Analysis of dosimetric leaf gap variation on dose rate variation for dynamic IMRT (동적 세기조절방사선 치료 시 선량률 변화에 따른 선량학적엽간격 변화 분석)

  • To evaluate the position accuracy of the MLC. This study analyzed the variations of the dosimetric leaf gap(DLG) and MLC transmission factor to reflect the location of the MLC leaves according to the dose rate variation for dynamic IMRT. We used the 6 MV and 10 MV X-ray beams from linear accelerator with a Millennium 120 MLC system. We measured the variation of DLG and MLC transmission factor at depth of 10 cm for the water phantom by varying the dose rate to 200, 300, 400, 500 and 600 MU/min using the CC13 and FC-65G chambers. For 6 MV X-ray beam, a result of measuring based on a dose rate 400 MU/min by varying the dose rate to 200, 300, 400, 500 and 600 MU/min of the difference rate was respectively -2.59, -1.89, 0.00, -0.58, -2.89%. For 10 MV X-ray beam, the difference rate was respectively ?2.52, -1.69, 0.00, +1.28, -1.98%. The difference rate of MLC transmission factor was in the range of about ${\pm}1%$ of the measured values at the two types of energy and all of the dose rates. This study evaluated the variation of DLG and MLC transmission factor for the dose rate variation for dynamic IMRT. The difference of the MLC transmission factor according to the dose rate variation is negligible, but, the difference of the DLG was found to be large. Therefore, when randomly changing the dose rate dynamic IMRT, it may significantly affect the dose delivered to the tumor. Unless you change the dose rate during dynamic IMRT, it is thought that is to be the more accurate radiation therapy.

  • The use of cone-beam computed tomography(CBCT) has been proposed for guiding the delivery of radiation therapy. A kilovoltage imaging system capable of radiography, fluoroscopy, and cone-beam computed tomography(CT) has been integrated with a medical linear accelerator. A standard clinical linear accelerator, operating in arc therapy mode, and an amorphous-silicon (a-Si) with an on-board electronic portal imager can be used to treat palliative patient and verify the patient's position prior to treatment. On-board CBCT images are used to generate patient geometric models to assist patient setup. The image data can also, potentially, be used for dose reconstruction in combination with the fluence maps from treatment plan. In this study, the accuracy of Hounsfield Units of CBCT images as well as the accuracy of dose calculations based on CBCT images of a phantom and compared the results with those of using CT simulator images. Phantom and patient studies were carried out to evaluate the achievable accuracy in using CBCT and CT stimulator for dose calculation. Relative electron density as a function of HU was obtained for both planning CT stimulator and CBCT using a Catphan-600 (The Phantom Laboratory, USA) calibration phantom. A clinical treatment planning system was employed for CT stimulator and CBCT based dose calculations and subsequent comparisons. The dosimetric consequence as the result of HU variation in CBCT was evaluated by comparing MU/cCy. The differences were about 2.7% (3-4MU/100cGy) in phantom and 2.5% (1-3MU/100cGy) in patients. The difference in HU values in Catphan was small. However, the magnitude of scatter and artifacts in CBCT images are affected by limitation of detector's FOV and patient's involuntary motions. CBCT images included scatters and artifacts due to In addition to guide the patient setup process, CBCT data acquired prior to the treatment be used to recalculate or verify the treatment plan based on the patient anatomy of the treatment area. And the CBCT has potential to become a very useful tool for on-line ART.)

Effective Dose Evaluation using Clinical PET/CT Acquisition Protocols (전신 PET/CT 영상 획득 프로토콜을 이용한 유효선량 평가)

  • The purpose of this study was to evaluate the radiation dose for clinical PET/CT protocols in clinical environments using Alderson phantom and TLDs. Radiation doses were evaluated for both Philips GEMINI 16 slice PET/CT system and GE DSTe 16 slice PET/CT system. Specific organ doses with $^{137}Cs$ transmission scan, high quality CT scan and topogram in philips GEMINI PET/CT system were measured. Specific organ doses with CT scan for attenuation map, CT scan for diagnosis and topogram in GE DSTe PET/CT system were also measured. The organs were selected based on ICRP60 recommendation. The TLDs used for measurements were selected for within an accuracy of ${\pm}5%$ and calibrated in 10 MV X-ray radiation field. The effective doses for $^{137}Cs$ transmission scan, high qualify scan, and topogram in Philips GEMINI PET/CT system were $0.14{\pm}0.950,\;29.49{\pm}1.508\;and\;0.72{\pm}0.032mSv$ respectively. The effective doses for CT scan to make attenuation map, CT scan to diagnose and topogram in GE DSTe PET/CT system were $20.06{\pm}1.003,\;24.83{\pm}0.805\;and\;0.27{\pm}0.008mSv$ respectively. We evaluated the total effective dose by adding effective dose for PET Image. The total PET/CT doses for Philips GEMINI PET/CT (Topogram+$^{137}Cs$ transmission scan+PET, Topogram+high qualify CT+PET) and GE DSTe PET/CT (Topogram +CT for attenuation map+ PET, Topogram+diagnostic CT+ PET) are $7.65{\pm}0.951,\;37.00{\pm}1.508,\;27.12{\pm}1.003\;and\;31.89{\pm}0.805mSv$ respectively. Further study may be needed to be peformed to find optimal PET/CT acquisition protocols for reducing the patient exposure with good image qualify.

