Effect of hyaluronic acid on prevention of adhesion in rats (랫드에서 hyaluronic acid에 의한 유착 방지 효과에 관한 연구)

  • This study was conducted to investigate the effect of hyaluronic acid (HA) on prevention of abdominal adhesions depending on various concentrations thereof by inducing an abrasion experimentally in the cecum of rats. Each group was consisted of 10 rats, and 40 rats were divided into 4 groups comprising the saline treatment group, HA 0.4% treated group, 0.6% treated group, and 0.8% treated group. And abrasion was caused in the cecum by using dry gauze and thereby, adhesion was induced. On 7 days after the operation, adhesions of each region were evaluated into the range of 0-4. Significant difference was found in the adhesion score between the control group and each experimental group (P<0.05). Also, HA 0.4% treatment group showed the lowest adhesion score (P<0.05). In the light of the above results, HA 0.4% solution was more effective on prevention of adhesion than HA 0.6% and 0.8% solution.

Diagnostic Imaging Features of Abdominal Foreign Body in Dogs; Retained Surgical Gauze (개에서 복강내 잔존한 거즈 이물의 진단영상)

  • This study was performed to describe the radiographic and ultrasonographic features of retained surgical gauze known as gossypiboma in 9 dogs. Female dogs (n = 8) were at higher risk and seven out of the eight cases had a history of ovariohysterectomy. Seven dogs were symptomatic and the most common clinical signs were vomiting, anorexia, and inertia. A palpable abdominal mass was detected in six dogs. Radiographic signs included a localized abdominal mass with soft tissue density (n = 7) or a mass containing speckled gas (n = 1). Ultrasonography showed a hypoechoic mass with a hyperechoic center (n = 4), or a homogeneous hypoechoic mass (n = 3). The remaining dogs (n = 2) showed an intestinal wall surrounding a hyperechoic center. Regardless of the characteristics of a mass, an acoustic shadowing was accompanied from the center of a mass in all dogs. Ultrasonography also revealed complications such as adhesion between a mass and adjacent organs, and peritonitis and intestinal obstruction around a mass. The gossypiboma can be considered when a hypoechoic mass accompanying a hyperechoic center with acoustic shadowing is observed on ultrasound examination.

Evaluation of sodium carboxymethylcellulose for prevention of adhesion in intestinal anastomosis in dogs

  • Adhesions are the most common postoperative complication in intestinal surgery in dogs. This study was aimed at verify to the efficacy of sodium carboxymethylcellulose (SCMC) in the adhesion prevention during healing of intestinal anastomosis. Twenty three healthy dogs were taken with average 4.17 years and weight was 5.68 kg and divided into 4 groups. The dogs of group III and IV, a pedicle of greater omentum was wrapped around the suture line. In the animals of group II and IV, 1% SCMC was infused (5 ml/kg, IP) at just before closure of the abdominal cavity. Two weeks after surgery, animals were reoperated and the adhesions were evaluated and graded. We found various degrees of intraabdominal adhesions in animals of all the groups. The significantly (P<0.05) lower adhesion score was observed in group IV than that of other groups. Nevertheless, varying intra-peritoneal adhesions, healing of anastomotic site was normal in all groups. It could be concluded that intra-peritoneal administration of SCMC solution reduces postoperative adhesions without any adverse effects on healing, and a synergistic beneficial effect can be obtained by supplementation with omental graft for intestinal anastomosis.

