Dose Distribution of Wedge filter by Dose Rate in LINAC (선형가속기의 선량율에 따른 쐐기필터의 선량분포)

  • This study is aimed at assessing whether dynamic wedge filters are appropriate to be used instead of physical wedge filters. The X-ray energy generated from linear accelerator is commercialize 6 MV and 10 MV. $15^{\circ}$, $30^{\circ}$, $45^{\circ}$, and $60^{\circ}$ of physical wedge filter was irradiated by dose rate 100, 200, 300, 400, 500, and 600 MU/min for each angle and for comparison with a dynamic wedge filter, irradiate 96 times under the same conditions. The measurement conditions are as 100 cm source-film distance and $10{\times}10cm$ irradiated surface. A developed film was scanned and analyzed after a calibration through a dose analysis program and the dose rate was compared after calculating the standard deviation. Dynamic wedge filters make dose, scattered rays and treatment time reduced and very useful due to less irradiated doses to patients. The errors at each dose rate under the same conditions were irrelevant. Thus, treatment based on a high dose rate depending on the patient is expected to be feasible.

Comparison of Dose and Quality of Copper and Nickel Additional Filter Plate in Diagnostic X-ray Generator (진단용 엑스선 발생장치에서 부가 여과판에 따른 선량과 화질 비교)

  • The purpose of this study was to evaluate the difference of dose and image quality according to the material of the additional filter plate by selecting copper and nickel. First, the absorbed dose was measured using a Rando phantom setting the additional filter plates of copper and nickel None, 0.1 mm, 0.2 mm, and 0.3 mm under 120 kVp, and 6.3 mAs. Second, We acquired image according to filter thickness of copper and nickel. by changing the tube voltage of 90 kVp, 100 kVp, 110 kVp, 120 kVp and exposure indexes of 400, 800 and 1600. Third, we obtained the SNR and CNR values using the Image J program and evaluated quantitatively and then evaluated image quality. As a result, Absorbed dose measurements showed that nickel was higher than copper, and the absorbed dose decreased as the thickness increased(p<0.05). Furthermore, Quantitative analysis of images showed no significant difference between the two images according to change the voltage and the exposure index(p>0.05). In conclusion, this study confirms that the nickel addition plate can maintain the current image quality while reducing the exposure dose compared to copper.

Bioimpedence to Assess Breast Density as a Risk Factor for Breast Cancer in Adult Women and Adolescent Girls

  • Although high mammographic density is one of the strongest predictors of breast cancer risk, X-ray based mammography cannot be performed before the recommended screening age, especially not in adolescents and young women. Therefore, new techniques for breast density measurement are of interest. In this pilot study in Guam and Hawaii, we evaluated a radiation-free, bioimpedance device called Electrical Breast Densitometer$^{TM}$ (EBD; senoSENSE Medical Systems, Inc., Ontario, Canada) for measuring breast density in 95 women aged 31-82 years and 41 girls aged 8-18 years. Percent density (PD) was estimated in the women's most recent mammogram using a computer-assisted method. Correlation coefficients and linear regression were applied for statistical analysis. In adult women, mean EBD and PD values of the left and right breasts were $230{\pm}52$ and $226{\pm}50{\Omega}$ and $23.7{\pm}15.1$ and $24.2{\pm}15.2%$, respectively. The EBD measurements were inversely correlated with PD ($r_{Spearman}=-0.52$, p<0.0001); the correlation was stronger in Caucasians ($r_{Spearman}=-0.70$, p<0.0001) than Asians ($r_{Spearman}=-0.54$, p<0.01) and Native Hawaiian/Chamorro/Pacific Islanders ($r_{Spearman}=-0.34$, p=0.06). Using 4 categories of PD (<10, 10-25, 26-50, 51-75%), the respective mean EBD values were $256{\pm}32$, $249{\pm}41$, $202{\pm}46$, and $178{\pm}43{\Omega}$ (p<0.0001). In girls, the mean EBD values in the left and right breast were $148{\pm}40$ and $155{\pm}54{\Omega}$; EBD values decreased from Tanner stages 1 to 4 ($204{\pm}14$, $154{\pm}79$, $136{\pm}43$, and $119{\pm}16{\Omega}$ for stages 1-4, respectively) but were higher at Tanner stage 5 ($165{\pm}30{\Omega}$). With further development, this bioimpedance method may allow for investigations of breast development among adolescent, as well as assessment of breast cancer risk early in life and in populations without access to mammography.

