The Assessment of the Nasopharyngeal Soft Tissue of Normal Korean Adults in the lateral Neck Radiograph (경부 X-선 측면 사진상 정상 한국인 비인강 연부조직 계측)

  • Radiography of nasopharynx are routinely performed for nasopharyngeal soft tissue changes. Although CT scan is widely performed nowadays, the value of lateral neck radiograph is still important to detect the masses in the nasopharynx. The purpose of this study was to establish the constitution of the normal dimension of the nasopharyngeal soft tissue on the lateral neck radiograph and make a parameter of the nasopharyngeal soft tissue hypertrophy. We have made various measurements of the thickness of the nasopharyngeal soft tissue on the lateral skull films in 214 Korean adults (109 males and 104 females). We found that the diameter of the nasopharyngeal soft tissue was decreased by age and the value of males were always greater than that of females and the thickness of the roof was always less than the posterior wall.

  • The middle mediastinum contains several important organs and pluripotent cells. It is difficult to make a definitive diagnosis in patients with middle mediastinal tumors due to a wide range of diseases. The likelihood of malignancy is influenced primarily by the following factors: patient age, size, tumor location, and the presence or absence of symptoms. We describe a case of a middle mediastinal tumor, which was suspected on chest x-ray; chest computed tomography revealed the eccentric mass of distal esophagus. This case emphasizes the diagnostic importance of the chest x-ray to the physicians. The possible differential diagnoses are reviewed.

Noninvasive Evaluation of Coronary Artery Bypass Graft Patency by Electron Beam Tomography (전자선 단층 촬영을 이용한 관상동맥 우회로 개존의 비침습적 평가)

  • Recently non-invasive diagnostic imaging replaced the invasive catheter angiography in the diagnosis of vascular disease. Catheter methods are now almost confined to the purpose of intervention. Coronary artery or coronary artery bypass graft still needs catheter technique because of small diameter and the cardiac motion. The last challenge for radiologists in this domain is to obtain a non-invasive imaging. Electron beam tomography(EBT) for high temporal resolution is able to obtain a coronary arteriogram or coronary artery bypass graft (CABG), of which CABG imaging is quite useful for the evaluation of patency. In our experience as well as others, the accuracy of EBT angiogram in evaluating CABG patency revealed that the accuracy of patency of saphenous vein grafts(SVG) is high due to relatively wide lumen, short and straight course and less influence from cardiac motion. The sensitivity and specificity of patency of SVGs were 92%, 97% respectively in the prospective evaluat on and 100% each in the retrospective evaluation. A false positive and a false negative case are rudimentary errors in the initial learing period. In contrast the analysis of left internal mammary artery(LIMA) graft was difficult due to the inherent small size and the adjacent surgical clips provoking beam-hardening artifact; therefore, the method of combining 3 dimensional reconstruction and flow mode study was important in improving the accuracy of LIMA patency. The sensitivity and specificity of LIMA patency were 100% and 80% in both prospective and retrospective evaluation. Therefore, EBT angiography is an accurate non-invasive diagnostic modality for evaluating the patency of CABG, particularly in SVGs. The accuracy can be improved with the improvement of the EBT and the development of the image reconstruction software.

Evaluation of the Radiopacity of Contemporary Luting Cements by Digital Radiography (디지털방사선촬영술을 이용한 합착용 시멘트의 방사선불투과성 평가)

  • This study examined the radiopacity of eight contemporary luting cements by direct digital radiography. Five disc-shaped specimens ($5mm{\times}1mm$) were prepared for each material tested (BisCem, Clearfil SA Luting, Duolink, Maxcem Elite, Multilink Speed, Panavia F 2.0, RelyX Unicem Clicker, V-link). The specimens were radiographed using a Kodak CS 7600 image plate (Carestream Health, Inc., Rochester, NY, USA) and an aluminum step wedge with a range of thicknesses (1.5 to 16.5 mm in 1.5 mm increments) and a 1 mm tooth used as a reference. A dental X-ray machine Kodak 2200 Intraoral X-ray System (Carestream Health, Inc., Rochester, NY, USA), operating at 70 kVp, 4 mA, 0.156 s and a source-to-sample distance of 30 cm, was used. According to international standards, the radiopacity of the specimens was compared with that of an aluminum step wedge using NIH ImageJ software (available at http://rsb.info.nih.gov/ij/).The data was analyzed by ANOVA and a Tukey's post hoc test. Maxcem Elite (5.66) showed the highest radiopacity of all materials, followed in order by Multilink Speed (3.87) and V-link (2.83). The radiopacity of Clearfil SA Luting (1.35), BisCem (1.33), Panavia F 2.0 (1.29) and Duolink (1.10) were between enamel (1.79) and dentin (0.19). RelyX Unicem Clicker (0.71) showed the lowest radiopacity, which was higher than that of dentin. All materials showed a radiopacity above the minimum recommended by the International Organization for Standardization and the American National Standards/American Dental Association with the exception of RelyX Unicem Clicker.

Surgical Treatment of Pulmonary Actinomycosis Mimicking Pulmonary tuberculosis (폐결핵으로 오인된 폐방선균증의 수술적 치험 1례)

  • Pulmonary actinomycosis is a rare disease entity to undergo thoracic surgery. We experienced a 49-year-old man with pulmonary actinomycosis who was admitted due to recurrent hemoptysis. Prior to admission, he was diagnosed as pulmonary tuberculosis on the basis of his clinical manifestations and chest radiological findings. The plain chest x-ray and chest computed tomography(CT) showed a cavitary lesion in left upper lobe and was given anti-tuberculous medication, but the x-ray revealed no imprcovement. He underwent left upper lobe lobectomy with segmentectomy of lower lobe and the surgical specimen showed no evidence of mycobacterial infection, but revealed sulfur granules which is a typical pathological finding of actinomycosis. He was discharged uneventfully and is scheduled to receive 6 months of antibiotic treatment.

