Factors Influencing Quality of Life of Elderly People with Dementia (치매노인의 삶의 질에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify the factors that affect quality of life in elderly people with dementia. Methods: The participants for this descriptive correlational study were 97 elderly patients with dementia who were attending a daycare center or were being cared for at home. Data were collected from the participants using the Mini-Mental Status Examination-Dementia Scale, the Geriatric Depression Scale (Short Form), Activity of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) Scale. Results: There were statistically significant differences in quality of life of elders with dementia according to mobility, using dementia day-care center, and cohabitation. In multiple regression analysis, depression, activity of daily of living, using dementia day-care centers and cohabitation were significant predictors of quality of life and the four variables explained 60% of the variance in quality of life. Conclusion: Findings indicate that to enhance the quality of life for elders with dementia, depression needs to be decreased and regular use of daycare service recommended. Further, various programs enhancing physical ability need to be developed and used with these elders.

A Study of Activities do Daily Living Performance on Arthritic Elderly Patients (노인의 퇴행성 슬관절염과 일상생활 수행력에 관한 연구)

  • To know the effect of index of social ability (ISA) and function of social life (FSL) with activities of daily living (ADL) and instrumental activities of daily living (IADL), we carried out a study on the elderly with osteoarthritis daily living in Daegu city for 4 months, from April to July 2002. The followings were observed: 1. The score of functional disability of knee, ADL and ISA were better at lower age (p < 0.01). 2. The younger they are, the better FSL they have. The lower age group has good FSL (p < 0.01). 3. Weight affects more to the under 49Kg group than 50-59Kg group and 60-69Kg group in functional disability of knee, while it doesn't make much difference for the other groups (p > 0.05) and other assessment index doesn't show any relationship with weight. 4. Regarding relat ionship with height , taller group has better knee functions: it's proved by post hock examination that the over 160cm group has much higher knee function than the other groups (p < 0.05). In case of ADL, the over 160cm group and under 149cm group were better than 150$\∼$159cm group. (p < 0.01) . 5. ISA was also different depending on height . It was proved that FSL was the better for the over 160cm group than for the others as a result of post hock examination (p < 0.05). 6. In regard of how many months have passed since the attack of functional disability of knee, knee joint function was better for the 12mon$\∼$36mon group than for the others and the score of ADL was lower for the 12mon$\∼$36mon group than for the others (p < 0.01). ISA was lower for the 12mon$\∼$36mon group and the under 6mon group than for the other groups (p < 0.01). FSL was lower for the 12mon$\∼$36mon group and the under 6mon group than the other groups. (p < 0.01). 7. Comparing left knee with right one, the problem with left knee has lower score of functional disability in all items (p < 0.01). 8. Considering friction noise, knee joint function was slightly better for non-friction noise-group, but it's not statistically significant though. ADL and ISA were better for non-friction noise-group (p < 0.01), while no statistical significance was found in FSL, 9. Knee joint function was better for the group who doesn't have limitation on range of knee flexion motion (p,0.05) but no other difference was found in other items from this group. 10. Limitation on range of knee flexion motion has no statistical difference in ADL and FSL. However, ISA was better for the non-limitation group (p < 0.05) . 11. Knee joint function score has high correlation with ISA (r=0.812, p < 0.01) and FSL (r=0.732, p < 0.01) , which shows the better knee joint function, the higher the score. 12. ISA and FSL were highly correlated: the better ISA, the better FSL (r=0.893, p < 0.01), while ADL has no correlation with either knee joint function or ISA or FSL.

The Association of Physical and Mental Function with Quality of Life among the Elderly at Care Facilities (요양시설 노인의 신체적 및 정신적 기능과 삶의 질과의 관련성)

  • This study was conducted to investigate the relationship between physical function (ADL, IADL) and mental function (depression, cognitive dysfunction) and quality of life in the elderly. The subjects of this study were 524 elderly people aged 65 or older who were admitted to 15 care facilities located in D metropolitan city. Data were collected through a personal interview conducted by interviewers that visited each care facility from November 2015 to January 2016. T-tests and ANOVA were used to compare the quality of life score for each independent variable, while multiple regression was used to determine the explanatory power of independent variables that affected quality of life. Quality of life was significantly lower among those of older age, lower educational level, living alone, with lower relationships with children, lower subjective health status, disability, lower ability for mastication, without regular eating habits, without regular exercise, and without regular health checkups. In addition, quality of life was significantly lower in the ADL and IADL, as well as among those with a higher depression level and lower cognitive impairment scores than their respective counterparts. The results of this study suggest that the quality of life among elderly that have been admitted to care facilities is significantly related to physical and mental functions as well as demographic characteristics, health status and health related behavioral characteristics.

