Health characteristics and symptom of workers in reactive dye industries (염료공장 근로자의 직업성 천식에 관한 역학적 연구)

  • This study investigated the symptoms, medical and occupational history of 424 workers of 5 reactive dye Industries in the Inchon area in Korea. The study was performed on March 6 and July 19, 1989. The tests applied to the subjects were : serum total IgE, specific IgE, skin prick test with 7 inhalatory antigens, pulmonary function test, chest X-ray, methacholine test, and bronchoprovocation test. The workers were classified according to these tests into 4 groups (healthy, realtively healthy, need careful medical observation, and occupational asthma), and were compared in terms of the group characteristics and the symptom prevalence. The prevalece of occupational asthma of workers in reactive dye was 5.9% Significant differences were observed among the 4 groups. The groups were significantly different in the variables of sex and duration of smoking among their general characteristics ; and dyspnea, wheezing, chest pain, cough, nasal symptoms and sore throat among symptoms ; asthma, bronchitis, and other respiratory diseases with respect to their past medical history. This study suggests that we should pay special attention to the workers exposed to the risk of occupational asthma.

  • In adults, aspiration of foreign bodies into tracheobronchus is sometimes presented atypically, mimicking chronic pulmonary diseases such as bronchial asthma, pneumonia, or empyema, especially without notice of aspiration or choking. In such cases, diagnosis and adequate treatments are often delayed. Suspicion of foreign body aspiration and computed tomography and bronchoscopic examination make correct diagnosis more early. We report a case of bronchial foreign body, which had been misdiagnosed as bronchial asthma with wheezing in the whole lung filed, and reversible airway obstruction in the spirometry.

Effects of Inhalable Microparticles of Seonpyejeongcheon-Tang in an Asthma Mouse Model - Effects of Microparticles of SJT -

  • Objectives: Allergic asthma generally presents with symptoms of wheezing, coughing, breathlessness, and airway inflammation. Seonpyejeongcheon-tang (SJT) consists of 12 herbs. It originated from Jeong-cheon-tang (JT), also known as Ding-chuan-tang, composed of 7 herbs, in She-sheng-zhong-miao-fang. This study aimed to evaluate the effects of local delivery of SJT via inhalable microparticles in an asthma mouse model. Methods: Microparticles containing SJT were produced by spray-drying with leucine as an excipient. SJT microparticles were evaluated with respect to their aerodynamic properties, in vitro cytotoxicity, in vivo toxicity, and therapeutic effects on ovalbumin (OVA)-induced asthma in comparison with orally-administered SJT. Results: SJT microparticles provided desirable aerodynamic properties (fine particle fraction of $48.9%{\pm}6.4%$ and mass median aerodynamic diameter of $3.7{\pm}0.3{\mu}m$). SJT microparticles did not show any cytotoxicity against RAW 264.7 macrophages at concentrations of 0.01 - 3 mg/mL. Inhaled SJT microparticles decreased the levels of IL-4, IL-5, IL-13, IL-17A, eotaxin and OVA-IgE in bronchoalveolar lavage fluid (BALF) in mice with OVA-induced asthma. These effects were verified by histological evaluation of the levels of infiltration of inflammatory cells and collagen, destructions of alveoli and bronchioles, and hyperplasia of goblet cells in lung tissues. The effects of SJT microparticles in the asthma model were equivalent to those of orally-administered SJT extract. Conclusion: This study suggests that SJT is a promising agent for inhalation therapy for patients with asthma.

  • Gastroesophageal reflux disease(GERD) refers to the varied clinical manifestations of reflux of stomach and duodenal contents into the esophagus and is preferable to the 'reflux esophagitis'. Clinical symptoms of reflux esophagitis are heartburn, dysphagia, blood loss, abdominal pain, nocturnal wheezing, coughing, hoarseness. We applied oriental medicines to patient who had reflux esophagitis. we treated them with herb-medication, acupuncture, moxa therapy and aroma therapy for about 2 months. During this period, we observed that symptoms had been improved.

