Effects of garlic intake on cancer: a systematic review of randomized clinical trials and cohort studies

  • BACKGROUND/OBJECTIVES: Due to the rapid increase of global cancer incidence and mortality and a high level of interest in cancer prevention, a systematic review of garlic intake and cancer risk is needed. SUBJECTS/METHODS: We implemented a systematic review to examine the effects of varying levels of garlic intake on cancer. We conducted comprehensive literature searches in three electronic databases (MEDLINE, Embase, and Web of Science) for studies published between database inception and July or September of 2018. Two investigators independently screened abstracts and full-texts, extracted data, and assessed risk of bias (RoB). A total of one medium-quality randomized controlled trial (RCT) and 13 cohort studies graded as high RoB were included. RESULTS: The 1-year follow-up results from a RCT showed that a significant decrease in the number and size of colorectal adenomas among participants with colorectal adenomas who received high-dose aged garlic extract (AGE) compared with those who received low-dose AGE (P < 0.05). The results of prospective observational studies provided inconsistent associations of colorectal cancer risk with garlic supplements and garlic intake as food. CONCLUSIONS: In summary, the AGE was effective in reducing the number and magnitude of colorectal adenomas in one RCT, but there were inconsistent associations between garlic intake and colorectal cancer in cohort studies. Therefore, we could not draw a firm conclusion regarding the effects of garlic on cancer, because the current strength of evidence is inadequate due to a lack of number of high-quality RCTs.

Text Mining and Visualization of Unstructured Data Using Big Data Analytical Tool R (빅데이터 분석 도구 R을 이용한 비정형 데이터 텍스트 마이닝과 시각화)

  • In the era of big data, not only structured data well organized in databases, but also the Internet, social network services, it is very important to effectively analyze unstructured big data such as web documents, e-mails, and social data generated in real time in mobile environment. Big data analysis is the process of creating new value by discovering meaningful new correlations, patterns, and trends in big data stored in data storage. We intend to summarize and visualize the analysis results through frequency analysis of unstructured article data using R language, a big data analysis tool. The data used in this study was analyzed for total 104 papers in the Mon-May 2021 among the journals of the Korea Institute of Information and Communication Engineering. In the final analysis results, the most frequently mentioned keyword was "Data", which ranked first 1,538 times. Therefore, based on the results of the analysis, the limitations of the study and theoretical implications are suggested.

The global prevalence of Toxocara spp. in pediatrics: a systematic review and meta-analysis

  • Background: Toxocariasis is a zoonotic parasitic disease caused by Toxocara canis and Toxocara cati in humans. Various types of T. canis are important. Purpose: The current study aimed to investigate the prevalence of Toxocara spp. in pediatrics in the context of a systematic review and meta-analysis. Methods: The MEDLINE (PubMed), Web of Sciences, Embase, Google Scholar, Scopus, and Cumulative Index of Nursing and Allied Health databases were searched to identify peer-reviewed studies published between January 2000 and December 2019 that report the prevalence of Toxocara spp. in pediatrics. The evaluation of articles based on the inclusion and exclusion criteria was performed by 2 researchers individually. Results: The results of 31 relevant studies indicated that the prevalence of Toxocara spp. was 3%-79% in 10,676 cases. The pooled estimate of global prevalence of Toxocara spp. in pediatrics was 30 (95% confidence interval, 22%-37%; I2=99.11%; P=0.00). The prevalence was higher in Asian populations than in European, American, and African populations. Conclusion: Health policymakers should be more attentive to future research and approaches to Toxocara spp. and other zoonotic diseases to improve culture and identify socioeconomically important factors.

MicroRNAs and periodontal disease: a qualitative systematic review of human studies

