A Cross-sectional Study between Herbal Medicine Intake and Abnormal Liver Function Test Results (간기능검사이상의 위험요인으로써 한약복용영향에 관한 단면연구)

  • Objectives : The aim of this study was to evaluate effects of herbal medicine and other associated factors for abnormal liver function tests(especially total bilirubin, AST, ALT, and ${\gamma}$GT) levels in outpatients at an oriental medical clinic. Methods : A cross-sectional study based on clinical records was conducted on 504 patients at one Oriental medical clinic in Seoul, Korea. They were tested a liver function tests for screening and 497 patients ranging from the age of 4 to 74 were chosen for the study. Patients with basal liver disease or DM were excluded during the screening process. Patients were classified into case(abnormal) and control(normal) groups by normal liver function test references. Results and Conclusions : In this study, Significant correlations for abnormal total bilirubin levels was age, for abnormal AST was gender, for abnormal ALT were gender, smoking, marriage status, and job, for abnormal ${\gamma}$GT were age, gender, obesity, alcohol, smoking, marriage status, and job using the Chi-square test(p<0.05). There was no significant correlations for abnormal LFTs by using herbal medication history, and intake duration. After controlling other covariates, logistic regression analysis showed that the significant associated factors for abnormal total bilirubin levels were age(odds ratio[OR]=0.87, 95% confidence intervals [CI]=0.87-0.99). For abnormal AST levels was obesity(OR=2.45, CI=1.2-4.9), for abnormal ALT levels were age(OR=1.01, CI=0.99-1.03), male gender(OR=3.31, CI=1.62-6.76), and obesity(OR=3.71 CI=2.28-6.08). For abnormal ${\gamma}$GT levels were male gender(OR=4.83 CI=1.34-17.43), obesity(OR=3.29, CI=1.74-6.22), alcohol (OR=2.51, CI=1.19-5.28), and smoking(OR=2.29, CI=1.16-4.50). Herbal medication history was not as a risk factor for all abnormal liver function tests. Therefore, people who are male, obese, alcohol drinker and smoker will have higher liver function test levels. But intake of herbal medication or the duration of intake may not be the risk factor for abnormal liver function test results.

A Study on Abdomen Ultrasonography Classified by Particular Disease Practiced in Health Promotion Center of a University Hospital (한 대학병원 종합건강진단센터에서 시행한 복부 초음파검사의 유소견 연구)

  • This study is to get preliminary data for an effectiveness evaluation of abdominal examination and improvement of it. Abnormal cases of abdominal ultrasonography are classified by sex, frequency, diagnosis and age. 4,924 examinees were included at a university hospital of health promotion center from January to December in 1999. The results are as follows. 1. According to the distribution of sex, there are more male patients(55.0%) than females patients(48.0%). For men, 40's showed the highest percentage among examinees. For women, 50's were the highest. 2. The reason that they visited the health promotion center was that 'they wanted to check their health status'. This answers were reported the highest(59.3%). 3. Patients that had abnormal cases of abdominal ultrasonography were 48.3%. Liver, kidney, gallbladder showed the highest percentage of abnormal cases in order of organs. Additionally, abnormal cases were discovered in liver cases. 4. According to the frequency of abnormal cases among examinees, the slight fatty liver were the highest regardless of sex. Men had the slight fatty liver, kidney simple cyst, liver calcification and liver simple cyst in order of abnormal cases. Women showed the slight fatty liver kidney simple cyst, kidney calcification, liver simple cyst, and blood vessel tumor in order of abnormal cases. 5. For the abnormal cases of liver by sex and age, the 50's reported the highest number of abnormal cases in men(299 patients). In addition, 60's had the highest of disease rata 47.8%. For women, 50's reported the highest number of abnormal cases(361 patients). Over 70's patients had the highest of disease rata 52.6%. For kidney, men and women showed the highest number of abnormal cases -62 vs 44 respectively. Over 70's patients had the highest percentage of disease rata-23.2% vs 14.0% respectively. For gallbladder, the number of abnormal cases were the most in men's 60's (31 patients) and in women's in the same age group (32patients). Disease showed the highest percentage in men's 60's(7.6%) and in women's 70's (14.0%). 6. According to malignant tumor, 17patients were liver cancer, 2patients stomach ca and 1pt kidney cancer. 7. The relationship between the malignant tumor and the examination motive was that 'they wanted to check their health status(41.0%)' and 'regular checkup (24.0%)'.