Influence of Gungguitang-gamibang on the Regulation of Melanogenesis through JNK Signaling Pathway in B16 Melanoma Cells

  • Gunggui-tang has been used for the therapy of blood disorders in Hangbang medicine for long time. Also, Glycyrrhiza uralensis has been used for deficientblood patterns with an irregular pulse or palpitations, coughing and wheezing, and heat or cold in the lungs. Melanogenesis is a physiological process resulting in the synthesis of melanin pigments. We investigated whether the water extract of Gunggui-tang plus G. uralensis inhibited melanogenesis in B16 melanoma cells. Because the molecular events connecting the regulation in tyrosinase activity remain to be elucidated, we also aimed to determine whether Gunggui-tang gamibang(GTG) affects tyrosinase at the gene activation level in the cells. First, we showed that GTG inhibited the tyrosinase promoter activity and further, down-regulated the tyrosinase protein activity in ${\alpha}-melanocyte-stimulating$ hormone $({\alpha}-MSH)-treated$ B16 melanoma cells. GTG also resulted in a decrease of melanin content in MSH-induced melanogenesis, indicating that GTG may be a useful drug in studying the regulation of melanogenesis. The pretreatment of GTG significantly prevented phosphotransferase activity of c-Jun N-terminal kinase (JNK1) and transcriptional activation of activating protein-1 (AP-1) in MSH-treated B16 melanoma cells. These findings indicate that GTG inhibits melanogenesis of B16 melanoma cells via suppression of phosphotransferase activity of JNK1 and transcriptional activation of AP-1.

A Comparative Study on Responses to Korean Version Questionaires on Respiratory Symptoms (호흡기계(系) 질환의 역학적(疫學的) 조사방법 개발에 관한 연구(I) -번역설문서 응답양상(應答樣相)에 대한 비교평가-)

  • Korean versions of British Medical Research Council (MRC), Cornell Medical Index(CMI), and American Thoracic Society 78 (ATS-DLD-78) respiratory questionaires were compared with each other, and were tested the stability, in terms of test-retest reliability, of each questions by self-administration of those to 156 medical students. The results obtained and conclusions drawn are as follow: 1. The degree of agreements between responses to the comparable questions of CMI vs MRC, and of CMI vs ATS-DLD-78 were not satisfactory. There were, however, $71{\sim}100$ per cent of agreement between responses to the questions on Cough, Wheezing, Phlegm, Breathlessness, and Chest illness of ATS-DLD-78 vs MRC questionaire. And the ATS-DLD-78 tended to yield greater number of positive responses than MRC (See Table 4). 2. All of the coefficient of stability of each questions in 3 questionaires were statistically significant, ranged $77{\sim}100$ per cent, except that of the question on episode of cough and phlegm in ATS-DLD-78 questionaire (See Table 5-1). The question is composed of two collateral conditions, 'lasting for 3 weeks or more' and 'each year'. 3. It can be insisted that the Section-B questions of CMI is not proper for use in epidemiologic survey on respiratory illness. And rather than MRC, the ATS-DLD-78 questionaire deserves to prefer to be used in epidemiologic studies on respiratory illness. 4. In question-wording, especially, of inquiring past experience, it is possible to lessen the reliability of the question that including collateral conditions such as 'the duration lasted of symptoms', and moreover, of which words are not common usage. For example, for Korean '10days' or 'half a month' is more familiar time unit rather than 'week'.

  • A functional upper airway obstruction due to a vocal cord dysfunction(VCD) is characterized by a paradoxical adduction of the vocal cords throughout the respiratory cycle with no obvious organic cause for the obstruction. It commonly occurs paroxysmally and imitates acute asthmatic attacks, often in patients with coexisting asthma. They present with episodes of dyspnea associated with inspiratory wheezing that persists despite conventional asthma treatment and a flattening of the inspiratory limb of the flow-volume curve ; an adduction of the vocal cord during inspiration. Failure to recognize concurrent vocal cord dysfunction and asthma has led not only to the excessive use of bronchodilators and corticosteroids, but also to intubation and tracheostomy. Here, we report a case of coexistent obstructive pulmonary disease and functional upper airway obstruction due to a vocal cord dysfunction where a bronchoscopy showed a paradoxical vocal cord motion and typical features of a variable extrathoracic obstruction and a lower airway obstruction on the Flow-volume loop during a symptomatic period.