  • Purpose: MicroRNAs (miRNAs) are epigenetic post-transcriptional regulators that modulate gene expression and have been identified as biomarkers for several diseases, including cancer. This study aimed to systematically review the relationship between miRNAs and periodontal disease in humans, and to evaluate the potential of miRNAs as diagnostic and prognostic biomarkers of disease. Methods: The review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines (reference number CRD42020180683). The MEDLINE, Scopus, Cochrane Library, Embase, Web of Science, and SciELO databases were searched for clinical studies conducted in humans investigating periodontal diseases and miRNAs. Expression levels of miRNAs across the different groups were analysed using the collected data. Results: A total of 1,299 references were identified in the initial literature search, and 23 articles were finally included in the review. The study designs were heterogeneous, which prevented a meta-analysis of the data. Most of the studies compared miRNA expression levels between patients with periodontitis and healthy controls. The most widely researched miRNA in periodontal diseases was miR-146a. Most studies reported higher expression levels of miR-146a in patients with periodontitis than in healthy controls. In addition, many studies also focused on identifying target genes of the differentially expressed miRNAs that were significantly related to periodontal inflammation. Conclusions: The results of the studies that we analysed are promising, but diagnostic tests are needed to confirm the use of miRNAs as biomarkers to monitor and aid in the early diagnosis of periodontitis in clinical practice.

Korean Domestic Trends in Clinical Research on Rheumatoid Arthritis since 2008 (류마티스 관절염의 임상 연구 동향: 2008년 이후 국내 학술 문헌을 중심으로)

  • Objectives To investigate the trends in clinical study on rheumatoid arthritis in all academic fields in Korea. Methods We searched seven Korean web databases for clinical studies published from 2008 to 2021. All identified studies were reviewed and classified by the year of publication, academic field, and study design. Results A total of 347 clinical studies were selected. An average of 26.7 studies were published each year. Regarding the distribution of studies by academic fields, 309 studies were published in the medical field and 17 studies were published during the same period in Korean medicine, and 21 studies were published in other fields. The distribution of studies by study design was as follows: 8 studies, meta-analyses; 5 studies, systematic reviews; 16 studies, experimental studies; and 318 studies, observational studies. Conclusions Multidisciplinary access and management are required for the early diagnosis of rheumatoid arthritis, effective management of disease progression, and prevention of rheumatoid joint deformities. Additionally, study design with high-level of evidence on the intervention of rheumatoid arthritis are warranted.

The Effects of Volunteering on Life Satisfaction and Depression among the Korean Elderly: A Systematic Review and Meta-Analysis (한국 노인의 자원봉사활동이 생활만족도와 우울에 미치는 효과: 체계적 리뷰 및 메타분석)

  • The purpose of this research is to provide synthesized findings on the effect of volunteering among the elderly on their life satisfaction and depression. In order to do that, we conducted a systematic review and meta-analysis from selected studies which were published in Korea. From five databases and three government web sites, 47 research papers were selected based on the eligible criteria and the 53 effect size data were extracted. The results are: The effect size of elderly volunteering on life satisfaction was 0.348 with 95% confidence interval of 0.286 to 0.408. The summary effect of elderly volunteering on depression was -0.310 with 95% confidence interval of -0.439 to -0.169. These findings suggest that elderly volunteering is an effective intervention for their psychosocial health, providing some evidence in the area of volunteering policies and practices.

An Intensive Review On Clinical Thesis about Hwangryunhaedok-tang Pharmacopuncture Treatment: Focused on Case Reports and Controlled Studies in Korean Academic Journals (황련해독탕약침 치료에 대한 임상 논문 고찰: 국내 학술지의 증례보고와 대조군 연구 중심으로)

  • Objectives The purpose of this thesis is to compare and analyze outcomes of case studies and controlled studies about Hwangryunhaedok-tang (HHT) pharmacopuncture in clinical studies. Based on these studies reporting clinical effects by using HHT pharmacopuncture, we object to deduct the comparative review in applying clinical disorders limitedly. And conclusionally, by using this comparative review, we aim to make the fine evidence in treating clinical specific diseases. Methods We searched 106 studies about HHT pharmacopuncture from 6 Korean web databases by using in Korean word 'Hwangryunhaedok-tang pharmacopuncture'. This search had been progressed throughout 1 month (January, 2019). There was 106 thesis and we excluded studies that are not related to HHT pharmacopuncture, experimental research, not the case report or randomized controlled trials, non-randomized controlled trials, not using HHT pharmacopuncture, not using only one pharmacopuncture, and not using HHT pharmacopuncture for major treating method. As a result, 39 thesis are selected as a population. Results We analyzed 34 case reports and 5 controlled studies. As the final outcome, HHT pharmacopuncture was used in musculoskeletal diseases, dermatopathy, and 32 specific diseases. Conclusions After analyzing 39 studies, effective symptom changes clinically and statistically by using HHT pharmacopuncture herbal acupuncture were identified. However, these thesis designs were reported deficiently in many parts such as not excluding other confounding variables. After modifying defects of thesis design and conduct study, it can play a role as the evidence using HHT pharmacopuncture more precisely.

