Waterborne parasite infections are considered a reemerging threat. Most studies on the epidemiology of human cryptosporidiosis, giardiasis, and amebiasis have been carried out in developed countries, and there is little data on the occurrence of these infections in other areas. The objective of this study was to investigate the presence of waterborne parasites such as Cryptosporidium parvum, Giardia lamblia and Entamoeba histolytica in various water samples in Ankara, turkey. A total of 85 samples were examined, 43 from the municipal water supply, 34 from wells, 6 from the Ankara River, and 2 from two untreated dams; by conventional microscopy, immunologically and by polymerase chain reaction (PCR). Oocysts of C. parvum and cysts of G. lamblia were detected by using an indirect fluorescence (antigen) assay, whereas an enzyme linked immunosorbent assay was used to detect the cysts of E. histolytica and E. dispar. In addition, PCR was used for E. histolytica, E. dispar, C. parvum and G. lamblia detection. G. lamblia was found in 2 of the 34 well water samples, and parasites were found in 3 of the 6 Ankara River samples. The 1$\^$st/ contained E. histolytica cysts and Strongyloides stercoralis larvae. the 2$\^$nd/ E. histolytica cysts, and Trichuris trichiura eggs, and the 3$\^$rd/ C. parvum oocysts only. No parasite was observed in the municipal water samples and untreated dam water samples. These results extend our knowledge on waterborne parasites, such occurrence information on waterborne pathogens assists the management and treatment of municipal water.
수인성 병원성 미생물에 의한 공중목욕탕의 오염은 질병발생의 원인이 된다. 본 연구에서는 공중목욕탕내에 존재하는 수인성 병원성미생물들을 확인하고자 하였다. 서울지역내의 30 곳 공중목욕탕에서 욕조수 시료를 채수하여 진행하였다. 수인성 병원성미생물의 검출은 0.45 ${\mu}m$의 여과막을 이용하여 전통적인 배양방법으로 분리 및 동정하였다. 분자생물학적 기법을 사용하기 위해 미생물학적인 배양을 하지 않고 핵산을 추출하여 16S rRNA유전자를 표적으로 polymerase chain reaction-reverse blot hybridization (PCR-REBA)을 실시하였다. 미생물학적 배양방법에서는 지표세균인 Escherichia coli와 Shigella spp.가 검출되었으며, 분자생물학적 기법인 PCR-REBA을 수행한 결과 E. coli, Shigella spp., Salmonella spp., Pseudomonas spp., Mycobacterium spp. 등의 수인성 병원성미생물이 7곳에서 검출되었다. 본 연구결과를 토대로 공중목욕탕의 욕조수내에 수인성병원성미생물에 의한 감염을 줄이기 위해 적절한 위생관리과 E. coli를 포함한 유해미생물을 선정하여 지속적인 모니터링이 필요할 것으로 사료된다.
A total of 30 water samples were collected from 30 different public baths in Seoul, Korea. Contamination of public bath water by waterborne pathogens can cause disease outbreaks and contribute to increase background rates of disease. Pathogens in water was filtered by nitrocellulose membrane with $0.45{\mu}m$ pore size. The membrane filters were analyzed by both cultivation and polymerase chain reaction (PCR) amplification of partial 16S rRNA gene. Various microorganisms including 4 Escherichia coli/Shigella spp. 1 Salmonella spp. 3 Pseudomonas aeruginosa and 2 Mycobacterium spp. were identified by reverse blot hybridization assay (REBA). PCR-REBA was able to identify many bacterial genera in one assay. Our results suggest that appropriate hygiene practice and continuous monitoring is needed for reducing health risk associated with public bath houses.
Waterborne infectious disease is induced by several pathogenic microbes such as bacteria, viruses and protozoans, and the cases caused by viral infection is currently increasing. Water treatment process could reduce the number of virus in the water, but there were many difficulties to completely remove the virus particles from water. Therefore, the membrane separation technology which was reported to effectively remove pollutants from raw water has attracted increasing attention and demand. Since its efficiency has been introduced, demands for evaluation method toward the membrane filtration process are increasing. However, progression of the method development is slow due to the difficulties in cultivation of several waterborne viruses from animal models or cell culture system. To overcome the difficulties, we used adenovirus, one of the commonly isolated pathogenic waterborne viruses which can grow in cell culture system in vitro. The adenovirus used in this study was identified as human adenovirus C strain. The adenovirus was spiked in the raw water and passed through the microfiltration membrane produced by Econity, a Korean membrane company, and then the viral removal rate was evaluated by real-time PCR. In the results, the amount of virus in the filtered water was decreased approximately by 5 log scale. Because coagulant treatment has been known to reduce filtering function of the membrane by inducing fouling, we also investigated whether there was any interference of coagulant. In the results, we confirmed that coagulant treatment did not show significant interference on microfiltration membrane. In this study, we found that waterborne virus can be effectively removed by membrane filtration system. In particular, here we also suggest that real-time PCR method can rapidly, sensitively and quantitatively evaluate the removal rate of virus. These results may provide a standard method to qualifying membrane filtration processes.
