Background: The purpose of this study was to investigate differences in balancing ability and weight bearing between paretic and non-paretic. Methods: Subjects of the study were consisted of 30 acute hemiplegia patients(paretic 15, non-paretic 15) who were receiving rehabilitation therapy in hospital. Weight shifting was quantitatively evaluated to determine the ability of patients to and during conscious weight shifting on the Gaitview. Independent t-test was utilized to detect the mean difference between the groups. Results: 15 patients with stroke were able to shift more weight onto the paretic leg. And pressure ratio of right hemiplegia was significantly difference on the nonparetic leg, pressure ratio of left hemiplegia was significantly difference onto the paretic leg. Conclusions: Patients with left hemiplegia who was cortical lesions in the right hemisphere were able to shift more body weight onto their paretic leg. These patients should be encouraged to practice shifting their weight towards their non-paretic leg to improve their balance.
The purpose of this study was to determine the effect of shoe lift of the affected limb in subjects with hemiplegia. The subjects of this study were 18 post-stroke hemiplegics. For the study, insole of the paretic side was lifted 10 mm higher, and static weight bearing was measured before and after the lift application. For the measurement of carry-over effect of lift, we got data of those three items prior to and 3 weeks after lift application and 3 days after removal of the lift. Static weight bearing was significantly increased both just after and continuous application of lift for 3 weeks than before. According to this study, lift applied to the shoe of the paretic limb was effective in inducing static weight bearing in the paretic limb. This study suggests that symmetry, induced by shoe lift applied to the paretic limb, could help correct abnormal posture that would be caused in standing and prevent development of abnormal muscle tone in subjects with hemiplegia caused by unilateral stroke.
The purpose of this study was to investigate the effects of a task-oriented approach on weight-bearing distribution and muscular activities of the paretic leg during sit-to-stand movement in 18 chronic stroke patients. Both groups were received neurodevelopmental treatment for 30 min/day and then the experimental group ($n_1$=9) followed additional a task-oriented approach (sit-to stand training with controlled environment) and the control group ($n_2$=9) followed a passive range of motion exercise for 15 min/day, five days/week, for four weeks. Weight-bearing distribution and muscular activities of the paretic leg during sit-to-stand movement were measured before and after four weeks of training. There was significantly improved weight-bearing distribution of the paretic leg during sit-to-stand movement in the experimental group compared with that of the control group after four weeks of training (p<.05). But electromyographic activities of the quadriceps and the tibialis anterior of the paretic leg were not significantly different (p>.05). Thus, it is necessary to apply a task-oriented approach to improve the weight-bearing distribution of the paretic leg during sit-to-stand movement in chronic stroke patients.
Hemiplegia-induced immobilizatoin and reduction of mechanical loading in chronic stroke limbs are common cause of disuse osteoporosis. The purpose of this study was to investigate the effects of asymmetrical weight bearing on the loss of bone mineral in the individual with chronic stroke. Sixteen hemiplegic patients with strokes were evaluated. The measurements of bone mineral density (BMD) were evaluated with the quantitative ultrasound system on the calcaneus region of the paretic and non-paretic side. Plantar pressure was measured using the Mat-Scan system. The paretic side showed significantly smaller values in the T-score of BMD, and peak value of plantar pressure, which included forefoot, midfoot, and hindfoot, than the non-paretic side (p<.05). Results from the pearson correlation analysis showed statistically significant correlation between the BMD difference and the peak-pressure difference of midfoot pressure (p<.05). This finding indicated that BMD loss depended on decrease of body weight born on the paretic leg.

The purpose of this study was to investigate whether the standing balance could be influenced by the different foot positions. Seventeen patients with hemiplegia were tested for the static and dynamic balance under the different foot positions. In the balance test, subject stood by bearing weight on one foot, and the other foot was positioned in three different positions (symmetric, $45^{\circ}$ anterolateral, and anterior position). This study used the Kinesthetic ability trainer (KAT2000) to measure the standing balance. The results were as follows: 1) There were significant differences in the static standing balance in different foot positions with both weight-bearing on the paretic limb and on the nonparetic limb (p<.05). 2) There were also significant differences in the dynamic standing balance in different foot positions with both weight-bearing on the paretic limb and on the nonparetic limb (p<.05). 3) There was a significant difference when the paretic weight-bearing and the nonparetic weight-bearing were compared (p<.01). 4) when the paretic weight-bearing and the nonparetic weight-bearing were compared, anterior foot position showed a significant difference in the dynamic standing balance (p<.05), but $45^{\circ}$ anterolateral foot position did not show a significant difference (p>.05). In this study, the standing balance showed a significant difference according to different foot positions in hemiparetic patients, and standing balance was better when they stood by bearing weight on the nonparetic limb. These results indicate that it is a necessary to consider both weight-bearing limb and foot position not only in the rehabilitation program but also in achieving the stability in the independent life.

