사업장 MSDS 영업비밀 적용실태 및 제도 개선방안에 관한 연구 (A study on the actual conditions of trade secrets in the MSDS and the improvement of the relevant system)

  • Objectives: It has been pointed out that there are problems in handling and managing chemicals due to trade secrets of Material Safety Data Sheet(MSDS). To date, some company intentionally leave out of several parts and use an inaccurate expression in MSDS. In particular, with the result of the inspection of the government and labour community, it is required that the 2nd section in MSDS included the composition and information on ingredients has to be provided with comprehensive expression to secure more reliability. Methods: Therefore, this study is aimed: 1) to recognize the current status of trade secrets of MSDS in workplaces with both domestic manufacturers of chemical products and multinational corporations; 2) to make contributions to prevention of the industrial accidents by providing the accurate information of MSDS; 3) to improve the risk communication system related with chemicals; 4) to impress workers on the importance of right of known for MSDS. With the result we analyzed the status of trade secrets in MSDS in 73 companies, such as petrochemistry production, paint production, metal processing oil production, detergent production, and international company related with chemicals, we have found that 38,150 (45.5%) have the trade secrets parts in the total number of 83,832 in MSDS. Also, based on the 288 MSDS gathered by the Occupational Safety and Health Research Institute (OSHRI) from 2005 to 2009, 41.7% of the MSDS are classified into the trade secrets. Conclusions: Therefore, to procure an assurance system of MSDS, we suggest that a MSDS picking up and checking system be legislated in the Occupational Safety and Health Act to protect workers from the unidentified chemical hazards due to the secret trade of MSDS.

한국 성인 변형근로자의 소화성궤양 환자 관련요인 (Adult Patients with Peptic Ulcer in Korea-related Factors, Flexible Workers)

  • To identify health behavior of Peptic ulcer patients among all employees in Korea and examine relevant factors to present basic data for improving Korean adult employees' health conditions and quality of living, 3,515 employees aged 20 to 69 were sampled from the group that responded to the question concerning the presence of Peptic ulcer in the fourth Korea National Health and Nutrition Examination Survey for the third year (2009). After logistic regression analysis was carried out to see the effects of working conditions, harmful factors, shift work, stress perception, and experience of depression on the presence of Peptic ulcer, it was found that unsafe or unpleasant working conditions led to higher risk of getting Peptic ulcer, that flexible workers were more likely to get Peptic ulcer, and that depression or stress perception exerted almost no effect on Peptic ulcer. With the increase in the number of flexible workers, increasingly more interest is taken in the effects of shift work on health; occupational Peptic ulcer tends to become chronic as compared with the condition in the general population and reappear even after recovery in many cases; therefore, it is necessary to take special management measures by estimating the number of those at a risk of getting Peptic ulcer and by finding out its risk factors.

Association with Combined Occupational Hazards Exposure and Risk of Metabolic Syndrome: A Workers' Health Examination Cohort 2012-2021

  • Background: This study aimed to evaluate the association between exposure to occupational hazards and the metabolic syndrome. A secondary objective was to analyze the additive and multiplicative effects of exposure to risk factors. Methods: This retrospective cohort was based on 31,615 health examinees at the Pusan National University Yangsan Hospital in Republic of Korea from 2012-2021. Demographic and behavior-related risk factors were treated as confounding factors, whereas three physical factors, 19 organic solvents and aerosols, and 13 metals and dust were considered occupational risk factors. Time-dependent Cox regression analysis was used to calculate hazard ratios. Results: The risk of metabolic syndrome was significantly higher in night shift workers (hazard ratio = 1.45: 95% confidence interval = 1.36-1.54) and workers who were exposed to noise (1.15:1.07-1.24). Exposure to some other risk factors was also significantly associated with a higher risk of metabolic syndrome. They were dimethylformamide, acetonitrile, trichloroethylene, xylene, styrene, toluene, dichloromethane, copper, antimony, lead, copper, iron, welding fume, and manganese. Among the 28 significant pairs, 19 exhibited both positive additive and multiplicative effects. Conclusions: Exposure to single or combined occupational risk factors may increase the risk of developing metabolic syndrome. Working conditions should be monitored and improved to reduce exposure to occupational hazards and prevent the development of the metabolic syndrome.

