Therapeutic effect of the traditional herbal formula, Hwanglyeonhaedok-tang, on rhinitis: A Review of the experimental study (황련해독탕이 비염 개선에 미치는 효과: 기초실험 연구 문헌 분석)

  • Objectives : This study aimed to assess the scientific evidence for the use of Hwanglyeonhaedok-tang, a traditional herbal formula, in the treatment of rhinitis, and prepare the basis for the investigational new drug application by analyzing the experimental studies. Methods : Ten electronic databases were searched up to December, 2016 without language limitation. Experimental studies on the anti-inflammatory and anti-allergic effects of Hwanglyeonhaedok-tang against rhinitis were included. We extracted data about study design, characteristics of intervention, outcomes, and pharmacological effects from the included studies and summarized them. Results : Eight hundred and thirty-three potentially relevant studies were identified, of which 18 experimental studies met our inclusion criteria. Of 18 included studies, 5 had conducted cell viability test, and all studies had reported that Hwanglyeonhaedok-tang was non-cytotoxic. Hwanglyeonhaedok-tang exhibits anti-inflammatory effect by regulating the inflammation-related cytokines including nitric oxide(NO), prostaglandin $E_2(PGE_2)$, interleukin-6(IL-6), and tumor necrosis $factor-{\alpha}(TNF-{\alpha})$ in vitro and in vivo. Hwanglyeonhaedok-tang exhibits anti-allergic effect by suppressing eosinophil, and histamine levels. Hwanglyeonhaedok-tang helps in the recovery of nasal mucous membrane by supressing goblet cells, heat shock protein 70, and substance P. Conclusions : This study suggests that Hwanglyeonhaedok-tang has the potential to be developed as therapeutic agent for rhinitis. Further experimental and clinical studies needed to be performed to prove the safety and efficacy.

Associations between Work-Related Musculoskeletal Pain, Quality of Life and Presenteeism in Physical Therapists (물리치료사의 업무관련성 근골격계 통증, 삶의 질과 프리젠티즘의 관계)

  • Background: The purpose of this study was to analyze the associations between physical therapists' work-related musculoskeletal pain, quality of life (QoL), and presenteeism. Methods: Self-reported questionnaires were given to 402 physical therapists in Korea. Variables examined included the prevalence of pain sites related to work, pain intensity, pain pattern, pain frequency, pain duration, pain rating score (PRS), general characteristics, QoL, and presenteeism. Results: Of the 314 physical therapists who responded, 251 (79.9%) reported work -related musculoskeletal pain in at least one body part. The most affected regions included the wrist and hand (21.0%), shoulder (19.6%), and lower back (19.1%). On average patients, QoL and presenteeism differed significantly between physical therapists who reported pain and those who did not (P < 0.05). QoL showed a negative correlation with the work-related musculoskeletal pain (P < 0.01) and presenteeism (P < 0.01), while the presenteeism and work-related musculoskeletal pain showed positive one(P < 0.01). Factors influencing a physical therapist's QoL included presenteeism, Working venues (Convalescent hospital), Work status (Contra ct worker) and professional experience, while the presenteeism included QoL, work status (Intern), PRS, working venues (Convalescent hospital), Area of specialty (Pediatrics), age, sex and working hour. Conclusions: Results show that physical therapists appear to be at a higher risk for work-related musculoskeletal pain. Therefore, administrators need to use the intervention strategies to manage the presenteeism and QoL of physical therapists.

Clinical Study on Curative Effect of Uterus Myoma (자궁근종의 치료효과에 관한 임상적 연구)

  • The purpose of this study is to find out oriental medical management and treatment of uterus myoma patients by investigating and analyzing them. We analyzed the results of medical records for 132 patients who were diagnosed as uterus myoma and visited Dong-Eui University Oriental Medical Center from March 2001 to December 2004. We investigated the frequency of symptom, the company frequency of symptom, the variety of treatment method. The frequency of symptom was dysmenorrhea 79 persons 59.85%, menorrhagia 61 persons 46.21%, subjective symptom of mass 12 persons 9.09%, infertility 4 persons 3.03%, lumbago 24 persons 18.18%, dysuria 10 persons 7.58%, dyschezia 22 persons 16.67'/), and the average number was 2.33. We investigated the company frequency of symptom, silent patients were 16 persons 12.12%, 116 persons 87.88(/) had myoma symptom. The treatment method; The frequency of taking herb medicine was 115 persons 87.12% and the average period was $50.65{\pm}5757$. The frequency of taking pill medicine was 56 persons 42.42% and the average period was $54.34{\pm}74.02$. The frequency of taking acupuncture treatment was 83 persons 62.88% and auricular acupuncture treatment was 30 persons 22.73%. We could find out the possibility of oriental medical management and treatment of uterus myoma.