Duration of stance phase was not changed among anytime of application. According to this study, lift applied to the shoe of the peretic limb was effective in inducing static weight bearing in the paretic limb, but did not significantly effect dynamic weight bearing on gait patterns. The purpose of this study was to reduce the spasticity of plantarflexion. the subjects of this study were 30 hemiplegic patients with stroke who received of physical therapy in JinJu o o hospital from May to July 2000. the subjects were divided into three groups(FES groups 10, FES + tilt table-wedge board standing groups 10, & tilt table-wedge board standing groups 10). The result were as follow 1. FES therapy was a effective method to reduce the spasticity of plantarflexor. there was a significant difference in modified Ashworth scale(p<.01). there was a significant difference in weight bearing ratio between nonparetic and paretic side(p<.01). 2. Tilt table-wedge board standing therapy was a effective method to reduce the spasticity of plantarflexor to a degree but there was a no significant difference in modified Ashworth scale(p<.05). there was a significant difference in weight bearing ratio between nonparetic and paretic side(p<.01). 3. Tilt table-wedge board standing therapy + FES therapy was a effective method to reduce the spasticity of plantarflexor. there was a significant difference in modified Ashworth scale(p<.01). there was a significant difference in weight bearing ratio between nonparetic and paretic side(p<.01) 4. There was a significant difference in weight bearing ratio between nonparetic and paretic side according to the grade spasticity(p<.01). 5. For normal persons vs hemiplegic patients, there was a significant different in weight bearing ratio between nonparetic and paretic side(experimental subjects 1 p<.01, experimental subjects 2 p<.01, control subjects p<.05).
Purpose: This study aimed to determine the interaction among the neck, trunk, and lower extremities on the non-paretic side in head rotation along with non-paretic-side weight shifting of stroke patients. To compare stroke patients' ability to control posture through muscle activity variation related to pertubation during head rotation along with the non-paretic limb. Methods: We tested 15 hemiplegic patients and 15 normal individuals. Each group's muscle activity was measured by electromyography in neutral head position and head rotation position. We compared each group's resu lt based on measured values in patients' non-paretic neck muscles, trunk muscles, and lower limbs muscles activation. Results: The study showed that muscle activity increased in the sternocleidomastoid muscle (102.26%, 53.00%), splenius capitis muscle (97.93%, 54.93%), erector spinae muscle (241.00%, 127.60%), external oblique abdominal muscle (256.66%, 152.00%), and internal oblique abdominal muscle (252.80%, 152.6%), peroneus longus muscle (117.53%, 137.13%) and gastrocnemius muscle (119.06%, 137.20%), while the results for the sternocleidomastoid muscle, splenius capitis muscle, erector spinae muscle, external oblique abdominal muscle, internal oblique abdominal muscle, peroneus longus muscle, and gastrocnemius muscle showed a statistically significant difference (p<0.05). Conclusion: It is hard for stroke patients to engage in normal movement control under suggested conditions because of the insufficient movement against gravity on the stroke patient's non-paretic side and impaired cooperative patterns. To solve these problems, patients need their bodies to improve through effective movement, resulting in advanced control of their effective and functional activity.
Patients with hemiplegia usually show different body weight distribution as compared with normal subjects. Asymmetrical posture during static stance has been identified as a common problem in patients with hemiplegia. The purpose of this study was to identify the effects of vision and visual feedback on body weight distribution while standing under three conditions: eyes-closed, eyes-open and visual feedback condition. Fourteen patients with hemiplegia participated in the study. Their body weight distribution during standing for 20 seconds was measured by Limloader. The data were analysed by repeated measure of one-way ANOVA. The weight bearing on the paretic limb in the eyes-open condition was significantly higher than that of the eyes-closed condition. The weight bearing on the parietic limb in the visual feedback condition was significantly higher than that of the eyes-open condition. These results suggest that patients with hemiplegia can improve their symmetrical stance ability using visual feedback.
