댄디워커증후군에 동반된 점막 밑 입천장갈림증 치험례 (Dandy-Walker Syndrome with Submucous Cleft Palate: A Case Report)

  • Purpose: Dandy-Walker syndrome is a rare congenital brain malformation that occurs one in every 25,000-35,000 live births, mostly in females. It is characterized by cystic enlargement of the fourth ventricle, agenesis or hypogenesis of the cerebellar vermis and enlargement of the posterior fossa. In this report, the authors aimed to address a rare case of a 14-months-old female Dandy-Walker syndrome patient that is presented with submucous cleft palate. Methods: A 14-months-old female patient admitted to our outpatient clinic, via the department of pediatrics, with the complaints of nasal regurgitation, choking and breathing difficulties. She was diagnosed as Dandy-Walker syndrome by magnetic resonance imaging evaluation, at another hospital and underwent a shunt operation for the hydrocephalus continuing treatments. On physical examination, she had structural abnormality of bifid uvula, and palpable notch in the posterior surface of the hard palate. Her submucous cleft palate was corrected, which used a double opposing Z-plasty under general anesthesia. Results: In a follow-up period of 2 months, no complications, such as wound dehiscence, necrosis and infection occurred, which shows satisfactory results. She consulted with pediatric neurologists and physical therapists for further evaluation and management of the abnormalities in the central nervous system. Conclusion: Dandy-Walker syndrome patient with a cleft palate is a very rare case to find, which only a few cases are reported around the world. Authors would like to share this case of Dandy-Walker syndrome patient, with submucous cleft palate, who underwent a double opposing Z-plasty that shows satisfactory results.

Two Species of the Genus Discorsopagurus (Malacostraca: Decapoda: Paguridae) New to Korea

  • The genus Discorsopagurus and two species belonging to this genus, Discorsopagurus maclaughlinae and Discorsopagurus tubicola, are herein first reported in Korean waters. Species belonging to Discorsopagurus are easily distinguished from other hermit crabs of the family Paguridae by their telsons that lack indentations and other characteristics such as straight abdomens, fused third and fourth abdominal tergites, and symmetrical uropods. Discorsopagurus maclaughlinae and D. tubicola were found to live in a tube made by a polychaete tubeworm. The two species differ in the shape of the posterior margin of the telson, the length of the pereopod, and the presence of setae on the antennal flagellum. Discorsopagurus tubicola is reported for the second time in the world. In this paper, there will be provided plates and descriptions of the two species.

신종교의 개벽사상 비교 (A Comparative Study on a New Religion, the Idea of the Gaebyok)

  • 이 연구는 우리나라의 근대 신종교의 핵심 사상인 개벽사상에 대한 상호 비교를 통해 개벽사상의 총체적 특징들과 그 의미들을 밝히는 것을 목적으로 한다. 지금까지의 대부분의 연구의 한계점은 신종교 각각의 개벽사상에 대한 단편적 연구에만 머물러 있다는데 있다. 이를 극복하기 위해 먼저 주역의 우주론적 시간관을 포괄하는 철학적 관점에서 개벽사상을 구명해보았다. 그리고 이러한 방법론적 기초 위에서 기존의 연구 성과를 정리하여 동학과 정역, 원불교, 그리고 강증산 각각의 개벽사상의 전개 과정과 성격을 살펴보았다. 그리고 각 개벽사상을 비교 분석하여 서로간의 공통점과 차이점을 정리해보았다. 신종교 창교자들은 자신들이 살았던 시점을 기준으로 선천과 후천의 획기적 전환 즉 천지와 자연의 운도(運度)의 커다란 변화가 이루어진다고 보았다. 운도론의 개념은 원시반본(元始返本)하는 우주자연의 운도에 따른 변화 양상을 설명하는 것과 같은 개념인데, 후천개벽사상 속에는 이러한 내용과 개념들이 고스란히 들어와 있다. 이 논문에서는 이 같은 선천후천 전환의 운도론적 관념이 수운(水雲), 일부(一夫), 소태산(少太山), 증산(甑山)의 교설 속에 상당히 많이 나타나있음을 밝혔다. 신종교 개벽사상에서의 선천은 인간의 한계상황을 대변하는 위기의 세계이며, 후천은 그 한계상황을 절대자의 힘 또는 극적인 우주변화로 인해 실현된 종교적 이상세계를 뜻한다. 그렇기에 개벽사상은 내일의 이상세계의 도래를 바라는 일종의 우주론적 말세론이며 종교적 역사관이다. 또한 우주론적 말세론은 일종의 시간관이며 역사관이기 때문에 필연적으로 그것에 포함되어 있는 선천과 후천이 각각 얼마나 기나긴 시간을 가졌는가 하는 문제를 지니게 된다. 그리고 특히 선천과 후천에서 사회의 조건들과 인간의 삶이 어떻게 달라지는가 하는 내용을 전제하게 된다. 따라서 이 논문에서는 우주론적 차원의 시간관에 대한 이해를 바탕으로 이러한 문제들에 대한 각각의 신종교에서 밝히고 있는 답변 내용들을 나열하고 비교 분석해보았다. 수운은 초월적 존재라 할 수 있는 하늘님과의 접신을 통해 구원의 메시지를 전달하는 사명을 맡았고, 일부는 역학적 원리를 통해 선천을 넘어서는 후천 세계의 도래를 밝혔으며, 소태산은 그 후천의 시대를 맞이하고 살아가기 위한 정신개벽론을 제기한 것으로 볼 수 있으며, 증산은 그와 같은 후천을 만들고 가져다주는 절대자임을 선언하였던 것이다.

