as well as improved patient recovery.

체질량지수와 디지털유방촬영술상의 유방밀도와의 연관성 (Association Between Body Mass Index and Breast Density Using Digital Mammography)

  • 이 연구의 목적은 무증상 여성을 대상으로 폐경 여부에 따라 체질량지수에 따른 유방밀도의 분포양상을 분석하여 선별유방검사방법 선택에 도움을 주는 것이다. 496명의 여성을 대상으로 유방촬영술을 시행하여 유방밀도를 분류해 분석한 결과는 다음과 같다. 40대 이하의 젊은 여성의 경우 치밀유방의 비율이 높은 것으로 나타났으며, 연령이 증가할수록 유방밀도는 감소하는 것으로 나타났다. 체질량지수에 따른 유방밀도 분포는 체질량지수가 증가함에 따라 유방밀도가 감소하는 것으로 나타났다. 폐경여부에 따라 유방밀도에 영향을 미치는 비만관련 요인을 알아 본 결과 폐경 후 여성에서는 연령과 체질량지수가 상관관계가 있는 것으로 나타났으나, 폐경 전 여성에서는 뚜렷한 상관관계가 있는 요인이 나타나지 않았다. 따라서 유방암 선별검사 선택 시 젊은 여성의 유방암의 발생비율과 치밀유방의 비율이 높은 우리나라의 특성을 고려한 개별적인 검진방법을 선택하는 것이 필요하겠으며, 젊고 마른 여성의 경우 유방초음파를 병행한다면 검사의 민감도를 보완할 수 있을 것이다.

지중 시공 벽체의 매립장 침출수 차단성 연구 (The Study on Cutting-off the Leachate Leakage or Infiltration from Waste Landfill by Wall Mass Constructed in Underground)

  • 원호활동파괴가 발생한 해안 폐기물 매립장에서 기초지반 전단강도의 보완과 폐기물 침출수가 지하수나 인접지반으로 유동되는 것을 차단하는 최종 시설로서 시멘트계 지중벽체를 시공한 후, 침출수 차단효과를 확인하였다. (1) 육안 확인 및 (2) 압축강도 시험에 의하면 지중에 시공된 벽체의 품질이 상당히 양호하다는 사실을 확인할 수 있고 (3) 현장 투수시험과 (4) 성분 및 성분비 분석과 (5) 질량 분석기를 통해서 얻게 되는 크로마토그램의 분포패턴 판정 등 중복적인 실험 및 조사 분석에 의해서 벽체의 수밀성이나 침출수의 이동에 대한 차단성이 매우 크다는 것을 확인할 수 있었다. 따라서, 폐기물 매립장 내에서 발생하는 침출수와 같은 고농도 환경오염수가 매립장 밖으로 유출되거나 침투하는 것에 대한 신뢰적인 차단 여부는 이 5가지의 조사 및 시험을 실시하는 방안에 의하여 객관적으로 판단될 수 있다고 사료된다.

Percutaneous Ultrasound-Guided Fine-Needle Aspiration Cytology and Core-Needle Biopsy for Laryngeal and Hypopharyngeal Masses

  • Objective: To evaluate the feasibility and diagnostic performance of ultrasound (US)-guided fine-needle aspiration cytology and core-needle biopsy (US-FNAC/CNB) for the diagnosis of laryngo-hypopharyngeal masses. Materials and Methods: This was a single-center prospective case series. From January 2018 to June 2019, we initially enrolled 40 patients with highly suspicious laryngo-hypopharyngeal masses on laryngoscopic examinations. Of these, 28 patients with the mass involving or abutting the pre-epiglottic, paraglottic, pyriform sinus, and/or subglottic regions were finally included. These patients underwent US examinations with/without subsequent US-FNAC/CNB under local anesthesia for evaluation of the laryngo-hypopharyngeal mass. Results: Of the 28 patients who underwent US examinations, a laryngo-hypopharyngeal mass was identified in 26 patients (92.9%). US-FNAC/CNB was performed successfully in 25 of these patients (96.2%), while the procedure failed to target the mass in 1 patient (3.8%). The performance of US caused minor subclinical hematoma in 2 patients (7.7%), but no major complications occurred. US-FNAC/CNB yielded conclusive results in 24 (96.0%) out of the 25 patients with a successful procedure, including 23 patients with squamous cell carcinoma (SCC) and 1 patient with a benign mass. In one patient with atypical cells in US-FNAC, additional direct laryngoscopic biopsy (DLB) was required to confirm SCC. Among the 26 patients who received US-FNAC/CNB, the time from first visit to pathological diagnosis was 7.8 days. For 24 patients finally diagnosed with SCC, the time from first visit to the initiation of treatment was 25.2 days. The mean costs associated with US-FNAC/CNB was $272 under the Korean National Health Insurance Service System. Conclusion: US-FNAC/CNB for a laryngo-hypopharyngeal mass is technically feasible in selected patients, providing good diagnostic performance. This technique could be used as a first-line diagnostic modality by adopting appropriate indications to avoid general anesthesia and DLB-related complications.

