A Case Study of Burning Mouth Syndrome with Oro-facial Dyskinesia Treated with Korean Medicine using Se-sim Pharmacopuncture (심화상염(心火上炎)으로 진단된 설통(舌痛)과 구강안면부 진전(振顫)의 세심약침을 이용한 한의치료 1례)

  • Objectives: This clinical report describes one patient with the clinical characteristics of burning mouth syndrome accompanied by oro-facial dyskinesia. Methods: The patient with burning mouth syndrome and jaw tremor was treated using the following Korean medicine treatments: the herbal medicines Yukgunja-tang-gami (六君子湯加味) and Sibak-tang-gami (柴朴湯加味) and acupuncture for 16 days, together with 10 Se-sim (Uncaria Ramulus et Uncus) pharmacopuncture procedures. Patient symptoms were evaluated daily using a Visual Analog Scale questionnaire and twice using the STAI (State-Trait Anxiety Inventory) questionnaire. Results: After the treatments, the oro-facial dyskinesia and burning sensation on the tongue were improved by 62.5%. The mental state of anxiety, regarded as the reason for these complaints, was also improved, based on the State-Trait Anxiety Inventory results (67/61 versus 57/52). We found that the improvement was better when combining Se-sim (Uncaria Ramulus et Uncus) pharmacopuncture with the conventional herbal medicine and acupuncture used in Korean medicine. Conclusion: Korean medicine treatments may be valuable for patients with burning mouth syndrome and oro-facial dyskinesia.

Case Report of Chemotherapy-Induced Peripheral Neuropathy Treated with Korean Medicine (항암화학요법 유발 말초신경병증에 대한 한방치험 1례)

  • Introduction: The aim of this study is to report on the effectiveness of Korean medicine to improve symptoms of chemotherapy-induced peripheral neuropathy (CIPN). Some patients are treated with medication, but medications may have no effect in some patients. Korean medicine treatments have been used in such patients, but few reports exist. Case Presentation: A 58-year-old female patient with CIPN reported symptoms of bilateral limb pain and tingling sensations as well as cold sensations in bilateral lower legs and hands, fatigue, and bilateral limb weakness. To reduce the patient's symptoms, we provided Korean medical treatment, including herbal medicine (Uchashinki-hwan), acupuncture, and moxibation. To evaluate the results of this treatment, we used a numeric rating scale (NRS), the manual muscle test (MMT), the National Cancer Institute Common Toxicity Criteria for Adverse Events (CTCAE), and the European Organization for Research and Treatment of Cancer quality-of-life questionnaire about CIPN (EORTC-QLQ-CIPN20). After 49 days of treatment, the NRS scores decreased for bilateral limb pain and tingling, for bilateral lower leg and hand cold sensation, and for fatigue. The MMT results for the upper and lower extremities were improved. CTCAE Grade and EORTC QLQ-CIPN20 score were also decreased. Conclusion: According to these results, Korean medicine treatment may be considered an effective treatment for CIPN. Prospective studies are needed in the future to confirm and expand these findings.

Three Cases of Postpartum Hyperhidrosis Patients Treated with Korean Medical Treatment Including Sanhuhyulpung-tang (산후혈풍탕을 포함한 한방치료로 호전된 산후다한 치험 3례)

  • Objectives: The purpose of this study is to report the effects of Korean medical treatment including Sanhuhyulpung-tang on three postpartum patients who suffered from hyperhidrosis. Methods: We treated three patients complained of postpartum symptoms including hyperhidrosis. The patients were treated by Traditional Korean Medicine including herbal medicine, acupuncture, moxibustion, and cupping therapy. Sanhuhyulpung-tang is a herbal medicine prescription which is used in clinical practice for the treatment of postpartum sweating, anxiety, flushing, and joint pain. Three patients were hospitalized for treatment for 6, 8, and 19 days respectively. During hospitalization, patients took Sanhuhyulpung-tang three times a day, and acupuncture, moxibustion, and cupping were performed once a day. To evaluate the degree of improvement, we used VAS score of the patient's chief complaint during hospitalization. Results: After the treatment, symptoms including hyperhidrosis were improved. The degree of excessive sweating decreased to 30% in two patients, and 55% in one patient. Combined symptoms including lower coldness, burning sensation, chilling, and multiple site pain also improved on discharge day. Conclusions: This case report shows that Korean medicine treatment including Sanhuhyulpung-tang can improve postpartum symptoms including hyperhidrosis.

