Fatty Liver Diagnostics from Medical Examination to Analyze the Accuracy Between the Abdominal Ultrasonography and Liver Hounsfield Units (건강검진에서 지방간 진단의 상복부초음파검사와 간 Hounsfield Units 측정값과의 정확성 분석)

  • In abdominal Ultrasonography, the fatty liver is diagnosed through hepatic parenchymal echo increased parenchymal density and unclear blood vessel boundary, and according to many studies, abdominal Ultrasonography has 60~90% of sensitivity and 84~95% of specificity in diagnosis of fatty liver, but the result of Ultrasonography is dependent on operators, so there can be difference among operators, and quantitative measurement of fatty infiltration is impossible. Among examinees who same day received abdominal Ultrasonography and chest computed tomography (CT), patients who were diagnosed with a fatty liver in the Ultrasonography were measured with liver Hounsfield Units (HU) of chest CT imaging to analyze the accuracy of the fatty liver diagnosis. Among 720 subject examinees, those who were diagnosed with a fatty liver through abdominal Ultrasonography by family physicians were 448, which is 62.2%. The result of Liver HU measurement in the chest CT imaging of those who were diagnosed with a fatty liver showed that 175 out of 720 had the measured value of less than 40 HU, which is 24.3%, and 173 were included to the 175 among 448 who were diagnosed through Ultrasonography, so 98.9% corresponded. This indicates that the operators' subjective ability has a great impact on diagnosis of lesion in Ultrasonography diagnosis of a fatty liver, and that in check up chest CT, under 40 HU in the measurement of Liver HU can be used for reference materials in diagnosis of a fatty liver.

Cysts of Gastrointestinal Origin in Children: Varied Presentation

  • Purpose: Abdominal cysts of gastrointestinal origin are rare. Their rarity and varied clinical presentations make their pre-operative diagnosis difficult. Methods: Fourteen patients with histological diagnosis of cysts of gastrointestinal origin admitted between 2009 and 2015 were retrospectively analyzed with respect to age, sex, clinical presentation, diagnostic modality, site and type of cyst, management, outcome and follow-up. Results: The mean age at presentation was 4 years and there were six males and eight females. Abdominal pain was the most common presenting symptom. Five patients had an acute presentation-three had distal ileal mesenteric cysts and two had ileal duplication cyst sharing a common wall with ileum. Six patients presented with chronic abdominal pain and lump-three patients had omental cysts and three had mesenteric cysts-two of these in distal ileum and one in sigmoid colon. Two patients presented with antenatally diagnosed palpable abdominal lump. One had a mesenteric cyst of the ileum and the other had a distal ileal duplication cyst which required excision with resection and anastomosis. One patient had an atypical presentation. He was a known case of sickle cell trait and had presented with vague abdominal pain, recurrent cough and multiple episodes of haemoptysis over a period of one year. At laparotomy, gastric duplication cyst was found which was excised completely. Histopathology confirmed the diagnosis. Conclusion: Cysts of gastrointestinal origin are rare and have varied presentation. Surgical excision is the mainstay of treatment. The results and prognosis are good.

Adrenal Hemorrhage in a Neonate (신생아의 부신 출혈)

  • Neonatal adrenal hemorrhage is frequently associated with birth trauma or perinatal hypoxia. Hemorrhagic necrosis of the adrenal glands is often found at autopsy and many small lesions are usually asymptomatic. A palpable abdominal mass and jaundice are the usual presenting signs. Ultrasound is very useful in the diagnosis of this lesion; however, if the mass has mixed echoic pattern, magnetic resonance imaging (MRl) is helpful for the differential diagnosis from neuroblastoma. We present the case of a female newborn who was found to have a abdominal mass on physical examination. The patient showed anemia and hyperbilirubinemia. An ultrasonogram disclosed a $3.8{\times}3.0$ cm suprarenal mass with mixed echoic pattern. The mass was initially suspected to be neuroblastoma. An abdominal computed tomogram was not able to differentiate the mass. Magnetic resonance imaging revealed markedly increased signal intensity on T1 and T2-weighted sequences. This finding was consistent with adrenal hemorrhage. Serial sonogram demonstrated the mass that resolved completely by 12 weeks of age.

  • 현재까지 개발된 대부분의 규칙기반 의료 진단시스템에서는 의사들이 환자들을 진단하는데 필요한 지식을 정형화된 규칙만으로 표현해야 하기 때문에 어려움이 있으며, 시스템의 성능개선을 위해 규칙들의 수정 및 추가가 이루어져야 할 뿐 아니라, 예외적인 상황에서 진단시 문제점율 지니게 된다 본 논문에서는 일반적인 급성복통 진단을 위한 지식은 규칙으로 표현하고, 기존 규칙으로 처리할 수 없는 예외적인 급성복통 진단을 위한 지식은 사례로 표현함으로써 규칙과 사례가 서로 보완적인 역할을 할 수 있는 통합 방법을 제안한다. 또한 기존의 규칙 기반 DS-DAAP와 사레기반 추론에 의해 확장된 CDS-DAAP(Combined Diagnosis System for Diseases associated with Acute Abdominal Pain)의 비교를 통해, 제안하는 접근 방법이 진단율을 향상시킴을 보였다.

