The effects of Yukmijihwang-Tang(UJT) on Dementia induced by focal brain ischemic injury in rats. (육미지황탕(六味地潢湯)이 국소(局所) 뇌허혈(腦虛血)로 유발된 기억력(記憶力) 손상(損傷) 백서(白鼠)에 미치는 영향(影響))

  • Objective: Yukmijibwang-Tang(UJT) has been used for Dementia derived by deficiency of Kidney-Yin in the oriental medicine. This study was planned to examine the effects of UJT on the memory loss induced by focal brain ischemic injury in the rats. Methods : Experimental groups were divided into 4 groups ; Normal group, Control group, UJT1 group and UJT2 group. Control group were no treated after focal brain ischemic injury. UJT1 group were administered UJT 0.3 $m{\ell}/kg$ to focal brain ischemic injuried rats for 21 days, UJT2 group were administered UJT 1.2 $m{\ell}/kg$ to focal brain ischemic injuried rats for 21 days. The present author observed the number of errors on the eight-arm radial maze task, the rate of correct choice on the eight-arm radial maze task, the values of density of Cresy1 violet- stained sections in the hippocampal CA1 and the values of density of Acetlycholine Esterase (AchE)stained nuclei in the hippocampal CA1. Results : The number of errors in the Eigth-arm radial maze task was significantly decreased in UJT1 group on 1, 2, 3, 5, 6days, And it was significantly decreased in UJT2 group on 1-6days compared with control group. The rate of correct choice in the eight-arm radial maze task was significantly increased in UJT1, UJT2 group compared with control group. The values of density of Cresyl violet-stained stained sections in the hippocampal CA1 were significantly increased in UJT1, UJT2 group compared with control group. The values of density of AchE in the hippocampal CA1 were increased in UJT1, UJT2 group compared with control group, but the values were not significant. Conclusions: The present author thought that Yukmijihwang-Tang could he used for curing dementia induced by focal brain ischemic injury.

Study on vasorelaxant activities of various Traditional Herbal Prescriptions in rat thoracic aortas (수종 전통 한약 처방에 대한 혈관 이완 활성 연구)

  • Objectives : High blood pressure (also called Hypertension), which is the blood pressure that is higher than normal, is a chronic disease and causes various complications. Historically, Traditional Herbal Prescriptions (THP) have treated many diseases. However, there are not many studies on the treatment of hypertension with THP, very few studies have investigated the interactions between the co-administration of synthetic anti-hypertensives and THP. Therefore, the objective of the present study was to investigate the vasorelaxant activities of 10 THP in rat thoracic aortas pre-contracted with potassium chloride (KCl). Methods : An electric extractor was used to extract THP in distilled water for 2h. Rat thoracic aortas were isolated and pre-contracted using KCl in organ chambers containing 10 ml Krebs Henseleit (KH) buffer. THP extracts were added in increasing concentrations (10-1000 ㎍/mL) to investigate vasorelaxant activities. The vasorelaxant activities induced by THP were expressed as a percentage in response to contraction generated by KCl. Results : Among the 10 THP, Dangguisu-san, Mahwang-tang, Bulwhangeumjeonggi-san, Jakyakgamcho-tang, and Hyangsapyeongwi-san showed significant vasorelaxant activities. Maekmundong-tang, Bojungikgi-tang, Samryeongbaekchul-san, Yukmijihwang-tang, and Insampaedok-san showed no significant effect. Also, in co-administration with amlodipine, Mahwang-tang showed higher vasorelaxant activities than amlodipine alone, and Hyangsapyeongwi-san showed greater vasorelaxant activities at low concentrations but inhibited amlodipine's vasorelaxant activities at high concentrations. Conclusion : The results of these experiments are expected to provide useful data to establish guidelines for THPs and co-administration with western antihypertensive drugs to treat hypertension.

