리더십 유형이 구성원의 조직몰입과 직무만족에 미치는 영향 : 종합병원 의무기록실을 대상으로 (Leadership Style of Medical Record Directors at General Hospitals and it's Effect on the Organizational Commitment and Job Satisfaction)

  • 1) Background: The hospitals of modem society, like any other business entities, have to constantly strive to secure their survival from aggressive changes and competition outside. In this unstable environment, effective leadership is one of the most effective strategies for securing organization's growth as well as stability. This study investigated types of leadership (transformational or transactional) that is dominant in medical record departments and compared it's effects on organizational commitment and job satisfaction of their organizational members by types. 2) Method: A questionnaire was developed and mailed to all medical record administrators working at general hospitals throughout the country except department directors (N=450). Of these, 150 useable questionnaires were returned and analyzed by t-test, multiple regression analysis using SPSS. 3) Results: The organizational commitment and job satisfaction were a little bit higher than moderate level, and that of leadership perceived by medical record administrators was also in moderate level throughout types. Significant characteristics (positively) related to organizational commitment and job satisfaction by univariate analysis were marital status (married), position (middle management) and both type of leadership. However transformational leadership was the only significant factor in leadership styles after considering all the factors related to organizational commitment and job satisfaction together by multivariate analysis. 4) Conclusion: The average organizational commitment and job satisfaction of medical record administrators was just in moderate level. Efforts should be made to increase them by improving leadership capacity of medical record directors, primarily by using transformational leadership approach.

119 구급대원의 근골격계 질환 (Musculoskeletal Diseases' of the 119 Rescue party's)

  • 본 연구는 병원 전 응급의료 현장을 담당하고 있는 119 구급대원의 근골격계 질환에 대해 조사하기 위해 2012년 7월 2일부터 7월 31일까지 전북지역의 119 구급대원 216명을 대상으로 설문조사한 결과이다. 조사결과 대상자의 57.9%가 근골격계 질환과 관련한 자각증상을 호소하였으며 신체부위별로는 허리와 엉덩이 부분에 통증이 있다는 응답이 47.2%로 가장 많았다. 또한 이러한 통증의 원인이 구급업무와 관련이 있는 것으로 조사되었다. 또한, 일반적 특성에 따른 근골격계 증상에서 성별(t=16.579, p=.000), 연령(t=102.344, p=.000), 학력(t=5.363, p=.027), 음주(t=6.999, p=.030), 흡연(t=6.266, p=.009)에 따른 유의한 차이가 있었으며 직업적 특성에서는 경력(t=67.684, p=.000), 구급차량 탑승인원(t=7.717, p=.004), 자격 및 면허(t=25.480, p=.000), 직급(t=74.615, p=.000)에서 유의한 차이가 있는 것으로 나타났다.

임상간호사의 공감만족, 공감피로, 소진이 긍정심리자본에 미치는 영향 (Influences of Compassion Satisfaction, Compassion Fatigue, and Burnout on Positive Psychological Capital of Clinical Nurses)

  • 본 연구는 임상간호사의 공감만족, 공감피로, 소진, 긍정심리자본의 관련성을 파악하고, 임상실무현장에서 임상간호사의 긍정심리자본에 영향을 미치는 요인을 규명하기 위한 서술적 조사연구이다. 본 연구의 대상자는 Y시에 소재하고 있는 3개 종합병원에 근무하는 간호사를 대상으로 하였으며, 193부를 최종 분석에 사용하였다. 수집된 자료는 IBM SPSS Statistics 20.0 프로그램을 사용하여 기술통계, t-test, one-way ANOVA, Scheffe, Pearson's correlation coefficient, 다중회귀분석을 통해 분석하였다. 연구결과 긍정심리자본은 공감만족(r=.662, p<.010), 소진(r=-.387, p<.010)에 유의한 상관관계가 있는 것으로 나타났다. 다중회귀분석결과 긍정심리자본에 영향을 미치는 요인은 공감만족, 소진, 직위였으며 이들의 설명력은 55.5%였다(F=30.88, p<.001). 본 연구결과를 바탕으로 임상간호사의 긍정심리자본을 높이기 위해서는 공감만족을 증진시키고 소진을 감소시키는 중재 프로그램 개발이 필요할 것으로 생각된다.

