Biomechanical Testing of Anterior Cervical Spine Implants: Evaluation of Changes in Strength Characteristics and Metal Fatigue Resulting from Minimal Bending and Cyclic Loading

  • Objective: To achieve optimal fit of implant, it is necessary to bend the implant during spine surgery. Bending procedure may decrease stiffness of plate especially made of titanium and stainless steel. Typically titanium suffers adverse effects including early crack propagation when it is bent. We investigate whether 6 degree bending of titanium plates would decrease the stiffness after full cyclic loading by comparing with non-bending titanium plates group. Methods: Authors experimented 40 titanium alloy plates of 57mm in length, manufactured by 5 different companies. Total 40 plates were divided into two groups (20 bent plates for experimental group and 20 non-bent plates for control group). Twenty plates of experimental group were bent to 6 degree with 3-point bending technique and verified with image analyzer. Using the electron microscope, we sought for a initial crack before and after 3-point bending. Mechanical testing by means of 6000 cyclic axial-compression loading of 35N in compression with moment arm of 35mm-1.1 Nm was conducted on each plate and followed by the electron microscopic examination to detect crack or fissure on plates. Results: The stiffness was decreased after 6000 cyclic loading, but there was no statistically significant difference in stiffness between experimental and control group. There was no evidence of change in grain structure on the electron microscopic magnification. Conclusion: The titanium cervical plates can be bent to 6 degree without any crack or weakness of plate. We also assume that minimal bending may increase the resistance to fatigue fracture in cervical flexion-extension movement.

  • Through the study on judgment of Body form and settle Energy flow(立形定氣) before diagnose the patients, the results are as follows. The observation of the body form is to determine prosperity and deficiency of each internal organ. It is necessary to distinguish Body form loss(形脫) and Body form fullness(形充). Fat man(肥人), Thin man(瘦人), Creamy man(膏人), Muscular man(肉人), Small Fat man(脂人) are discriminated by fat distribution, fat content, and muscle mass. The observation of the body form means the observation of structure disorder, color change, develop part at body, head and face. The observation of the body form that is to determine prosperity and deficiency of each internal organ is from the limited knowledge of the anatomy. The observation of face color is considered by blood perfusion, blood oxygenation and accumulation of carotinoid, bilirubin and change of melanin in the facial skin. The prosperity and the deficiency of energy flow is considered by symptom combined with growth (<40 years) and aging (>40 years). The prosperity of energy flow includes the anger, anxious emotion and the deficiency of energy flow includes the fear, depressive emotion. The breathing type is expiratory exhalation like asthma patients in the prosperity of energy flow. The deficiency of energy flow is weakness to overcome the disease. The prosperity and the deficiency of energy flow are considered by body metabolic ratios (Basal metabolic Rate: BMR, Resting metabolic rate: RMR, Physical activity ratios: PASs). Development of subcutaneous fat is good in the person of prosperous energy flow. The person of prosperous energy flow is hard to overcome to heat weather than cold weather. The person of deficiency of energy flow has tendencies of low blood pressure, insufficiency of blood flow in the peripheral and being shocked. The person of deficiency of energy flow has tendencies of chronic fatigue syndrome or automatic nerve disorder. If the patient who has deficiency of energy flow has severe weight loss should be checked for the presence of disease. The observation of small and large of bone is to check the development and disorder of bone growth and aging. The observation of thickness and weakness of muscle is to check the development of muscle, particularly biceps, gastrocnemius, and rectus abdominal muscle. The observation of thickness and weakness of skin is to check the ability of regulating body temperature by sweating.

The Effectiveness of Ink Foot Print Analysis by Using Pre-post Oriental Medicine Music Therapy on Patient One Case with Cerebral Infarction (뇌경색 환자에게 실행한 한방음악치료 전후(前後)의 Ink foot print analysis 1례)

  • Objectives : Oriental Medicine Music Therapy is a new way of treating and preventing various diseases and promoting health by means of music specially picked and designed to produce Qi(氣) fitting to an individual person in the areas of the structure of human body, physiological function, pathological change, diagnosis and treatment. Therefore, it can facilitate the efficiency of the treatment much better and faster than only with herb or acupuncture. Equilibrium between body and mind is its originality. Methods : A 38-year old man came to hospital suffering from left side weakness, dysarthria because of cerebral infarction. After one week's medication in the part of neurology, he was sent to the oriental medicine music therapy center, and he took ten times' medical treatments once a day. Before and after the treatments, implementing 'Ink foot print analysis', as for the movement capacity and walking of left side weakness, these results are acquired. Results and Conclusions : 1. Before the oriental medicine music therapy, except the acceleration and deceleration section, within three meters, a right foot's step was measured 30.55 cm and a left foot was measured 15.2 cm, and the step distance was 15.2 cm. After the 10 times treatments, the right foot's step was 112.6 cm, the left foot's step was 113 cm, and the step distance improved to 51.8 cm. 2. The normal person's average step is about 38 cm, and it could be decreased owing to a decreptitude, pain, fatigue, and the weakness of the legs. In spite of all, from the increased 50 cm step measurement with the patient's body condition, it means that his movement capacity is recovered. 3. In the measurement of walking time, before 10 times treatments, it took 46.35 seconds within 6M, but after the treatments, it increased to 7.47 seconds as same as a normal person's walking speed. 4. In insomnia, and the abnormality of a facial muscle, it shows lots of improvement, and even the accuracy of a pronunciation, but despite of the 10 times treatments, the 'ㄹ(r)' sound placed under a vowel is unclear. 5. Before the 10 times' treatments, he can move the left side shoulder with pain and cannot move below the left elbow. After the treatments, the pain was disappeared, and he was able to move over 60 cm without pain. And improving of the movement of fingers, he can beat over 8 times with a drumstick.