일개 전자제품 제조업 근로자에서 교대근무와 불안증상: 비타민 D와 수면의 질의 매개효과 (Shift Work and Anxiety Symptoms at an Electronic Manufacture Company: The mediating effect of vitamin D and sleep quality)

  • Objectives: The purpose of this study was to compare vitamin D levels, sleep quality, and anxiety symptoms between shift workers and daytime workers and analyze the mediating effect of vitamin D and sleep quality between shift work and anxiety symptoms. Methods: The subjects of the study were 6744 people who received a medical examination while working for an electronics manufacturing company in 2016. We evaluated their shift work, vitamin D levels, sleep quality, and anxiety symptoms with blood samples and questionnaires. Chi-square test, t-test and path statistical analysis were performed. Results: The shift workers had significantly higher symptoms of anxiety than the day workers, significantly lower levels of vitamin D in the blood, and significantly poorer quality of sleep. The results of the pathway analysis confirmed that blood vitamin D and sleep quality had a mediating effect between shift work and anxiety symptoms. Conclusions: Shift workers should be constantly paying attention to mental health effects such as anxiety symptoms. Shift workers should also be alerted of the risk of vitamin D deficiency, encouraged about sun exposure and vitamin D intake, and required to undergo training and counseling on sleep hygiene.

  • In Vol. 31, No. 2, our journal prepared a review article, two original papers, and three case reports. First, autonomous systems are increasingly being introduced in aircraft systems. Therefore, it is crucial that flight crews are trained to adapt and handle these autonomous systems better. Consequently, we discussed crew resource management for handling autonomous systems. The fatigue of aviation workers is one of the biggest threats to aviation safety. Therefore, we analyzed the degree of fatigue in commercial aircraft pilots and evaluated the relationship between fatigue and their workload and the effect of fatigue on sleep. Since obesity has a significant impact on the health of aviation workers, we also comparatively analyzed the prevalence of obesity in Korea and Japan based on the data from the National Health and Nutrition Examination Survey. We defined cases that are difficult for aviation medical examiners to evaluate. This issue included cases of peripheral dizziness and tinnitus, cases of atopic dermatitis treated with biological agents, and cases of malignant tumors.

종합건강진단 수진자들의 건강진단에 대한 인식 및 태도에 관한 연구 (A Study on Perception and Attitudes of Examinees of Health Screening Center towards Health Examination)

