Central Localization of Facial Nerve and L14 related to the Large Intestine Meridian (수양명대장경(手陽明大腸經)과 관련(關聯)된 경혈(經穴)과 안면신경(顔面神經)의 표식영역(標識領域)에 관한 연구(硏究))

  • The purpose of this morphological study was to investigate the relationship to facial nerve and LI4 related to the large intestine meridian. The common locations of the spinal cord and brain projecting to the LI4 and facial nerve were observed fallowing injection of transsynaptic neurotropic virus, pseudorabis virus(PRV), into the LI4 and facial nerve of the rat. After survival times of 96 hours following injection of PRV, the rats were perfused, and their spinal cord and brain were frozen sectioned(30${\mu}m$). These sections were stained by PRV immunohistochemical staining method, and observed with light microscope The results were as follows: 1. The PRV labeled spinal cord segments projecting to the LI4 and facial nerve were founded in cervical, thoracic, lumbar and sacral segments. Dense labeled areas of each spinal cord segment were founded in lamina IV, V, X, lateral spinal nucleus, intermediolateral nucleus and dorsal nucleus. 2. The PRV labeled medulla oblongata projecting to the LI4 and facial nerve were founded in the A1 noradrenalin cells/C1 adrenalin cells/caudoventrolateral reticular nucleus, rostroventrolateral reticular nucleus, medullary reticular nucleus, nucleus tractus solitarius, raphe obscurus nucleus, raphe pallidus nucleus, raphe magnus nucleus, gigantocellular nucleus, lateral paragigantocellular nucleus, and spinal trigeminal nucleus.

The Clinical Study on Preference of TongBiEum and GunYoEum (통비음(通痺飮)과 건요음(健腰飮)의 선호도에 대한 임상연구)

  • Objective: In oriental medicine, development of new formulations has been demanded. But, there are few reports about the improvement of formulations of herbal medicine in Korea. Nowadays soft Ex has been exploited and used comparing with retort pouch. This study was designed to study on preference on TongBiEum(通痺飮)/GunYoEum(健腰飮) and to investigate which OMD(oriental medical doctor) and non-OMD prefer soft Ex or retort pouch. Methods: As pilot study we administerd TongBiEum(通痺飮) once to 30 persons who serviced Gumi Oriental Medical hospital, Daegu Haany University from April 1, 2003 to April 10, 2003. And then we administerd GunYoEum(健腰飮) once to final test group(73 persons) who serviced Daegu, Gumi, Pohang Oriental Medical hospital, Daegu Haany University from April 20, 2003 to May 10, 2003. We made up questionaires about feeling during and after taking medicine, convenience and preference ect. Results: Convenience and preference of soft Ex is significantly different between OMD and non-OMD and in age bracket, but there is no significance between men and women group. Portable state of soft Ex is very convenient, dose of soft Ex is a little amount, color of soft Ex is regular in OMD & non-OMD, men & Women and an age bracket. Conclusions: From above study it was revealed that preference of herbal medicine formulations was significantly different between OMD and non-OMD. So development of new herbal medicine formulations is needed for consumers such as non-OMD.

Protocol for Systematic Review of Controlled Trials of Korean and Chinese Herbal Treatments for Stroke

  • Background: Many stroke patients receive traditional medical care in Korea and China. Stroke patients comprise the largest proportion of inpatients in traditional Korean medicine and traditional Chinese medicine hospitals. The purpose of this study is to identify the types and effects of Chinese medicine widely used in China and Korea, and to apply them to future studies of stroke. Methods: Nine Korean and Chinese databases will be surveyed for clinical studies of herbal medicines for stroke, published between the inception of the database and August 2017. Clinical studies of decoctions or modified decoctions will be included without restriction on study type. The frequencies and patterns of formulas or single herb usage and the any type of herbal medicine utilization will be analyzed. Dissemination: The results of the systematic review will be published in a peer-reviewed journal and disseminated electronically and in print. Updates of the review will be conducted to inform and guide healthcare practices.

