지지면에 따른 체중 이동 시 체간 근육의 근활성도에 미치는 영향 (Effects of the Support Surface Condition on Muscle Activity of Trunk Muscles during Weight Shifting Exercise)

  • Purpose: This study examined the muscle activity of the trunk muscles during weight shifting exercises on a stable and unstable surface. Methods: Ten healthy young subjects (4 males, 6 females) with no medical history of lower-extremity or lumbar spine disease were enrolled in this study. The muscle activity was recorded using surface electromyography (EMG) electrodes from the both sides of the external obliques and erector spinae muscles. Results: The EMG activities of the both rectus abdominalis, both external obliques and erector spinae muscles were significantly higher when the weight shifting exercise was performed on a firm surface than when a balance pad with a balance ball was used. In weight shifting exercises, the EMG activity of the contralateral external obliques and erector spinae muscles was significantly higher than that of the ipsilateral external obliques and erector spinae muscles under all three support surface conditions. Conclusion: Performing weight shifting exercise using an unstable surface is a useful method for facilitating the trunk-muscle strength and trunk stability.

Effect of Weight Ball Throw Training on Weight Shifting of Lower Body, Head Speed of Club, and Driving Distance of Amateur Golfers

  • PURPOSE: To determine the effect of weight ball throw training as a preparatory exercise before golf practice for 8 weeks on back muscle strength, weight shifting of lower body, head speed of club, and driving distance of amateur golfers. METHODS: A total of 18 subjects were randomly assigned to the experimental group (n=9) and the control group (n=9), respectively. For the experimental group, Weight ball throw training was provided to the height of waist and shoulder similar to golf swing with the following schedule: 3 kg weight ball throw training from the first week to the 4th week; 5 kg weight ball throw training from the 5th week to the 8th week. Before and after 8 weeks of training, back muscle strength, weight shifting of lower body, head speed of club, and driving distance of subjects in the two groups were measured. RESULTS: The experimental group showed significant differences in rotational back extension torque, weight shifting of lower body, head speed of club, and driving distance during golf swing (p<.05). However, the control group only showed significant difference in driving distance during golf swing (p<.05). Back extension torque, weight shifting of lower body, and head speed of club showed significant differences between the two groups during golf swing (p<.05). CONCLUSION: Weight ball throw training can positively change rotational back muscle strength, weight shifting of lower body, head speed of club, and driving distance of amateur golfers. Therefore, it might be used as an effective warming up exercise for amateur golfers.

Effects of trunk control robot training on balance and gait abilities in persons with chronic stroke

  • Objective: To investigate the effects of training using a trunk control robot (TCR) system combined with conventional therapy (CT) on balance and gait abilities in persons with chronic stroke. Design: Two-group pretest-posttest design. Methods: Thirty-five subjects with chronic stroke were randomly assigned to either the TCR group (n=17) or the trunk extension-training (TET) group (n=18). Both groups performed CT for 30 minutes, after which the TCR group performed TCR training and the TET group performed trunk extension training for 20 minutes. Both groups performed the therapeutic interventions 3 days per week for 6 weeks. Balance ability was evaluated using the Berg Balance Scale (BBS), and the Timed Up-and-Go (TUG) test. Gait ability was measured using the 10 m Walk Test (10MWT) and the NeuroCom Smart Balance Master. Results: TCR group showed significant improvements in static balance (weight bearing) and dynamic balance (weight shifting speed, weight shifting direction, BBS, and TUG), 10MWT, gait speed, and step width (p<0.05); step length was not significant. The TET group showed a significant partial improvement of dynamic balance (weight shifting speed, weight shifting direction, BBS, and 10MWT (p<0.05), but the improvements in static balance, TUG, gait speed, and step width and step length was not significant. Additionally, significant differences in static balance, dynamic balance (weight shifting speed, weight shifting direction, BBS, and TUG), 10MWT, gait speed, and step width were detected between groups (p<0.05). Conclusions: TCR training combined with CT is effective in improving static and dynamic balance, as well as gait abilities in persons with chronic stroke.

