Papillary thyroid carcinoma is known as its relatively high cure rate after surgical treatment. But invasion of the trachea by thyroid carcinoma is poor prognostic factor and the best management is en bloc surgical resection of the tumor invading the trachea. A 55-year-old man was diagnosed as papillary thyroid cancer with tracheal invasion. We treated the patient by total thyroidectomy with window resection of invading trachea followed by immediate reconstruction with sternohyoid muscle flap and tracheostomy. At 48 days after surgery, tracheostoma was closed and the patient had no functional complication by the surgical process. Until 10 months after surgery, there was no sign of recurrence and the patient led social life without any discomfort. We present this case with a review of the related literatures.

Objective : The purpose of this study is to report the patient with peripheral neuropathy, who improved by Oriental medical treatment. Methods : The patient was managed by acupuncture, moxibustion, physical treatment and herbal medicine. We checked digital infrared thermographic imaging(D.I.T.I.), electromyography(E.M.G.), nerve conduction velocity(N.C.V.), deep tendon reflex(D.T.R.), sensory recover area and range of motion(R.O.M.) of knee & ankle. Results : After 6 week treatment, clinical sign(the movement and sense of leg) of improvement was appeared. Also E.M.G., N.C.V., D.T.R. and D.I.T.I. was recovered to nearly normal range. Conclusion : The results suggest that combination of acupunture, moxibustion and herbal medicine is good method for treatment of peripheral neuropathy. But further studies may be required to concretely prove the effectiveness of this methods for treating peripheral neuropathy.

The purpose of this report is to describe a treatment method of dentigerous cyst associated with unerupted permanent teeth in mixed dentition patients. In our cases, extraction of infected primary teeth was followed by decompression of the cyst. At the same time, parts of the cystic walls were sent for histopathological examination. Decompression was performed by inserting a rubber tube into the cystic cavity through the extraction socket. The cystic cavity was kept open by means of vigorous use of a syringe by patient. Postoperative panoramic radiograph was taken bimonthly. After $5{\sim}12$ months, the impacted permanent teeth were erupted on the desired position. All cases presented favorable result. By extracting the infected primary teeth, and opening the cyst for continuous drainage, it was possible to achieve spontaneous eruption of the involved permanent teeth into the proper position. In all our cases, there was no sign or symptom of recurrence of the cyst up to postoperative 18 months.

Ceramide has been suggested as an important mediator of the effects of extracellular agonists on cell growth inhibition, differentiation, apoptosis. However the biochemical sign aling mechanism involved in transducing the effects of ceramide on leukemia cell differentiation is still unclear. In these respects, we examined the regulatory effects of ceramide on c-jun gene expression during differentiation. In U-937 cells. ceramide increased c-jun mRNA levels in a time-dependent manner. The half life, of c-jun mRNA was 30 min. In contrast, inhibition of protein synthesis with cycloheximide in the absence, of transcription with actinomycin D increased the half-life of c-jun mRNA in ceramide-treated U-937 cells to more than 90 min. In order to examine whether ceramide-inhibited c-jun gene expression is regulated through ceramide-activated protein phosphatase (CAPP), a direct target for the action of ceramide, okadaic acid were treated to the cells. Okadaic acid inhibited enhancement of c-jun mRNA induced by C2-ceramide in a dose-dependent manner. These results suggested that ceramide increases c-jun mRNA level during differentiation in U-937 cells and regulates the gene expression on posttranscriptional level. In addition, we provide the evidence that CAPP is involved in ceramide-induced c-jun gene expression in U-937 cells.

The purpose of this study was to develop an accurate frequency-dependent type apex locator. To compensate the impedence differences of different canal contents, voltage differences were pre-measured and stored for saline, HOsb1/$2O_2$ and NaOCl. The circuit was adjusted to recognize the different voltages of each solution, thereby the machine could automatically compensate the voltage differences. In the process of this study, the following informations were observed. 1. Most stable apical reading was obtained at 500Hz and 10kHz. 2. There were no differences in accuracy among three different frequency types, sign wave, triangular wave and rectangular wave(p>0.05). 3. Before the compensation of voltage differences, saline, H2O2 and NaOCl showed different readings at the apex even with the frequency-type (p<0.05). 4. After compensating the voltage difference, the accuracy was enhanced from + 0.42 to +0.32 in H2O2(p<0.05), from -0.34 to -0.12 in NaOCl(p<0.05).