2018년 6월 12일 충청북도 옥천군 소재 A고등학교에서 노로바이러스의 유행을 지연신고하는 사례가 발생하였고, 이에 대한 원인과 전파양식 등을 규명하고 예방 및 관리대책을 마련 하기 위하여 역학조사를 수행하였다. A고등학교 학생 183명을 대상으로 설문조사, 환례자 60명, 조리종사자 10명을 대상으로 세균 10종 및 바이러스 5종에 대한 검체검사를 실시하였다. 설문조사는 최초환자 발생일 6월 5일을 기준으로 3일 전인 6월 2일부터 12일까지의 식단을 이용하여 환자-대조군 조사를 시행하였다. 학생, 교직원, 조리종사자 785명 중 환례는 61명으로 노로바이러스의 발병률은 7.8% 이었다. 위험요인 분석에서 정수기 음용수가 유의한 변수이었다. 검체검사에서 학생 2명, 정수기 음용수, 환경검체에서 동일 유형의Norovirus GI-8이 검출되었다. 이번 유행의 원인으로 본관, 기숙사, 급식실 정수기 음용수가 오염되고 그로 인해 원인병원체 노로바이러스의 전파가 이루어졌다고 판단하였다. 이번 연구는 6월 5일 첫 환례자가 발생했음에도 신고가 7일이 경과한 6월 12일 지연신고 되어 장관감염증 확산 조기차단이 이루어지지 않아 환례자가 더 많이 발생한 것으로 추정된다. 향후 학교급식 시 발생하는 수인성 및 식품매개성질병으로부터 학생을 보호하고 집단발생을 예방하기 위하여 학생의 증상을 가장 우선적으로 파악이 가능한 담임선생님과 보건교사의 공조체제 개선, 보건교사 부재 시 대응방안, 보건교사의 학교 감염병 집단발생에 대한 인식제고를 위한 시스템 보완이 이루어져야 할 것이다. 또한 질병관리본부의 기본 역학조사서 서식에 지연신고에 대한 사유를 기록하는 항목을 추가하여 지연신고에 대한 원인에 대한 규명과 그에 따른 대책을 마련해야 할 것이다.
Background: The occurrence of environmental disease is known to be associated with chronic exposure to toxic chemicals, including waterborne contaminants, air/indoor pollutants, asbestos, ingredients in humidifier disinfectants, etc. Objectives: In this study, we reviewed toxicological studies related to environmental disease as defined by the Environmental Health Act in Korea and toxic chemicals. We also suggested a direction for future toxicological research necessary for the prevention and management of environmental disease. Methods: Trends in previous studies related to environmental disease were investigated through PubMed and Web of Science. A detailed review was provided on toxicological studies related to the humidifier disinfectants. We identified adverse outcome pathways (AOPs) that can be linked to the induction of environmental diseases, and proposed a chemical screening system that uses AOP, chemical toxicity big data, and deep learning models to select chemicals that induce environmental disease. Results: Research on chemical toxicity is increasing every year, but there is a limitation to revealing a clear causal relationship between exposure to chemicals and the occurrence of environmental disease. It is necessary to develop various exposure- and effect-biomarkers related to disease occurrence and to conduct toxicokinetic studies. A novel chemical screening system that uses AOP and chemical toxicity big data could be useful for selecting chemicals that cause environmental diseases. Conclusions: From a toxicological point of view, developing AOP related to environmental diseases and a deep learning-based chemical screening system will contribute to the prevention of environmental diseases in advance.