  • 본 연구는 종합건강진단 수진자들의 건강진단에 대한 인식 및 태도를 조사하여 예방진료 행위에 대한 이용형태를 분석하고자 1995년 3월부터 1995년 5월까지 영남대학교 의과대학 부속병원 종합건강진단센터를 방문한 총 468명을 대상으로 설문조사를 실시하였으며, 분석한 결과는 다음과 같다. 조사대상자 468명 중 40-49세가 48.3%로 가장 많았으며, 대졸이상이 68.6%이었다. 학력이 높고 사회경제적으로 안정된 연령층에서 건강진단에 대한 관심도가 높았다. 현재 종합건강진단을 이용하고 있는 대다수가 자발적 동기보다는 기업체의 직원복지 차원에서 이루어지고 있고, 자발적으로 건강진단을 받은 137명 중에서는 건강유지를 위해 검진을 받았다고 응답한 경우가 43%로 제일 높았다. 응답자의 대부분인 95.7%가 종합건강진단이 질병 조기발견에 효과가 있다고 응답했으며, 92.5%는 종합건강진단이 유익성이 있는 것으로 인식하고 있었다. 종합건강진단에 소요되는 시간과 절차가 적당하다고 생각하고 있었으며, 적정건강진단 간격을 묻는 물음에서는 1년이 가장 많았다. 또한 검진 추가항목으로는 뇌검사, 신경계통 검사, 위내시경검사, 각종 암검사에 대한 의견이 높게 나타났다. 건강유지를 위한 실천여부와 수진 횟수와의 관계 및 적정 건강진단 간격과 수진횟수와의 관계는 통계학적으로 유의한 관련성이 있었다(p<0.01). 종합건강진단 수가에 대한 의견과 적절 비용을 묻는 물음에서는 현재비용이 비싸다고 응답한 경우가 61.8%로 가장 높았으며, 적절한 비용으로는 15만원 정도가 적당하다고 보는 의견이 가장 많았다. 건강진단 결과에 대한 판정시 의사와 상담하고 싶은 내용으로는 각종 성인병에 대한 정보가 가장 많았으며, 다음으로 정기적 건강관리 내용에 대한 문의나 가족들의 건강문제에 대한 상담 등이 많은 수를 차지하였다. 요망사항으로는 예약기간의 단축, 검진비용의 인하 및 상세한 설명에 대한 요구의 순으로 나타났다. 이상의 결과로 보아 종합건강진단을 질병의 예방 및 조기발견의 측면에서 예방진료기능이 높은 것으로 인식되고 있으며, 수진자들의 의견을 반영하여 상담내용의 내실화를 기하고 수진자가 선택할 수 있는 건강진단 항목의 개발과 수진자 교육을 위한 보건교육 교재 등을 개발함으로써 단순한 검사 결과의 판정이나 일회성 건강진단의 수준을 극복하고 보다 지속적이고, 포괄적인 건강관리를 위한 기전과 방안이 필요하다고 생각된다.

MCV와 RDW를 이용한 복합유기용제 취급 근로자의 빈혈 유형에 관한 조사 (The Type of Anemia in Organic Solvent Workers by MCV and RDW)

  • The hematologic effect by low-concentration and repeated exposure to mixed organic solvent remains obscure. This study was performed to evaluate the hematologic effect by mixed organic solvent exposure, especially on the type of anemia by mean corpuscular volume(MCV) and red blood cell distribution width(RDW). The subject were 64 organic solvent workers(male 4, female 60) and 78 general workers(male 18, female 60) who showed anemic tendency in worker's health examination which carried out by Pusan Paik Hospital from January to December, 1988. The author gained some hematologic findings(hemoglobin, hematocrit, RBC, WBC, MCV, MCHC, platelet count, ESR, RDW) from auto-analysis method by coulter counter S plus IV, and compared the type of anemia by MCV and RDW. In the pilot study for estimating the prevalence of anemia according to the type of b, the prevalence rate was higher in organic solvent workers than in public officials as 10.9% and 4.1% respectively. There were statistical significance in the value of hemoglobin, hematocrit, MCV, MCHC, platelet count, ESR, RDW between the two study groups with more severe anemic tendency in organic solvent workers. The type of anemia in organic solvent workers was microcytic and anisocytic with the mean value of $68.28{\pm}8.3fL$ of MCV and $19.1{\pm}4.0%$ of RDW.

남성근로자의 대사증후군 유병에 영향을 미치는 직업군 및 생활습관 위험인자: 후향적 코호트 조사연구 (Influence of Occupational Type and Lifestyle Risk Factors on Prevalence of Metabolic Syndrome among Male Workers: A Retrospective Cohort Study)

  • Purpose: This study examined the influence of occupational type and lifestyle habits on the prevalence of metabolic syndrome (MetS) among Korean male workers. Methods: Through secondary analysis of their four-year health examination data, 3,892 subjects were divided into four subgroups according to the presence of MetS now and four years ago. Results: Nineteen percent (n=739) suffered from MetS and these 739 subjects were classified into following occupations: 7.1% were office workers, 17.6% were non-office workers, and 42.2% were drivers. Multiple logistic regression analyses showed that when the data adjusted for age, the predicting factors on the prevalence of MetS were heavy drinking (OR 1.34, 95% CI 1.09~1.64) and the occupation of non-office workers (OR 2.99, 95% CI 2.13~4.18) and drivers (OR 7.97, 95% CI 4.89~10.83) among workers without MetS four years ago. Among workers already with a history of MetS, the predicting factors were less exercise (OR 1.55, 95% CI 1.02~2.35) and drivers (OR 2.21, 95% CI 1.03~2.94). Conclusion: Heavy drinking and less exercise and drivers were reported as influencing factors on the prevalence of MetS by this sample. The findings suggest that employers need to provide their employees with screening and management program for those at risk of MetS.