Effect of Experimental Muscle Fatigue on Muscle Pain and Occlusal Pattern (실험적으로 유발되는 근피로가 근통증 및 교합양상에 미치는 영향)

  • This study aimed to make an analysis of the occlusion in the state of muscle fatigue produced by excessive mouth opening and clenching during the dental treatment to control the dental pain and to evaluate the sensory nerve in the muscle pain state. Most of the reasons why patients visit the dental office result in pain-either conceivably the dental origin pain or the non-dental origin pain. The dental offices have many therapeutic actions to produce the masticatory muscle fatigue for the treatment. Dental treatment with long minutes of mouth opening can cause some headaches, masticatory muscle pain and mouth opening difficulties. Patients with mastication problems who visits a dental office to alleviate pain run against another unexpected pain with other aspects. This study uses T-scan II system(Tekscan Co., USA) for the evaluation on the occlusal pattern in the experimental muscle fatigue after clenching, opening the mouth excessively and chewing gum. The occlusal contact pattern is analyzed by the contact timing, namely first, intercuspal, maximum and end point of contact. This inspection was performed at frequencies of 2000Hz, 250 Hz and 5 Hz before and after each experimental muscle pain was produced to 24 subjects who had normal occlusion without the orthodontic treatment or a wide range of the prosthesis by using $neurometer^{\circledR}$ CPT/C(Neurotron, Inc. Baltimore, Maryland, USA). The measuring sites were mandibular nerve experimental muscle fatigue respectively. This study could obtain the following results after the assessment of occlusion and sensory nerve of the experimental muscle fatigue. 1. There were the fastest expression after the excessive mouth opening in muscle fatigue and after tooth clenching in muscle pain. In the visual analog scale that records the subjective level, there was the highest scale after the clenching in the muscle fatigue in jumping off the point of pain. 2. Tooth contact time, contact force, relative contact force on the point of the first contact had no difference, and there were decreases in the contact force after the excessive mouth opening on intercuspal position point, after the excessive mouth opening and the gum chewing on the point of the maximum, and in the contact time after all the experimental muscle fatigue state on the point of the end contact. 3. There was no statistic significance in the current perception threshold before and after the experimental muscle fatigue. 4. There was no significant difference in the contact number, the maximal contact number on the point of the first contact, and the contact number after the mouth opening and gum chewing on the point of the intercuspal position and the contact number after the experimental muscle fatigue on the maximum point, and showed significant decreases. In conclusion, it was found that the occlusal pattern can cause the changes on the case of the clinical muscle weakness by intra-external oral events. It was important that the sedulous attention to details is required during dental treatment in case of excessive mouth opening, mastication and clenching.

The Effect of McKenzie and Mulligan Exercise Training on the Cervical Spine (맥켄지 운동과 멀리건 기법이 경추에 미치는 효과)

  • Background: Many people are suffering from neck and shoulder pain due to muscle weakness, muscle fatigue, lack of exercise and accidents during exercise. In this study we compared the effects of McKenzie method and Mulligan technique method. Method: we randomly selected men(10) and women(10) and they are the 20s students of Busan material K college. We divided for McKenzie exercise group and Mulligan exercise group. McKenzie exercise and mulligan exercise applied cervical area three times a week for three weeks. We compared with pre-experiment and after 1~3 week through measuring the electromyography (EMG) change and neck range of motion (ROM). Results: The changes in the EMG at each exercise group, there was a statistically significant difference in each week. Although there was a significant difference after 1 week between the exercise group, the changes in the neck ROM at each exercise group there was no statistically significant difference. Conclusion: After experiment, EMG and neck ROM were increased in McKenzie exercise group and Mulligan exercise group. But Mulligan exercise group showed the change better than McKenzie exercise group in early stage. We suggest the Mulligan exercise method for recovering in early stage of neck ROM.