A Dumbbell-Shaped Meningioma Mimicking a Schwannoma in the Thoracic Spine

  • A 50-year-old man presented bilateral hypesthesia on and below the T6 dermatome and paresthesia. Magnetic resonance imaging (MRI) showed an intraspinal extradural tumor, which located from the 6th thoracic vertebral body to the upper margin of the 7th vertebral body, continuing dumbbell-like through the intervertebral foramen into the right middle thorax suggesting a neurogenic tumor (neurofibroma or neurilemmoma). With the patient in a prone position, we exposed and excised the tumor via a one stage posterior approach through a hemi-laminictomy of T6. Histologic examination showed a grade 1 meningothelial meningioma, according to the World Health Organization classification. Initially, we assumed the mass was a schwannoma because of its location and dumbbell shape. However, the tumor was actually a meningioma. Postoperatively, hypesthesia resolved completely and motor power of the leg gradually full recovered. A postoperative MRI revealed no evidence of residual tumor.

A case of Moniliformis moniliformis (Acanthocephala) infection in Iran

  • Only a few cases of Acanthocephala infections have been reported in humans, and Moniliformis moniliformis is the most common species around the world. We report here a case of infection with M. moniliformis, which passed in the stool of a 2-year-old girl in Iran. The patient had abdominal pain, diarrhea, vomiting, and facial edema. According to her mother, the patient had habit of eating dirt and once a cockroach was discovered in her mouth. In stool examination, eggs of M. moniliformis were not found. She was treated with levamisole and the clinical symptoms reduced within 2 weeks. The specimen contained 2 pieces of a female worm with a total length of 148 mm lacking the posterior end. The spiral musculature of the proboscis receptacle and the shape of the trunk allowed its generic determination. Previously 2 cases of M. moniliformis infection were reported in Iran. This is the 3rd case of M. moniliformis infection in Iran.

Collapsed L4 Vertebral Body Caused by Brucellosis

  • Brucellosis is caused by gram-negative, aerobic, non-motile, facultative, intracellular coccobacilli belonging to the genus Brucella. A 50-year-old man working as an employee was admitted to neurosurgery clinic with severe low back, radicular right leg pain and hypoesthesia in right L4-5 dermatomes for 2 months. Brucella tube agglutination (Wright) test was positive in serum sample of the patient with a titer of 1/640. Brucella melitensis was isolated from blood culture. X-ray and MRI of the lomber spine showed massive collapse of L4 vertebral body. Neural tissue was decompressed and then posterior L3-5 short segment transpedicular screw fixation and stabilization was performed. Brucella melitensis was isolated from microbiologic culture of pathologic specimen. Antibiotic therapy was given as doxycycline 200 mg/day and rifampicin 600 mg/day for 6 months. Brucellosis is a systemic zoonotic infection and still an important public health problem in many geographical parts of the world. Vertebral body collapse caused by brucellosis occurs very rarely but represents a neurosurgical emergency because of its potential for causing rapidly progressive spinal cord compression and permanent paralysis. Neurosurgeons, emergency department personnel as well as infectious disease specialists should always keep a high index of suspicion and include brucellosis in the differential diagnosis of vertebral body collapse.