Ventricular septal defects (VSDs) is an opening in the ventricular septum that allows blood to flow between the ventricles. Most ventricular septal defects are located in the upper ventricular septum and can be identified by auscultation. In this report, 2 dogs with heart murmur were diagnosed as VSD using radiography and echocardiography. In radiographs, bulging sign of the main pulmonary artery or the enlargement of the left ventricle was observed. The color Doppler examination showed the left-to-right shunting of blood via interventricular septal defect.

A 7-month-old female Shih-tzu dog was presented with intermittent trembling, dyspnea, generalized muscle weakness, and unconsciousness after exercise. No remarkable findings were shown in the complete blood counts and the radiographic examination. On serum biochemical profiles, alkaline phosphatase and creatine phosphokinase were mildly elevated. Based on history takings, physical examination, and neurological findings, presumptive diagnosis was made as a myasthenia gravis (MG). Clinical signs of this patient were dramatically improved after administration of neostigmine. The result of acetylcholine receptor antibody test in serum was 0.89 nmol/L and the histopathology of muscle were normal. Clinical sign of the patient evaluated in this study is stabilized with long-term administration of pyridostigmine at this time. This case report here describes clinical and clinicopathological findings of a juvenile onset acquired MG in a Shih-tzu dog.

Purpose: The purpose of this research was to develop and evaluate the effect of a mind map for relief of emotional labor and burnout among nurses in outpatient departments in cancer hospitals. Methods: We developed a mind map to reduce emotional labor and burnout. A quasi-experimental study was used with a nonequivalent control group pretest-posttest design. Data were collected from December 2012 to April 2013. Participants were 35 nurses working in the outpatient department of a cancer hospital. The experimental group participated in the mind map program biweekly for 10 weeks. Data were analyzed using $x^2$-test, Mann-Whitney U test, paired t-test, and Wilcoxon sign rank test with the SPSS 21.0 program. Results: The physical burnout and total burnout scores decreased significantly in the intervention group which took the mind map program. Conclusion: Findings indicate that the mind map is an effective intervention to reduce burnout in outpatient department nurses.

A ranula is a bluish, transparent, and thin-walled swelling in the floor of the mouth. They originate from the extravasation and subsequent accumulation of saliva from the sublingual gland. Ranulas are usually limited to the sublingual space but they sometimes extend to the submandibular space and parapharyngeal space, which is defined as a plunging ranula. A 21-year-old woman presented with a complaint of a large swelling in the left submandibular region. On contrast-enhanced CT images, it dissected across the midline, and extended to the parapharyngeal space posteriorly and to the submandibular space inferiorly. Several septa and a fluid-fluid level within the lesion were also demonstrated. We diagnosed this lesion as a ranula rather than cystic hygroma due to the location of its center and its sublingual tail sign. As plunging ranula and cystic hygroma are managed with different surgical approaches, it is important to differentiate them radiologically.

The high energy fluid leakage from the high temperature and high differential pressure drop system of NPPs (Nuclear Power Plants) decreases efficiency and consequently leads to considerable economic loss due to less power production. Also, the leakage possibly damages critical parts of components such as valve and trim with the effect of cavitation, flashing, and erosion, etc. and deteriorates its performance. Thus, in this study, we diagnosed the as-is leakage for four (4) main steam drain valves and two (2) steam traps of Yonggwang 1,2 units during normal operation by using multi-measuring technique and observed the occurrence of fine leakage. In the course of measuring fluid leakage, the sign of fine leakage is estimated to be the leakage from orifice. By converting the leakage to energy loss, it is equivalent to the amount of several hundred thousand won per each unit, which supports the basis for the justification of fine leakage.