급성 뇌졸중환자의 마비측 체중지지 임상양상 (Weight-bearing on Paretic in Patients with Acute Stroke)

  • Background: The purpose of this study was to investigate differences in balancing ability and weight bearing between paretic and non-paretic. Methods: Subjects of the study were consisted of 30 acute hemiplegia patients(paretic 15, non-paretic 15) who were receiving rehabilitation therapy in hospital. Weight shifting was quantitatively evaluated to determine the ability of patients to and during conscious weight shifting on the Gaitview. Independent t-test was utilized to detect the mean difference between the groups. Results: 15 patients with stroke were able to shift more weight onto the paretic leg. And pressure ratio of right hemiplegia was significantly difference on the nonparetic leg, pressure ratio of left hemiplegia was significantly difference onto the paretic leg. Conclusions: Patients with left hemiplegia who was cortical lesions in the right hemisphere were able to shift more body weight onto their paretic leg. These patients should be encouraged to practice shifting their weight towards their non-paretic leg to improve their balance.

체중이동 과제 학습시 효과적인 운동학적 되먹임 유형과 상대적 빈도 (Kinetic Feedback Frequency Effects on Learning Weight Shifting Skills in Nondisabled Subjects)

  • Physical therapists have been using balance and weight shifting training to induce improvements in standing and walking. This study compared the effects of kinetic feedback frequency and concurrent kinetic feedback on the performance and learning of a weight shifting skill in young, nondisabled adults. Sixteen young adults without known impairment of the neuromusculoskeletal system volunteered for the study. Subjects in each of three kinetic feedback groups performed a weight shifting task in an attempt to minimize error between their effort and a center of pressure (COP) template for a 12 second period. Feedback was provided: 1) concurrently (concurrent feedback), 2) after each trial (100% feedback), 3) after every other trial (50% feedback). Immediate and delayed (24 hour) retention tests were performed without feedback. During acquisition phase, the concurrent feedback group exhibited less error than either of the post response feedback group. For the immediate retention test, the 50% feedback group exhibited less error than did the 100% feedback and concurrent feedback. During the delayed retention, 50% feedback group displayed less error than did the other groups. But no significant differences were found between groups. These results suggest that practice with concurrent feedback is beneficial for the immediate performance, but not for the learning of this weight shifting skill. Lower frequency of feedback resulted in more permanent changes in the subject's ability to complete the task.

Effect of Changes in Knee Angle and Weight-Shifting of the Sole on the Lower Extremity Muscle Activity during the Bridge Exercise

  • PURPOSE: This study examined the effect of changes in the knee angle and weight shifting of the sole on the activity of the lower extremity muscles during bridge exercise. METHODS: The subjects of this study included 20 healthy adult women (mean age 29.8 ± 4.32). The subjects performed the bridge exercise under three weight-shifting conditions general bridge (GB), hindfoot press bridge (HPB), and fore-foot bridge (FPB) and at two knee angles (90° and 60°). During the bridge exercise, the activity of the quadriceps femoris (rectus femoris, vastus medialis oblique, and vastus lateralis) and biceps femoris muscles were measured using an electromyography sensor. RESULTS: In the quadriceps femoris, the muscle activity of HPB and FPB was significantly higher than that of the GB at knee angles of 90° and 60° (p < .05). In the biceps femoris, the muscle activity increased significantly in the order of GB < HPB < FPB, and the knee angle increased significantly at 60° rather than at 90° (p < .05). There was no significant difference according to the knee angle in all muscles except for the biceps femoris. CONCLUSION: These findings suggest that the weight-shifting bridge of sole bridge exercise was more effective in increasing the activation of the lower extremity muscles than the GB.