  • Objectives: Obesity is regarded as the aggregation of needless risk factors, for instance, cardiovascular disease, joint disease, induce cancer. We studied on interrelation between abnormal liver function and obesity. Methods: We analyzed liver function, T.Cholesterol, Triglyceride before and after lose weight treatment. The collateral condition is over 6 weeks period on obesity treatment, no liver injury and no complicated another disease on personal past history and found out abnormal impression on biochemical liver function blood test. Results: The improvement rate of LFT, compare with before treatment is 10.6% in T.Bilirubin, 11.1% in ALP, 21% in AST, 38% in ALT, 37.3% in r-GTP, 9.2% in LDH and decreased 2.7% in T.Protein, increased 2.3% in Albumin. Hyperlipidemia is 19.4% in T.Cholesterol, 42.5% in Triglyceride. Conclusions: LFT and Hyperlipidemia of abnormal liver function patients, caused by obesity, is improved to normal limit in proportion to reduce patient's weight.

  • Objectives: This case study reports on improvements after treating patients who had abnormal liver function values in Soyangin gyolhyungjeung with Soyangin hyeongbangdojeok-san-gami.Methods: The study was carried out with three patients who showed abnormal liver function values in Soyangin gyolhyungjeung. Liver function tests (LFT) and numerical rating scales (NRS) were used to evaluate symptoms.Results: After we treated three patients with Soyangin hyeongbangdojeok-san-gami, their symptoms and LFT results had improved considerably.Conclusions: Soyangin hyeongbangdojeok-san-gami appears to have a useful effect on abnormal liver function values in Soyangin gyolhyungjeung. Further clinical studies are needed to verify these findings.

The Association of Cardiovascular Risk Factors with Nonalcoholic Fatty Liver Disease in Health Checkup Examinees (일부 종합검진 수검자 중 비알코올성 지방간과 심혈관질환 위험요인과의 관련성)

  • Objectives: The purpose of this study was to evaluate the relationship of nonalcoholic fatty liver and cardiovascular risk factors. Methods: This study was conducted to investigate the association of nonalcoholic fatty liver and cardiovascular risk factors for adult men (n=2976) and women (n=2442) who were over 19 years old, after excluding the HBsAg(+) or anti-HCV(+) patients and the men and women with increased alcohol intake (men: 40g/week, women: 20g/week). Results: Compared with the normal liver subjects, the nonalcoholic fatty liver subjects showed a significantly increased frequency of abnormal systolic blood pressure (${geq}120mmHg$), fasting blood sugar (${\geq}100mg/dL$), total cholesterol ($({\geq}200 mg/dL$), triglyceride ($({\geq}150mg/dL$), high density lipoprotein cholesterol (<40 mg/dL), low density lipoprotein cholesterol ($({\geq}130g\; m/dL$) and abdominal obesity in men, and all these measures were significantly increased in the women except for abnormal HDL cholesterol. After adjusting for the body mass index, age, smoking, exercise and a nonalcoholic liver, the odds ratios of an abnormal waist hip ratio were 1.35(95% Confidence Interval=1.05-4.72) in the mild fatty liver, 1.61 (1.19-2.18) in the moderate fatty liver, 2.77(1.57-4.92) in the severe fatty liver compared with a normal liver. The adjusted odds ratios for abnormal fasting blood sugar were 1.26(1.03-1.53) in the mild fatty liver, 1.62(1.27-2.06) in the moderate fatty Iiver and 1.77(1.12-2.78) in the severe fatty liver. The adjusted odds ratios for abnormal triglyceride were 1.38(1.11-1.72) in the mild fatty liver, 1.73(0.33-2.24) in the moderate fatty liver and 1.91 (1.17-3.10) in the severe fatty liver of men. Adjusted odds ratios for abnormal triglyceride were 1.50(1.04-2.15) in mild, 1.71(1.07-2.68) in moderate, 1.81(0.69-4.38) in severe fatty liver of women. Conclusions: The nonalcoholic fatty liver subjects had more cardiovascular risk factors compared with the normal liver subjects. Thus, prevention and treatment of the nonalcoholic fatty liver is necessary by lifestyle modifications such as restriction of alcohol intake, no smoking, exercise and adequate eating habits.