A Case Report of a Patient with Suspected Secondary Raynaud Phenomenon Managed with Danggwisaeyeok-tang (이차성 레이노 현상 의심소견에 대한 당귀사역탕 투여 경과 : 증례보고)

  • Objective: The aim of this report is to report the effects of Danggwisaeyeok-tang (當歸四逆湯) on Raynaud phenomenon in a patient. Methods: A 57-year-old male diagnosed in 2016 with Raynaud's phenomenon complained of pain, digital ulceration, and discoloration of his fingers. He underwent acupuncture and electroacupuncture treatment, but the symptoms gradually worsened, so we administered herbal medicine, Danggwisaeyeok-tang (當歸四逆湯). Visual observation of the affected part, changes in subjective symptoms, and a verbal numerical rating scale (VNAS) were used to assess the clinical response. Results: After treatment with Danggwisaeyeok-tang, the patient reported improvement in his symptoms of cold sensation, ulceration, and sclerosis of his digits. Furthermore, the patient exhibited no general adverse effects. Conclusions: Danggwisaeyeok-tang (當歸四逆湯) significantly improved the patient's clinical symptoms. This case suggests that Danggwisaeyeok-tang (當歸四逆湯) may represent a good option for the treatment of Raynaud's phenomenon.

  • Objectives : Pruritus is a skin disorder that usually occurs in the elderly. The purpose of this study is to report the effect of external preparation containing Ponciri fructus and Radix lithospermi on senile pruritus and xeroderma with liver cirrhosis.Methods : We treated 85-years-old liver cirrhosis patient suffered from itching sensation on the whole body with external preparation containing Ponciri fructus and Radix lithospermi and oral administration of Sodalgeonbitang for 37 days.Results : Despite discontinuing western medical treatment of topical steroids and oral antihistamine, pruritus and erythema of the patient were significantly improved.Conclusions : As for external application, decoction of Ponciri fructus and Radix lithospermi might be effective used for treatment senile pruritus and xeroderma.

  • Uterine myoma is benign tumor formed from muscular and connective tissues, which is one of the most common tumors found in women's reproductive organs. Uterine myoma is the most common pelvic tumor and they are diagnosed in up to15 to 20% of women in pubertal period. Although myoma is generally considered to be a slowly growing tumor, in 20-40% of women at the age of 35 and more have uterine myoma of significant sizes with severe clinical symptoms. The most common symptoms include abdominal pain or discomfort, low abdominal pressure sensation, hypermenorrhea and urinary symptoms. The treatment of uterine myoma is just observation, operation and hormone replacement therapy. We treated 6 outpatients who has uterine myoma and visited Kyungwon InCheon Oriental Hospital with Herb-medication and acupuncture. So we have gotten good result from them. After 6 outpatients had taken Guichulpazing-tang(歸朮破?湯), the symptoms were improved and the size of the uterine myoma was reduced significantly, It proves that oriental medical treatment is effective on the patients with uterine myoma, but continuous studies are needed.

A Case of Erythema Nodosum-like Lesions of a Behcet's Disease Patient treated with Wolbi-Tang (월비탕(越婢湯)으로 치료된 베체트병의 결절홍반양 병변 치험 1례)

  • Background and Objective : Erythema nodosum-like lesions in patients with Behcet's disease is a reactive dermatosis that occurs in response to various stimuli, such as bacterial, fungal and viral infections, drugs and malignancies. We observed and treated a case of erythema nodosum-like lesions with Behcet's disease. We present this case because there were no former reports of erythema nodosum-like lesions with Behcet's disease treated by herbal medicine. Methods : The patient was a 28-year-old. The chief complains were erythema, nodose, edema, burning sensation, tenderness on both legs, forearms and feet, arthralgia and genital ulcers. She was treated with Wolbi-Tang for eleven days. We diagnosed the medication according to the results of manual abdominal examination of the patient. We used the Visual Analogue Scale(VAS). Results : After the treatment the grade of VAS was decreased and clinical symptoms were improved. Conclusions : Wolbi-Tang can be used on the treatment of erythema nodosum-like lesions with Behcet's disease.