Characterization of Carboxymethylcellulase(CMCase) Produced by Recombinant E. coli Containing CMCase Gene for Cellulomonas sp. YE-5

  • CMCase produced by recombinant E. coli JM109 (pCEH#4) containing CMCase gene from Cellulomonas sp. YE-5 was purified to 24.3 fold and 2.6% yield by ammoniumsulfate precipitation, DEAE-cellulose column chromatography and gel filtration on Sephadex G-100. The optimum pH and temperature for CMCase activity were pH 7.0 and 5$0^{\circ}C$. The enzyme was stable between pH 5.0 and 10.0, and up to 6$0^{\circ}C$. The molecular weight of he enzyme was estimated to be approximately 40,000 daltons by SDS-PAGE. Analysis of the amino acid composition showed that the enzyme contained many glycines and acidic amino acids. The enzyme was an endo-type CMCase and the final enzyme reaction product from hydrolysis of Cm-cellulose by the enzyme was cellobiose. {TEX}$K_{M}${/TEX} value determined with CM-cellulose was 1.28mM.

Columnar variant of papillary carcinoma in the thyroglossal duct cyst with progression to lung metastasis

  • Thyroglossal duct cyst (TGDC) carcinoma generally shows a favorable prognosis. If metastasis is present latently, it may not threaten the patient's life immediately. It has been shown, however, that larger than 1 cm papillary carcinoma (PC), level VI metastasis to the lymph node (LN), which is the nearest to the thyroid, independently predicts a worse prognosis. In the case presented herein, a 61-year-old female patient was diagnosed with an about 3 cm PC in the TGDC, particularly the columnar variant subtype, one of the aggressive variants. She had occult papillary thyroid microcarcinoma, but no LN metastasis. Even though she underwent the Sistrunk procedure and total thyroidectomy with central compartment neck dissection followed by high-dose radioactive iodine remnant ablation, however, the cancer cells spread to level IV neck LN, and finally to the lung. Therefore, when a patient is diagnosed with an aggressive histologic variant of PC in the TGDC, even without LN metastasis, the invasive surgical approach and close postoperative surveillance are necessary, with consideration of the risk of disease progression. Therefore, if it is possible to stratify the risk for patients, higher-risk patients can be offered a more invasive therapeutic approach.

Effects of Drenching Treatment of Growth Retardants on Growth in Datura suaveolens 'Orange Star' (생장억제제 관주처리가 'Angel's Trumpet'의 생육에 미치는 영향)

  • This study was conducted to identity the effect of plant growth retardants such as paclobutrazol, ethephon, uniconazole and daminozide by drenching on growth of Datura suaveolens 'Orange Star'. Datura suaveolens 'Orange Star' treated with the growth retardants except ethephon was gradually shortened in proportion to the treated concentration of growth retardants. Especially, uniconazole treatment dramatically short ed stem height and width of plant, diminished leaf width and length and also shortened the days to flowering, and increased flowering times per year. On the contrary, ethephon treatment showed the opposite affect such as higher plant width and plant height, increased days to flowering, and decreased the number of flower times. Dwarfing by plant growth retardants in Datura suaveolens 'Orange Star' was effective in orders of uniconazole, daminozide and paclobutrazol.

A cross-sectional survey of clinical factors that influence the use of traditional Korean medicine among children with cerebral palsy

  • Background: Traditional Korean medicine (TKM) is widely used to treat children with cerebral palsy (CP) in Korea; however, studies investigating factors that influence the use of TKM are scarce. Thus, we investigated the clinical factors that might influence the use of TKM. Methods: A population-based, cross-sectional, multicenter survey was performed from August 2014 to May 2016. The history of TKM use, type and severity of CP, current treatment characteristics, presence of accompanying disabilities or other health problems not directly related to CP, and monthly cost for the treatment of CP were surveyed. Results: In total, 182 children were recruited, and 78 children (42.9%) had used TKM. Among these 78 children, 50 (64.1% of the TKM-use group) had used both acupuncture and herbal medication, 15 (19.2%) had used acupuncture only, and 13 (16.7%) had used herbal medication only. Children with non-typical CP, accompanying disabilities and general health problems tended to use TKM. The monthly cost of treatment for CP was significantly higher in the TKM-use group than that in the no-TKM-use group, suggesting that economically disadvantaged children may have difficulty in accessing TKM. Dietary supplements, conventional pharmacological treatments, and rehabilitation therapies did not affect TKM use. Conclusion: Children with non-typical symptoms or those with poor overall health status are likely to use TKM. Additionally, TKM use leads to increased treatment costs. Studies investigating the motivation for starting or ceasing TKM therapy, socioeconomic factors and the attitude of parents towards complementary and alternative medicine should be performed.

