PURPOSES: Design of approach roads of rest areas in highway has many drawbacks such as geometric design elements. There has been traffic accidents occured in these approach roads of rest areas. Thus, design criteria is required in order to protect accidents from being occurred. In case of Korea, geometric structure design criteria of entry facilities, such as toll-gate, interchange, junction etc was established. However there are no presence in a detailed standards for geometric structure of the rest area which affiliated road facilities. METHODS: In this study, analytic on accidents was carried out in regards to the entry of geometric structure of resting areas by utilizing a sight survey and an investigation research of traffic accidents. The survey was targeting 135 general service areas. Collisions with physical channelization and safety facilities occurred due to speeding, rapid entry, and etc at the entrance nose section. At the entrance connector roads, accidents caused by speeding, negligence, over-operation of handle of drivers were main reason of accidents. Discriminant analysis were conducted about geometric elements to distinguish influencing factors for traffic accidents. the lengths and access angles of the entrance connector roads were regarded as to have the high relation with traffic accidents. RESULTS: After classifying the design section of resting areas' entry as well as derive design elements on each section, a speed measurement by targeting entry of rest areas and car behavior surveys were performed, then each element's minimum standard was derived through the analyses. According to the speeds at the starting/end point of entrance connector road, the range of the junction setting angle of the entrance connector road is defined as $12^{\circ}{\sim}17^{\circ}$ and the connector length model was suggested. CONCLUSIONS: Suggest improvement plans for existing rest areas that can be applied realistically. This should be corresponded to the standards of entry and exit of developed rest areas.
Purpose: This study was to compare the effects of early ambulation to usual care of late ambulation in patients with transarterial chemoembolization on back pain, discomfort, and puncture site complications. Methods: A nonequivalent control pretest-posttest design was used. Total 40 patients were recruited from a gastrointestinal unit of an urban general hospital in Seoul, South Korea. The experimental group began to ambulate after 4 hours of bed rest with a compression bandage after receiving transarterial chemoembolization. The control group stayed in bed with a compression bandage overnight. Results: Back pain was not significantly different between the two group. The experimental group reported significantly lower discomfort than the control group. There was no incidence of bleeding complications on puncture site between two groups. Conclusion: The results of this study showed that early ambulation with four hours of bed rest after femoral sheet removal did not cause bleeding complications compared to the usual care and even decreased patients' level of discomfort due to bed rest. Repetitive research on the effect of short bed rest is warranted for its clinical utilization.
This survey provides, at a participation rate of 70%, 4,790 examinees. The purpose of this study is to study the association of the failed rest after work with 34 diseases including cardiovascular diseases. The index of the failed rest after work was composed of 4 questions about "thinking of work for several hours", "feeling exhausted", feeling unsatisfied or depressed", and "needing to go to bed early for next day′s work". Estimation of correlation among 4 variables, factor analysis, and ANCOVA adjusted for sex, age and job were carried out. A self-rating questionnaire of one′s own disease history and the "London School of Hygiene Cardiovascular Questionnaire" were used in order to discriminate each morbid group from the opposite group. Brief explanations of the result are as follows: 1) Every variable of failed rest after work shows significant difference between the morbid group and the no morbid group for possible infarction; for angina pectoris in the total, and men. 2) Among 4 variables ′exhaustion′ best discriminates the infarction group from the no infarction group, and the angina group from the no angina group. 3) The factor of failed rest after work is a significant factor that distinguishes the infarction group from the no infarction group, and the angina pectoris group from the no angina group. Therefore, stress management through health education and promotion such as behavioral modification can be used to reduce cardiovascular diseases and stress as perceived by an individual.