Cutaneous Rosai-Dorfman Disease Confused with Vascular Mass

  • Rosai-Dorfman disease is a rare histiocytic disorder, clinically characterized by massive, bilateral painless cervical lymphadenopathy with potential for extranodal manifestations. We report a 45-year-old male patient who presented with a slowly growing erythematous nodule of the left chin. The mass appeared non-vascular on computed tomography study, but ultrasonogram was suggestive of a vascular lesion. The lesion was excised with presumptive diagnosis of a hemangioma. However, histopathologic examination of the surgical biopsy revealed histiocytic infiltration with emperipolesis, which was pathognomic for Rosai-Dorfman disease. Additional imaging studies did not reveal lymph node enlargement or other extranodal manifestation. The patient was diagnosed with cutaneous form of the Rosai-Dorfman disease and was discharged home. He remains free of local recurrence at 8 months.

  • We have analyzed the stability for cut-slopes at main street 3-2 line section in Pa-ju local industrial complex. After studying an additional boring test, laboratory test and face mapping etc., we have determined the extent of reinforcement, slope inclinations and soil strength parameter from the analysis of test results. After changing the inclination of slopes for ground limit and carrying out the analysis of slope stability, we applied the Mass Nailing Method to the site because of need for reinforcement to soil and weathered rock slopes. In slope for soft and hard rock sections, we also reinforced the sections that are difficult to obtain the safety without reinforcement in alteration zone.

Comparative Proteome Analysis of Celastrol-Treated Helicobacter pylori

  • Various preclinical and clinical trials have been conducted the efficacy of celastrol. In data presented in the current manuscript is the first trial to inhibit Helicobacter pylori with celastrol. In this study, the quantitative change of various H. pylori proteins including CagA and VacA by the anti-bacterial effect of celastrol was determined. The anti-H. pylori effects of celastrol was investigated by performing 2-dimensional electrophoresis and additional supporting experiments. After 2-dimensional electrophoresis analysis, spot intensities were analyzed and then each spot was identified using matrix assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF-MS) or peptide sequencing using Finnigan LCQ ion trap mass spectrometer (LC-MS/MS). The results show that celastrol has multiple effects on protein expression in H. pylori.

  • The data set was collected on fifty-eight different days with a 24-h sampling period from October 27, 1995 through August 25, 1996. From the chemical mass balance (CMB) analysis of $PM_{2.5}$ in the Chongju area, the contributions from soil, gasoline, diesel, light and heavy oil combustion were 2.6%, 15.4%, 9.0%, 28.8% and 1.5%, respectively. Residual $NO_{3}^{-}$), residual $SO_{4}^{2-}$ and residual OC, possibly formed in the atmosphere. represented additional 8.0, 10.2, and 1.6% of the $PM_{2.5}$, respectively. Other unidentified sources constituted the remaining 22.9%. From the CMB analysis, the $PM_{2.5}$ source contribution for fall, winter, spring and summer were 92, 76.8, 77.5 and 59.2%, respectively.

분석기기지원을 위한 원격 데이터 분석 시스템 개발 (Development of Remote Data Analysis System for the Joint Use of Equipments)

  • In Korea Basic Science Institute(KBSI) the remote data analysis system is developed for the joint use of advanced equipments. This system enables the researchers to access the datas which are produced at KBSI and analyse them by Java program on the Web,. Except Web browser such as Internet Explorer or Netscape Navigator no additional softwares are required for analysing data. We have developed remote data analysis systems for five major equipments which KBSI supports for the researchers, The systems which are developed are those for NMR spectrometer High Reso-lution Tandem mass Spectrometer Microscopic Imaging System DNA Sequencer and Natural Ra-dioactivity Measruement System, These programs work on any computer platform and any operat-ing system only if the internet is available. This remote data analysis system will be served as a part of Collaboratory the remote collaborative system.