In Vitro: Antimicrobial Effect of Lactobacillus salivarius on Staphylococcus pseudintermedius

  • Lactobacillus spp. are the bacteria most commonly used as probiotics and it has been proven that they inhibit pathogenic bacterial growth and improve skin repair in humans. This study was conducted to investigate the growth inhibitory effect of Lactobacillus on Staphylococcus pseudintermedius, the most commonly isolated pathogen in canine pyoderma, and whether Lactobacillus could inhibit the adhesion capability of S. pseudintermedius to canine corneocytes. For this study, L. salivarius and S. pseudintermedius were isolated from healthy beagle fecal samples and the skin surface of dogs with skin infection, respectively. S. pseudintermedius was co-cultured with L. salivarius to assess the inhibitory effect. For the adhesion assay, corneocytes were collected from healthy beagle ventral abdominal skin. Both bacterial species attached to corneocytes and were assessed in number. As a result, L. salivarius significantly inhibited the growth of S. pseudintermedius in the culture medium. Moreover, L. salivarius reduced attachment of S. pseudintermedius in the adhesion assay. These results suggest that L. salivarius has an inhibitory effect on S. pseudintermedius and may be effectively used in the topical therapy of canine skin infections.

Acupuncture, ST-36 Electroacupuncture and Moxa Treatment of Abdominal Pain due to Bowel Obstruction in Cancer Patients : Case Report (말기 암 환자의 장폐색성 복통에 대한 침구 및 족삼리 전침 치험 2례)

  • Here we described 2 cancer patients treated by acupuncture for abdominal pain as a complication of bowel obstruction. The patient 1, 43-year-old man was treated with surgery, radiotherapy and chemotherapy for rectal cancer. Two years after surgery, he complained of acute abdominal pain due to small bowel adhesion band. Bowel obstruction and symptoms were not improved by conservative therapy like as fasting, keeping Levin tube, and fluid therapy. 4 months later, he could eat a little rice water, but ileus and abdominal pain persisted despite of applying opiod drug and patch. Got started on acupuncture, electroacupuncture(ST-36) and moxa treatment, pain was relived gradually. He could stop opioids 1 month later and ileus was improved after acupuncture therapy for 2 months. The patient 2, 65-year-old woman got hepatectomy, cholecystectomy, and chemotherapy with cholangiocarcinoma. 6 months after surgery, she got laparotomy again for biopsy of new mass around ascending colon. She started eating after gas passing, but felt abdominal distension. Diffuse paralytic ileus was diagnosed by abdominal X-ray, and she got started conservative therapy. During ST-36 electroacupuncture therapy, symptoms like abdominal pain and distension were improved and could stop opiod patch. But eating aggravated ileus again and clinical finding of mechanical bowel obstruction was appeared. Based on these cases, acupuncture and moxa therapy could be helpful for improving abdominal pain and ileus, but possibility of malignant bowel obstruction should be considered especially in cancer patients.

Cysteine antagonism of captafol induced toxicities in rats 1. Effects on hematological and serum biochemical values (랫트에 있어서 captafol의 독성에 대한 cysteine의 방어 작용 1. 혈액학 및 혈청 생화학적 성상에 미치는 영향)

  • This experiment was carried out to study the preventive effect of cysteine on the toxicities of captafol to the hematological and serum biochemical values. A single dose of captafol(5mg/kg BW, ip) was given to male Sprague-Dawley rats and its toxicities were evalutated by body weitht changes, autopsy findings, absolute organ weight, and hematological and serum biochemical parameters. The single dose of captafol caused significant decreases in body weitht, and absolute liver weight, as-cites, fibrin clot in abdominal cavity, adhesion of liver lobes significant elevation of number of RBC, hemoglobin concentration and serum AST activity, and decreased of serum phospholipid level. Where as cysteine(over 58mg/kg BW) given immediately after captafol appeared to prevent the ascites, fibrin clot in abdominal cavity and liver lobe adhesion. It also prevented the liver and blood, especially RBC toxicites. The results suggest that cysteine and other compounds containing sulfhydryl groups may protect the subjects from captafol-induced toxicity.