We performed 1st day Tc-99m-sestamibi gated SPECT with dipyridamole/rest T1-201 SPECT and 2nd day 24 hour delay T1-201 SPECT/rest Tc-99m-sestamibi gated SPECT in 27 patients with coronary artery disease(24) or having chest pain(3). Stress and rest Tc-99m-sestamibi gated SPECT was acquired at 60min post-injection. A 4-point scoring system(0 to 3 for normal to absent tracer uptake) for 17 segments was used. Wall motion was scored on another 4 point scale(0 to 3 for normal to dyskinesia) in the 1st day post-stress gated and the 2nd day rest gated SPECT. Post-stress gated SPECT showed wall motion abnormality in 94 segments(20%). Fifty-five segments among these 94 showed the same wall motion between post-stress and rest gated SPECT: i.e. 1-1 23 segments, 2-2: 29 segments, 3-3: 3 segments. Remaining 39 segments(41.5%) showed different wall motion between post- stress and rest Tc-99m-sestamibi gated SPECT. Twenty one segments with wall motion abnormality had normal perfusion(rest : 15 segments, 24 hour delay: 6 segments) at either rest or 24 hour delay. Fifteen among these 21 segments showed persistent post-stress and the 2nd day rest wall motion abnormality(persistent stunning). However, in 6 segments with pro-longed (1 hour after stress) stunning, abnormal wall motion did improve in the 2nd day rest Tc-99m-sestamibi gated SPECT(transient prolonged stunning). These 6 segments had normal perfusion at rest(n=4) or at 24 hour delay(n=2). Post stress wall motions showed significantly higher scores in persistent stunning than in prolonged transient stunning(P value<0.05). It was concluded that we could find stunned myocardium with gated Tc-99m-sestamibi SPECT at either post-stress or rest and that some myocardial walls of post-stress 1 hour gated SPECT did not show truly rest wall motion. So, we should be cautious if we use post-stress Tc-99m-sestamibi wall motion to assess rest wall motion.
Background: The socioeconomic burden of musculoskeletal disorders (MSDs) is significant, and kitchen work is a high-risk occupation for MSDs due to the intensive manual workload and repetitive movements that are involved. However, there are very few studies on MSDs and rest breaks as a workplace intervention among kitchen workers. This study examined the relationship between insufficient rest breaks and increased MSD risk among Korean kitchen workers. Methods: Sociodemographic and occupational factors of 1,909 kitchen workers were collected from the 3rd-4th Korean Working Conditions Survey data. Five items on rest breaks at work were categorized into two groups, "sufficient" and "insufficient." The number of MSDs and work-related MSDs (WMSDs), an outcome variable, was obtained from the sum of MSDs/WMSDs in three anatomical sites (back, neck, and upper limb, lower limb). The association between rest breaks and MSDs was estimated using zeroinflated negative binomial analyses, with adjustments for age, education level, and weekly working hours, and the analyses were stratified by sex. Results: After adjustment, significant associations were found between insufficient rest breaks and an increased risk of MSDs (odds ratio [OR] 1.68 95% confidence interval [CI] 1.11-2.54) and WMSDs (OR 1.40 95% CI 1.01-1.96) among female kitchen workers. Insufficient rest breaks were significantly associated with MSDs in female kitchen workers in all three anatomical sites. Conclusion: This study emphasizes the need for rest breaks as a workplace intervention for preventing MSDs in kitchen workers. Further studies to reveal the causality of this relationship are required.
This study aims to identify the actual condition of local food stores that serve as a regional activation space within the highway rest area. It analyzes and examines user awareness, and presents desirable directions for the establishment of new local food stores and improvement of existing facilities. The research method was conducted as follows: the status of 226 rest areas were examined which were installed on national highways, location conditions and convenience facilities. Second, a physical survey was conducted on 116 local food stores(floor area, location) and a user awareness survey on 63 local food stores. The results of the analysis and review have been concluded as follows. First, it is necessary to consider the establishment of a local food store in order to promote the local community by using various cultural and environmental conditions in the highway rest area, and to secure a public space to promote exchanges with local residents and revitalize the local economy. Second, considering the continuous increase of highway users, location, and traffic conditions, diversification of convenience functions including local food stores should be considered even if it is a medium and small highway rest area. Third, the floor area of local food stores is generally planned within a range of $75m^2$ to $125m^2$, and its location should be integrated with the rest area as much as possible. Inevitably, when you need to construct separately at the exit or entrance, you need to consider a unique design that ensures accessibility and visibility.