We have known that the symptom of shortness of breath due to fluid retention are similiar to cardiac rapid respiration, and the symptom of dyspnea acused by fire-evil alike 'dyspnea at moving' acused at mild case of cardiac rapid respiration.'

The Impact of Air Pollution, Including Asian Sand Dust, on Respiratory Symptoms and Health-related Quality of Life in Outpatients With Chronic Respiratory Disease in Korea: A Panel Study

  • Objectives: Air pollution is a growing concern in Korea because of transboundary air pollution from mainland China. A panel study was conducted to clarify the effects of air pollution on respiratory symptoms and health-related quality of life (HR-QoL) in outpatients with and without chronic obstructive pulmonary disease (COPD) in Korea. Methods: Patients filled out a questionnaire including self-reported HR-QoL in February and were followed up in May and July. The study was conducted from 2013 to 2015, with different participants each year. Air quality parameters were applied in a generalized estimating equation as independent variables to predict factors affecting HR-QoL. Results: Lower physical fitness scores were associated with Asian sand dust events. Daily activity scores were worse when there were high concentrations of particulate matter (PM) less than $10{\mu}m$ in diameter ($PM_{10}$). Lower social functioning scores were associated with high PM less than $2.5{\mu}m$ in diameter and nitrogen dioxide ($NO_2$) concentrations. High $NO_2$ concentrations also showed a significant association with mental health scores. Weather-related cough was prevalent when $PM_{10}$, $NO_2$, or ozone ($O_3$) concentrations were high, regardless of COPD severity. High $PM_{10}$ concentrations were associated with worsened wheezing, particularly in COPD patients. Conclusions: The results suggest that PM, $NO_2$, and $O_3$ cause respiratory symptoms leading to HR-QoL deterioration. While some adverse effects of air pollution appeared to occur regardless of COPD, others occurred more often and more intensely in COPD patients. The public sector, therefore, needs to consider tailoring air pollution countermeasures to people with different conditions to minimize adverse health effects.

Mahaenggamsuktang for treating Mycoplasma pneumonia in Children: Systematic Review and Meta-analysis of Randomized Controlled Trials (RCTs) (마행감석탕의 소아 마이코플라즈마 폐렴 치료에 대한 체계적 문헌 고찰 및 메타분석)

  • Objectives The purpose of this study is to evaluate efficacy and safety of Mahaenggamsuktang for treating mycoplasma pneumonia in children based on the randomized controlled trials (RCTs). Methods Literatures were searched from OASIS, KISS, NDSL, CNKI, Cochrane, Embase and Pubmed, and the search was conducted on January 29, 2020. Only RCTs published since 2000 were included. Trials comparing Mahaenggamsuktang combined with antibiotics or antibiotics treatment alone for the treatment of mycoplasma pneumonia in children were included. Results 17 trials, including 2,241 participants with mycoplasma pneumonia were included in this review. As a result of the meta-analysis, total effective rate of combination of Mahaggamsuktang and antibiotics was 1.24 times higher than that of the antibiotics alone, which was statistically significant. Symptoms with fever, lung sounds, cough, chest X-ray lesion findings, wheezing were also significantly reduced in the treatment group with Mahaenggamseoktang and antibiotics. Also, Serum CRP level was significantly lower with combination treatment. The incidence of adverse reactions was lower in the treatment group with Mahaenggamseoktang and antibiotics, but it was not statistically significant. Conclusions As a result of meta-analysis, combination treatment of Mahaenggamseoktang and antibiotics seems significantly effective for the treatment of mycoplasma pneumonia in children. In order to have a higher level of evidence for efficacy and safety of Mahaenggamsuktang in treating mycoplasma pneumonia, additional RCTs with good qualities are required.