전북 인천강 하구역에서 출현한 망둑어과(농어목) 한국미기록종, Gymnogobius cylindricus (First Record of Gymnogobius cylindricus (Perciformes: Gobiidae) from Incheon River Estuary, Jeonbuk-do, Korea)

  • 우리나라 서남부의 인천강 하구역에 설치된 실뱀장어잡이 그물에서 망둑어과 꾹저구속 Gymnogobius cylindricus 1개체(표준체장 49.9 mm)가 채집되어 한국미기록종 및 분포의 서방한계로 보고한다. 본 종은 머리가 납작하며 몸이 가늘고 긴 점, 등지느러미 기조수가 VI-I, 12, 뒷지느러미 기조수가 I, 11, 체측의 어두운 반점열이 몸의 복부에 도달하지 않는 점 및 꼬리지느러미 하방에 반점열이 없는 점이 특징이다. 본 종의 신한국명으로 "가는살망둑"을 제안한다.

섬수약침 국소시술 이후 발생한 단순 피부 발진: 3 이상사례 보고 (Three Adverse Events Reports of Simple Exanthematous Eruption after Sumsu (Bufonis Venenum) Pharmacopuncture Topical Treatment)

  • Three cases of simple exanthematous eruption were suspected during Sumsu (Bufonis Venenum) pharmacopuncture (SP) topical anesthesia for acupotomy. Patients had skin rash with pruritus on both ankle, posterior neck, and left shoulder after 11, 12, and 7 times of SP treatment, respectively. There were no cases of systemic manifestations or changes in vital signs. As a result of using the World Health Organization-Uppsala Monitoring Centre (WHO-UMC) causality assessment, all the cases were evaluated as 'unlikely'. However, the results of using the Korean algorithm for assessing the causality of drug adverse reactions version 2.0 were evaluated as 'possible'. This report is the first case report on adverse events suspected of occurring after SP treatment. Although the causal relationship between suspected intervention and the adverse event is not clear, there was a difficulty in completely excluding the possibility. Additional safety studies will be required to make SP more widely available.

Three Cases of Intracranial Clear Cell Meningioma

  • The clear cell meningioma[CCM] is a rare and recently described as a histologic variant of meningioma. It has been identified and included in new World Health Organization[WHO] classification of the Central Nervous System[CNS] tumors recently. The CCMs are histologically characterized by sheets of spindled to polygonal cells with clear cytoplasm, which is the expression of high glycogen concentration. The CCMs occur in younger patients and usually are located in the spinal canal and posterior fossa. The most interesting aspect of CCM is the high recurrence rate and aggressiveness. Poor outcome has been shown in intracranial and spinal tumor location, but the indicators that predict outcome have not been established. Until now 22 intracranial CCM cases had been reported in English literature and 3 cases in Korean. The authors report two cases of CCM located at cerebral convexity and one at cavernous sinus those were totally removed [Simpson Grade $I{\sim}II$] by subfrontal, frontal and orbitocranial approaches. The clinical, radiological, histopathological, and neurosurgical features of these cases are discussed with the relevant literatures.

악성 흉막액을 동반한 원발성 종격동 지방육종 1예 (The Primary Mediastinal Liposarcoma with Effusion)

  • Primary liposarcoma of the mediastinum is a very rare and relatively slow growing tumor. Since the original description by Pallase and Roubier in 1916, there have been about 55 reported cases in world literature until 1985. Recently, we experienced one case of a primary mediastinal liposarcoma with malignant effusion. A 51-year-old man complained of dyspnea and chest discomfort. The chest plain films and computerized tomogram showed a huge mass of the posterior mediastinal space. The needle aspiration biopsy was done in the huge mass and the histologic examination revealed mediastinal round-cell type liposarcoma. Patient refused surgery or chemotherapy after establishing the diagnosis. About 6 months later, the metastatic pleural effusion was noted. After discharge, he was lost to follow up since then. The clinical and therapeutic features of the previously reported cases of primary liposarcoma arising in the mediastinum have been reviewed. Surgery may served to establish a tissue diagnosis, to relieved the patient's symptoms and result occasionally in a cure but radiotherapy or chemotherapy is ineffctive.
