The risks of homologous blood transfusion are well documented and recently increased with the emergence of acquired immunodeficiency syndrome. This is a report concerning 29 consecutive adult with no blood conservation patients (Group I) who had an elective cardiac operations. A similar group of 29 patients operated on 1992 but with blood conservation (Group II). In I of Group I patients and 15 of Group II patients, no homologous blood products were required. Group II patients used significantly less fresh frozen plasma (2.05${\pm}$0.68 unit versus 6.52${\pm}$0.72 unit, p<0.05) and the homologous blood transfusion (0.42${\pm}$0.9 unit versus 3.64${\pm}$0.17 unit, p<0.05) than Group I patients. Group II patients had also significantly less postoperative bleeding (338${\pm}$39.9 ml versus 585${\pm}$93.0 ml, p<0.05) than Group I patients. Group II patients recieved 460${\pm}$62.6 ml of mediastinal shed blood in acquired group. In conclusion, a simple and inexpensive blood conservation program, mainly combining autologous blood removal before bypass, retransfusion of the volume remaining in the oxygenator, and consistent autotransfusion of mediastinal shed blood has enabled us to avoid infusion of homologous blood in 15/29 patients of Group II patients. No side effects or complicatinos could be related to the blood conservation program.
목적: '연명의료결정법'의 시행으로 호스피스 완화의료의 대상이 암뿐 아니라 후천성면역결핍증(acquired immunodeficiency syndrome, AIDS)을 포함한 비암성 질환에까지 확대된다. 하지만, AIDS에 대해서는 부정적인 인식과 낙인, 편견이 많아 호스피스 완화의료 확대에 대한 우려가 많다. 이에 본 연구에서는 말기 AIDS 환자들에 대한 자원봉사 제공의향 정도에 영향을 주는 요인들을 알아보고자 하였다. 방법: 우리나라의 19개 기관의 326명의 호스피스 자원봉사자들을 대상으로 자가 기입식 설문지를 이용해 조사하였다. 말기 AIDS 환자들에 대한 자원봉사 제공의향 정도는 11단계 숫자등급을 통해 파악하였다. 인구사회학적 변수들, 자원봉사와 관련된 변수들, 자원봉사에 대한 만족도, AIDS에 대한 지식 수준, AIDS에 대한 태도('AIDS 환자에 대한 두려움', 'AIDS 환자에 대한 부정적인 태도', '개인적인 낙인', '낙인적 태도')를 조사하였다. 다중회귀분석을 이용하여 말기 AIDS 환자들에 대한 자원봉사 제공의향 정도에 영향을 주는 요인들을 알아보았다. 결과: 말기 AIDS 환자들에 대한 자원봉사 제공의향 정도 점수는 암환자에 대해서보다 평균 2.82점 낮았고 이는 통계적으로 유의하였다(P<0.001). 다중회귀분석 결과 자원봉사에 대한 만족도가 높을수록(P=0.002) '개인적인 낙인' 점수가 낮을수록(P<0.001) 말기 AIDS 환자들에 대한 자원봉사 제공의향 정도는 높아졌다. 결론: 본 연구의 결과 호스피스 자원봉사자들의 말기 AIDS 환자들에 대한 자원봉사 제공의향 정도와 관련이 있는 요인은 자원봉사에 대한 만족도와 AIDS에 대한 태도 중 '개인적인 낙인'이었다.

Objectives: Acquired immunodeficiency syndrome (AIDS) is a disease of the human immune system caused by the human immunodeficiency virus (HIV). People with AIDS are much more vulnerable to infections, including opportunistic infections and tumors, than people with a healthy immune system. The objective of this study was to correlate oral lesions associated with HIV/AIDS and immunosuppression levels by measuring clusters of differentiation 4 (CD4) cell counts among patients living in the middle western regions of Ghana. Materials and Methods: A total of 120 patients who visited the HIV clinic at the Komfo Anokye Teaching Hospital and the Regional Hospital Sunyani of Ghana were consecutively enrolled in this prospective and cross-sectional study. Referred patients' baseline CD4 counts were obtained from medical records and each patient received an initial physician assessment. Intraoral diagnoses were based on the classification and diagnostic criteria of the EEC Clearinghouse, 1993. After the initial assessment, extra- and intraoral tissues from each enrolled patient were examined. Data analyses were carried out using simple proportions, frequencies and chi-square tests of significance. Results: Our study included 120 patients, and was comprised of 42 (35.0%) males and 78 (65.0%) females, ranging in age from 21 to 67 years with sex-specific mean ages of 39.31 years (males) and 39.28 years (females). Patient CD4 count values ranged from 3 to 985 cells/mL with a mean baseline CD4 count of 291.29 cells/mL for males and 325.92 cells/mL for females. The mean baseline CD4 count for the entire sample was 313.80 cells/mL. Of the 120 patients we examined, 99 (82.5%) were observed to have at least one HIV-associated intraoral lesion while 21 (17.5%) had no intraoral lesions. Oral candidiasis, periodontitis, melanotic hyperpigmentation, gingivitis and xerostomia were the most common oral lesions. Conclusion: From a total of nine oral lesions, six lesions that included oral candidiasis, periodontitis, melanotic hyperpigmentation, gingivitis, xerostomia and oral hairy leukoplakia were significantly correlated with declining CD4 counts.