Using of Polypropylene Mesh for Peritoneal Defect induced Gossypiboma in a Shih-Tzu Dog (시츄견에서 Polypropylene mesh를 이용한 gossypiboma에 의한 복막 결손 수복 증례)

  • A 10-year-old spayed female Shih-tzu dog with a left-sided caudal abdominal swelling of 2-month clinical duration was referred to the Konkuk University Teaching Hospital (KUVTH). We confirmed the presence of an abdominal mass by radiography and ultrasongraphy. A gossypiboma was identified between bladder and small intestines at surgery and removed. The resulting peritoneal adhesion was severe. But, the peritoneal defect did not allow easy repair of the abdominal wall. Therefore, polypropylene mesh (PPM) was used in repair. The patient was good condition after surgery with no caudal abdominal swelling. We followed the patient for the next 6 months with no complication.

Reduction of Postsurgical Adhesion Formation of Reproductive Tract with Sodium Carboxymethylcellulose in Korean Black Goats (Sodium Carboxymethylcellulose가 개복 수술한 한국흑염소의 생식기 유착 방지에 미치는 영향 평가)

  • This study was performed to evaluate the efficacy of sodium carboxymethylcellulose (SCMC) for the prevention of postoperative uterus and ovary adhesion formation in the Korean black goats. Twenty adult female Korean black goats randomly were divided into five groups with four animals in each group. After the routine laparotomy, catheter for injection was inserted into the abdomen. Before abdominal closure, saline, 1% SCMC, 2% SCMC or 0.4% HA solution (50ml/10kg of body weight/head) were injected in the abdominal cavity in each group. Three weeks after surgery, second laparotomy was performed and the adhesions were scored on a scale of 0 to 10 according to their vascularity and adhesion size in uterus and ovary. This trial was repeated the four times with the interval of three weeks. In the first and second trial, the group treated with 2% SCMC significantly reduced the adhesion formation than other treatment groups (P<0.05). In the third trial, the adhesion formation was significantly reduced in 2% SCMC and 1% SCMC (P<0.05). In the fourth trial, 2% SCMC reduced the adhesion formation. However, there was no significant difference among other groups. This study showed that the 2% SCMC administered at the end of the surgery reduced the adhesion formation in the Korean black goats.

Single Small Bowel Metastasis after Curative Operation in a Pleomorphic Lung Carcinoma (근치적 수술 후 소장으로 단독 전이된 폐의 다형성 암종 1예)

  • Small bowel metastasis of pleomorphic carcinoma of the lung is very rare. A 58-year-old man was admitted to our hospital with abdominal palpable mass in the right upper quadrant area. He underwent right middle and lower lobectomy for early stage pleomorphic carcinoma of the lung approximately 3 months ago. USG-guided biopsy was performed for abdominal mass. Pathologic examination revealed a metastatic pleomorphic carcinoma from the lung. He received chemotherapy followed by radiation therapy but died due to septic shock caused by intestinal stenosis and adhesion. We report the first case of small bowel metastasis by pleomorphic carcinoma of the lung after curative surgery.

  • A 15 years Old girl was admitted with chief complaints of intermittent claudication of lower extremity, dizziness, and headache for 5 years. On admission, malignant hypertension was noted in the upper part of body [190-150/120-110] but femoral & dorsalis pedis pulse could not palpate. Once she had experienced C. V. A. due to hypertension of upper part, about years ago. On auscultation, systolic murmur was audible along the left sternal border. E.C.G. Showed left ventricular hypertrophy pattern, and others within normal limit. Retrograde aortography demonstrated diffuse narrowing of entire thoracic aorta with underdeveloped lower abdominal aorta [below the renal artery] & both common lilac artery, and rich collaterals, but normally visualized greater arteries in the aortic arch. On left posterolasteral thoracotomy, entire descending thoracic aorta revealed marked narrowing with mild perivascular adhesion, but no mediastinal pleura adhesion. These findings suggest as congenital type of atypical coarctation in the entire thoracic aorta with mild secondary change. But histopathology was showed the findings of chronic non-specific aortitis, later. Dacron by pass graft was performed with end to side anastomosis between graft and aortic wall. After operation, all her preoperative symptoms & signs were disappeared, and discharged with good general condition.