Chuna Manual Therapy for Pediatric Allergic Rhinitis : A Systematic Review and meta-analysis (소아 알레르기 비염에 대한 추나요법의 효과 : 체계적 문헌 고찰과 메타분석)

  • Objectives : This review determines the evidence of effectiveness and safety of Chuna manual therapy (CMT) for pediatric allergic rhinitis. Methods : We searched 12 electronic databases (Pubmed, EMBASE, Cochrane Library, Web of Science, CAJ, J-STAGE, RISS, DBpia, NDSL, KISTI, KISS, and KMBASE) and two related journals up to the end of April, 2019. We only included randomized controlled trials (RCTs) investigating CMT for the treatment of pediatric allergic rhinitis. The methodological quality of the included RCTs was evaluated using the Cochrane risk of bias tool 1.0. Results : Twelve RCTs were eligible in our inclusion criteria. A meta-analysis of five studies demonstrated positive results for the using CMT for the treatment of pediatric allergic rhinitis. When used in conjunction with traditional Chinese medicine, CMT treatment significantly improved total efficacy rate compared with traditional Chinese medicine alone (P<0.02, n=5). Conclusions : Based on a published meta-analysis, there is reliable evidence for the use of CMT in treating pediatric allergic rhinitis. However, it should be noted that the studies included in this systematic review were heterogeneous and were of low quality, warranting further investigation using well-designed RCTs.

A Study on the Research Trends on Obese People with Disabilities: Focused on Domestic Journal (장애인 비만에 대한 연구 동향 고찰: 국내 학술지를 중심으로)

  • Objectives: The purpose of this review is to analyse the trend in papers related to obese people with disabilities. Methods: We reviewed Korean Medicine papers by searching 4 Korean web databases and classified the papers by the year of publishment, the title of journals, the type of study, main treatment, assessment for outcomes. Results: There were 18 studies about the intellectually disabled, which accounted for most of the study. It is expected that intensive research on obesity for the intellectually disabled will be conducted in the future. Also, study on obesity for the disabled will require special attention. Disabled person tend to have a higher obesity rate and secondary diseases due to disabilities can be easily developed. Their health care capabilities are more likely lower than those of the general population. Given these considerations, it would be necessary to study metabolic syndrome in parallel in the study of obesity for the disabled. Disabled person are exposed to obesity more easily than ordinary people, and there is a higher risk of health degradation due to complications from obesity, which requires more attention and research. However, since there is not research in the oriental medical and medical circles on obesity in the disabled, medical approaches and studies on obesity in the disabled are necessary. Conclusions: We expect that more interest and research will be carried out on obesity for the disabled in the Korean medicine to achieve clinical application and to develop treatment protocols for the disabled obesity disease.

A Review of Bee Venom Acupuncture for Spinal Diseases in the Journal of Korean Medicine (국내 임상연구 문헌고찰을 통한 척추 질환의 봉약침요법 응용례 분석)

  • Objectives This review aims to analyze the current research trend in the usage of bee venom (BV) acupuncture for spinal diseases. Methods We collected 80 articles on BV acupuncture clinical study for spinal diseases by searching Korean web databases 'Journal of Korean Medicine Rehabilitation', 'Oriental Medicine Advanced Searching Integrated System', 'Korean Traditional Knowledge Portal' from 1999 to 2018. Results We summarized the most frequent clinical treatment methods of BV acupuncture. 1. Herniated intervertebral disc was treated for 40% and the treated spinal diseases were various. 2. '1:10,000' in the type of BV was chosen for 21%. 3. 29% of studies used 6-7 acupoints per session and 23% of studies used 'BL' as the injection site. 3. The highest volume of injected BV into each acupoint was 0.1 cc for 34% and the highest total volume of injected BV was 1.0 cc for 39%. Conclusions We found the limited evidence on the lack of consensus and detailed description in the reported experimental design. The results in this review suggest that future works should focus on more consistent and systematic study for the reproducible and standardized treatment methods.