Effects of Home Visiting Physical Therapy on Activites of Daily Living and Function in Disabled Persons Living at Home (가정방문물리치료가 재가장애인의 일상생활동작과 기능에 미치는 영향)

  • Purpose: The purpose of this study was to provide basic data for the effectiveness and the necessity of home visiting physical therapy through examining performance ability and functional changes in daily life activities by investigating the life of disabled persons living at home. Methods: After selecting 100 people with physical disabilities greater than level 3 and brain damage related disabilities living in Youngam-Gun, we conducted mobility tests according to a Modified Bathel Index (MBI), the Berg Balance Scale (BBS), and floor and bed movements? for Activities of Daily Living evaluation. We did this before the experiment and after doing home visiting physical therapy for 90 minutes at a time, once a week for 6 months Results: 1) Persons who live in a bed are higher than those who live in a floor. There was a significant difference between persons who live in a floor and those who live in a bed in the change in MBI and BBS scores after home visiting physical therapy. 2) Those with a musculoskeletal disorder had a significantly different change in MBI and BBS according to the type of disease and kind of disability. Those with neuropathy had a significantly different change in MBI. Those with physical disabilities showed a significant difference in MBI and BBS. Conclusion: A home visiting physical therapist can cause an improved performance ability and lead to the ability of disabled persons living at home to carry out activities of daily livingin a large part and the study for brain lesion disabilities except physical disabilities and stroke which occupy large proportion of disabled persons should be done.

Computer-Aided Cognitive Rehabilitation of Stroke Patients in Cognitive Function and Research on the Ability to Their Daily Living

  • Purpose: The purpose of this study was to evaluate the impact of differences in computer-aided cognitive rehabilitation aimed at stroke patients on cognitive function and daily life. Methods: The study subjects were Gwangju, Jeonnam including the attention group (group I), memory group (group II), and visual perception group (group III), who were randomly placed in each of a 10 by one problem solving group (group IV). The PSSCogRehab was applied to all subjects who underwent eight weeks once a week after 20 minutes in each group by strengthening mediation 10 minutes 30 minutes total, with a five minute break in the middle. Results: Cognitive function and activities of daily living is security and inter-group differences before intervention, and post-test results in the memory training group II, the intervention group I, III, IV and more on cognitive function and activities of daily living compared to it was effective. Conclusion: The results of computer-aided cognitive rehabilitation measuring cognitive function and ADL in patients with stroke in accordance with the group I, III, and IV group cognitive function and activities of daily living compared to IV showed that it was more effective.

A Study on the Determinants of the Elderly's ADL/IADL: Focused on the Comparison of Urban and Rural Areas (노인의 일상생활수행능력(ADL/IADL) 결정요인에 관한 연구: 도시와 농촌 비교를 중심으로)

  • This study examined the differentiation and influence of socio-demographic factors that affect the daily living ability of the elderly living in urban and rural areas. The research data is 「Korean Social Life, Health and Aging Project」. The analysis target was 400 urban elderly people (K-gu, Seoul: surveyed Apr. ~ Jun. 2017) and 524 rural elderly people (B-myeon, Gyeonggi-do: surveyed Dec. 2015 ~ Feb. 2016). The research methods included descriptive statistical analysis, chi-square test through cross-analysis, correlation analysis, and logit analysis. The summary of the research results is as follows. First, the ratio of 'complete independence' in ADL/IADL was 'ADL(city)> ADL(rural)> IADL(city)> IADL(rural)". Second, the probability that the elderly living in rural areas will experience a lower ADL level and lower IADL level was 7.1 times and 3.25 times higher than that of the urban elderly. Lastly, the statistically significant variable affecting the ADL level of urban elderly was depression, and the IADL level was gender, age, economic activity, spouse presence, and depression. Age and economic activity were statistically significant variables for the ADL level of the rural elderly, and the IADL level was gender, age, and economic activity.

The Effects of Time-use on the Elderly for Facilities in Activity of Daily Living (시설 노인들의 시간 사용이 일상생활 수행능력에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effects of the activities of daily living on time-use of the Elderly in facility. This study is also to provide basic data of the redesign time management and lifestyle as occupational therapy. Methods : The subjects were 20 elderly people (over 65 years) who live in S institution of Daejeun from August 11th, to August 22th in 2008. We used time table, interview and FIM which was to evaluate the ability to collect the general characteristics of these subjects(gender, age, moving period, education). Results : There was no significant different between the general characteristics of the subjects and activity of daily living. The more they spent time in Active BADL and IADL, the more they got higher scores in ADL performance ability(p<0.01). The more they spent time in the more they got higher score in ADL performance ability(p<0.05). Conclusion : We could know the amount of spending time of the elderly in Daejeun area facility and it related to activity of daily living. To improve the efficiency of time-use of the elderly, it is needed the role of occupational therapists. They should prepare a intervention to maintain active and positive life of the elderly.

