Direct and Indirect Costs of Chronic Obstructive Pulmonary Disease in Korea

  • Background: Understanding the burden of disease is important to establish cost-effective treatment strategies and to allocate healthcare resources appropriately. However, little reliable information is available regarding the overall economic burden imposed by chronic obstructive pulmonary disease (COPD) in Korea. Methods: This study is a multicenter observational research on the COPD burden in Korea. Total COPD costs were comprised of three categories: direct medical, direct non-medical, and indirect costs. For direct medical costs, institutional investigation was performed at 13 medical facilities mainly based on the claims data. For direct non-medical and indirect costs, site-based surveys were administered to the COPD patients during routine visits. Total costs were estimated using the COPD population defined in the recent report. Results: The estimated total costs were approximately 1,245 million US dollar (1,408 billion Korean won). Direct medical costs comprised approximately 20% of the total estimated costs. Of these, formal medical costs held more than 80%. As direct non-medical costs, nursing costs made up the largest percentage (39%) of the total estimated costs. Costs for COPD-related loss of productivity formed four fifths of indirect costs, and accounted for up to 33% of the total costs. Conclusion: This study shows for the first time the direct and indirect costs of COPD in Korea. The total costs were enormous, and the costs of nursing and lost productivity comprised approximately 70% of total costs. The results provide insight for an effective allocation of healthcare resources and to inform establishment of strategies to reduce national burden of COPD.

  • The anterior guidance is one of the important factors in prosthodontic treatment of anterior teeth. The lingual surface shape of anterior restorations is so critical that small errors of treatment procedure can cause discomfort of the patient and disharmony of the dentition. If the anterior restorations are achieved harmonious anterior guidance through the fabrication and adjustment of provisional restorations, it is important to accurately reproduce the lingual surface shape of provisional restorations as the final prosthesis. In this case report, it was used the modified Dahl principle and copy-milled technique of computer-aided design/computer-aided manufacturing (CAD/CAM) system for systematic diagnosis and treatment. Therefore, we tried to reconstruct the restoration shape more precisely by setting the appropriate anterior guidance and superimposing the digital image of the abutment teeth and the provisional restorations. Thus, by promoting functional and esthetic recovery, this case report demonstrates satisfying results to both the patients and dentist.

Effects of Task Training for Cognitive Activation of Stroke Patients on Upper Function and Activities of Daily Living (뇌졸중 환자의 인지활성화를 위한 과제 훈련이 상지기능 및 일상생활동작에 미치는 영향)

  • The purpose of this study is to investigate the effect of task training for cognitive activation of upper extremity on upper limb function and activities of daily living and to suggest intervention in rehabilitation treatment. From January to February of 2018, nine stroke patients were arbitrated 30 minutes a day, five days a week, for four weeks. For the experimental group, the therapist has induced the group using the linguistic guidance to patients, so they utilize the cognition strategy. The control group conducted active exercises in a range of motion using the instruments and passive exercises in a range of motion to reduce the stiffness of joints and upper limbs. As muscle strengthening exercises, the patients were assigned to work on the biceps muscle of arm, triceps muscle of arm, and deltoid according to the individual patient's muscular strength level. For the experimental group, the MBI was improved by ten points at maximum, and K-AMPS motor skills showed the improvements of 1.0 logit at maximum, and processing skills showed improvements of 0.6 logits at maximum. In MFT, the maximum improvement was by two points. For the control group, MBI was improved by five points at maximum, and 0.2 maximum improvements were shown in K-AMPS' motor skills and 0.3 maximum improvements in processing skills. MFT showed no change. The conclusion is that the challenges to enable training for stroke patients give a positive impact on upper limb function and activities of daily living.