The Effects of Metal Plate loaded on TLD chip in 6 MV Photon and 6 MeV Electron Beams (6 MV 광자선과 6 MeV 전자선 하에서 TLD 기판 위에 얹힌 금속 박막의 효과)

  • There is necessity for making a smaller and more sensitive detector in small field sizes. This report assesses the suitability of metal-loaded thermoluminescent dosimeters for this purpose. Measurements were performed in the 6 MV photon and 6 MeV electron beams of a medical linear accelerator with LiF thermoluminescence dosimeters (TLD-100) embedded in solid water phantom. TLD-100 chips(surface area 3.2 $\times$ 3.2 $\textrm{mm}^2$) loaded with a metal plate(Tin or gold respectively) were used to enhance dose readings to TLD-100. Surface dose was measured for field size 10 $\times$ 10 $\textrm{cm}^2$ and 100 em SSD. Measurements have been made of the enhanced signal intensity and good linearity for absorbed dose with each metal. Using a 1 mm each metal on TLD-l00 in the beam increased the surface dose to 14% and 56% respectively for 6MV photon. In the case of 6 MeV electron, gold plate enhanced the TL response to 13%, but there is no difference for tin plate. The specific dose response of TLD-100 with thin metal plate increases with electron concentration of metal film, this is most likely due to increased electron scattered from the additional material with electron density higher than TLD-100. This emphasizes the role of TL dosimeters with metal as amplified dosimeters for therapeutic high energy x-ray beams. Due to the enhanced dose reading of TLD-100 with metal plate, it could be possible to develop smaller TL dosimeter with high sensitivity.

Clinical Study on 32 Patients of Lumbar Herniated Intervertebral Disc with Spondylolisthesis (척추전방전위증을 동반한 요추간판 탈출증 환자 32례에 대한 한의학적 임상고찰)

  • The purpose of this study is to survey the effectiveness of oriental medicine treatment on lumbar herniated intervertebral disc and spondylolisthesis. The clinical study was performed on 32 cases of patients with lumbar herniated intervertebral disc and spondylolisthesis as diagnosed by X-ray, Magnetic resonance imaging(MRI) exams amongst the patients who were admitted to Jaseng Hospital of Oriental Medicine from January 2010 to June 2010. 32 cases of patients were analyzed according to the distribution of sex, age, duration of disease, contributory factor, symptoms, admission period, disc herniation type and level of spondylolisthesis. The efficacy of treatment was evaluated respectively. The number of the female patients were larger than that of male with the ratio of 1:0.6. Most of the patients were in their fifties(43.8%), choronic phase(46.9%), those patients suffered from lower back pain and leg pain radiation(90.6%), reasons unknown(56.3%). Most of them also had mixed disc(37.5%), bulging disc(37.5%) and spondylolisthesis of LS on L5(50.0%). Most of them stayed in the hospital for 21-25days. After treatment, the percentage with good improvement was 62.5%, fair 18.8%, poor 12.5%, and excellent 6.3%, respectively. Oriental medicine treatment has on useful effect on the recovery of lumbar herniated intervertebral disc with spondylolisthesis.

A Study on the Luminescence Properties of LiGd9(SiO4)6O2:Ce3+ (LiGd9(SiO4)6O2:Ce3+ 형광 특성 연구)

  • $LiGd_9(SiO_4)_6O_2:Ce^{3+}$ phosphors were synthesized by solid-state reaction method. The structural characteristic was investigated by X-ray powder diffraction analysis. The emission and excitation spectra of the $Ce^{3+}$ ions doped $LiGd_9(SiO_4)_6O_2$ phosphors were obtained under the UV excitation. The emission spectra of $LiGd_9(SiO_4)_6O_2:Ce^{3+}$ shows the band at 410 nm corresponding to the $^2F_{5/2}$ and $^2F_{7/2}$ states of $Ce^{3+}$. The red shift of $Ce^{3+}$ emission is found as the $Ce^{3+}$ concentration increases, which could be explained by the change in crystal-field symmetry and strength with increasing $Ce^{3+}$ concentration. Fluorescence decay time of $Ce^{3+}$ was about 20 ns. When the concentration of $Ce^{3+}$ increases life time was slightly reduced.

Study on the Change of Absorbed Dose and Image Quality according to X-ray Condition of Detector in Digital Radiography(DR) (Digital Radiography(DR)에서 검출기의 X선 조건에 따른 흡수선량 및 영상화질 변화에 관한 연구)

  • This study focused on the issue that when a diagnostic detector is found to have a defect, a patient would be exposed to radiation and image quality would be degraded. Though dose analysis, an experiment was conducted to evaluate detector performance as Signal to Noise Ratio (SNR) and Contrast to Noise Ratio (CNR). Absorbed dose, SNR and CNR were measured using a dosimeter and a tissue equivalent phantom. The experiment was conducted to compare whether the dose value shown after being attached to the back side of the phantom matches the dose value attached behind the detector, where in the conditions of skull, chest and abdomen were set at 75 kVp, 25 mAs, 110 kVp, 8 mAs, and 80 kVp, 20 mAs, respectively. As a result, there was a difference in that the dose values attached to the back side of the detector were 0.004 mGy, 0.006 mGy, 0.003 mGy, whereas those of the back side of the phantom were 0.006 mGy, 0.016 mGy, 0.017 mGy. In order to match both values, the condition was increased and SNR and CNR also increased from 88.32, 88.10, 4.09, 1.63, 87.94, 79.97 to 93.87, 93.75, 4.91, 4.03, 92.02, 84.92. Though this study, we found that when a detector is found to have a aging, it shortens the life of equipment and increases the dose of a patient, also the improvement effect of image quality is inadequate.