A Case Report of Dyskinesia in Lower Limbs caused by Parkinson's Disease Diagnosed as Shinjeonghyuson (신정휴손(腎精虧損)으로 변증한 파킨슨병 환자의 이상운동증 치험(治驗) 1례(例))

  • Objectives : This study was designed to research the effects of oriental treatment on dyskinesia with lower limbs caused by Parkinson's Disease. Methods : We prescribed Yukmijihwang-tang (六味地黃湯) and Jaumganghwa-tang (滋陰降火湯) and treated acupuncture through Shinjungkyuk (腎正格) every day. It was used to treat the Shinjeonghyuson (腎精虧損) symptoms. We analyzed the frequency of dyskinesia over one night at intervals of three days and measured the score of 'Unified Parkinson's Disease Rating Scale' every week to evaluate the entire symptom's improvement. Results and Conclusion : He had been treated with western medicine to improve the dyskinesia symptoms, but couldn't get a noticeable result. Having oriental treatment for 43 days, dyskinesia in the lower limbs mostly disappeared and all symptoms of Parkinson's disease (tremor, rigidity, dysphagia, hallucination, anorexia, etc.) improved significantly.

A Case Report of Multiple System Atrophy in a Cerebellar Ataxia Patient Suffering from Gait Disturbance Treated with Korean Medicine (보행장애를 주소로 하는 소뇌형 다계통 위축증 환자의 한방 치료 1례)

  • Objective: To describe the effect of Korean medicine on gait disturbance in a patient with multiple system atrophy and cerebellar ataxia. Methods: We observed a 75-year-old female patient who had multiple system atrophy and cerebellar ataxia. The patient was treated with Korean medicine, including acupuncture, moxibustion, and herbal medicine (Yukmijihwang-tang-gamibang and Boyanghwano-tang-gamibang) for 38 days. We evaluated the patient with the Korean version of the Berg Balance Scale (K-BBS), the Unified Multiple System Atrophy Rating Scale (UMSARS), and the Numeric Rating Scale-11 (NRS-11). Results: After treatment, the patient’s symptoms were improved. The K-BBS score increased from 9 to 19, while the UMSARS score decreased from 16 to 12 in Part I, from 15 to 7 in part Ⅱ, and from 4 to 3 in part Ⅳ. The NRS-11 score of gait disturbance decreased from 10 to 5. Conclusions: Korean medicine may be an effective treatment for gait disturbance in multiple system atrophy with cerebellar ataxia.

Literature Review on Korean Medicine Treatment for Alopecia

  • This study aimed to analyze the use of Korean medicine treatments for alopecia in among clinical studies. We identified and analyzed 22 studies from Korean databases; Oriental Medicine Advanced Searching Integrated System (OASIS), Science ON, Korean Studies Information Service System (KISS), and Research Information Sharing Service (RISS) and international database; PubMed. We analyzed the Korean medical treatment in each case and determined the tendency to use each intervention. We analyzed 1,464 patients from 22 selected studies. Herbal medicine, acupuncture, external medicine or products, pharmacopuncture, and phototherapy were used for alopecia treatment. The herbal medicines mainly used to treat alopecia were Gagam Cheongyoung-tang, Gagam Hwajung-hwan, and Yukmijihwang-tang·hwan. The acupoints primarily used were GV20, EX-HN1, GB5, KI3, PC6, ST36, GV22, and A-shi. The most commonly used pharmacopuncture therapies were Hwangryunhaedoktang (HH), Carthami Fructus (CF), Bee Venom (BV), and Hominis placenta (HP). The Korean medical treatment for alopecia improved the condition of patients. However, seven studies reported the occurrence of side effects such as pruritus, dazed, drowsiness, headache, pain, and diarrhea. This study shows the potential of Korean medicine for the treatment of alopecia. Further studies with a large sample size and long-term follow-up are warranted to establish the primary treatment guidelines and objective outcome measures for alopecia.