Therapeutic Effects of Electroacupuncture on Cholecystokinin-octapeptide-induced Acute Pancreatitis Models (급성 췌장염모델에서 전침의 치료효과)

  • Objectives : We examined the effects of electroacupuncture on the cholecystokinin-octapeptide-induced acute pancreatitis in rats. Methods : Rats were administered with $75{\mu}g/kg$ cholecystokinin-octapeptide subcutaneously three times (1, 3 and 5h after shaving) for 5days. Three days after finishing cholecystokinin-octapeptide administration, high frequency electroacupuncture (100Hz) and low frequency electroacupuncture (2Hz) were applied to acupoint equivalent to ST36 (Zusanli) for 7 days. The author determined the pancreatic weight/body weight ratio, the levels of pancreatic heat shock protein HSP60 and HSP72. The author also assay the secretion of ${\beta}-amylase$, lipase and pro-inflammatory cytokines in serum. Repeated cholecysokinin-octapeptide treatment resulted in the typical laboratory and morphological changes of experimentally induced pancreatitis. Results : Eelectroacupuncture significantly decreased the pancreatic weight/body weight ratio in cholecystokinin-octapeptide-induced acute pancreatitis, increased the pancreatic levels of HSP60 and HSP72, and decreased ${\beta}-amylase$ and lipase levels in cholecystokinin-octapeptide-induced acute pancreatitis. Additionally, the secretion of $Interleukin-1{\beta}$ and tumor necrosis $factor-{\alpha}$ was decreased in the animals treated with electroacupuncture. Conclusion : These results suggest that electroacupuncture may have protective effects against cholecystokinin-octapeptide-induced acute pancreatitis.

Consumptive Disease and Chronic Fatigue Improved by Nokyonggunbi-tang and Other Korean Medical Treatments: A Case Report (녹용건비탕을 비롯한 한방치료로 호전된 허로 환자 1례)

  • Objective: This study investigated the efficacy of using Nokyonggunbi-tang and other Korean medical treatments for a patient with consumptive disease and chronic fatigue. Methods: A 59-year-old female patient with fatigue, headache, dizziness, and neck pain due to consumptive disease and chronic fatigue was treated with herbal medicine (Nokyonggunbi-tang), together with acupuncture, pharmacopuncture, cupping, and chuna manual therapy, for 22 days. The treatment effects on the pain were evaluated using the scores for the Fatigue Severity Scale (FSS), European Quality of Life Five Dimensions (EQ-5d) Scale, Neck Disability Index (NDI), and Numerical Rating Scale (NRS). Results: Following treatment, the patient showed a decrease in the FSS, NDI, and NRS scores and an improvement in the EQ-5d score. Conclusions: Nokyonggunbi-tang and Korean medical treatment significantly improved fatigue symptoms in a patient with no substrate disease.

Acupuncture Attenuates Cocaine-induced Dopamine Release in the Nucleus Accumbens and Voluntary Cocaine Intake in Rats (침자극이 코카인 투여로 인한 측핵내 도파민 유리와 자발적 코카인 섭취량에 미치는 효과)

