Control Strategy for Industrial Safety Based on Dynamic Modeling of Safety Budget-Industrial Accidents Relationship (안전예산-산재간 동적모델을 이용한 산업안전제어 전략)

  • This study focuses on the control strategy for industrial safety in Korea. Specifically, the effect of safety budget such as the industrial accident prevention fund on the safety performance to prevent and reduce industrial accidents is statistically examined first and modeled as a second order system. The effectiveness of such a dynamic model is also explained with a simple PI control mechanism in a feedback loop. The simulated model, however, suggests that, without improving the efficiency of the safety system, extra safety budget needed to decrease the accident rate to a level in advanced countries is far beyond the social consensus. An efficient way of reducing industrial accidents based on such a dynamic model with more internal damping but with less elastic nature in a feedback loop framework needs to be implemented.

A basic research on risk control measure for reducing the fishermen's occupational accident in offshore and coastal fishing vessel (연근해 어선원 재해 현황과 저감 대책 마련을 위한 기초 연구)

  • The aim of this study is to research attributes of fishermen's occupational accidents for investigating the measure of risk control on situational condition in the Korean offshore and coastal fishing vessel. Using data of fishermen's occupational accidents are from National federation of fisheries cooperatives for 2013. The results were as belows; Occupational accident occurrence rate was 29.5‰, slips & trips and struck by object and contact with gear were shown severe occurrence pattern. Occupational accident occurrence rate of offshore fisheries was 130.2‰, coastal was 16.9‰, specially the risk rates were severely high in several type of danish seine, stow net and offshore trap. Death rate by accidents was 10.6‰ and by fall into the water in occurrence pattern was 5.5‰.

  • Compared with other transport means, safety and timeliness are the merits of railways. Unexpectedly when accident happens, much time and human strength are required to cope with the accident. And for swift recovery, systematic rehabilitation is needed. Recently using MTS(Moving picture Transmission System), we can perform accident rehabilitation and recording work efficiently. MTS is the device that transmits continuous picture information from accident field to control center. We are developing the appropriate system to railway situation to make use of the existing information communication technology, processing technology of video-tex, super high speed transmission technology through fiber-optic, copper cable and network description of information Technology, etc. If these communication-based can technologies are applied to railway system, railway managers can control the accident by inspecting the picture of accident field and can contribute to the safe train operation and the improvement of railway management. In this paper, we investigate the connecting methods when optical fiber is used for moving picture data transmission of train accidents, and its problems. And, we validate MTS's performance through about 28km section of field test.

  • Accident analysis models were developed to improve the construction site safety and case studies was conducted. In 2016, 86% of fatality accidents occurred due to simple unsafe acts. Structure related accidents are less frequent than the non structure related causes, but the number of casualties per accident is two times higher than non structure one. In the view of risk perception, efforts should be given to reduce accidents caused by low frequency - high consequence structure related causes. In case of structure related accident, structural safety inspection and management (including quality), ground condition management / inspection technology, and provision of risk information delivery system in case of non structure related accident were proposed as a solution. In analysis of relationship between safety related stakeholder, the main problem were the lack of knowledge of controller and player, loss of control due to duplicated controls, lack of communication system of risk information, and relative position error of controller and player.

A Study on the Re-establishment of Selection Criterion on the Frequency of Foggy Area in Highway (고속도로 안개 잦은 구간 선정 기준 재정립에 관한 연구)

  • There is a high potentiality of large traffic accident due to the dense fog when road is developed along the coast or river. The establishment of national level control system against the fog is necessary because the accident due to the creation of fog has a high fatality ratio than other weather conditions. The selection method for the frequent foggy area on highway was suggested to control the fog on the highway effectively because the establishment of the countermeasure against the fog in every range in highway is difficult practically. 44 ranges where the fog control is necessary throughout the year and the 45 ranges where the control is necessary in specific months were selected from the result of application of the weighted value on each visible distance data except the fog with beyond 250 m visible distance which does not affect on the safe driving out of the surveyedjsh fog visible distances. The preferential fog control countermeasure shall be provided to prevent the traffic accident and to reduce the severeness of the accident in case of fog creation for 89 ranges which were selected for frequent foggy area in highway.

A Case Study on the Human Error Analysis for the Prevention of Converter Furnace Accidents (전로사고 예방을 위한 인적오류 분석)

  • Occupational fatal injury rate per 10,000 population of Korea is still higher among the OECD member countries. To prevent fatal injuries, the causes of accidents including human error should be analyzed and then appropriate countermeasures should be established. There was an severe converter furnace accident resulting in five people death by chocking in 2013. Although the accident type of the furnace accident was suffocation, many safety problems were included before reaching the death of suffocation. If the safety problems are reviewed throughly, the alternative measures based on the review would be very useful in preventing similar accidents. In this study, we investigated the converter furnace accident by using human error analysis and accident scenario analysis. As a result, it was found that the accident was caused by some human errors, inappropriate task sequence and lack of control in coordinating work by several subordinating companies. From the review of this case, the followings are suggested: First, systematic human error analysis should be included in the investigation of fatal injury accidents. Second, multi man-machine accident scenario analyis is useful in most of coordinating work. Third, the more provision of information on system state will lessen human errors. Fourth, the coordinating control in safety should be performed in the work conducting by several different companies.