A Case Report with Long-term Korean Medical Treatment of Systemic Psoriasis Patient (장기간의 한의 치료를 시행한 전신성 건선 환자 치험 1례)

  • Objectives: The aim of this study is to present one case report on the treatment of psoriasis. Methods: We did retrospective study for one psoriasis patient who had been treated by herbal medicine and acupuncture for more than one year. The patient was treated with Bojungikgi-tang hap Gamisoyo-san, Yukmijihuang-tang hap Gamisoyo-san, Saengmaek-san hap Gamisoyo-san, Gamisoyo-san hap Samul-tang and Gamisoyo-san hap Yukmijihwang-tang. Evaluation was done by PASI score and pictures. Results: After the treatment, the patient showed improvements in PASI. PASI of the patient changed from 28.2 to 0. Conclusions: This study suggests the clinical use of Korean medical treatment in systemic psoriasis.

Clinical Study for YMG-1, 2's Effects on Learning and Memory Abilities (육미지황탕가감방-1, 2가 학습과 기억능력에 미치는 영향에 관한 임상연구)

  • The aim of this study was to examine the memory and attention enhancement effect of YMG-1 and YMG-2, which are modified herbal extracts from Yukmijihwang-tang (YMJ). YMJ, composing six herbal medicine, has been used for restoring the normal functions of the body to consolidate the constitution, nourishing and invigorating the kidney functions for hundreds years in Asian countries. A series of studies reported that YMJ and its components enhance memory retention, protects neuronal cell from reactive oxygen attack and boost immune activities. Recently the microarray analysis suggested that YMG-1 protects neurodegeneration through modulating various neuron specific genes. A total of 55 subjects were divided into three groups according to the treatment of YMG-1 (n=20), YMG-2 (n=20) and control (C; n=15) groups. Before treatments, all of subjects were subjected to the assessments on neuropsychological tests of K-WAIS test, Rey-Kim memory test, and psychophysiological test of Event-Related Potential (ERP) during auditory oddball task and repeated word recognition task. They were repeatedly assessed with the same methods after drug treatment for 6 weeks. Although no significant effect of drug was found in Rey-Kim memory test, a significant interaction (P = .010, P < 0.05) between YMG-2 and C groups was identified in the scores digit span and block design, which are the subscales of K-WAIS. The very similar but marginal interaction (P = .064) between YMG-1 and C groups was found too. In ERP analysis, only YMG-1 group showed decreasing tendency of P300 latency during oddball task while the others tended to increase, and it caused significant interaction between session and group (p= .004). This result implies the enhancement of cognitive function in due to consideration of relationship between P300 latency and the speed of information processing. However, no evidence which could demonstrate the significant drug effect was found in neither amplitude or latency. These results come together suggest that YMG-1, 2 may enhance the attention, resulting in enhancement of memory processing. For elucidating detailed mechanism of YMG on learning and memory, the further studies are necessary.

Effect on Gene Expression Profile of Rat Hippocampus Caused by Administration of Memory Enhancing Herbal Extract (육미지황탕가미방이 흰쥐의 기억능력과 중추신경계 유전자 발현에 미치는 영향)