A Case Report of Yeoldahanso-tang on Sudden Hearing Loss and Tinnitus after Trigeminal Schwannoma Surgery (열다한소탕의 삼차신경초종 수술후 돌발성 난청 및 이명 치험 1례)

  • Objective The case report showed that sudden hearing loss and tinnitus after trigeminal schwannoma surgery were improved with Yeoldahansotang-gamibang. Method The patient was diagnosed with Taeeumin interior heat (Ganyeol) disease based on the original symptoms of Taeeumin. He was treated with herbal medicine and acupuncture treatment. Puretone audiometry (PTA), speech audiometry, tinnitus handicap inventory (THI) and the original symptoms were investigated before and after the treatment. Results Right side PTA score was significantly reduced, speech audiometry was improved in speech reception threshold (SRT), word recognition score (WRS) and most comfortable level (MCL) and THI score decreased from 40 to 0, which showed normalizing hearing function. And the patient revealed improvements in sleeping, digestion, stooling, perspiration and facial sensation after treatment. Conclusion This study suggests that Yeoldahansotang-gamibang is effective on sudden hearing loss and tinnitus after surgery by correcting the imbalanced energy of Taeeumin.

  • Objectives : The purpose of this study is to report the effect of Bangpoongtongseong-san Gagam and Yukmijihwang-tang Gagam on whole-body xerotic eczema. Methods : We treated a 63-year-old male patient who has whole-body xerotic eczema by Korean medicine. He was treated from August 23rd, 2023 to November 22th, 2023. The treatment applied was acupuncture, pharmacopuncture and herbal medicine including 'Bangpoongtongseong-san Gagam' and 'Yukmijihwang-tang Gagam'. The evaluation was conducted by photo comparison, Visual Analogue Scale(VAS) and Dermatology Life Quality Index(DLQI). Results : After three months of treatment, the symptoms were disappeared. The itching and heat sensation decreased, and redness also disappeared. VAS of discomfort was decreased from 10 to 0. DLQI was also decreased. Conclusions : Bangpoongtongseong-san Gagam and Yukmijihwang-tang Gagam helped to relieve the itching and heat of whole-body xerotic eczema.

A Case Report of Korean Medicine Treatment Focused on Acupotomy Therapy for Acute Cervical Radiculopathy Presenting with Shoulder Pain (어깨 통증 양상으로 나타난 급성 경추부 신경뿌리병증 환자의 침도치료를 중심으로 한 한의치료 1례)

  • This case report aims to illustrate the diagnostic challenge and effective management of acute cervical radiculopathy presenting as shoulder pain, differentially diagnosed through physical examinations, and subsequently treated with acupotomy. A male patient in his late 20s with no significant medical history or comorbidities reported acute left shoulder pain upon awakening, rated at a severity of 7 on the numeric rating scale (NRS). Initial physical evaluations of the shoulder assembly revealed no abnormalities, but a positive Spurling test, relief with cervical distraction, and diminished sensation across the left C5~C8 dermatomes pointed towards a cervical spine origin. Immediate acupotomy at identified tender points significantly reduced pain, with additional therapies enhancing recovery. Pain was eliminated after the second treatment, and this resolution was sustained at the 4-week follow-up, confirming the treatment's effectiveness and verifying the absence of adverse effects. This case presents the critical need for precise diagnosis in cases of neck and shoulder pain. Additionally, the employment of acupotomy provided a swift and effective resolution of symptoms in acute cervical radiculopathy, showing its value in clinical practice.