Bed rest is recommended to prevent postlumbar puncture headaches(PLPHA), but the period of bed rest varies in the literature from 6 hours to 24 hours. In clinical practice the period of bed rest varies but nursing methods for adults and children have little difference. In Seoul National University Hospital, children have been given at least 6 hours bed rest after a lumbar puncture. Pediatric oncology patients require a lumbar puncture for an initial diagnosis, follow up treatment or administration of chemotherapeutic agent. But it is difficult for young children to lie supine or to refrain from their usual activities in any way, and unpleasant problems related to a shortage of beds often occurs during discharge or in an outpatient setting. The purpose of this study is to substantiate the preventive effect of PLPHA by the period of bed rest, to identify the other factors that influence PLPHA, and to use the nursing methods proper to children. The subjects were 65 children, ages 1-17, undergoing treatment in the children's cancer center at SNUCH during the period June 1, 1995, to Aug. 31, 1995. The team nurses asked questions about PLPHA of the parents and children in order to fill out a questionnaire. The data were evaluated by percent, t-test, Chi-square test and Mann-Whitney U test. Result; 1. There was no significant difference relating the bed rest time spent to the occurrence of postspinal headaches (t-test). 2. There was a significant risk of PLPHA in the children who were irritable before procedure and/or had experienced previous PLPHA(p<0.05, ${x^2}-test$). 3. The following factors were not found to be associated with increased risk of PLPHA: previous puncture experience, giving analgesics, the choice of puncturist, inpatient/outpatient status, gauge of needle, purpose, the amount of CSF removed, gender, diagnosis, the number of peripheral WBCs, previous lumbago experience after LP, position after bed rest, age, the number of aural puncture at the time. A longer period of bed rest is unlikely to be more effective to prevent PLPHA and seems impractical. A shorter period will save time and effort. Perhaps it will also allay some of the fears which surround LP. So 1 hour bed rest after LP is suggested and nursing methods for emotional support should be investigated to reduce PLPH.
Purpose - This study reveals antecedent factors to help develop expressway rest areas in which employees were exposed to inferior environments, using Hackman and Oldham's Job Characteristics Theory. Research design, data, and methodology - The survey was conducted on 148 expressway rest area employees in Gyeonggi-do. After excluding questionnaires with invalid responses, 141 questionnaires were analyzed. Results - The sub-factors of job characteristics had a positive influence on the organizational commitment of employees, and organizational commitment had a positive influence on organizational citizenship behavior (OCB). Further, the results of mediation effects showed that organizational commitment and self-leadership were partially mediated. Finally, the results of the moderation effect of self-leadership showed that self-leadership was thought to be vital to let members endeavor to promote the competitiveness of expressway rest areas. Conclusions - The expressway rest area company needed to design jobs to let employees be self-motivated, productive, positive, responsible, and aspirational; job redesign, including job enrichment and job enlargement can be considerably helpful for affirmative influence.
Purpose: Many researches have clearly begun to address the relationship between people and their physical environments. These represent that children experience traditional hospital environments as unpleasant and scary. More specifically, children in hospitals face many psychological and emotional challenges: isolation, painful procedures, separation from family members, all in unfamiliar settings. For this reason, some have argued that the children's experiences within hospital environments can lead to poorer health outcomes. This study has been started in order to provide basic informations for the planning of Healing rest space in pediatric ward. Methods: Literature reviewes and field surveys to rest space at pediatric ward in Korea and foreign country have been conducted for the data collection. Results: The result of this study can be summarized into eight environment factor for healing rest space in pediatric ward: family, noise, safe, choice opportunity, light, privacy, social support and distract from disease or death. Implications: In the rest space in pediatric ward, it is necessary to consider healing rest space for promoting healing from disease and normal developing of children.
The author compared and analysed the roentgenocephalograms of one hundred Korean adults with the normal occlusion (50 males and 50 females), which were taken on the centric occlusion and the rest position of the mandible respectively for every subject, and then researched the relations among the relaxed muscle of the mandible, lightly touched of the lips, and the position just after the swallowing of saliva, of which 38 among 50 males had the rest position of the mandible. The results are as followings: 1. The anterior facial height increases more at the rest position of the mandible than at the centric occlusion, while the posterior facial height decreases. 2. The mandible moves more backward and downward at the rest position of the mandible than at the centric occlusion. 3. The facial procumbency and the incisor tooth inclination increases more at the rest position of the mandible than the centric occlusion in terms of the facial plane. 4. There are no differences between males and females at the rest position of the mandible and the centric occlusion in the meaning of variation. 5. There are no differences among the three methods from the view of lines and angles of the roentgenocephalogram.