  • The herbal extract (YMT_02) is a modified herbal extracts from Yukmijihwang-tang (YMJ) to promote memory-enhancing. The YMJ extracts has been widely used as an anti-aging herbal medicine for hundred years in Asian countries. The purpose of this study is to; 1) quantitatively evaluate the memory-enhancing effect of YMT_02 by behavior task, 2) identify candidate genes responsible for enhancing memory by cDNA microarray and 3) assess the anti-oxidant effect of YMT_02 on PC12 cell. Memory retention abilities are addressed by passive avoidance task with Sprague-Dawley (SD) male rat. Before the training session, the rats are subdivided into four groups and administrated with YMT_02, Ginkgo biloba, Soya lecithin and normal saline for 10 days. The retention test was performed. 24 hours after the training session. The retention time of the YMT_02 group was significantly (p<0.05) delayed (~100%), whereas Ginkgo biloba and Soya lecithin treatment delayed 20% and 10% respectively. The hippocampi of YMT_02 and control group were dissected and mANA was further purified. After synthesizing cDNA using oligo-dT primer, the cDNA were applied to Incyte rat GEMTM 2 cDNA microarray. The microarray results show that prealbumin(transthyretin), phosphotidylethanolamine N-methyltransferase, and PEP-19 are expressed abundantly in the YMT_02 treated group. Especially, PEP-19 is a neuron-specific protein, which inhibits apoptotic processes in neuronal cell. On the other hand, transcripts of RAB15, glutamate receptor subunit 2 and CDK108 are abundant in control group. Besides, neuronal genes involved in neuronal death or neurodegeneration such as neuronal-pentraxin and spectrin are abundantly expressed in control group. Additionally, the YMT_02 shows an anti oxidative effect in the PC12 cell. The list of differentially expressed genes may implicate further insight on the action and mechanism behind the memory-enhancing effect of herbal extracts YMT_02, for example, anti-apoptotic, anti-oxidative, and neuroprotective effects.

Clinical Study of 25 Interstitial Cystitis Patients (간질성 방광염 환자 25예에 대한 임상적 고찰)

  • Background : Interstitial cystitis is a disorder of the bladder characterized by urgency and frequency of urination, and pelvic pain. The cause of interstitial cystitis is not clear. Modern medical treatments include behavioral therapy, medication, intravesical infusion therapy, and surgical treatment, but the goal of treatment is based on symptom relief rather than a cure. According to a recent report, in an animal model, oriental medicine treatment, Yukmijihwang-tang improved the symptoms of interstitial cystitis. However, clinical studies and experimental research are lacking. Objectives : In this study, We investigated Chuknyojetong-tang, acupuncture, moxibustion and bee venom effect on interstitial cystitis. Methods : We targeted 25 patients diagnosed with interstitial cystitis at the university hospital and treated them for more than one month. We used acupuncture, Chuknyojetong-tang, and bee venom. The effects were evaluated using ICSI and ICPI at the begin of treatment and at the end of treatment. Results : All of the 25 patients were female, average age was $53.3{\pm}13.5$ years, and the average month of disease-duration was $69.0{\pm}59.8$. The duration of treatment was $8.0{\pm}5.4$ months, and 60% of patients were improved. After treatment, we used correlation coefficient method, multiple regression analysis. ICSI difference increased as treatment period increased. ICPI difference increased as age decreased. Total difference also increased as treatment period increased. These results imply that treatment effect was improved. Conclusions : These findings suggest that oriental medicine therapy using Chuknyojetong-tang, acupuncture, moxibustion, and bee venom would be very effective on interstitial cystitis.

Studies on Gene Expression of baicalin treated in HL-60 cell line using High-throughput Gene Expression Analysis Techniques (Baicalin을 처리한 HL-60 백혈병 세포주에서 대규모 유전자 분석 발현 연구)

  • Baicalin, a biologically active flavonoid form the roots of Scutallaria baicalensis (Skullcap), have been reported to not only function as anti-oxidants but also cause anticancer effect. We investigated the mechanism of baicalin-induced cytotoxicity and the macro scale gene expression analysis in leukemia cell line, HL-60 cells. Baicalin (10 μM) were used to treat the cells for 6h, 12h, 24h, 48h and 72h. In a human cDNAchip study of 65,000 genes evaluated 6, 12, 24, 48. 72 hours after treated with Baicalin in HL-60 cells. Hierarchical cluster against the genes which showed expression changes by more than two fold. One hundred one genes were grouped into 6 clusters according to their profile of expression by a hierarchical clustering algorithm. For genes differentially expressed in response to baicalin treatment, we tested functional classes based on Gene Ontology (GO) terms. This study provides the most comprehensive available survey of gene expression changes in response to baicalin treatment in HL-60 cell line.