Effect of Radiation Dose for Radiotherapy on Ovarian Follicle Atresia in Rat (치료 방사선량이 쥐의 난포 퇴축에 미치는 영향)

  • In previous studies, ovarian follicle in rat has been used a higher radiation dose than that for cancer radiotherapy in clinical practice. The aim of this study was to evaluate the effect of radiation dose used for cancer radiotherapy on ovarian follicle atresia in rat. Mice of 4-week-old female were whole body irradiated with 2 cGy or 2 Gy (Mevatron 67, Siemens, Germany) and sacrificed by cervical dislocation. Ovaries were collected at 24 hours after irradiation to observe the degree of follicular atresia. Ovaries were fixed in neutral formaldehyde solution for 24 hours and embedded with paraffin. Cutted in $5{\mu}m$ thickness with microtome and stained with hematoxylin and eosin (H&E) and TUNEL immunohistochemical stain, and examined histologically under a light microscope. All data were presented as mean ${\pm}SD$, calculating the ratio of normal or atretic follicles to total ovarian follicles. Statistical analysis was performed by the Mann Whitney test using the SPSS ver 19.0. Ratio of atretic to total follicles of 2 Gy group was significantly higher than control or 2 cGy groups (p<0.05). Ratio of normal to total follicles of 2 Gy group was significantly lower than control group in preantral follicle (64.0 vs. 87.7, p=0.027). Ratio of normal to total follicles of 2 cGy group was significantly increased more than control or 2 Gy groups in antral follicle, and there were no significant difference between control and 2 Gy groups (p=0.522). Radiation dose of 2 Gy for cancer radiotherapy have a significant effect on ovarian follicle atresia in rat.

Scintigraphic Evaluation of Inhalation Injury in Fire Victims (화재사고시 흡입에 의한 기도손상의 핵의학적 평가)

  • Purpose: Conventional chest X-ray and pulmonary function test cannot sensitively detect inhalation injury. Bronchoscopy is known to be the gold standard but it is invasive method. We evaluated whether lung inhalation/perfusion scans can sensitively detect inhalation injury of fire victims. Materials and Methods: Nineteen patients (male 9, female 10, mean age 31.6 yr) of fire victims were enrolled in this study. Inhalation lung scan was performed 2 days later after inhalation injury with $^{99m}Tc$-technegas. Perfusion lung scan was performed 4 days later with $^{99m}Tc$- MAA (macroaggregated albumin). Follow up lung scans were performed 16 and 18 days later for each. Chest X-ray was performed in all patients and bronchoscopy was performed in 17 of 19 patients at the same period. Pulmonary function test was performed in 9 patients. Results: Four of 19 patients showed inhalation and perfusion defects and one showed inhalation defect but, normal perfusion scan findings. These five patients with abnormal scan findings showed abnormal bronchoscopic findings and severe respiratory symptoms. On chest X-ray, 2 of them had pulmonary tuberculosis and one of them showed pulmonary congestion. FEV1 /FVC was abnormal in 3 patients. On the follow up scan, all patients with abnormal initial scan findings showed improved findings and they had improved clinical state. Conclusion: Inhalation/perfusion lung scans can detect inhalation burn injury noninvasively in early stage and may be useful in therapeutic decision making and follow up of patients.

