Multilevel Dumbbell Tumor of the Posterior Mediastinum -1 Case Report- (다범위 종격동 Dumbbell종양 - 1례 보고 -)

  • A 45 year old man was admitted for further examination of an abnormal shadow of the right posterior mediastinum. The patient suffered from dysesthesia in the right thoracic wall of dermatome T7. CT scan and MRI revealed that two separate tumors had developed in the right paravertebral area linked to the vertebral canal via an intervertebral foramina. One-stage removal of the tumors were performed safely through the right posterolateral thoracotomy following the resection of the rib head and vertebral pedicle. The tumors were confirmed as histologically neurilemomas. The postoperative course was uneventful.

  • At this study, we are developing a possible control system through remote monitoring for fire protection in various chemical factory facilities. It's possible to do real time confirmation of a normal operation presence of the various equipment installed in a chemical factory through the internet network at a fire fighting head office, an area fire department and a chemical factory situation room using this remote monitoring control system. When occurring, abnormal operation is the remote monitoring control system, which can check this immediately and notify the situation room administrator. After it was tested using developed remote monitoring control system, the remote monitoring for which the internet network was used confirmed possible.

  • Behçet disease (BD) is rare in childhood. We report a 9-year-old boy with neuro-Behçet disease who presented diplopia and weakness on the left side after a cerebral concussion. Brain magnetic resonance imaging (MRI) revealed hyperintensity of the right mesodiencephalic junction on T2-weighted and fluid attenuated inversion recovery images. Prednisolone administration resulted in complete remission and normalization of abnormal MRI finding. Brain MRI is a useful diagnostic tool when the neurological sign is the first symptom of subclinical BD.

A Clinical Observation of the Traumatic Sternal Fracture (흉골 골절에 대한 임상적 고찰)

  • Clinical observations were performed on 17 cases of the traumatic sternal fracture, those were admitted and treated at the department of thoracic and cardiovascular surgery in Chosun University Hospital during the past 6 years 5months period from January 1983 to May 1989. Obtained results were as follows: 1. The frequency was about 4.8% of the nonpenetrating chest trauma. 2. The ratio of male to female was 16: 1 in male predominance and age distribution was from 24 to 62 years old. 3. The common cause were high decelerating injury [impact of the steering column] and falling down[more than 3 m in high]. 4. The most common fracture site was sternal body and next was sternomanubrial junction. 5. Associated intrathoracic organ injuries were cardiac contusion [6 cases], hemopneumothorax[1 Case], mediastinal bleeding[1 case], and thoracic cage and extrathoracic organ injuries were rib fracture, head injuries, thoracic spinal fracture, and long bone fracture. 6. Abnormal EGG findings were sinus bradycardia[1 case], bundle branch block [2 cases], and sinus tachycardia[3 cases]. 7. The operative reduction and fixation was necessary in only one case and the others were treated with conservative treatment.

  • Temporomandibular joint (TMJ) subluxation and dislocation are uncommon but very unpleasant and distressing conditions to patients. Subluxation of the TMJ is an excessively abnormal condylar excursion secondary to flaccidity and laxity of the capsule. When the condyle head excurses anterior to the eminence upon wide opening, it can return to the fossa by self-manipulation or non-surgical conservative reduction. Surgery is recommended if a complete dislocation, so-called open lock, occurs as a chronic or recurrent protracted condition that cannot be reduced voluntarily. A range of surgical procedures can be performed to limit condylar hypermobility inclusing soft tissue tethering, creation of articular obstacles, removal of mechanical blockade and augmentation of articular eminence using different kinds of grafts. In the present case, a 74-year-old woman was diagnosed with a chronic TMJ dislocation that had lasted for 6 months. Bilateral condylectomy was performed and the post-operative results were good without functional limitations or recurrence.

Wafer Dicing State Monitoring by Signal Processing (신호처리를 이용한 웨이퍼 다이싱 상태 모니터링)

  • After the patterning and probe process of wafer have been achieved, the dicing process is necessary to separate chips from a wafer. The dicing process cuts a wafer to lengthwise and crosswise direction to make many chips by using narrow circular rotating diamond blade. But inferior goods are made under the influence of complex dicing environment such as blade, wafer, cutting water and cutting conditions. This paper describes a monitoring algorithm using feature extraction in order to find out an instant of vibration signal change when bad dicing appears. The algorithm is composed of two steps: feature extraction and decision. In the feature extraction, two features processed from vibration signal which is acquired by accelerometer attached on blade head are proposed. In the decision. a threshold method is adopted to classify the dicing process into normal and abnormal dicing. Experiment have been performed for GaAs semiconductor wafer. Based upon observation of the experimental results, the proposed scheme shown a good accuracy of classification performance by which the inferior goods decreased from 35.2% to 12.8%.

Effects of Cadmium on Embryo Hatchability, Larval Development and Survival of the Olive flounder, Paralichthys olivaceus

  • The cadmium (Cd) toxicological effects on the fertilized eggs, embryos and larvae were investigated in olive flounder, Paralichthys olivaceus water-borne exposed to Cd. The survival rate and hatching success of the embryos significantly diminished in treated groups in dependence of the Cd concentration. Significant differences were found at ${\geq}30{\mu}g\;L^{-1}$ exposed groups compared to the control group. A significant increase of malformation of the embryo was observed at ${\geq}20{\mu}g\;L^{-1}$ exposed groups. They usually include such symptoms as clouded yolk-sac abnormality, fin erosion and spinal curvature. A significant reduction in the survival rate of the larvae was observed in ${\geq}20{\mu}g\;L^{-1}$ exposed groups with accompanied by the disorder. Notably, in larvae, a concentration as low as $10{\mu}g\;L^{-1}$ exposed groups caused significant elevated abnormalities that is incidences of spinal cord deformation, abnormal eyes, deformation of the head region and severe developmental delay.

  • Sandifer syndrome is a rare manifestation of gastroesophageal reflux in children, occurring in association with abnormal movement of the head and neck and recurrent episodes of seizure after feeding. This syndrome may be misdiagnosed as infantile seizure and musculoskeletal disorder and may be associated with failure to thrive, anemia, aspiration pneumonia, and esophagitis. In this paper, Sandifer syndrome in a four months old male with recurrent aspiration pneumonia, apnea, and seizure after feeding is presented.

  • Thanatophoric dysplasia (TD) is a short-limb neonatal dwarfism syndrome that is usually lethal in the perinatal period. It is characterized by shortening of the limbs, severely small thorax, large head with a prominent forehead, macrocephaly, curved femur, and flattened vertebral bodies. These malformations result from the mutation in fibroblast growth factor receptor 3 (FGFR-3) gene which is located on the short arm of chromosome 4. A definite diagnosis should be established by molecular genetic analysis to find out the abnormal mutations in the $FGFR3$ gene. We confirmed by detection of a R248C mutation in the $FGFR3$ gene in DNA analysis.

  • Spasmodic dysphonia is a focal dystonia of the larynx and breathy voice is a typical sign of abductor spasmodic dysphonia. A group of patients with abductor spasmodic dysphonia have a number of acoustic characteristics including abnormal fundamental frequency fluctuations and abnormally long word duration. We report a abductor spasmodic dysphonia case have enlongated voice onset time voiceless consonants and breathy voice in wide band spectrogram. The patient have the acoustic characteristics only in telephone speaking at work time. We treated the patient with anticholinergic and anticonvulsant drug and supplementary voice therapy. The breathy voice and enlongated VOT were disappeared after those treatment.
