협착된 관상동맥에 시술된 스텐트형상이 벽면 전단응력에 미치는 영향 (Effects of Stenting Shapes on the Wall Shear Stress in the Angulated Coronary Stenosis)

  • The objective of the present study is to evaluate the effects of the stenting shapes on flow velocity and wall shear stress in angulated coronary stenosis by computer simulation. Coronary angiogram and Doppler ultrasound measurement in the patients with angulated coronary stenosis were obtained. Inlet wave velocity distribution obtained from in vivo intracoronary Doppler data was used for the numerical simulation. Spatial pattern of blood flow velocity and recirculation area were drawn through out the selected segment of coronary models. Wall shear stresses in the intracoronary stent models were calculated from three-dimensional computer simulation. A negative shear stress region, which is consistent with re-circulation area on flow pattern, was noted on the inner wall of post-stenotic area of pre-stenting model. The negative shear stress was disappeared after stenting. Shear stress in the post-stenting model was markedly reduced up to about two orders of magnitude compared to that of the pre-stenting model.

  • Direct numerical simulation database of an axial turbulent boundary layer is used to compute frequency and wave number spectra of the wall shear-stress fluctuations in a low-Reynolds number axial turbulent boundary layer. One-dimensional and two-dimensional power spectra of flow variables are calculated and compared. At low wave numbers and frequencies, the power of streamwise shear stress is larger than that of spanwise shear stress, while the powers of both stresses are almost the same at high wave numbers and frequencies. The frequency/streamwise wave number spectra of the wall flow variables show that large-scale fluctuations to the ms value is largest for the streamwise shear stress, while that of small-scale fluctuations to the rms value is largest for pressure. In the two-point auto-correlations, negative correlation occurs in streamwise separations for pressure and spanwise shear stress, and in spanwise correlation for both shear stresses.

The Wall Shear Rate Distribution Near an End-to-End Anastomosis : Effects of Graft Compliance and Size

  • The patency rates of small diameter vascular grafts are disappointing because of the formation of thrombus and intimal hyperplasia. Among the various factors influencing the success of graft surgery, the compliance and the size of a graft are believed to be the most important physical properties of a vascular graft. Mismatch of compliance and size between an artery and a graft alters anastomotic flow characteristics, which may affect the formation of intimal hyperplasia. Among the hemodynamic factors influencing the development of intimal hyperplasia, the wall shear stress is suspected as the most important one. The wall shear stress distributions are experimentally measured near the end-to-end anastomosis models in order to clarify the effects of compliance and diameter mismatch on the hemodynamics near the anastomosis. The effects of radial wall motion, diameter mismatch and impedance phase angle on the wall shear rate distributions near the anastomosis are considered. Compliance mismatch generates both different radial wall motion and instantaneous diameter mismatch between the arterial portion and the graft portion during a flow cycle. Mismatch in diameter seems to be affecting the wall shear rate distribution more significantly compared to radial wall motion. The impedance phase angle also affects the wall shear rate distribution.

  • In the present study, flow characteristics of turbulent oscillatory flow in a square-sectional $180^{\circ}$ curved duct are investigated experimentally. In order to measure wall shear stress and pressure distributions, experimental studies for air flow are conducted in a square-sectional $180^{\circ}$ curved duct by using the LDV system with the data acquisition and the processing system. The wall shear stress measuring point bend angle of the $150^{\circ}$ and pressure distribution of the inlet (${\phi}=0^{\circ}$) to the outlet (${\phi}=180^{\circ}$) at $10^{\circ}$ intervals of the duct. The results obtained from the experimentation are summarized as follows: A wall shear stress value in an inner wall is larger than that in an outer wall, except for the phase angle (${\omega}t/{\pi}/6$) of 3, because of the intensity of secondary flow. The pressure distributions are the largest in accelerating and decelerating regions at the bend angle(${\phi}$) of $90^{\circ}$ and pressure difference of inner and outer walls is the largest before and after the ${\phi}=90^{\circ}$.