이단계 크리깅 모델을 이용한 조(Jaw)의 구조최적설계 (Structural Optimization for a Jaw Using the Kriging model)

  • 컨테이너의 양 ${\cdot}$ 하역 작업 시 크레인을 정 위치에 고정시키고, 돌풍으로 인해 크레인이 레일방향으로 미끄러지는 것을 방지하는 장치가 레일클램프이다. 쐐기형 레일클램프는 풍속이 증가하면 쐐기작용에 의해 압착력이 증가하므로 구조적으로 안정성과 내구성이 있게 설계해야한다. 따라서 본 연구에서는 순차적 표본방법을 기반으로 하는 크리깅 모델을 이용하여 조 형상최적설계를 수행하였다. 크리깅은 표본점의 반응치를 기초로 내삽법에 의해 임의의 점에서의 반응치를 예측하는 근사기법의 하나이다. 이것은 최적설계 분야에서 자주 사용되는 근사기법인 반응표면법에 비해 반응치의 보다 정확한 예측이 가능하며 특히 비선형성이 강한 함수의 예측에 적합하다고 알려져 있다. 순차적 크리깅 모델에 의하여 구해진 최적해를 상용프로그램을 이용하여 구한 최적해와 비교하고 제안된 방법의 유용성을 검토하였다.

터널천단변위와 암석 또는 암반의 일축압축강도를 이용한 시공 중인 터널의 예비 안정성 평가 (The Pre-Evaluation of Stability during Tunnel Excavation using Unconfined Compression Strength of Intact Rock or Rock Mass and Crown Settlement Data)

  • PURPOSES : It is difficult to estimate tunnel stability because of lack of timely information during tunnel excavation. Tunnel deformability refers to the capacity of rock to strain under applied loads or unloads during tunnel excavation. This study was conducted to analyze a methods of pre-evaluation of stability during tunnel construction using the critical strain concept, which is applied to the results of tunnel settlement data and unconfined compression strength of intact rock or rock mass at the tunnel construction site. METHODS : Based on the critical strain concept, the pre-evaluation of stability of a tunnel was performed in the Daegu region, at a tunnel through andesite and granite rock. The critical strain concept is a method of predicting tunnel behavior from tunnel crown settlement data using the critical strain chart that is obtained from the relationship between strain and the unconfined compression strength of intact rock in a laboratory. RESULTS : In a pre-evaluation of stability of a tunnel, only actually measured crown settlement data is plotted on the lower position of the critical strain chart, to be compared with the total displacement of crown settlement, including precedent settlement and displacement data from before the settlement measurement. However, both cases show almost the same tunnel behavior. In an evaluation using rock mass instead of intact rock, the data for the rock mass strength is plotted on the lower portion of the critical strain chart, as a way to compare to the data for intact rock strength. CONCLUSIONS : From the results of the pre-evaluation of stability of the tunnel using the critical strain chart, we reaffirmed that it is possible to promptly evaluate the stability of a tunnel under construction. Moreover, this research shows that a safety evaluation using the actual instrumented crown settlement data with the unconfined compression strength of intact rock, rather than with the unconfined compression strength of a rock mass in the tunnel working face, is more conservative.

Body Sway as a Possible Indicator of Fatigue in Clerical Workers

  • Background: Fatigue has a strong impact on workers' performance and safety, but expedient methods for assessing fatigue on the job are not yet available. Studies discuss posturography as an indicator of fatigue, but further evidence for its use in the workplace is needed. The purpose of the study is to examine whether posturography is a suitable indicator of fatigue in clerical workers. Methods: Thirty-six employees (${\emptyset}$ 34.8 years, standard deviation = 12.5) participated in postural tasks (eyes open, eyes closed, arm swinging, and dual task) in the morning and afternoon. Position of their center of pressure (COP) was registered using a Nintendo Wii Balance Board and commercial software. From registered COP time series, we calculated the following parameters: path length (mm), velocity (mm/s), anterior-posterior variance (mm), mediolateral variance (mm), and confidence area ($mm^2$). These parameters were reduced to two orthogonal factors in a factor analysis with varimax rotation. Results: Statistical analysis of the first factor (path length and velocity) showed a significant effect of time of day: COP moved along a shorter path at a lower velocity in the afternoon compared with that in the morning. There also was a significant effect of task, but no significant interaction. Conclusion: Data suggest that postural stability of clerical workers was comparable in the morning and afternoon, but COP movement was greater in the morning. Within the framework of dynamic systems theory, this could indicate that the postural system explored the state space in more detail, and thus was more ready to respond to unexpected perturbations in the morning.

임상간호사의 간호근무환경과 자아존중감이 비판적 사고성향에 미치는 영향 (Effects of Nursing Practice Environment and Self-esteem on Critical Thinking Disposition among Clinical Nurses)

  • Purpose: This study was a cross-sectional study done to determine the relevance and impact factors of nursing practice environment and self-esteem on critical thinking disposition in clinical nurses. Methods: A survey was conducted from March to May 2015 with self-report questionnaire. Participants were 281 registered nurses working in one tertiary hospital. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation, and multiple regression with SPSS/WIN 21.0. Results: Factors affecting the nurses' critical thinking disposition included age (F=7.23, p<.001), educational background (F=7.82, p<.001), position (F=14.95, p<.001), clinical career (F=7.66, p<.001). Further, critical thinking disposition had a positive correlation with nursing practice environment (r=.60, p<.001) and self-esteem (r=.41, p<.001). Self-esteem and nursing practice environment accounted for 43% of the variance in critical thinking disposition. Conclusion: The study findings show that critical thinking disposition is influenced by nursing foundations for quality of care and the collegial nurse-physician relations of nursing practice environment. Therefore, it's necessary to provide continuing education for clinical nurses to reconstruct the organizational culture of nurses and physician partnerships. In addition, increasing self-esteem through various motivational programs should increase critical thinking disposition.

