  • Although Behcet`s disease, characterized by oro-genital ulceration and ocular abnormalities, is rare and unfamiliar disease to thoracic surgeon, one must remember that it can cause serious complications on the vascular system such as thrombophlebitis,thrombotic obstuction and aneurysm.We report a case of multiple arterial aneurysms with Behcet`s disease. This 31 year-old female had five times surgical interventions during short period[19 months]. 6 aneurysms were developed in this case, 3 were spontaneous aneurysms, 2 were anastomotic aneurysms and 1 was false aneurysm due to arterial puncture.

Games Application Methodology for History Education: Case Study of Developing a Serious Game for History Education (역사교육에 대한 게임 활용 방안: 기능성 게임 개발사례를 중심으로)

  • Although computer games seem to be efficient tools for facilitating and supporting situated learning in Europe, GBL(game based learning) is less likely in Korea. The objectives of this paper are to address the causes of the problem and evaluate a serious game for history GBL. We review various cases of history GBL projects in European School Net, commercial games and serious games which are related to history. We draw a demand of our game's structural type, learning criterion, basic model of instruction, and executable prototype from the analysis results. Scene management educators pursue and dependencies of turning points in the history are critical differentiator. An evaluation team of educators, learners, and edu-game managers evaluates that the prototype is suitable model for application in history instruction.

  • One of the serious long-tenn complications of diabetes mellitus is the diabetic foot lesion. Diabetic foot lesions are a major cause of morbidity, mortality, disability in patients with diabetes and frequent cause of lower-extremity amputations. The treatment of diabetic foot lesions is based on the strict control of diabetes mellitus. A patient with diabetic foot lesions was treated with Cheunhwasangamibang and improvement was seen. Results in this case support a role for Cheunhwasangamibang in controlling diabetes mellitus, and are here reported to encourage further study.

  • It is known that an unbiased estimator of f(p) for binomial B(n,p) exists if and only if f is a polynomial of degree at most n, in which case the unbiased estimator of a real-valued function $f(p), p = (p_0,p_1,\cdots,p_r)$ is unique. In general, this estimator has the serious fault of not being range preserving; that is, its value may fall outside the range of f(p). In this article, a condition on a real-valued function f is derived that is necessary for the unbiased estimator to be range preserving that this is sufficient when n is large enough.

The Safety Guidelines for use of Ma-huang in Obesity Treatment (비만처방에서의 안전한 마황사용 지침)

  • Objectives : Ma-huang (Ephedra sinica) is frequently prescribed for obesity management in oriental medicine. The main component is ephedrine alkaloids which can have serious adverse side effects such as heart attack, stroke, sudden death. There are no scientific guidelines for Ma-huang usage in the safe treatment of obesity in oriental medicine. We reviewed published studies on its safety to make evidence based guidelines. Methods : We searched electronic databases up to May 2006. We limited evidence to controlled trials for efficacy or safety, case reports for safety, and studies for Ma-huang contents analysis. Results and Conclusions : In clinical trials for weight loss, Ma-huang and ephedrine promote modest short-term weight loss but have no serious adverse effects, have only a few adverse effects associated with increased risk of psychiatric, autonomic, gastrointestinal symptoms and heart palpitations. In case reports, there have been serious adverse effects including stroke, heart attack, and death using typical doses of ephedrine or no associated illness. There are factors related to serious adverse effects, such as overuse, lack of standardization, individual sensitivity, and interactions with other drugs. Studies relating to these factors should be analyzed for safe use of Ma-huang and ephedrine. After analyzing related studies, we suggest guidelines for Ma-huang usage. We propose that the dosage should be within 4.5-7.5g per day for up to 6 months for generally healthy individual. It's use is contraindicated in individuals with heart disease, thyroid disease, diabetes mellitus, hypertension, psychiatric disorders, glaucoma, urination disorders, enlarged prostate, persons using MAOIs, methyldopa and sympathomimetic agents.

  • Intestinal T-cell lymphoma is a rare and aggressive form of non-Hodgkin lymphoma. Owing to its rarity and variable manifestations, intestinal T-cell lymphoma is usually diagnosed at an advanced stage. Therefore, it may be accompanied with serious complications at the time of diagnosis. Herein, I reports a case of intestinal T-cell lymphoma with multiple severe complications.