CdZnTe Detector for Computed Tomography based on Weighting Potential (가중 퍼텐셜에 기초한 CT용 CdZnTe 소자 설계)

  • Room-temperature operating CdZnTe(CZT) material is an innovative radiation detector which could reduce the patient dose to one-tenth level of conventional CT (Computed Tomography) and mammography system. The pixel and pixel pitch in the imaging device determine the conversion efficiency of incident X-or gamma-ray and the cross-talk of signal, that is, image quality of detector system. The weighting potential is the virtual potential determined by the position and geometry of electrode. The weighting potential obtained by computer-based simulation in solving Poisson equation with proper boundaries condition. The pixel was optimized by considering the CIE (charge induced efficiency) and the signal cross-talk in CT detector system. The pixel pitch was 1-mm and the detector thickness was 2-mm in the simulation. The optimized pixel size and inter-pixel distance for maximizing the CIE and minimizing the signal cross-talk is about $750{\mu}m$ and $125{\mu}m$, respectively.

Spectroscopic Comparison of Photo-oxidation of Outside and Inside of Hair by UVB Irradiation (자외선B 조사에 의한 모발 외부와 내부의 광산화에 관한 분광학적 비교)

  • Hair is made of proteins containing various amino acids. Ultraviolet (UV) radiation is believed to be responsible for the most damaging effects of sunlight, and also plays an important role in hair aging. The purpose of this study was to investigate the changes in morphological and chemical structures after ultraviolet B (UVB) irradiation of human hair. The UVB-irradiated hair showed characteristic morphological and structural changes, compared to those of the normal hair. The result from a scanning electron microscope (SEM) equipped with an energy dispersive X-ray diffractometer (EDX) showed that the scale of UV-irradiated hair appeared to be rough and the amount of oxygen element was higher than that of the normal hair. Fluorescence and three dimensional (3D) topographical images were obtained by a confocal laser scanning microscope (CLSM). In 3D images, the green emission intensity of normal hair was much higher than that of fluorescing UVB-irradiated hair. The intensity of green emission reflects the intrinsic fluorescence of hair protein. Also, a fluorescent imaging method using fluorescamine reagent was used to identify the free amino groups resulting from a peptide bond breakage in UVB-irradiated hair. Strong blue fluorescence of UVB-irradiated hair, which indicates a very high level of amino groups, was observed by CLSM. Therefore, the fluorescamine as an extrinsic fluorescence could provide a useful tool to identify the peptide bond breakage in UVB-irradiated hair. Infrared image mapping was also employed to assess the cross-sections of normal and UVB-irradiated specimens to examine the oxidation of disulfide bonds. The degree of peak areas with strong absorbance for the disulfide mono-oxide was spread from the outside to the inside of hair. The spectroscopic techniques used alone, or in combination, launch new possibilities in the field of hair cosmetics.

A Study on the Distribution of Microorganisms in Department of Radiography (영상의학과 촬영실의 미생물 분포에 관한 연구)

  • To prevent the secondary hospital-acquired infection (cross-infection) from occurring in the general radiographic room in the department of radiology, the microbial measurement was conducted at the points making direct close contact with radiologists and patients. For the case of radiologists, the microbial measurement and incubation were focused on the x-ray tube handle of the radiation generating device, and, for the case of patients, the microbial measurement and incubation were focused on the chin supporting device, chest-contact point, and handle. Once disinfected with Aniosurf, the sterilized media were gathered and identified, and the microorganisms were confirmed. Based on the identification results, it was confirmed that the points making direct close contact with radiologists showed a value of 103 CFU for Proteus mirabilis, Staphylococcus epidermidis, Bacillus spp. and Candida spp., and that the points making direct close contact with patients showed a value of 103~5 CFU for Proteus mirabilis, Enterococcu faecium, Pseudomonas aeruginosa, NTM(Non-Tuberculosis Mycobacteria) and Candida spp.. It was also confirmed that the types and number of microorganisms gathered from the points making direct close contact with patients were greater. Fortunately, most of the involved microorganisms were observed to be on the skin surface and are known to become extinct when disinfected in accordance with the hospital-acquired infection control rules. However, since even minor exposure to such microorganisms may be lethal for patients with reduced immunity, caution must be taken. In particular, since the points making contact with patients showed a high level of microbial measurement, it was thought that it would be necessary for radiologists and personnel having frequent access to strictly disinfect the parts, such as instruments and handles, making frequent contact with patients. The purpose of this study was to announce the importance of safe microbial control in the radiographic inspection room in hospital, and this study is expected to be used as the baseline data for preventing hospital-acquired secondary infection and Nth infectious diseases.