Reliability and validity of new evaluation methods using static surface electromyography in persons with neck pain

  • Objective: The aim of this study was to evaluate the validity and reliability of using static surface electromyography (sEMG) on persons with neck pain and in healthy adults. Design: Cross-sectional study. Methods: Twenty-two female participants with neck pain and thirty healthy adults in the age group of 20-65 years were recruited in this study. To evaluate the validity and reliability of sEMG in subjects with neck pain, the subjects'characteristics were recorded and the Visual Analog Scale (VAS) and Neck Disability Index (NDI) were examined in addition to sEMG and algometer tests being carried out on the subjects. The site for using the sEMG and algometer was the upper trapezius. sEMG test-retest reliability was measured by intraclass correlation coefficients (ICCs). Independent t-tests were used to analyze the differences in the dependent variables between subjects with neck pain and healthy adults. The Pearson correlation coefficient was used to examine the linear relationship between measured variables. Results: sEMG and algometer tests were reliable according to the test-retest reliability results in subjects with neck pain and healthy adults (ICC=0.815-0.979). The results of this study showed that there were significant differences in respect to age, VAS, sEMG and algometer tests between persons with neck pain and healthy adults (p<0.05). The VAS and NDI were statistically correlated with sEMG and algometer results (p<0.05). Conclusions: In this study, we investigated the clinical usefulness of the static sEMG test in evaluating the pain scale of persons with neck pain with high reliability and validity.

Continuous wound infiltration of ropivacaine for reducing of postoperative pain after anterior lumbar fusion surgery: a clinical retrospective comparative study

  • Background: Local anesthetic infiltration at the site of a surgical wound is commonly used to control postoperative pain. In this study, we examined the effectiveness of continuous local infiltration at an abdominal surgical site in patients undergoing anterior lumbar interbody fusion (ALIF) surgery. Methods: Sixty-one patients who underwent ALIF surgery were enrolled. For thirtyone of them, a continuous local anesthetics infiltration system was used at the abdominal site. We collected data regarding the patients' sleep quality; satisfaction with pain control after surgery; abilities to perform physical tasks and the additional application of opioids in the postoperative 48 hours. Results: The On-Q system group showed reduced visual analogue scale scores for pain at the surgical site during rest and movement at 0, 12, 24, and 48 hours; and more was satisfied with pain control management at the first postoperative day (7.0 ± 1.2 vs. 6.0 ± 1.4; P = 0.003) and week (8.1 ± 1.6 vs. 7.0 ± 1.8; P = 0.010) than the control group. The number of additional patient-controlled analgesia (PCA) bolus and pethidine injections was lower in the On-Q group (PCA: 3.67 ± 1.35 vs. 4.60 ± 1.88; P = 0.049 and pethidine: 2.09 ± 1.07 vs. 2.73 ± 1.38; P = 0.032). Patients who used the On-Q system performed more diverse activity and achieved earlier ambulation than those in the control group. Conclusions: Continuous wound infiltration with ropivacaine using an On-Q system may be effective for controlling postoperative pain after ALIF surgery.

IoT-based Smart Switchboard Development for Power Supply of Entertainment Devices (엔터테인먼트 장치의 전원 공급을 위한 IoT 기반의 스마트 배전반 개발)

  • In this study, a smart switchboard for power supply of entertainment devices was developed for the following purposes. First, the heat generated when the high-temperature and humid air inside is cooled by the thermoelectric module is smoothly discharged to the outside of the switchboard, thereby maximizing the cooling effect. So, it is possible to prevent excessive temperature rise inside the switchboard. Various problems such as condensation inside the switchboard can be prevented by controlling the temperature of the switchboard in which a fire occurs due to excessive heat in summer, removing moisture due to the cooling effect, and generating heat instead of cooling in winter. Second, it is a smart switchboard control system that can reduce the salt that may permeate inside the switchboard. Third, the smart switchboard system is an IoT-controlled switchboard that collects environmental data using a variety of sensors and can remotely control devices through a smartphone, and can be easily used in various fields.