A Case Study on the Potential Severity Assessment for Incident Investigation in the Shipbuilding Industry (잠재 심각도 평가 기반의 조선업 재해 분석 활동에 관한 사례 연구)

  • Korean shipbuilding companies have taken many efforts for safety over the years by developing Health, Safety & Environment (HSE) Management Systems, Procedures, Training, and studying Programs for prevention of incidents. As a result, the shipbuilding industry has succeeded in reducing overall injury rates. Nevertheless, the industry also noticed that incident rates are still not at zero and more importantly, serious injuries and fatalities are still occurring. One factor that may be attributing to this is the lack of managing potential severity during incident investigations, most incident investigations are implemented based on the actual result. Generally, each shipbuilding company develops their customized incident investigation programs and these are also commonly being focused on actual result. This study aimed to develop a shift in strategy toward safety to classify the criteria of potential severity from any incidents and manage that to prevent any recurrence or causing any serious injuries or fatalities in the shipbuilding industry. Several global energy companies have already developed potential severity management tools and applied them in their incident investigations. In order to verify the necessity of improvement for current systems, a case study and comparative analysis between a domestic shipbuilding company and several global energy companies from foreign countries was implemented and comparison of two incident investigation cases from specific offshore projects was conducted to measure the value of a potential severity system. Also, a checklist was established from the data of fatalities and serious injuries in recent 5 years that occurred in Korea shipbuilding industry and a proposal to verify high potential incidents in the incident investigation process and comparative analysis between the assessment by appling proposed checklist and the assessment from a global energy company by using their own system was implemented. As a measure to prevent any incidents, it is required to focus on potential severity assessment during the incident investigation rather than to only control actual result. Hence, this study aims to propose a realistic plan which enables to improve the existing practices of incident investigation and control in the shipbuilding industry.

A Study on key attributes to congenital disease factors regarding the patients of the four most-common serious chronic illnesses and their relatives in the light of Myeongrihak -centering on the elderly in Seoul nursing homes- (4대 중증질환자와 혈연 관계자에 대한 명리학적 선천성질환 특성요인 분석 -서울시 요양원 입소노인을 중심으로-)

  • The objective of this study is to identify key attributes to congenital disease factors regarding the patients of the four most-common serious chronic illnesses and their relatives in the light of Myeongrihak. For the purpose of this study, a survey was conducted from March 15 to July 31, 2013, of 147 elderly people with the four most-common serious chronic illnesses admitted to nursing homes located in Seoul. Data were analyzed by frequency analysis and Kai-square tests by use of SPSS/WIN 18.0. The analysis found that the patients and their relatives had very similar disease factors, and cerebrovascular diseases presented the strongest correlations. The subjects also showed meaningful differences according to the health factors of their relatives. As this study found that meaningful correlations regarding congenital diseases between patients of the four most-common serious chronic illnesses and their relatives in the light of Myeongrihak, this will serve as basic data to predicts health conditions of families and relatives.

Reconstruction of tracheal stenosis: report of one case (기관협착증 치험 1)

  • For the treatment of acute respiratory failure and emergency care of an urgent patient, tracheostomy in itself may have been a life saving procedure. But, tracheal stenosis gives serious clinical manifestation which can only be corrected by surgical intervention in many occasions. We experienced one case of tracheal stenosis following tracheostomy for assisted ventilation. Tracheogram showed a 4.0 cm segmental narrowing below the tracheostoma. Before reconstruction, we tried to T-tube cannulation, but the result was not satisfactory. So we resected the narrowed segment and tracheal reconstruction was performed with uneventful result.

Pinch-Off Syndrome, a Rare Complication of Totally Implantable Venous Access Device Implantation: A Case Series and Literature Review

  • Background: Pinch-off syndrome (POS) is a rare complication after totally implantable venous access device (TIVAD) implantation. In cancer patients, it is important to prevent this rare complication and to recognize it early if it does occur. We present a case series of POS after TIVAD implantation and the results of a literature search about this complication. Methods: From July 2006 to December 2015, 924 permanent implantable central venous catheter implantation procedures were performed. The most common indication was vascular access for chemotherapy. Results: POS occurred in 5 patients in our clinic. Two patients experienced POS within 2 weeks, and the other 3 patients were admitted to department of surgery, Istanbul Faculty of Medicine at 6 to 14 months following implantation. The catheters were found to be occluded during medication administration, and all patients complained of serious pain. The transected fragments of the catheters had migrated to the heart. They were successfully removed under angiography with a single-loop snare. Conclusion: POS is a serious complication after TIVAD implantation. It is important to be aware of this possibility and to make an early diagnosis in order to prevent complications such as drug extravasation and occlusion events.