Role of F-18 FDG PET/CT in the Management of Infected Abdominal Aortic Aneurysm due to Salmonella (살모넬라 감염성 복부 대동맥류 환자에서 F-18 FDG PET/CT의 역할)

  • We present a case of infected abdominal aortic aneurysm due to salmonella enteritidis. F-18 FDG PET/CT was performed to diagnosis and during follow-up after antibiotic treatment. Computed tomography (CT) is considered to be the best diagnostic imaging modality in infected aortic lesions. In this case, a combination of CT and FDG PET/CT provided accurate information for the diagnosis of infected abdominal aortic aneurysm. Moreover, FDG PET/CT made an important contribution to monitoring disease activity during antibiotic treatment.

  • Diagnostics of unborn baby is mainly aimed at prediction and detection of occurrence of intrauterine hypoxia. Consequences resulting from fetal hypoxia appear in its heart activity. In this study, we have developed a new non-invasive system for fetal hypoxia diagnosis which provides systolic time interval(STI) parameters on the basis of analysis of electrical and mechanical heart activity together. For this we have worked on 1) the proper lead system for the acquisition of abdominal ECG, 2) the independent component analysis based signal processing and fetal ECG separation, 3) the development of a hardware which consists of an abdominal ECG amplifying module and an ultrasound module and 4) the detection of characteristic points of FECG and Doppler signal and the extraction of diagnostic parameters. The developed system was evaluated by the clinical experiments in which 33 subjects were participated. The acquired STI by the system were distributed within the ranges from the well-established invasive results of other researchers. From this, we can conclude that the developed non-invasive fetal hypoxia diagnosis system is useful.

Study on Characteristics of Abdominal Obesity among Acute Stroke Patients (뇌졸중 환자의 복부비만에 따른 제특성 연구)

  • Objectives : This study was aimed to assess characteristics in acute stroke patients according to abdominal obesity. Method : 1,506 subjects were included from the patients admitted to the Internal Medical Department at Kyunghee University Oriental Medical Center, Kyunghee University East-West Neo Medical Center, Kyungwon University Songpa Oriental Medical Center, Kyungwon University Incheon Oriental Medical Center, and D ongguk University Ilsan Oriental Medical Center from April 2007 to August 2009. Results : 1. Considering the demographic variables of the patients, the gender, older age, diabetes, hyperlipidemia and multiple infarction were significantly higher in the abdominal obesity group than in the control group. 2. Sasang constitution and oriental medical diagnosis showed significant difference in the abdominal obesity group. By oriental medical diagnosis, the D amp ness-Phlegm group was significantly higher in abdominal obesity group than in the control group. According to this analysis, we observed the general disposition of various characteristic distributions according to abdominal obesity in acute stoke patients These results can be utilized in the future as a basis material.

  • Non-traumatic acute abdominal pain in children presents a diagnostic dilemma. Numerous disorders can cause abdominal pain. Although many etiologies are benign, some require a rapid diagnosis and treatment in order to minimize morbidity. This review concentrates on the clinical office evaluation of acute abdominal pain in infants and children and details the clinical guideline for the diagnostic approach to imaging and the salient clinical features of some of the conditions. The clinical outcomes of children presenting with acute abdominal pain and the risk factors of the high risk underlying diseases would be provided for the diagnostic algorithm.

  • Abdominal pregnancy is one of the rare but extremely harzardous complications of childbearing. The diagnosis of the abdominal pregnancy is rarely made before surgery and the management of it also has many difficult problems. I experienced a case of abdominal pregnancy developed after the previous unilateral adnexectomy and it was a suspicious primary abdominal pregnancy as though did not fulfilled the Studdiford's the following criteria for primary abdominal pregnancy. 1) normal tubes and ovaries with no evidence of recent or remote injury, 2) absence of any evidence of uteroplacental fistula, and 3) presence of a pregnancy related exclusively to the peritoneal surface and young enough to eliminate the possibility of secondary implantation following primary nidation in the tube. And now I present a case with brief review of literatures.

  • The subject of Abdominal syndrome in the field of Shanghanlun takes a quarter of the whole research quantity, and has greatly contributed to the development of diagnoses due to the well-growth of syndrome differentiation through the differentiation of symptoms and signs based on prescription-centered abdominal syndrome. Since then, while the diagnostic has been developed mainly in the field of pulse fee ling and the inspection of the tongue, application of the abdominal diagnostic has not been continuously improved because of special historical and social environments. Recently, since interest in the differentiation of symptoms and signs based on abdominal syndrome has been raised by emphasis of Oriental diagnostic methods and medical treatment which have led to the study of Abdominal Syndrome in Shanghanlun. The following is the results of the study. 1. Shanghanlun abdominal syndrome is categorized into all abdominal symptoms. Epigastric symptoms, Hypochondrium symptoms, and Lower abdomen symptoms. 2. Subjective symptoms and Objective symptoms have been found in Abdominal syn drome, and Subjective symptoms have been more often than Objective symptoms. Both of the symptoms have been found more to co-exist in abdominal syndromes. 3. more cases of fullness of abdomen symptoms in All abdominal symptoms, a smaller number of cases in Taiyang disease, Yangming disease, disease, disease involving all three yang, Tayin disease and Jueyin disease have been found, but there have not been found in shaoyin disease. 4. More cases of Epigastric fullness and rigidity in Epigastric symptoms, Epigastric throbs in Palpitation symptoms, and sense of fullness-in-chest in Abdominal syndrome of chest and hypochondrium have been recognized. 5. Any regularity caused by abdominal symptoms has not been identified. 6. Diagnosis of the abdomen caused by abdominal symptoms has been identified in Epigastric fullness, Epigastric pain, Epigastric procrastination, Epigastric throb, fullness of abdomen and distension of lower abdomen.