Influence of Prescribed Herbal and Western Medicine on Patients with Abnormal Liver Function Tests: A Retrospective Quasi-Experimental Study

  • Objectives: The aim of this study was to investigate the safety and the efficacy of Korean herbal, western and combination medicine use in patients with abnormal liver function tests. Methods: We investigated nerve disease patients with abnormal liver function tests who were treated with Korean herbal, western and combination medicine at Dong-Eui University Oriental Hospital from January 2011 to August 2011. We compared aspartic aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP) and total bilirubin (T-bil) levels before and after taking medicine and excluded patients who had liver-related disease when admitted. Results: AST and ALT were decreased significantly in patients who had taken herbal, western medicine. AST, ALT and ALP were decreased significantly in patients who had taken combination medicine. Compare to herbal medicine, AST, ALT and ALP were decreased significantly in patients who had taken western medicine, and ALT and ALP were decreased significantly in patients who had taken combination medicine. There were no significant differences between western and combination medicine. Conclusions: This study suggests that prescribed Korean herbal medicine, at least, does not injure liver function for patients', moreover, it was shown to be effective in patients with abnormal liver function tests.

Effects of the Obesity Therapy with Korean Herbal Medicine on Liver Function: Case Series (한방비만치료가 비만환자의 간기능에 미치는 영향 증례보고)

  • Many obesity patients were affected by depressed liver function including hepatosteatosis. The aim of this study was to investigate the clinical effect of the obesity therapy with Korean herbal medicine (KHM) on obese patient with abnormal liver function. This is a retrospective observational study performed on obesity patients with abnormal liver function. All subjects have taken serum liver function tests (LFTs) before treatment. Ten patients showed abnormal LFTs. They were treated with KHM at least 15 days. During this treatment, LFTs were evaluated afterwards. Body weight and body mass index of the patients were decreased during treatments. Five of 10 subjects recovered normal levels of LFTs. While other subjects remained at abnormal level but their levels were decreased. This study shows that the obesity therapy including KHM could be effective to improve liver function on obese patient. Additional large scale studies are necessary for more evidences.

Clinical and Histopathologic Findings on the Abnormal Liver Function Complicated with Kawasaki Disease (가와사끼 병에 합병된 간기능 이상소견에 대한 임상적 및 병리조직학적 소견)

  • Purpose: The aim of this research is to examine the abnormal liver function complicated with Kawasaki disease (KD) and to analyze its clinical characteristics and complications. Methods: Ninty eight cases were diagnosed as having KD, among which thirty four cases had abnormal alanine aminotransferase (ALT) (>30 IU/L). These abnormal ALT cases were evaluated in terms of age and sex distribution, major symptoms, complications, laboratory and histopathologic findings. Results: Male to female ratio was 2.4:1 and most patients (91.2%) were under 5 years of age. Cervical lymphadenopathy was observed in 3 cases (8.8%); and recurred cases were two (5.8%). Average fever duration was 8 days; average length of hospitalization was 9 days; and average recovery period was 13 days. Immediately after admission, positive CRP was observed in 31 cases (91.2%), leukocytosis (> $10,000/mm^3$) in 26 cases (76.0%), thrombocytosis (> $450,000/mm^3$) in 7 cases (20.6%), and anemia (<10 gm/dl) in 7 cases (20.6%), respectively. GB hydrops or cholecystitis were noted in 3 cases (8.8%), abnormal ECG finding in 1 case (2.9%), coronary dilatation or aneurysm in 2 cases (5.9%). Liver biopsy was done in four cases and revealed mild infiltration of lymphocytes on the portal area and mild bile duct proliferations. Conclusion: The abnormal liver function was noted in 34.7% of KD patients, and subsided all within one month. But the liver function test should be checked closely in patients of the abnormal liver function test complicated with KD despite of its good prognosis.