철도 발리스 응용을 위한 DBPL 인코더 설계 연구 (The Study on DBPL Encoder Design for Railway Balise Application)

  • 발리스는 열차의 안전한 운행을 위하여 철도의 레일 사이에 설치하여 운영하는 장치로, 텔레그램이라 칭하는 정보(거리 구배 속도 임시 속도제한 등의 가변정보)를 무선으로 통과하는 열차에 전송하는 기능을 갖는다. 본 연구는 이에 필요한 DBPL 인코더의 설계에 대한 것이다. 일반적으로 발리스를 위한 DBPL 인코더는 전용 ASIC이나 FPGA를 통하여 구현하고 있으나, 본 연구에서는 범용의 저전력 마이크로컨트롤러(STM32L4 Series)를 활용하여 설계하였다. 본 연구의 DBPL인코더는 펌웨어 방식의 논리적 1차 처리 수단과 마이크로컨트롤러 내장 SPI Bus 등을 이용한 물리적 출력수단으로 구분하여 설계하였다. 본 연구의 DBPL 인코더는 유로발리스 표준에서 요구하는 564.48Kbps의 속도로 동작 가능함을 확인하였다.

말기 암 환자를 간호하는 간호사의 고통경험 정도에 영향을 미치는 특성요인 (The Influencing Factors on the Degree of Nurse's Suffering Experience Caring for Terminal Cancer Patient)

  • Purpose: The study was undertaken to examine the degree of nurse's suffering experience and to identify the influencing factors on nurses' suffering experience in Korea. Method: Data were collected using a questionnaire for 271 nurses working at 5 general hospitals in Daegu and Kyung-book province from Sep. 1, to Sep. 30, 2003. The questionnaire consists of 54 items, general characteristics(10) and nurse's suffering experience(44). All surveys were sorted and studied by frequency analysis, mean score, standard deviation, range, independent t-test, one way ANOVA, Pearson's correlation coefficient and Multiple regression. Result: The findings of this survey indicate 1) The degree of suffering experienced by nurses caring for terminal cancer patients was 2.96; 2) Demographic variables affecting the degree of nurses' suffering experience were age(F=5.62, p=.000), marital status(F=20.53, p=.000), religion(F=5.44, p=.020), career of clinical experience(F=6.96, p=.000), and feelings of end-life care(F=3.11, p=.016); 3) There were slight correlation between the subitem of nurse's suffering experience and general characteristics of subjects. For 'expanding self consciousness', age, career duration, and position; for 'forming empathy with family', age and career duration ; for 'spiritual sublimation', age, and career duration were affected variables. 4) As a result of the multiple regression analysis for predictable variables affecting nurses' suffering, it was found that 'career of clinical experience' was most significant(F=23.100, p=.000). The explanatory power of this regression formula was 17.6%. Conclusion: This study can provide the basic data useful towards improvement of nursing services for terminal cancer patients and the health of the nurse.

방사선사의 전직의사와 관련된 요인분석 (Factors affecting the Intention of transfering of Radiology Technologists to Different Institutions)

  • This study attempts to analyze the factors affecting the intention of transferring to another hospitals among radiology technologists. 344 cases were reviewed in 5 university hospitals and 1 general hospital. Self-administered questionaire were given to study the socioeconomic characteristics, working conditions, job satisfaction level, and the reasons for transfer among the technologists. The major findings were as follows : 1. Job position and hospital characteristics had a statistically significant relationship with the intention of transferring to another hospital. 2. Those who were not satisfied with their salaries and promotional opportunity showed a higher tendency towards to transfer. 3. Those who were less satisfied with the opportunity for developing the personal ability and had the negative attitude on their job showed a higher tendency to transfer. 4. Those who did not sustain good relationship with their superiors and co-workers scored high on the tendency to transfer. 5. In the result of mutiple regression, recognition of radiation hazard, job satisfaction, satisfaction with salary levels, job attitude were significantly related to transfer. The above indicate that besides economic incentives, job satisfaction and organizational culture to promote their ability and form a good relationship with organization members were very important to decrease the intention of transfer. Since these results represent only 6 hospitals from a limited area, more hospitals nationwide, especially small and medium-sized institutions where there is a high turnover rate of employment, need to be examined in order to investigate the various factors that affect the intention of transferring.