An Analysis of Burnout Factors and Coping Methods on Continuous Peritoneal Dialysis Patients (복막투석환자의 소진요인과 대응양상에 관한 분석)

  • This study was done to identify burnout factors and coping methods surveyed upon 59 out-patients who have continuous peritoneal dialysis. Raw data was collected after being filled the questionnaire by subjects or directly interviewing at the peritoneal dialysis room of the K hospital. Taegu. Then. data was classified and analyzed according to categories of questions and revealed facts. The following results are obtained: The average age of subjects is 43.8 years old and the average duration of continuous peritoneal dialysis is 16.9 months. The reasons of choosing continuous peritoneal dialysis are 61. 0% by recommendation of doctor and nurse ranked as the first, 32. 2% by patient own decision ranked as the second. The 57.6% of subjects were experienced complications related to continuous peritoneal dialysis. The major psychosocial burnout factors because of the dialysis are confinement, wasting time, change in responsibility and role as a member of family, worriment for infection, interference in job: and the major physiological burnout factors are fatigue and weakness. Among the main coping methods, the problem-oriented coping methods are "accept the given situation itself, "actively try to change the situation", "try to maintain some controls over the situation", and the affective-oriented coping method are "give up and accept as fate", "pray", "ask comfort or help from family and friends", "sleep", "cry". Conclusively, it can be said that effective nursing intervention is needed to prevent continuous peritoneal dialysis burnout.

  • Tyroid gland is the biggist endocrine organ which is located in front of trochea. The role of the thyroid gland in the total body economy comprises the synthesis, storage, and secretion of thyroid hormones, which are necessary to growth, development, and normal bodymetabolism. Hypothyroidism is the state of a dificiency in the secretion of Thyroid hormone. This may result from idiopathic myxedema, or distruction or removal of thyroid, or goiter, or thyroid adenomas. Hypothyroidism corresponds to thick lips, dry skin, puffy eyelids, coars hair, megaloglossia, and pudgy hands etc. On one case of Hypothyroidism who was hospitalized for the chief complaints of fatigue, general weakness, coldness, edema in head, neck site, lumbago. I diagnosed this case as dificiency of Qi and blood(氣血兩虛) or dificiency of warm-Qi in spleen and kidney(脾腎陽虛). And then I used Moxa treatment to increase Qi and blood, and Acupuncture treatment to reduce shoulder and neck pain, lumbago. And Herb-med is used to increase Qi and blood, and recover of general condition. As a result, this patient got better more quickly.

  • Objectives The aim of this study was to suggest basic data of health and medical status in Ethiopia for globalization of Korean Medicine. Methods This study was conducted from 26 to 28, Dec, 2018 in the MCM General Hospital in Addis ababa of Ethiopia. The questionnaire was developed from open form of review of system questionnaire and some question was changed. It was composed of 3 parts: chief complaint, review of system and medical policy. For more information, we interviewed the director of a public health clinic. The 94 questionnaires was collected. Three of them was incomplete, and 91 was analyzed. Results (1) Seventeen people appealed for head problem, and thirteen people for abdomen, and eleven for chest in chief complaint. (2) The number of 22 people had problem in nervous system. Twenty also had diseases of respiratory and digestive system respectively. (3) They also appeal for pain in head, mouth, eye, and neck and complained for fatigue, weight gain or loss, and weakness in review of system. (4) They put the priority on disease prevention than management or treatment. Conclusions This study suggests information on medical status of the people in the medical hospital of Ethiopia which is valuable for globalization of Korean Medicine.

The Meaning of Quality of Life for Bone Marrow Transplant Survivors (골수이식생존자의 삶의 질 의미)

  • The purpose of this study was to explore the concept of quality of life for bone marrow transplant (BMT) survivors and to gain understanding of nursing interventions that may improve QOL in this population. The data was gathered from 32 BMT survivors using seven open-ended questions. The items were based on previous research of Ferrell et al., (1992). Content analysis was performed on written responses to seven questions regarding BMT and QOL. The results were as follows : 1. The meaning of QOL for BMT survivors were "being healt", "being able to take a role", "having relationships", "self-accomplishment", "peace of mind", "spiritual well-being", "economic stability" and "being alive". 2. The impact of BMT on physical well-being were "skin impairment", "digestive problems", "infections ", "fatigue/weakness", "arthralgia", "eye dryness". "weight gain", "amenorrhea" and "hand tremor". 3. The impact of BMT on psychological well-being were "fear of recurrence", "sence of peace" and "hope". 4. The impact of BMT on socioeconomic status were "financial burden", "limitation of social activities" and "sence of withdrawal". 5. The impact of BMT on spiritual well-being were "dependency on Supreme Being", "spiritual arousal " and "spiritual maturity". Health Problems and Psychosocial Adaptation of Children with Cancer (소아암 환아의 건강문제와 사회심리적 적응)

  • Purpose: The purpose of this study was to describe health problems and psychosocial adaptation in children with cancer and to determine the relationship between these two variables. Method: The data were collected from parents of 61 children with cancer, aged 4-11 years at one university hospital in Seoul. The instruments for this study were the Korea Child Behavior Check List (KCBCL) and a physical symptom checklist developed by the researchers. Result: The major health problems were coughing, sleeping disturbances, fatigue and weakness. The total behavior problem score for children in the cancer group was higher than the score for those in the norm group. The scores on the scales for withdrawal, somatic complaints, social immaturity, and internalizing problems were higher in the children with cancer compared to normative findings. Scores on social and school competence in the cancer group were lower than the norms for healthy children. The relationship between health problems and psychosocial adaptation was significant in the subscales of withdrawal, anxiety/depression, internalizing problems and social involvement in the off therapy group. Conclusions: This study shows the various areas that need further study in caring for children with cancer.