  • 목적 : 침(鍼)은 정신이상 및 약물중독과 같은 많은 기능상의 장애질환을 치료, 조정하는데 널리 쓰이고 있으며, 중추신경계에서 생화학적인 균형을 유지하는데 기여한다고 밝혀져 있다. 동물 연구에서 코카인의 강화 및 민감화 특성과 중추의 도파민 활성과의 관련 가능성이 높아지고 있다. 본 연구에서는 약물중독 치료에 이용할 수 있는 침의 효과를 규명하기 위하여 침자극 후 급성 코카인 투여에 의한 도파민 유리 및 자발적 코카인 섭취량에 미치는 침자극의 효과를 측정하였다. 방법 : 웅성 SD 쥐에 코카인 (1mg/kg, i.v.)의 주사 직전 양측의 신문혈(HT7)을 1분간 침자극 한 후, 측핵내 세포외액의 도파민 및 대사산물의 함량변화를 미세투석법을 이용하여 HPLC로 분석하였다. 침자극 후 two-bottle, free choice protocol을 이용하여 자발적 코카인 섭취량의 변화를 관찰하였다. 별도의 쥐를 이용하여 1회 동량의 코카인 정맥주사하고 2주간의 철회를 거친 다음 two-bottle, free choice protocol을 실시하여 자발적 코카인 섭취량과 급성 코카인 투여 후 측핵 중 도파민유리증가율 사이의 상관관계를 조사하였다. 결과 : 신문혈(HT7) 자침이 코카인 1회 주사 후에 유도된 도파민 증가를 대조혈(내관혈(PC6), 또는 꼬리)에 비해 유의하게 억제하였으며 two-bottle choice protocol에 의한 동물의 자발적 코카인 섭취량을 억제하였다. 아울러 자발적 코카인 섭취량과 급성 코카인 투여 후 측핵 중 도파민유리증가율 사이에는 상관관계가 있었다. 결론 : 본 실험결과 침자극에 의해 자발적 코카인 섭취량이 억제되는 것은 측핵에서의 도파민 유리억제에 의해 중개되는 것으로 보여지므로 침자극은 코카인중독과 같은 약물의 중독치료에 이용될 수 있을 것으로 사료된다.

A Study on Anti-oxidative Effect of Electroacupuncture at Kokchi($LI_{11}$) in Rats (곡지(곡지)에 시술(施術)한 저주파 전침자극(電鍼刺戟)의 항산화(抗酸化) 효과(效果)에 대(對)한 실험적(實驗的) 연구(硏究)량)

  • Objectives: The purpose of this study is to investigate the anti-oxidative effect of electroacupuncture at Kokchi($LI_{11}$) in rats. Methods: A study administer AAPH to the abdominal cavity of rats and stimulate Kokchi($LI_{11}$) of the rat that lead to oxidative stress by electropuncture. And the study survey serum albumin, total bilirubin, LDL-cholesterol, LDH, Glucose, GOT, GPT and measure SOD activity, GSH concentration, catalase activity, NO concentration, MDA concentration. Results: 1. At the analysis of blood chemistry Albumin and Glucose significantly increase at $LI_{11}$-NR group, $LI_{11}$-EA group than at control group and holder group. LDL cholesterol, GOT, GPT decrease meaningfully. 2. As results of measurement at liver, SOD, Catalase represnet significantly increase at $LI_{11}$-NR group and $LI_{11}$-EA group than at control group and holder group. 3. Glutathione has some increases at the $LI_{11}$-NR group and $LI_{11}$-EA group than at the control group and the holder group. 4. As a result of measurement of NO and MDA's content, the content of NO decrease at the $LI_{11}$-NR group than at the control group. That reduces more meaningfully at the $LI_{11}$-EA group than at the control group. MDA has a significant decrease at the $LI_{11}$-NR group and the $LI_{11}$-EA group. 5. At the histological analysis, the study confirm that the density of intracellular cytoplasm in the liver tissue decreases at the control group as compared with the Normal group and the size of cell increases. Conclusions : These results suppose that electroacupuncture at $LI_{11}$ has an anti-oxidant effect in human.

The Study of $NF-{\kappa}B(P50)$ Suppression mechanism with main Component of Bee Venom and Melittin on Human Synoviocyte