Nurses are at the forefront of providing hands-on care to patients infected with HIV(Human Immunodeficiency Virus). Therefore, appropriate and sensible infection control precaution should be taken at all times. The purpose of this study is to find out nurse's loaming needs about HIV/AIDS ; and nurse's willingness to perform nursing care to HIV/AIDS patients ; and the degree of fellowing universal precautions in nursing care ; and the degree of knowledge of HIV/AIDS to provide information on the guide for the further continuing education. The respondents of this study were 280 nurses in a university hospital in Kwang-Ju city. Data were collected using a self-administered questionnaire and analyzed by using percentages, means, Pearson's correlation, t-test, ANOVA, Duncan's grouping. The findings were as follows ; 1. The mean age of the nurses was 28.9 years, and the mean duration of their clinical experience was 6.8 years. Majority of the nurses(83.9%) had no experience in taking care of the AIDS patients. 2. The content s nurse's need to learn were showed in order such as clinical manifestations, infection control, patient care, and the mode of transmission of HIV/AIDS. 3. Even though the nurses had quite low knowledge about HIV/AIDS(mean score was 12.2 from the total of 26), they are willing to take care of the AIDS patients (mean score was 39.5 from the total of 60), and the degree of following universal precaution was also not so bad (mean score was 40.8 from the total of 50). 4. Nurses who are over 35 years old and had informal education regarding HIV/AIDS better following universal precautions. Nurses who had over 12 years of clinical experiences have more willingness to perform nursing care to the AIDS patients. Nurses who had experiences in taking care of the AIDS patients were more knowledgeable about HIV/AIDS. These findings suggest that some strategies should be develope to increase the willingness to perform the nursing care to the AIDS patients. And also continuing education program should be develope and run for the clinical nurses so that they can provide effective and appropriate nursing care to the AIDS patients.
Intestinal parasitic infections are one of the major causes of diarrhea in human immunodeficiency virus (HIV) seropositive individuals. Antiretroviral therapy has markedly reduced the incidence of many opportunistic infections, but parasite-related diarrhea still remains frequent and often underestimated especially in developing countries. The present hospital-based study was conducted to determine the spectrum of intestinal parasitosis in adult HIV/AIDS (acquired immunodeficiency syndrome) patients with or without diarrhea with the levels of $CD4^+$ T-cell counts. A total of 400 individuals were enrolled and were screened for intestinal parasitosis. Of these study population, 200 were HIV seropositives, and the remaining 200 were HIV uninfected individuals with or without diarrhea. Intestinal parasites were identified by using microscopy as well as PCR assay. A total of 130 (32.5%) out of 400 patients were positive for any kinds of intestinal parasites. The cumulative number of parasite positive patients was 152 due to multiple infections. A significant association of Cryptosporidium (P<0.001) was detected among individuals with $CD4^+$ T-cell counts less than $200cells/{\mu}l$.
The purpose of this study was to provide the basic data for developing a program for effective health education about AIDS (Acquired Immunodeficiency Syndrome) by investigating the knowledge and attitudes of middle and high school students about AIDS. The subjects were 476 middle school students and 658 high school students from Seoul and Kang Won province. The data were collected from November 19 to December 18. 1996. using a 56-item questionnaire. and analyzed by SAS program for t-test. x2-test. and pearson correlation coefficients. The results were as follows : 1. High school students had more knowledge than middle school students about AIDS. 2. Male. students from Seoul. and students who had drinking experience and smoking experience among middle school students. and male. students from Seoul. and students of non-coeducation among high school students were more knowledgeable about AIDS. 3. Misconceptions about the transmission of AIDS through non-intimate contact were especially common among middle school students. And a high proportion of middle and high school students knew very little about the symptoms of AIDS. 4. High school students had more positive attitudes toward AIDS than middle school students. 5. Students from Seoul and students who had smoking experience among middle school students. and students from Seoul and non-coeducation and students who had substance use experience among high school students were more positive attitudes about AIDS. 6. Most of the middle and high school students agreed that there is a need for AIDS education. 7. Middle and high school students reported that had learned about AIDS mostly from TV. Since students in the middle-school age group are especially at risk for developing AIDSrelated behaviors. this study findings suggest that it is crucial to develop school-based AIDS education programs that help students acquire the knowledge and attitudes to adopt and maintain behaviors that reduce the risk of HIV (Human Immunodeficiency Virus) infection and other related health problems.