The Physical and Social Disability of Aged Persons Who Live Alone in Goksung Area (곡성지역(谷城地域) 독거노인(獨居老人)의 신체적(身體的) 사회적(社會的) 능력장애(能力障碍)에 관(關)한 조사(調査))

  • It is necessary that the old should have the physical and social ability to perform their daily life. This study is to grasp their degree of disability and problems and suggest their solutions. It surveyed the 87 old people over 65 years old from September 1st until September 30th, in 1997. The findings are as follows. 1) The activities of daily living(ADL) to find their degree of physical disability shows that their average performance ability is 75.9% of all the action while 24.1% of all the old people needs the others' help. As they get older and older, the aged drop off in their physical ability, which is related to a statistical sense (p<0.001). 2) The social disability shows that the aged have their great difference from 9.2% to 85.1% in their instrumental activities of daily living(IADL), intellectual ability and social role. 3) A simple analysis shows that the activities of daily living are, in a statistical sense, related to age(p<0.001), the use of elder's hall(p<0.001), the understanding degree of health(p<0.01) and so forth. 4) A simple analysis shows that the instrumental activities of daily living are, in a statistical sense, related to age(p<0.001), the degree of education(p<0.05), the life of leisure(p<0.001), the understanding degree of health and so forth. 5) A multivariate logistic regression analysis shows that the disability of daily living is related to age, the visit of elder's hall, the period of solitary living, instrumental activities of daily living is age and the visit of elder's hall, and social role is the visit of elder's hall and the decree of education, while intellectual activity has no related variables in a statistical sense.

Effects of Percutaneous Sacroplasty on Pain and Mobility in Sacral Insufficiency Fracture

  • Objective : Sacral insufficiency fracture (SIF) contributes to severe low back pain. Prolonged immobilization resulting from SIF can cause significant complications in the elderly. Sacroplasty, a treatment similar to vertebroplasty, has recently been introduced for providing pain relief in SIF. The purpose of this study is to investigate the clinical short-term effects of percutaneous sacroplasty on pain and mobility in SIF. Methods : This study is conducted prospectively with data collection. Sixteen patients (3 men and 13 women) with a mean age of 77.5 years (58 to 91) underwent sacroplasty. Patients reported visual analogue scale (VAS; 0-10) and Oswestry disability index (ODI; 0-100%) scores. VAS and ODI scores were collected preoperatively and again at one day, one month, and three months postoperatively. Questionnaires measuring six activities of daily living (ADLs) including ambulating, performing housework, dressing, bathing, transferring from chair, and transferring from bed were collected. Ability to perform ADLs were reported preoperatively and again at three months postoperatively. Results : The mean preoperative VAS score ($mean{\pm}SD$) of $7.5{\pm}0.8$ was significantly reduced to $4.1{\pm}1.6$, $3.3{\pm}1.0$, and $3.2{\pm}1.2$ postoperatively at one day, one month, and three months, respectively (p<0.01). The mean ODI score (%) also significantly improved from $59{\pm}14$ preoperatively to $15.5{\pm}8.2$ postoperatively at one month and $14.8{\pm}8.8$ at three months (p<0.01). All ADL scores significantly improved at three months postoperatively (p<0.01). Conclusion : Percutaneous sacroplasty alleviates pain quickly and improves mobility and quality of life in patients treated for SIF.

Application of the Development Model of Day Care Center for Dementia (치매주간보호사업 개발모형의 적용)

  • This study was executed to develop the model of the format of the day care center which supplies the total service on the health care in the day time and social program related with the work and to evaluate the processing courses which was adjusted for 3 years from March 1999 to December 2001 in order to improve the quality of life of the family who experience dementia, dementia prevention, and the old and the burden of daily activities. The processing courses of this business model were evaluated to the confirmation on the health condition, application of the nursing processes, and the precise medical examination, and 20 sorts of social programs. The methods used in this model were the simplified MMSE-K, Ability for ,Daily Life(ADL), Instrumental Activities of Daily Living(IADL), and nursing process, and so on. The execution effects of this model are as follows. 1. It can contribute to the delay in the dementia process of the old people who are related with the dementia and maintenance and improvement of the health by confirming the stopping of the process of the dementia as the objective estimation method while the dementia symptom is maintained as the current condition through the medical and social total services. 2. The reduction of the burden for the daily care of the subject people and their families for the old people who are concerned with the dementia helped to improve the quality of life of the subject people and their family by enabling them to have jobs. 3. It enabled them to positively cope with the demand for the health and nursing of the local residents related to the old people. 4. It enabled them to enhance the recognition for the socialization of the local social organization and residents for the old people. 5. It enabled them to reinforce the related system among the local social organizations, and develop and provide the various social programs which are proper for the old people.