From May to June 2012, a waterborne outbreak of 124 cases of cryptosporidiosis occurred in the plumbing system of an older high-rise apartment complex in Seoul, Republic of Korea. The residents of this apartment complex had symptoms of watery diarrhea and vomiting. Tap water samples in the apartment complex and its adjacent buildings were collected and tested for 57 parameters under the Korean Drinking Water Standards and for additional 11 microbiological parameters. The microbiological parameters included total colony counts, Clostridium perfringens, Enterococcus, fecal streptococcus, Salmonella, Shigella, Pseudomonas aeruginosa, Cryptosporidium oocysts, Giardia cysts, total culturable virus, and Norovirus. While the tap water samples of the adjacent buildings complied with the Korean Drinking Water Standards for all parameters, fecal bacteria and Cryptosporidium oocysts were detected in the tap water samples of the outbreak apartment complex. It turned out that the agent of the disease was Cryptosporidium parvum. The drinking water was polluted with sewage from a septic tank in the apartment complex. To remove C. parvum oocysts, we conducted physical processes of cleaning the water storage tanks, flushing the indoor pipes, and replacing old pipes with new ones. Finally we restored the clean drinking water to the apartment complex after identification of no oocysts.
A national survey for enteric virus contamination in treated water and its source water was performed from March to November 1999. The water samples were subjected to virus filtration at the major water plants producing over 10$^5$ tons treated water per day. Twenty surveyed sites encompass most of heavily populated residential area except for Seoul and Pusan. (omitted)
질병관리청에서는 「감염병예방법」에 따라 법정 감염병을 지정하여 감염병 감시체계를 운영하고 있으며 그 중 전수감시 대상 수인성·식품매개감염병은 총 6종으로 제2급감염병 중 콜레라, 장티푸스, 파라티푸스, 세균성이질, 장출혈성대장균 감염증과 제3급감염병인 비브리오패혈증이 있다. 전수감시체계는 1954년 국내에 도입되었고 질병관리청에서는 감염병 신고 활성화를 위해 많은 노력을 기울이고 있으며 최근까지 국가 감시체계 신고 현황을 분석한 연구는 부족하였기 때문에 본 연구를 통해 최근 감염병 신고 현황을 이해하고자 한다. 본 연구에서는 지난 10년 동안 전수감시 체계를 통해 수집된 수인성·식품매개감염병의 신고자료 특징을 살펴보았다. 질병관리청에서 운영하는 질병보건통합관리시스템을 통해 수집된 신고정보 중 2012년 1월 1일부터 2021년 12월 31일까지 신고된 것을 분석에 사용하였으며, 이 중 신고 기준에 적합했던 12,296건을 바탕으로 적시 신고 여부와 확인진단 검사 결과 3가지에 따라 자료를 구분하여 분석하였다. 전체 신고 건수는 10년 동안 꾸준히 증가하고 있으며, 그 중 장티푸스와 파라티푸스, 장출혈성 대장균감염증의 신고 건수가 크게 증가하였다. 전체 신고된 건의 96%가 진단일로부터 1일 이내에 적시 신고된 것을 확인하였으며, 지연 신고된 4% 중 비브리오패혈증이 지연 신고 비율이 가장 높았으며 콜레라에서 가장 낮았다. 또한 콜레라는 지연된 건의 평균 지연 일수가 2.7일로 가장 낮은 것으로 나타났다. 확인진단 검사 결과에 따라 살펴보았을 때, 장티푸스와 파라티푸스는 과거에 비해 검사 결과가 '음성'인 건의 비율이 증가한 것을 확인할 수 있었고, 비브리오패혈증은 검사 결과가 '양성'으로 확인된 건의비율이 높았으며, 장출혈성대장균감염증은 확인진단 검사를 실시하지 못한 경우가 특히 많았다. 최초 의심 신고된 이후 최종 검사 결과를 확인할 때까지 소요된 시간을 확인했을 때 검사 결과가 '양성'인 경우에서 소요 일수가 가장 작게나타났고, 검사를 실시하지 못한 경우에서 소요일수의 사분위수 범위가 가장 크게 나타났다. 특히 모든 감염병에서 검사 결과가 양성인 경우의소요 일수가 0∼1일이었으며, 그 중 파라티푸스는음성, 검사 미실시의 경우에서 다른 감염병에비해 높은 소요 일수를 나타냈다. 전반적인 수인성·식품매개감염병의 신고 건수 증가와 더불어 확인진단 검사 결과가 음성인 건의 비율이 증가함에 따라 전수감시체계의 모니터링 기능이 향상되고 있다고 추정할 수 있었다. 단, 일부 감염병의 경우 신고의 적시성과 확인진단 검사 결과 확인의 적시성 측면에서 개선이필요한 것으로 사료된다.