  • The present study attempted to identify how the knowledge and risk perception of nursing students regarding COVID-19 impact COVID-19 preventive behaviors. This study collected data from May 1st to September 20th, 2021, using a self-reported questionnaire targeting 184 nursing students. The collected data were analyzed using Independent t-test, One-way ANOVA, and Multiple regression. The research results indicated that the knowledge(r=.15, p=.019) and risk perception(r=.13, p=.036) of nursing students regarding COVID-19 had a positive correlation with COVID-19 preventive behaviors. The factor that impacted the COVID-19 preventive behaviors of the subjects was COVID-19 knowledge, which had an explanatory power of 4%. Therefore, it is necessary to develop an educational intervention program that enables nursing students to obtain the correct knowledge regarding COVID-19 and improves preventive behaviors.

  • Background: During squat exercises, Ankle joint stability during squat movement transfers forces produced by unstable supports or various loads to the leg joints and trunk, reducing the risk of injury; therefore, a reference is needed for correct ankle joint posture during squat exercises. The purpose of this study was to examine the effect of ankle joint stability on quadriceps femoris and hamstring activation during squat exercises. Design: Quasi-experiment one group pretest-posttest design. Methods: The participants in this study were 20 volunteers who performed squat exercises with and without kinesiology tape. We measured quadriceps femoris and hamstring activation using surface electromyography. The ankle joint was stabilized with kinesiology tape using the ankle balance taping (ABT) method. A paired t-test was performed to compare differences between taping conditions. Results: Vastus medialis and vastus lateralis activation were greater following squat exercises with kinesiology tape than without; however, the difference was not significant. Medial and lateral hamstring activation was not significantly different between taping conditions. Conclusion: Although it is difficult to maintain stability using kinesiology tape alone, ankle joint stability is believed to affect quadriceps femoris muscle activity, which increases the effectiveness of the squat exercise.

Management of prosthodontic patients with severe gag reflex using the questionaire and behavior technique: A case report (심한 구역반사를 보이는 환자에서 문진표와 행동 요법을 이용한 임플란트 고정성 보철 수복 증례)

  • Although gag reflex is an essential function and a useful physiological defense mechanism, it can become an obstacle in a dental treatment. In this case, a questionnaire was designed and used in consultations to objectify the factors associated with gag reflex. Based on the questionnaire, various treatment protocols were planned, such as behavioral control, which is a systematic desensitization that adapts by placing dental instruments in the oral cavity, and habit control to eliminate vomiting habits after drinking. Except for the placement of the implant fixture under general anesthesia, all restoration procedures were performed in the outpatient clinic in the same manner as the conventional implant fixed prosthetic restoration. The systematic desensitization and behavior technique were repeated until the patient was able to endure discomfort and relieve gag reflex in dental treatment. The patient was successfully received the implant-fixed prosthetic restoration in the partial edentulous region.

Photoreversibility of Fruiting and Growth in Oriental Melon (Cucumis melo L.)

  • BACKGROUND: Photoreversibility, a reversion of the inductive effect of a brief red light pulse by a subsequent far red light pulse, is a property of photo responses regulated by the plant photoreceptor phytochrome B. Plants use photoreceptors to sense photo signal and to adapt and modify their morphological and physiological properties. Phytochrome recognizes red light and far red light and plays an important role in regulating plant growth and development. METHODS AND RESULTS: The reversal responses of growth and fruiting characteristics were investigated to increase the yield of oriental melon (Cucumis Melo L. var. Kumsargakieuncheon) by means of controlling light quality in a plastic house. Red (R:660nm) and far red (FR:730nm) lights were subsequently irradiated on the whole stems and leaves of the oriental melon plant during growing periods, using red and far red LEDs as light sources, from 9:00 PM daily for 15 minutes. The intensities of R and FR light were 0.322-0.430 μmol m-2s-1 and 0.250-0.366 μmol m-2s-1, respectively. Compared to R light irradiation, combination of R and FR light irradiation increased the length of internode, number of axillary stems, number of female flowers, and fruit number of oriental melons. The results of treatment with R were similar to R-FR-R light irradiation in terms of length of internode, number of axillary stems, number of female flowers, and number of fruits. When FR treatment was considered, R-FR and R-FR-R-FR light irradiation had similarities in responses. These reversal responses revealed that oriental melon showed a photoreversibility of growth characteristics, flowering, and fruiting. CONCLUSION: These results suggested the possibility of phytochrome regulation of female flower formation and fruiting in oriental melon. The fruit weight of the oriental melon was the heaviest with the R light irradiation, while the number of fruits was the highest with the FR light. With the FR light irradiation, the fruit weight was not significantly higher compared to that of the control. Meanwhile, the yield of oriental melon fruits increased by 28-36% according to the intensities of the FR light due to the increases of the number of fruits.