편마비 환자의 골반경사 각도에 따른 하지체중지지 및 체중이동에 관한 연구 (A Study on Weight Bearing and Weight Shifting of Lower Extremity According to the Pelvic Tilt Angle in Hemiplegic Patients)

  • The purpose of this study was l)to compare a pelvic tilt angle between sound side and affected side in hemiplegic patients, 2)to determine the difference of weight bearing and weight shifting between sound and affected lower extremity according to the pelvic tilt angle. The subject for the study were 40 hemiplegic patients(mean age of 55.6 years)without orthopedic disability on pelvic bone. The data were analyzed by t-test, one way ANOVA, Multiple comparison and Range. The results of this study were as follows : 1. There was a significant difference in the pelvic tilt angle between sound side and affected side in hemiplegic patients(p<0.05). 2. There was a significant difference in the weight bearing value between sound and affected side in hemiplegic patients according to the posterior pelvic tilt angle(F = 12.43, df = 3/36, p<0.001). Therefore, the lesser the posterior pelvic tilt angle, the higher the weight bearing value on the affected side. 3. The lesser the posterior pelvic tilt angle, the higher the weight shifting value on the affected side. 4. There was a significant difference in weight bearing value between sound and affected side according to the grade spasticity(F = 3.61, df = 4/35, p<0.05). Therefore, the higher the grade spasticity, the lower the weight bearing value on affected side.

  • The aim of this study was to develop and verify gait training system for post-stroke hemiplegia patients with step length asymmetry. Most post-stroke hemiplegic patients show gait asymmetry and weight shifting training has been suggested as a useful method for improving the walking ability. However, verbal cue by physical therapist may be not effective. Therefore, our weight shift training system was designed to give a feedback to patients through precise plantar pressure and center of pressure (COP) measurement. This weight shifting biofeedback training system is composed of F-Scan plantar pressure measurement system and software development kit (SDK) for Windows operating system. Two post-stroke patients with step length asymmetry were enrolled in this study. After training for six weeks, the weight shift score and step length ratio of two all patients were improved and approached to them of non-disabled. This system developed in this study may improve the step length asymmetry, and therefore this system is also expected to improve a walking ability in hemiplegic patients.

테이핑이 편마비 환자의 결관절 아탈구에 미치는 효과 (The effect of taping therapy on shoulder in hemiplegic patients)

  • The purpose of this case study was to evaluate the taping therapy program designed to improve shoulder joint subluxation in hemiplegic patients. This study was 9 males who had hemiparesis with limitation of range of motion (ROM) and subluxation on affected side. Patients received proprioreptive facilitation using shoulder weight shifting and additional A type and B type taping. As a result of this study, Patients showed reduction of the subluxation on hemiplegic shoulder joint. but it was difficult to generalize. We concluded that proprioceptive facilitation using shoulder weight shifting and taping therapy was effective in reducing severe shoulder subluxation. Following studies must be evaluated its statistical significance.

과제 지향 훈련이 슬관절 전치환술을 시행한 퇴행성 관절염 환자의 균형 능력에 미치는 효과 (Effect of Task-Oriented Exercise on the Balance in Degenerative Arthritis Patients with Total Knee Replacement)

  • 본 연구는 슬관절 전치환술을 시행하고 난 후 3주 이내의 환자 24명에게 과제 지향 훈련과 체중 이동 훈련이 정적 균형과 동적 균형에 어떠한 영향을 미치는지 알아보기 위해서 실시하였다. 대상자는 과제 지향 훈련군(실험군) 12명과 체중 이동 훈련군(대조군) 12명을 2그룹으로 무작위 배정하였다. 각 군의 운동 효과를 알아보기 위하여 Gaitview 시스템과 기능적 팔 뻗기 검사를 통해 정적 균형을 측정하였고, Balance 시스템을 이용하여 동적 균형을 측정하였다. 통계 분석은 실험 전과 후의 차이를 알아보기 위하여 대응표본 t검정을 실시하였고, 그룹 간 차이를 알아보기 위하여 독립표본 t검정을 실시하였다. 그 결과 과제 지향 훈련군이 체중 이동 훈련군에 비해 정적, 동적 균형 능력에 유의한 차이를 보였다(p<0.05). 따라서 이번 연구 결과를 통해 슬관절 전치환술을 시행한 조기 환자들에게 과제 지향 훈련이 균형 능력에 효과적이라는 것을 알 수 있었다.