Mineral Composition and Grain Size Distribution of Fault Rock from Yangbuk-myeon, Gyeongju City, Korea (경주시 양북면 단층암의 광물 조성과 입도 분포 특징)

  • This paper is focused on mineral compositions, microstructures and distributional characters of remained grains in the fault rocks collected from a fault developed in Yongdang-ri, Yangbuk-myeon, Gyeongju City, Korea, using X-ray diffraction (XRD), optical microscope, laser grain size analysis and fractal dimension analysis methods. The exposed fault core zone is about 1.5 meter thick. On the average, the breccia zone is 1.2 meter and the gouge zone is 20cm thick, respectively. XRD results show that the breccia zone consists predominantly of rock-forming minerals including quartz and feldspar, but the gouge zone consists of abundant clay minerals such as chlorite, illite and kaolinite. Mineral vein, pyrite and altered minerals commonly observed in the fault rock support evidence of fault activity associated with hydrothermal alteration. Fractal dimensions based on box counting, image analysis and laser particle analysis suggest that mineral grains in the fault rock underwent fracturing process as well as abrasion that gave rise to diminution of grains during the fault activity. Fractal dimensions(D-values) calculated by three methods gradually increase from the breccia zone to the gouge zone which has commonly high D-values. There are no noticeable changes in D-values in the gouge zone with trend being constant. It means that the bulk-crushing process of mineral grains in the breccia zone was predominant, whereas abrasion of mineral grains in the gouge zone took place by continuous fault activity. Mineral compositions in the fault zone and peculiar trends in grain distribution indicate that multiple fault activity had a considerable influence on the evolution of fault zones, together with hydrothermal alteration. Meanwhile, fractal dimension values(D) in the fault rock should be used with caution because there is possibility that different values are unexpectedly obtained depending on the measurement methods available even in the same sample.

Evaluation of Mn Removal Efficiency from the Mine Drainage in the Presence of Fe Using Slag Complex Reactors (제강슬래그 복합매질체를 이용한 철 유입에 따른 광산배수내 망간 제거효율 평가)

  • For the treatment of heavy metals in the mine drainage from the closed mine area, various methods such as passive, active and semi-active treatments are considered. Among contaminated elements in the mine drainage, Mn is one of the difficult elements for the treatment because it needs high pH over 9.0 for its concentration to be reduced. In this study, the efficiency of various slag complex reactors (slag (S), slag+limestone (SL) and slag+Mn coated gravel (SG)) on Mn removal in the presence of Fe, which is a competitive element with Mn, was evaluated to investigate effective methods for the treatment of Mn in mine drainage. As a result of experiments on Mn removal without Fe during 358 days, using influent with $30{\sim}50Mn{\cdot}mg/L$ and pH 6.7 on the average, S reactor showed continuously high Mn removal efficiency with the average of 99.9% with pH 8.9~11.4. Using the same reactors, Mn removal experiments with Fe during 237 days were conducted with the influent with $40{\sim}60Mn{\cdot}mg/L$. The pH range of effluent reached to 6.1~10.0, which is slightly lower than that of effluent without Fe. S reactor showed the highest range of pH with 7.1~9.9, followed by S+L and S+G reactor. However, the efficiency of Mn removal showed S+L>S>S+G with the range of 94~100%, 68~100% and 68~100%, respectively in spite of relatively low pH range. S+L reactor showed the most resistance on Fe input, which means other mechanisms such as $MnCO_3$ formation by the carbonate prouced from the limestone or autocatalysis reaction of Mn contributed to Mn removal rather than pH related mechanisms. The evidence of reactions between carbonates and Mn, rhodochrosite ($MnCO_3$), was found from the X-ray diffraction analysis of precipitates sample from S+L reactor. From this study, the most effective reactors on Mn removal in the presence of Fe was S+L reactor. The results are expected to be applied for the Mn containing mine water treatment in the presence of Fe within the relatively low range of pH.

Evaluation and Comparison of Myocardial Perfusion Defects in Patients with Early Breast Cancer Subjected to Different Radiation Simulation Techniques (조기유방암 환자에서 방사선 모의치료 방법의 차이에 따른 심근관류결손의 비교 평가)

  • [ $\underline{Purpose}$ ]: The aim of this study is to evaluate and compare the incidence and aspects of myocardial perfusion defects in patients who were subjected to either two-dimensional or three-dimensional simulation techniques for early left-sided breast cancer. The myocardial perfusion defects were determined from using single photon emitted computerized tomography (SPECT) myocardial perfusion images. $\underline{Materials\;and\;Methods}$: Between January 2002 and August 2003, 32 patients were enrolled in this study. The patients were diagnosed as having early (AJCC stage T1-T2N0M0) left-sided breast cancer and were treated with tangential irradiation after breast-conserving surgery and systemic chemotherapy. The patients were divided into two groups according to the type of simulation received: two-dimensional simulation using an X-ray fluoroscope simulator or three-dimensional simulation with a CT simulator. All patients underwent technetium-99m-sestamibi gated perfusion SPECT at least 3 years after radiotherapy. The incidence and area of myocardial perfusion defects were evaluated and were compared in the two groups, and at the same time left ventricular ejection fraction and cardiac wall motion were also analyzed. The cardiac volume included in the radiation fields was calculated and evaluated to check for a correlation between the amount of irradiated cardiac volume and aspects of myocardial perfusion defects. $\underline{Results}$: A myocardial perfusion defect was detected in 11 of 32 patients (34.4%). There were 7 (46.7%) perfusion defect cases in 15 patients who underwent the two-dimensional simulation technique and 4 (23.5%) patients with perfusion defects in the three-dimensional simulation group (p=0.0312). In 10 of 11 patients who had myocardial perfusion changes, the perfusion defects were observed in the cardiac apex. The left ventricular ejection fraction was within the normal range and cardiac wall motion was normal in all patients. The irradiated cardiac volume of patients in the three-dimensional simulation group was less than that of patients who received the two-dimensional simulation technique, but there was no statistical significance as compared to the incidence of perfusion defects. $\underline{Conclusion}$: Radiotherapy with a CT simulator (three-dimensional simulation technique) for early left-sided breast cancer may reduce the size of the irradiated cardiac volume and the incidence of myocardial perfusion defects. Further investigation and a longer follow-up duration are needed to analyze the relationship between myocardial perfusion defects and clinical ischemic heart disease.