Clinical & Nutritional Assessments of Long-term Survivors of Biliary Atresia (수술후 10년 이상 장기 생존 담도폐쇄증 환장에서의 영양상태 및 임상적 특성에 관한 분석)

  • To assess the clinical and nutritional status of long-term survivors of biliary atresia, history taking, medical record review, physical examination (height, weight, MAC, TSF), blood tests (LFT, prothrombin time, platelet count, prealbumin, calcium) and liver needle biopsy were performed in 12 patients in whom Kasai procedure were performed more than 10 years ago at Department of Pediatric Surgery in Seoul National University Hospital. None were below the 5th percentile in height and weight. TSF was above the 75th percentile in all patients and showed good subcutaneous fat deposition. MAC was above the 5th percentile in all patients. Serum prealbumin level was abnormal in 2 patients with abnormal liver function and revealed visceral protein malnutrition. Serum calcium level was decreased below normal range in 4 patients with abnormal liver function. One patient had mild ascites. Five patients had abnormal liver function and 7 patients showed clinical manifestation of portal hypertension. Liver needle biopsy was performed in 5 patients and no cirrhotic change was observed. Although some patients who have survived for more than 10 years after Kasai procedure developed protein malnutrition and vitamin deficiencies, growth and development and nutritional status were generally satisfactory. Five patients(42%) showed normal liver function and no portal hypertension. In conclusion, Kasai procedure is satisfactory as a primary treatment in biliary atresia but significant portion of long-term survivors had abnormal liver function and portal hypertension. Continuous and careful follow-up is necessary to determine when liver transplantation may be indicated.

Clinical Characteristics of Factory Wokers with Asymptomatic Liver Function Test Abnormalities in Serial Health Examination (연속적으로 시행한 공장 근로자 건강 검진에서 무증상 간기능 검사 이상자의 임상적 특성)

  • Background/Aims The liver funtion tests(LFTs), such as aspartate aminotransferase(AST), alanine amino-transferase(ALT), r -glutamyl transferase( r -GT), have been widely used for screening tests but their low positive predictive value can cause many false positive results. To evaluate the clinical usefulness of these tests, we analyzed serial LFT results of single factory workers and compared the risk factor's in groups divided by the serial LFT results. Methods From June 2001 to October 2001, 1223 consecutive healthy workers in a single factory were enrolled and questionnaire, LFT and liver ultrasonography were performed. Previous LFT results were collected from Annual Health Examination Survey. According to the abnormalities in serial LFT, participants were classified into three groups (abnormal-in-both, alternating normal-in-both) and the risk factors were compared among these groups using multiple logistic regression Results The prevalence of LFT abnormality in a single test was 16.8% but, in serial LFT, only 5% of participants showed consistent abnormality. The risk factors for abnormal-in-both group, compared with alternating group, were liver ultrasonography abnormality such as fatty liver(odds ratio, 2.2; p=0.026) and heavy alcohol intake (more than 210g/week) (odds ratio, 7.2;P=0.064). HBsAg was not significant risk factor for any of the three groups. ConclusionIn factory workers with serial LFT abnormality, alcoholic liver disease could be the principal cause of abnormal LFT. Even if HBsAg were positive in patients with abnormal LFT, there is a possibility of another causes for LFT abnormalities such as alcoholic liver disease and nonalcoholic steatosis or steatohepatitis