혈관 유연성을 고려한 경동맥 분기부 모델 혈류역학 해석 (Numerical Study on Blood Flow Dynamics and Wall Mechanics in a Compliant Carotid Bifurcation Model)

  • Blood flow simulations in an realistic carotid bifurcation model with considering wall compliance were carried out to investigate the effect of wall elasticity on the wall shear stress and wall solid stress. Canonical waveforms of flow rates and pressure in carotid arteries were imposed for boundary conditions. Compared to a rigid wall model, we found an increased recirculation region at the carotid bulb and an overall reduction of wall shear stress in a compliant model. Additionally, there was appreciable change of flow rate and pressure wave in longitudinal direction. Both solid and wall shear stress concentration occur at the bifurcation apex.

우 대동맥 평활근 세포의 성장에 관한 shear stress의 영향 (Effect of Shear Stress on Bovine Aortic Smooth Muscle Cell Growth)

  • 평활근 세포에 대한 shear stress의 영향을 알아보기 위해 슬라이드 글라스위에 배양된 牛 대통맥의 평활근 세포를 120시간 동안 0~26 dyn/$cm^2$의 각기 다른 일정한 shear stress의 유체에 노출하였다. 실험 장치를 순환하는 배지의 lactate dehydrogen­a ase의 농도측정 결과, 본 연구에서 적용된 shear stress 범위에서는 유체의 흐름으로 인하여 세포가 슬라이드 글라스 표면으로부터 이탈되는 현상은 없었다. 표면 $cm^2$ 당 존재하는 세포의 수를 측정한 결과, 평활근 세포는 정체배양시 가장 빨리 성장하였다. 표변에서의 shear stress가 증가할수록 i영활근 세포의 성장은 늦었으며, shear stress가 17 dyn/$cm^2$ 이상에서는 세포의 성장이 관찰되지 않았다.

고리형 약물분출 스텐트 주위 벽전단응력의 영향에 대한 수치해석 (Numerical Analysis on the Effect of Wall Shear Stress Around the Ring Drug-Eluting Stent)

  • The use of drug-eluting stents has dramatically reduced the incidence of restenosis however, much remains to be teamed about the performance of these stouts. In the present study, we tested the hypothesis that the design of drug-eluting stents influences the efficacy of local drug delivery to the arterial wall and that this effect depends on both arterial geometry and the prevailing flow conditions. We performed computational simulations in which the coupled Navier-Stokes and advection-diffusion equations were solved to determine the flow field and drug concentration in the vicinity of model drug-eluting stouts It is found that the characteristics of flow phenomena can be influenced greatly by the ratio of stent diameter to vessel diameter. The presence of drug-eluting stent may have profound effect on wall shear stresses, recirculation sizes and drug distributions. The results show that recirculation zone is influenced by the imposed flow conditions and stent diameter. In pulsatile flow, the low wall shear stress and high drug concentration occur along the arterial wall during the decelerating flow conditions. These results could provide the guideline for future drug-eluting stent designs toward reducing restenosis by affecting local wall shear stress distributions associated with neointimal hyperplasia.

Effects of the Velocity Waveform of the Physiological Flow on the Hemodynamics in the Bifurcated Tube

  • The periodicity of the physiological flow has been the major interest of analytic research in this field up to now Among the mechanical forces stimulating the biochemical reaction of endothelial cells on the wall, the wall shear stresses show the strongest effect to the biochemical product. The objective of present study is to find the effects of velocity waveform on the wall shear stresses and pressure distribution along the artery and to present some correlation of the velocity waveform with the clinical observations. In order to investigate the complex flow phenomena in the bifurcated tube, constitutive equations, which are suitable to describe the rheological properties of the non-Newtonian fluids, are determined, and pulsatile momemtum equations are solved by the finite volume prediction. The results show that pressure and wall shear stresses are related to the velocity waveform of the physiological flow and the blood viscosity. And the variational tendency of the wall shear stresses along the flow direction is very similar to the applied sinusoidal and physiological velocity waveforms, but the stress values are quite different depending on the local region. Under the sinusoidal velocity waveform, a Newtonian fluid and blood show big differences in velocity. pressure, and wall shear stress as a function of time, but the differences under the physiological velocity waveform are negligibly small.