Fast Marker-based Registration of 3D CT and 2D X-ray Fluoroscopy Images (3차원 전산화 단층촬영영상과 2차원 X-선 투시영상간 표식기 기반 고속 정합)

  • This paper proposes a novel technique of marker-based 2D-3D registration to combine 3D information obtained from preoperative CT images into 2D image obtained from intraoperative x-ray fluoroscopy image. Our method is divided into preoperative and intraoperative procedures. In preoperative procedure, we generate CT-derived DRRs using graphics hardware and detect markers automatically. In intraoperative procedure, we propose a hierarchical two- step registration to reduce a degree of freedom from 6-DOP to 2-DOF which is composed of in-plane registration using principal axis method and out-plane registration using minimal error searching method in spherical coordinate. For experimentation, we use cardiac phantom datasets with confirmation markers and evaluate our method in the aspects of visual inspection, accuracy and processing time. As experimental results, our method keeps accuracy and aligns very fast by reducing real-time computations.

Study of the Production Method of the Framework of Lacquerware Inlaid with Mother-of-pearl Using X-ray Computed Tomography (엑스선 컴퓨터 단층면 조사를 통한 나전연상 백골의 제작기법 연구)

  • X-ray computed tomography (X-ray CT), which is often applied to the investigation of the interior structures of cultural properties, is very useful for studying production methods used in lacquerware that would otherwise be difficult to observe with the naked eye. Lacquerware inlaid with mother-of-pearl is an important subject in the study of traditional Korean craft. However, it can be a challenge to specify the production methods used in the framework of a lacquerware item unless the object has been structurally damaged. Studies of lacquerware inlaid with mother-of-pearl have thus far focused on the techniques of lacquering and of inlaying the mother-of-pearl. Production methods for the framework of lacquerware has received little academic attention. This study conducted X-ray CT on a lacquerware item from the Joseon period to reveal the materials and woodworking techniques used for the framework. The study revealed that softwood was used for the framework, and the post (gidungmok) and apron (soemok) were joined using a three-way miter joint (sambang yeongwi jjaim). Other pieces were joined with nails.

Infection Control of Computed Radiography Portable in Radiology (영상의학과 이동촬영장비의 감염 관리)

  • The purpose of this study is to prevent infection in the hospital by computed radiography portable and to provide basic data on infection-related education by investigating bacterial contamination level of computed radiography portable equipment using IP cassette. The results suggest that IP cassette No. 1 is infected with CNS and VRE, no. 2 with CNS, No. 3 with CNS and Pseudomonas aeruginosa, No. 4 with CNS, No. 5 with CNS and Bacillus sp., and No. 6 with enterococcus faecium. Enterococcus faecium and bacillus sp. were detected from the IP reader and Acinetobacter baumannii was detected on the mobile handle; Bacillus sp. on the control buttons, CNS and Bacillus sp. from the irradiation control handle, Acinetobacter baumannii on the x-ray generation switch, and CNS on the barcode scanner. In addition, Bacillus sp. Acinetobacter baumannii was found on the IP cassette mobile table and CNS and bacillus sp. were found on the lead apron. Acinetobacter baumannii and CNS were detected from the medical gloves worn by a radiological technologist during radiography. This suggests that IP cassette should be sterilized after use as it can hand over bacteria to IP reader and IP mobile table. Medical gloves that are in direct contact with patients should also be replaced after using them once and other supplies such as x-ray generation switch and lead apron should thoroughly be sterilized to prevent infection due to radiography as they are in a lot of contact with patients.

Experimental Study for Phase-contrast X-ray Imaging Based on a Single Antiscatter Grid and a Polychromatic X-ray Source (단일 비산란 그리드 및 다색광 x-선원 기반 위상대조 x-선 영상화 실험 연구)

  • In this work, we performed a proof-of-concept experiment for phase-contrast x-ray imaging (PCXI) based on a single antiscatter grid and a polychromatic x-ray source. We established a table-top setup which consists of a focused-linear grid having a strip density of 200 lines/inch, a microfocus x-ray tube having a focal-spot size of about $5{\mu}m$, and a CMOS-type flat-panel detector having a pixel size of $48{\mu}m$. By using our prototype PCXI system and the Fourier demodulation technique, we successfully obtained attenuation, scattering, and differential phase-contrast images of improved visibility from the raw images of several selected samples at x-ray tube conditions of $90kV_p$ and 0.1 mAs. Further, fusion image (e.g., the attenuation+the scattering) may have an advantage in displaying details of the sample's structures that are not clearly visible in the conventional attenuation image. Our experimental results indicate that single-grid-based approach seems a useful method for PCXI with great simplicity and minimal requirements on the setup alignment.

A Study to Apply the Neural Networks for Improvement of X-Ray Chest Image (흉부 X-Ray 영상개선을 위한 신경망 적용에 관한 연구)

  • Recently, X-ray chest rediography is showing a tendency to take an image of digital radiography so as to diagnose the pathology of chest in a usual. When the radiologist observes the chest image derived from digital radiography system on the monitor, he feels difficult to find out the pathological pattern because the quality of chest radiography is unequal. It takes amount of time to adjust the proper image for diagnosis. Therefore, we propose the method of the chest image equalization using neural networks and provide the compared result with histogram equalization method.