국내 지하철 정비 사업장의 근골격계질환 실태조사에 관한 연구 (A Study of Musculoskeletal Disorders at a Subway Train Repair Plant in Korea)

  • A research project was conducted to study work-related musculoskeletal disorders (MSDs) at subway train repair plant in Korea. The project was consisted of 4 main parts; education on the topics of MSDs for all workers, symptom survey, medical check-up, investigation of MSD risk factors. The result of symptom survey showed that 95.2% of the respondents complained pains on at least one part of the body. After cross-sectional analysis of various information and risk factors, 86.8% of respondents were considered as active health surveillance level 1 that require continuous monitoring on their symptoms and working conditions. And 57.1% of the respondents were considered as active health surveillance level 2 that needed medical check-ups for proper medical treatment. The analysis of occupational risk factors revealed that handling of heavy object (46.15%) and repeated awkward postures (46.15%) were two most contributing risk factors for the on-set of MSD at this work site followed by static strain (7.7%), and vibration and impact (3.8%). Medical examination was performed by an industrial medicine MD on 156 workers those considered as active health surveillance level 2. The result showed that 35 workers (20.7%) were considered as MSD patient group at severe level, and 68.6 workers (68.6%) were considered as moderate group and 18 workers (10.6%) were considered minor or normal group those have no symptom.

우리 나라의 사회경제적 사망률 불평등: 1998년도 국민건강영양조사 자료의 사망추적 결과 (Socioeconomic Mortality Inequality in Korea: Mortality Follow-up of the 1998 National Health and Nutrition Examination Survey (NHANES) Data)

  • Objectives : This study was conducted to examine the relationships of the several socioeconomic position indicators with the mortality risk in a representative longitudinal study of South Korea. Methods : The 1998 National Health and Nutrition Examination Survey was conducted on a cross-sectional probability sample of South Korean households, and it contained unique 13-digit personal identification numbers that were linked to the data on mortality from the National Statistical Office of Korea. Of 5,607 males and females, 264 died between 1999 and 2003. Cox's regression was used to estimate the relative risks (RR) and their 95% confidence intervals (CI) of mortality. Results: Socioeconomic differences in mortality were observed after adjustments were made for gender and age. Compared with those people having college or higher education, those people without any formal education had a greater mortality risk (RR=2.21, 95% CI=1.12-4.40). The mortality risk among manual workers was significantly greater than that for the non-manual workers (RR=2.73, 95% CI=1.47-5.06). A non-standard employment status was also associated with an increase in mortality: temporary or daily workers had a greater mortality risk than did the full-time workers (RR=3.01, 95% CI=1.50-6.03). The mortality risk for the low occupational class was 3.06 times greater than that of the high and middle occupational classes (95% CI=1.75-5.36). In addition, graded mortality differences according to equivalized monthly household income were found. A reduction of monthly household income by 500 thousand Korean Won (about 400 US dollars) was related with a 20% excess risk of mortality. Self-reported poor living standards were also associated with an increased risk of mortality. Those without health insurance had a 3.63 times greater risk of mortality than the insured (95% CI=1.61-8.19). Conclusions: This study showed the socioeconomic differentials in mortality in a national longitudinal study of South Korea. The existence of socioeconomic mortality inequalities requires increased social discussion on social policies in Korean society. Furthermore, the mechanisms for the socioeconomic inequalities of mortality need to be explored in future studies.