Comparing the Effects of Ventilation and Air Purification Plants on Radon Concentration in the Lower and Upper Floors of a Building (건물 저층과 고층에서 환기와 공기정화 식물을 통한 라돈 농도의 비교)

  • The objective of this study was to quantitatively measure the changes in radon concentration due to ventilation and air purification plants in the lower and upper floors of a building. This study measured and compared radon concentration in the lower and upper floors of the building by using a radon meter when the room was closed, it was ventilated, and air purification plants were installed at a specific time. One-way ANOVA was conducted to evaluate the effect of treatment (i.e., closure, ventilation, and air purification plants) on radon concentration. The results of this study showed that ventilation and air purification plants significantly decreased radon concentration in the lower and upper floors of the building, but the effect of ventilation and that of air purification plants were not significantly different. Therefore, it will be possible to reduce radon concentration effectively when ventilation and air purification plants are used appropriately.

The Relationship between Social Support, Positive Thinking and Subjective Happiness of Nursing College Students (간호대학생의 사회적 지지, 긍정적 사고와 주관적 행복감 간의 관계)

  • This is a descriptive study to investigate the relationship between social support, positive thinking and subjective happiness of nursing college students. Data collection was targeted to 140 nursing college students from 2nd, to 4th grade in two colleges in K area and the data collection period was from June 3, 2019, to July 9, 2019. The collected data were analyzed using SPSS WIN 21.0 program. The result of this study showed that the average degree of social support was 3.86 points, positive thinking was 3.52 points, subjective happiness was 4.68 points. Among the sub-domains of each variable, emotional support was the highest with 3.92 points in social support, and goal pursuit was the highest with 3.69 points in positive thinking. In addition, the highest score among the subjective happiness items was 'generally happy' with 4.92 points. The social support of nursing students showed a significant positive correlation with positive thinking(r=.48, p<.001) and subjective happiness(r=.49, p<.001). In addition, positive thinking also showed a significant positive correlation(r=.69, p<.001) with subjective happiness. Therefore, this study proposes the development of a program that can improve social support and positive thinking so as to improve the subjective happiness of nursing students.

Fusion correlation between the degree of depression and oral health behavior, autologous oral health symptoms, and quality of life due to COVID-19 (COVID-19로 인한 우울정도와 구강건강행위, 자가구강건강증상, 삶의 질의 융합적인 관련성)

  • This study aims to provide basic data to identify the effects of oral health behavior and autologous oral health symptoms on factors that affect quality of life of COVID-19 (coronavirus disease 19) depression. This study collected data from July 2020 to September 2020, the research method was finally analyzed for 89 people in the group of depression non-increase and 86 people in the group of depression due to COVID-19. As an analysis method, oral health behavior, autologous oral health symptom and quality of life were compared, and the hierarchical regression analysis was implemented to check the effect of depression on the quality of life. As a result, the effects of depression (��=-0.155, p=0.012) and autologous oral health symptoms (��=0.524, p<0.001) were significant as a result of controlling demographic characteristics and checking the effects on quality of life. Therefore, it is urgent to develop programs related to the mental health of the community to relieve depression and anxiety with COVID-19, and public relations activities will be needed to relieve anxiety about dental visits so that dental treatment can be received at an appropriate time.