Mineralogical Study on the Clay Formation and Heavy Metal Speciation in the Acidified Soil Profile of the Onsan Industrial Area (온산공업지역 산성 토양 프로화일 내에서의 점토광물의 생성과 중금속 이온의 거동에 관한 광물학적 연구)

  • The present study is focused on the granite weathering and soil formation, and the heavy metal contamination in soils in the Onsan industrial area. For profile study, soil sampling was conducted on each depth and experimental analyses have been conducted on those samples. X-ray diffraction analyses show that clay minerals consist mainly of kaolin minerals, vermiculite, and minor illite. Most of kaolin minerals in the lower kiwi of the profile consist of halloysite as confirmed by formamide intercalation, but the content of halloysite decreases gradually toward the surface since it has been transformed to kaolinite in the upper part of the profile. Thermal treatment by heating at $110^{\circ}C,\;300^{\circ}C,\;and\;550^{\circ}C$ shows a diffuse and broad peak the between 10 and $14\;{\AA}$ region in X-ray diffractograms. This suggests the possible existence of the hydroxy-Al interlayerecl vermiculite. Na-citrate extraction method reaconfirms this result showing transition of $14\;{\AA}$ peak to $10\;{\AA}$ In by removing the interlayer materials and restoring the vermiculite to its original state. The occurrence of hydroxy-Al interlayered vermiculite is also supported by soil pH distribution room 3.9 In the lower part to 3.6 in the upper part of the profile. Sequential extraction experiment was conducted to investigate the states of heavy metals in soils. The experiment shows that relatively high amounts of heavy metals are concentrated in the upper part of the profile and that most of them are bound to Fe/Mn oxides and organic matters while less concentration in clay minerals. The result indicates that most of heavy metal pollutants are concentrated in the surface soil and that the low concentrations of heavy metals in clays are mainly due to the low adsorption capacities of clay minerals such as kaolin minerals and hydroxy-Al interlayered vermiculite in acidified soil condition.

Surgical Experiences of Shone's Syndrome (숀 증후군의 외과적 치료)

  • Shone's syndrome is a congenital cardiac malformation that consists of multiple levels of left heart obstruction including supravalvular mitral ring, congenital mitral stenosis(parachute mitral valve), subaortic stenosis, and coarctation of aorta. This syndrome is a very rare congenital anomaly and its prognosis is poor. We experienced 9 patients with Shone's syndrome between 1985 and 1994. There were 8 male and 1 female patients, and mean age was 33.0$\pm$31.0 months ranged from 2 months to 1 1 years. The congenital mitral, stenosis and coarctation of aorta existed in all patients and the supravalvular mitral ring and subaortic stenosis in 4 patients. Two patients had all four anatomic lesions. 3 patients underwent one stage total correction and the other 6 patients underwent two staged operation that was initial coarctoplasty with thoracotomy and later correction of intracardiac anomalies with median sternotomy. A third operation was performed in 2 patients. These procedures included reoperation for coarctation and replacement of mitral valve for persistent mitral stenosis. There was no operative death at the first operation but two operative deaths at the second operation. The cause of death in two cases was severe heart failure secondary to left ventricular hypoplasia. There was no operative death at the third operation. The seven survivors have beeli followed from 11 months to 12 years(mean follow-up 6.7 $\pm$ 3.6 years). There was no late death and the New York HeArt Association activity level was class I for all patients. We conclude that a food lone-term outcome can be expected by proper surgical treatment tailred to each individual's anatomy and pathophysiology although the operative mortality and morbidity of Shone's syndrome are high.