Improvement Strategy for Management of Accident Preparedness Substances Causing Chemical Accidents (화학사고 유발 사고대비물질 관리 개선 방안)

  • Chemical accidents occur by a variety of causes and aspects. Accident preparedness substances (APS) are defined by toxic chemicals highly likely to cause accidents due to their high acute toxicity, explosiveness or likely to cause severe damage where a chemical accident occurs among the chemicals. APS is designated and announced by Presidential Decree. However, chemical accidents occurred for recent 7 years were arisen mainly from non-accident preparedness substances, and only 24 species were included as chemical accident trigger among total 69 APS. In addition, APS were designated in 2014 and the list has not been updated since then although it needs to be amended in several aspects. Accordingly, this study analyzes the necessity of modification of APS management, and presents improved plans.

  • On 7 December 2007, the Hebei Spirit, a 260,000 dwt VLCC, anchored near Korea's Daesan Port, was collided with a passing crane-carrying barge Samsung1, which was under tow of two tugs Samsung T5 and Samho T3. In this study, the behaviour of Hebei Spirit at the time of the accident has been reproduced and analyzed by simulation. This study precedes the study for the investigation of any available countermeasures for Hebei Spirit to prevent the accident. The simulation has been done only for Hebei Spiri and the motion of Samsung barge is just given with recorded AIS data. Dynamic characteristics of Hebei Spirit have been modeled based on empirical data and her sea trial data. Effects of current, wave and wind are also estimateed using empirical formula. Considering uncertainty of environmental condition and control of Hebei Spirit, simulation has been done by varying engine control method and holding power coefficients of the anchor. Finally, based on simulations, the most plausible scenario on the state of anchor and engine control could cause real accidents.

Impingement wastage experiment with SUS 316 in a printed circuit steam generator

  • The sodium cooled fast reactor (SFR) is one of the Gen-IV reactors with the most operating experience accumulated. Although the technology level is the most mature among the Gen-IV reactors, there is still a safety problem that has not been solved, which is the sodium-water reaction. Since sodium and water are separated only by a heat transfer tube with a thickness of only a few mm, there is inherently a risk of a sodium-water reaction (SWR) accident in the SFR. In this study, it is attempted to quantitatively evaluate the resistance of SWR accidents by replacing the shell and tube steam generator with printed circuit steam generator (PCSG) as a method to mitigate the SWR accident. To do this, a CATS-S (Compact Accident Tolerance Steam Generator-SWR) facility was designed and built. And for the quantitative evaluation of accident resistance, a methodology for measuring the impingement wastage rate was established. As a result of this research, the impingement wastage rate caused by SWR generated in a PCSG was measured first time. It was confirmed that the impingement wastage phenomenon was suppressed in the PCSG, and the accident resistance was higher than that of the SWR through comparison with the experimental results performed in the existing shell and tube steam generator. In conclusion, a PCSG is more resistant to impingement wastage as a result of the SWR accident than existing shell and tube steam generators, and it is estimated that a PCSG can mitigate SWR accidents, an inherent problem of SFR.

Effect of treatment with S.O.T block on musculoskeletal pain caused by Traffic Accident (교통사고 환자를 대상으로 한 S.O.T block의 치료 효과)

  • Objectives : The purpose of this study was to investigate the effect of treatment with Sacro-Occipital Technique block on musculoskeletal pain caused by traffic accident by analysis of the Visual Analogue Scale(VAS), Neck Disability Index(NDI), Pain Disability Index(PDI), Oswestry Low back Pain Disability Index(ODI) and Short Form - McGill Pain Qusetionnaire (SF-MPQ). Methods : This study carried out on 18 patients who have received hospital treatment in Daejeon Univ. Dun-San Oriental Hospital. Control group got acupunture-therapy, herbal medication, physical therapy and Experimental group got all the therapies and treatment with Sacro-Occipital Technique block. We measured VAS, NDI, PDI, ODI and SF-MPQ on 1st day and 7 days later. Results : After being treated by our methods, Both group were improved in VAS, NDI, PDI, ODI, and SF-MPQ. Especially, Experimental group was significantly meaningful improved in VAS, PDI, and ODI. Control group was significantly meaningful improved in VAS and SF-MPQ. But, differences between control and experimental group were nonsignificant. Conclusions : The results suggest that treatment with Sacro-Occipital Technique block is not significantly meaningful but gives a positive impact on musculoskeletal pain caused by traffic accident. But further long term study in a large scale is needed.
