Purpose: The purpose of this study was to deepen understanding the nature of the care experience by the mothers with adult children in persistent vegetative state. Methods: Participants were 7 mothers caring for their adult children in persistent vegetative state. Data were collected individually through in-depth interviews on their lived experiences. Also texts were included as data from literary works, novels, movies, essays, and arts containing on patients with persistent vegetative state and their mothers. Data were analyzed by van Manen's phenomenological methodology. Results: The essential themes of caring experiences of the mothers were as follows. The theme in relation to lived time has shown as back to the past and caring experience related lived body has emerged as locked the body in children. The theme related lived space was getting into the swamp and the theme in lived others was derived as lonely struggle into. Conclusion: The nature of mothers' caring experiences for adult children in persistent vegetative state is summarized as 'Do not off hand of hope in a locked state'. This study suggests long-term supports are necessary for mothers to care persistent vegetative state children.
The present study investigated the parenting alliance's mediating effects on the relationship between adult attachment and marital satisfaction. The participants were 157 married parents of young children(77 men and 80 women). We used the Experiences in Close Relationships Scales (Kim & Lee, 2005), Quality Marriage Index (Jang, 2001), and Parenting Alliance Inventory (Abidin, 1988) and analyzed data using descriptive statistics, Pearson's correlation, and regression analysis, via SPSS PC program(17.0 version). The results revealed that the effects of married males' avoidant dimension on marital satisfaction were partially mediated by their wives' parenting alliance. The effects of married females' anxious dimension on marital satisfaction were totally mediated, and the effects of married females' avoidant dimension on marital satisfaction were partially mediated, by their husbands' parenting alliance. In conclusion, the parenting alliance mediates the effects of adult attachment on marital satisfaction in couples with young children.
Purpose: The purpose of the study was to explore and describe the experience of adult Korean children who are caregivers for parents institutionalized with dementia. Methods: Participants were fourteen adult children caregivers of elders institutionalized with dementia. Data were collected through in-depth unstructured interviews with individual participants from August to November, 2012. Theoretical sampling was used to the point of theoretical saturation. Data were analyzed using Strauss and Corbin's Grounded Theory Method. Results: From open coding, 67 concepts, 29 sub-categories, and 14 categories were identified. Analysis revealed that the core category of the experience of adult children caring for their parents institutionalized with dementia was 'enduring the role of a prop' consisting of four phases: initial turmoil, exploration, role adjustment, and acclimation. To manage the role of a prop, participants utilized various action/interactional strategies such as overcoming the unfamiliarity, overseeing the nursing home care, and counterbalancing the caring roles. As a result, participants experienced ambivalence towards the existence of parents with dementia, changes in family relationships, altered viewpoint towards nursing homes, and restructuring of life. Conclusion: In-depth understanding of the experience will guide nurses to promote effective interventions in order to better support the Korean family caregivers of parents institutionalized with dementia.

The purpose of this study is to examine factors affecting the conflict between grandparents raising grandchildren and adult children. The data were based on the panel survey to explore korean retirement and income study conducted by National Pension Service in 2009. For this study, 287 parenting grandparents rearing grandchild are selected from the survey. Included variables are a demographic factor, a economic factor, a caregiving-related factor, a health factor, and a family relationship factor. The data were analyzed using descriptive statistics, t-test, co-relation and logistic regression, with SPSS WIN 18.0 program. The results are as follows: First, when grandparents are male, older, highly educated, having no religion, and unemployed, a conflict with their adult children grows. Second, when grandparents do not have earned income or financial income, but have private income transfers, a conflict with their adult children is high. Third, when grandchildren are younger, there are no caregiving rewards, economic activities suspension or reduction because of caregiving, a conflict with their adult children is strong. Also, when grandparents' physical health is good, but their mental health is poor, a conflict with their adult children increases. Fourth, when grandparents' satisfactions with family relationship and spouse relationship are low, but their satisfaction with adult children relationship is high, a conflict with their adult children rises. This study suggests that unlike in the past, grandparents raising grandchildren can no longer make sacrifice themselves for their adult children, but they expect proper exchange between parents and children. Therefore, there needs to be understanding of grandparents raising grandchildren and further studies of a conflict between grandparents raising grandchildren their adult children.
Obstructive sleep apnea (OSA) in children is a frequent disease for which optimal diagnostic methods are still being defined. Treatment of OSA in children should include providing space, improving craniofacial growth, resolving all symptoms, and preventing the development of the disease in the adult years. Adenotonsillectomy (T&A) has been the treatment of choice and thought to solve young patient's OSA problem, which is not the case for most adults. Recent reports showed success rates that vary from 27.2% to 82.9%. Children snoring regularly generally have a narrow maxilla compared to children who do not snore. The impairment of nasal breathing with increased nasal resistance has a well-documented negative impact on early childhood maxilla-mandibular development, making the upper airway smaller and might lead to adult OSA. Surgery in young children should be performed as early as possible to prevent the resulting morphologic changes and neurobehavioral, cardiovascular, endocrine, and metabolic complications. Close postoperative follow-up to monitor for residual disease is equally important. As the proportion of obese children has been increasing recently, parents should be informed about the weight gain after T&A. Multidisciplinary evaluation of the anatomic abnormalities in children with OSA leads to better overall treatment outcome.