Objectives: This study investigated factors associated with the retention of people living with human immunodeficiency virus (HIV) on antiretroviral therapy (ART) during the first 3 years of treatment. Methods: A retrospective study using electronic health records was conducted at a tertiary hospital in Jakarta, Indonesia. Adult HIV-positive patients who started ART from 2010 until 2020 were included. A binary logistic regression model was used to identify factors associated with ART retention in the first 3 years. Results: In total, 535 respondents were included in the analysis. The ART retention rates for the first, second, and third years were 83.7%, 79.1%, and 77.2%, respectively. The multivariate analysis revealed a negative association between CD4 count when starting ART and retention. Patients with CD4 counts >200 cells/mL were 0.65 times less likely to have good retention than those with CD4 counts ≤200 cells/mL. The year of starting ART was also significantly associated with retention. Patients who started ART in 2010-2013 or 2014-2016 were less likely to have good retention than those who started ART in 2017-2020, with adjusted odds ratios of 0.52 and 0.40, respectively. Patients who received efavirenz-based therapy were 1.69 times more likely to have good retention than those who received nevirapine (95% confidence interval, 1.05 to 2.72). Conclusions: Our study revealed a decline in ART retention in the third year. The CD4 count, year of enrollment, and an efavirenz-based regimen were significantly associated with retention. Patient engagement has long been a priority in HIV programs, with interventions being implemented to address this issue.
Purpose: to evaluate the effects of AIDS education for baccalaureate nursing students. Method: a one-time AIDS education was delivered to 175 nursing students and knowledge and attitude toward HIV/AIDS were measured before and after the AIDS education using a questionnaire. Result: 1) Before the AIDS education, the average knowledge score of the students was 64.30 points out of 103 points while the average attitude score was 25.77 points out of 36 points. 2) Before the AIDS education, school grade, former experience of AIDS education and religion were founded to be the significantly related to the student's knowledge on AIDS. 3) There was a significant increase in AIDS related knowledge (t=-24.21, p=.000). There was also a significant improvement in attitude toward HIV/AIDS (t=4.67, p=.000) after the AIDS education. 4) There was a significant correlation between the knowledge and the attitude toward HIV/AIDS before the AIDS education, while no correlations was found between the AIDS knowledge and attitude after the education. Conclusion: AIDS education is necessary and effective for baccalaureate nursing students. It is necessary to develop comprehensive AIDS education program to improve the level of knowledge and preventive behavior for HIV/AIDS as well as to allay the fears for AIDS.
The Korea now incarcerates a greater percentage of its Population than any other country : For 63,000 prisoners in 40 prisons and jails. Most inmates are mate. young, poor, and morbidity groups. Most are substance abusers with substantial physical and mental health needs. Corrections in general and correctional health care in particular have suffered negative consequence : severe overcrowding, insuffcient programs as the acquired immunodeficiency syndrome(AIDS). tuberculosis, and hepatitis. The large increase in the number of substance abusers and sick and terminally ill inmates has rendered our nation's prisons and jails physically or financially unable to deal with their current populations, much less the explosive increases the future holds. It is the magnitude of inmate health problems that threatens to overwhelm the substantial gains made in correctional health care over the past two decades. As measured by recognized standards of inmate health and health services, our correctional systems are in crisis. As a nation. we must respond to the problem of health problem with national strategies that do not overwhelm the capacity of our criminal justice system to care for its inmates.
The purpose of this descriptive study was to investigate the general knowledge and attitude of physical therapist to patient with HIV/AIDS. Many physical therapists are afraid of AIDS patients because they have a little information of AIDS and fear of HIV infection from AIDS patients at work site. This study was carried out from June to October in 2003 for collecting data. The 271 physical therapists volunteered for a this study and completed questionnaires. The findings of this study can be summarized as follows. 1)The knowledge of prevention of HIV/AIDS influences the attitude of physical therapists to patients with HIV/AIDS. 2)The knowledge of symptoms of HIV/AIDS influences the attitude of physical therapists to patients with HIV/AIDS. 3)The knowledge of transmission routes of HIV/AIDS influences the attitude of physical therapists to patients with HIV/AIDS. 4)84.9 percentage of participant in survey suggested college should have curriculum about HIV/AIDS. In conclusion, physical therapists with more knowledge of HIV/ADIS could positively treat HIV/AIDS patients.