  • Melittin,cationic 26-amino acid, is the principal component of the bee venom (BV) which has been used for treatment of inflammatory disease such as arthritis rheumatism NF-kB is activated by subsequent release of inhibitory IkB via activation of a multisubunit IkB kinase (IKK). We previously found that melittin bind to the sulfhydryl group of p50, a subunit of NF-kB. Since sulfhydryl group is present in kinase domain of IKKa and IKKb, melittin could modify IKK activity by protein-protein interaction. We therefore examined effect of melittin on IKK activities in sodium nitroprusside (SNP)-stimulated synoviocyte obtained from RA patients. Melittin suppressed the SNP-induced release of IkB resulted in inhibition of DNA binding activity of NF-kB and NF-kB-dependent luciferase activity. Consistent with the inhibitory effect on NF-kB activation, IKKa and IKKb activities were also suppressed by melittin. Surface plasmon resonance analysis realized that melitin binds to IKKa $(Kd\;=\;1.34{\times}10-9M)$ and IKKb$(Kd\;=\;1.0{\times}10-9M)$. Inhibition of IKKa and IKKb resulted in reduction of the SNP-induced production of inflammatory mediators NO and PGE2 generation. The inhibitory effect of melittin on the IKKs activities, binding affinity of melittin to IKKs, and NO and PGE2 generation were blocked by addition of reducing agents dithiothreitol and glutathione. In addition, melittin did not show inhibitory effect in the transfected Synoviocytes with plasmid carrying dominant negative mutant IKKa (C178A) and IKKb (C179A). These results demonstrate that melittin directly binds to sulfhydryl group of IKKs resulting in IkBrelease, thereby inhibits activation of NF-kB and expression of genes involving in the inflammatory responses.

Case Report of Chemotherapy-Induced Peripheral Neuropathy Treated with Korean Medicine (항암화학요법 유발 말초신경병증에 대한 한방치험 1례)

  • Introduction: The aim of this study is to report on the effectiveness of Korean medicine to improve symptoms of chemotherapy-induced peripheral neuropathy (CIPN). Some patients are treated with medication, but medications may have no effect in some patients. Korean medicine treatments have been used in such patients, but few reports exist. Case Presentation: A 58-year-old female patient with CIPN reported symptoms of bilateral limb pain and tingling sensations as well as cold sensations in bilateral lower legs and hands, fatigue, and bilateral limb weakness. To reduce the patient's symptoms, we provided Korean medical treatment, including herbal medicine (Uchashinki-hwan), acupuncture, and moxibation. To evaluate the results of this treatment, we used a numeric rating scale (NRS), the manual muscle test (MMT), the National Cancer Institute Common Toxicity Criteria for Adverse Events (CTCAE), and the European Organization for Research and Treatment of Cancer quality-of-life questionnaire about CIPN (EORTC-QLQ-CIPN20). After 49 days of treatment, the NRS scores decreased for bilateral limb pain and tingling, for bilateral lower leg and hand cold sensation, and for fatigue. The MMT results for the upper and lower extremities were improved. CTCAE Grade and EORTC QLQ-CIPN20 score were also decreased. Conclusion: According to these results, Korean medicine treatment may be considered an effective treatment for CIPN. Prospective studies are needed in the future to confirm and expand these findings.

A Case Report of Combining Korean Medicine Treatment with Chuna Manual Therapy for Cervical and Lumbar Pain and Hypoesthesia of the Limbs After Total Spondylectomy for Giant Cell Tumor of the Cervical Spine (경추거대세포종에 대한 전척추제거술 후 발생한 경요추부 통증 및 사지부 감각저하에 대한 추나요법을 포함한 한방치료 증례보고 1례)

  • Objectives This study aimed to report the effects of giant cell tumor treatment to a patient who had cervical and lumbar pain and hypoesthesia of the limbs after total spondylectomy with Korean medicine. Methods A 67-year-old female patient with cervical and lumbar pain and hypoesthesia of the limbs after total spondylectomy was treated with herbal medicine (Cheongpa-jeon, Yukkongbaro-hwa), acupuncture, pharmacopuncture, and physical therapy for 36 days. The effects were evaluated using a numerical rating scale (NRS), Neck Disability Index (NDI), Oswestry Disability Index (ODI), and EuroQoL-5D (EQ-5D). Results After treatment, changes were observed in the patient's pain and hypoesthesia. NRS, NDI, ODI, and EQ-5D scores decreased. Conclusions This case study suggests that Korean medicine treatment may be effective for symptomatic treatment in patients with giant cell tumor following total spondylectomy.