Recently increasing young adults have experienced serious economic difficulties in their transition to adulthood and so a great deal of parents have to help their children financially even well after children finish their schools. Utilizing the 2008 Social Survey, a nationwide representative survey collected by the Statistics Korea, this paper examines how children, mothers, and fathers think about parental support to adult children. Respondents of the 2008 Social Survey answer how much parents support economically to their children about the following four items: college education, graduation school education, spending money while searching for jobs, and marital preparation. Analyzing a sample of 1,727 high school students and their parents, I find that the three family members generally accept economic support to the above items as part of parental duties. Also comparisons of the three family members' attitudes indicate that they usually reach the consensus about the dutiful scope of parental support to adult children. Logistic regressions reveal that male teens and children with conservatism are more likely to believe in wider scope of parental economic duties. And parents who are on good terms with children and are high in educational and occupational levels are more willing to support adult children. I conclude with implications of the findings for intergenerational relationships.
Significant advances in the diagnosis and medical care of children with chronic kidney disease (CKD) are major reasons for the better survival rates of children and adolescents with CKD than the survival rates reported in previous decades. These patients are reaching adulthood, and therefore require a transition to adult medical care. This transition phase is well-recognized to be associated with considerably increased morbidities and medical problems, such as non-adherence, graft loss after transplantation, and loss to follow-up. Low adherence increases morbidity and medical complications and contributes to poorer qualities of life and an overuse of the health care system. However, these tragic outcomes may be avoidable through a structured and well-defined transition program. In the last decade, there has been increasing interest to resolve these medical and psychological problems that occur during the transfer of young adult patients from pediatric to adult renal units. The aims of a successful transition from pediatric to adult medical care include enhancing the individual development of better health-competence and stabilizing, or even improving, the state of health. This review will focus on various aspects of the transition phase of adolescents who have CKD or who underwent kidney transplantation from pediatric to adult nephrology care.
Nutritional assessment is based on anthropometric, clinical, dietary and biochemical data. There is a lack of studies about the propriety of biochemical indexes for the nutritional assessment in children despite biochemical data in pediatric population are different from them in adult's in many respects. Serum albumin is useful index to evaluate the severity of malnutrition. Hemoglobin and hematocrit tend to decrease in malnutrition on account of defect of iron metabolism and to increase in metabolic syndrome on account of enhancement of erythropoiesis. But, unlike adult, total lymphocyte count is not so useful biochemical indexes in children. We should consider pediatric characteristic when interpret biochemical indexes for nutritional assessment in children, and nutritional status in children should be assessed comprehensively with anthropometric, clinical, dietary and biochemical data.
The purpose of this study is to investigate the relationship between adult children's perceived interaction pattern with their parents and their marital satisfaction. The subjects are a total of 646 male and female who been married for 3 years or less, living in Seoul, and whose fathers or mothers are still alive. The questionaire, including of attachment, autonomy, marital satisfaction scales, are distributed to collect the data. The main results are as follows: 1. Intimate-Dependent type is 31% each in relation to fathers and mothers. Distant-Independent type is 30% in relation to fathers, 27% in relation to mothers. Balanced type is 21% in relation to fathers, 23% in relation to mothers. Undeveloped type is 18% in relation to fathers, 19% in relation to mothers. 2. there is interaction effect between parental sex and adult children's sex as related to attachment levels, autonomy levels. 3. There is statistically significant difference among attachment-autonomy patterns as related to adult children's marital satisfaction. 4. Attachment and autonomy in relation to fathers and attachment in relation to mothers affect sons' marital satisfaction positively. Attachment in relation to fathers and mothers affect positively to daughters' marital satisfaction.
Background: The aim of the present study was to evaluate the presentation, clinical course and outcome between children and young adults with differentiated thyroid cancer (DTC) treated in our hospital. Materials and Methods: The medical records of 145 patients with DTC who underwent surgery followed by radioiodine and thyroid hormone (TSH) suppression were retrospectively reviewed. The follow up was between January 2006 and June 2012. These patients consisted of 38 children (age${\leq}18y$) and 107 young adult patients (age${\leq}30y$). The clinical characteristics and outcome were analyzed and compared, and the progression-free survival (PFS) was evaluated using the Kaplan-Meier method. Results: At initial diagnosis, a greater degree of extra thyroidal extension was found in children than adults patients (p<0.001). However, there was no significant difference between the two groups with regard to the tumor size and the presence of lymph node or distant metastasis (p=0.172, p=0.050 and p=0.068, respectively). The extent of surgery and the cumulative or mean dose of radioiodine were similar in both groups. During the follow up, the overall survival rate was 100% for both groups, and the PFS rate was similar in children and in young adults group (log rank test, ${\chi}^2$=0.126, p=0.723). Conclusions: In comparison to the young adult patients, DTC in children presents with more aggressive behavior, but outcomes are similar between the two groups after the intensive management of surgery followed by radioiodine and TSH suppression therapy.
