Purpose: This study aims to compare patients in whom fixation failure occurred via cut-out (CO) or cut-through (CT) in order to determine patient factors and radiographic parameters that may be predictive of each mechanism. Materials and Methods: This retrospective cohort study includes 18 patients with intertrochanteric (IT) hip fractures (AO/OTA classification 31A1.3) who underwent treatment using a single lag screw design intramedullary nail in whom fixation failure occurred within one year. All patients were reviewed for demographics and radiographic parameters including tip-to-apex distance (TAD), posteromedial calcar continuity, neck-shaft angle, lateral wall thickness, and others. Patients were grouped into cohorts based on the mechanism of failure, either lag screw CO or CT, and a comparison was performed. Results: No differences in demographics, injury details, fracture classifications, or radiographic parameters were observed between CO/CT cohorts. Of note, a similar rate of post-reduction TAD>25 mm (P=0.936) was observed between groups. A higher rate of DEXA (dual energy X-ray absorptiometry) confirmed osteoporosis (25.0% vs. 60.0%) was observed in the CT group, but without significance. Conclusion: The mechanism of CT failure during intramedullary nail fixation of an IT fracture did not show an association with clinical data including patient demographics, reduction accuracy, or radiographic parameters. As reported in previous biomechanical studies, the main predictive factor for patients in whom early failure might occur via the CT effect mechanism may be related to bone quality; however, conduct of larger studies will be required in order to determine whether there is a difference in bone quality.
The prediction of pressure drop for a droplet flow in a confined micro channel is presented using FE-FTM (Finite Element - Front Tracking Method). A single droplet is passing through 5:1:5 contraction - straight narrow channel - expansion flow domain. The pressure drop is investigated especially when the droplet flows in the straight narrow channel. We explore the effects of droplet size, capillary number (Ca), viscosity ratio ($\chi$) between droplet and medium, and fluid elasticity represented by the Oldroyd-B constitutive model on the excess pressure drop (${\Delta}p^+$) against single phase flow. The tightly fitted droplets in the narrow channel are mainly considered in the range of $0.001{\leq}Ca{\leq}1$ and $0.01{\leq}{\chi}{\leq}100$. In Newtonian droplet/Newtonian medium, two characteristic features are observed. First, an approximate relation ${\Delta}p^+{\sim}{\chi}$ observed for ${\chi}{\geq}1$. The excess pressure drop necessary for droplet flow is roughly proportional to $\chi$. Second, ${\Delta}p^+$ seems inversely proportional to Ca, which is represented as ${\Delta}p^+{\sim}Ca^m$ with negative m irrespective of $\chi$. In addition, we observe that the film thickness (${\delta}_f$) between droplet interface and channel wall decreases with decreasing Ca, showing ${\delta}_f{\sim}Ca^n$ Can with positive n independent of $\chi$. Consequently, the excess pressure drop (${\Delta}p^+$) is strongly dependent on the film thickness (${\delta}_f$). The droplets larger than the channel width show enhancement of ${\Delta}p^+$, whereas the smaller droplets show no significant change in ${\Delta}p^+$. Also, the droplet deformation in the narrow channel is affected by the flow history of the contraction flow at the entrance region, but rather surprisingly ${\Delta}p^+$ is not affected by this flow history. Instead, ${\Delta}p^+$ is more dependent on ${\delta}_f$ irrespective of the droplet shape. As for the effect of fluid elasticity, an increase in ${\delta}_f$ induced by the normal stress difference in viscoelastic medium results in a drastic reduction of ${\Delta}p^+$.

From now on, in order to meet more stringer diesel emission standard, diesel vehicle should be equipped with emission after-treatment devices as NOx reduction catalyst and particulate filters. Urea-SCR is being developed as the most efficient method of reducing NOx emissions in the after-treatment devices of diesel engines, and recent studies have begun to mount the urea-SCR device for diesel passenger cars and light duty vehicles. That is because their operational characteristics are quite different from heavy duty vehicles, urea solution injection should be changed with other conditions. Therefore, the number and diameter of the nozzle, injection directions, mounting positions in front of the catalytic converter are important design factors. In this study, major design parameters concerning urea solution injection in front of SCR are optimized by using a CFD analysis and Taguchi method. The computational prediction of internal flow and spray characteristics in front of SCR was carried out by using STAR-CCM+7.06 code that used to evaluate $NH_3$ uniformity index($NH_3$ UI). The design parameters are optimized by using the $L_{16}$ orthogonal array and small-the-better characteristics of the Taguchi method. As a result, the optimal values are confirmed to be valid in 95% confidence and 5% significance level through analysis of variance(ANOVA). The compared maximize $NH_3$ UI and activation time($NH_3$ UI 0.82) are numerically confirmed that the optimal model provides better conversion efficiency of $NH_3$. In addition, we propose a method to minimize wall-wetting around the urea injector in order to prevent injector blocks caused by solid urea loading. Consequently, the thickness reduction of fluid film in front of mixer is numerically confirmed through the mounting mixer and correcting injection direction by using the trial and error method.
Background : We assessed the accuracy of staging in evaluation of bronchial invasion, thus found the role of CT in patients who underwent resective surgery in primary lung cancer. Materials and Methods : Authors retrospectively analized the preoperative CT scans of 156 patients receiving pneumonectomy(n = 95) and lobectomy(n = 61). Among lobectomy patients, 7 patients subsequently performed pneumonectomy because of positive resection margin of bronchus in frozen biopsy. We also retrospectively analized CT scans of non-operated 60 patients who performed sufficient bronchoscopic biopsy. Bronchial wall thickness more than 3mm, irregular wall thickening and reduction of diameter by CT were defined as bronchial invasion. The pathologic examination of resection margin were positive in 20, stump recurrence occurred in 6 of the operated group, and the pathologic examination of biopsy of bronchial wall were positive in 34 of the non operated group, and these were an regarded as bronchial invasion. Results : The CT assessment of bronchial invasion revealed low sensitivity (11.5%), low positive predictability(38%), but high specificity(96%) and relatively high accuracy (84%) in the operated group and higher sensitivity (62%), higher positive predictability(95%) in non-operated group. Conclusion : In lung cancer patients who underwent operation, CT showed very low sensitivity and positive predictability in evaluation of bronchial invasion. Because the usefulness of CT in evaluation of bronchial invasion is limited, therefore aggressive fiberoptic bronchoscopic biopsy is thought to be necessary before surgical attempt.

This study evaluated the construction costs of 11 design cases to decrease the horizontal forces applied to an abutment. They include two kinds of abutment types, which are used to improve the backfill materials for reversed T-shaped abutment and geosynthehtic reinforced abutment for railways (RAR). In the first economic analysis, the internal friction angles of the backfill materials were increased from ${\Phi}=35^{\circ}$ to ${\Phi}=40^{\circ}$ and $50^{\circ}$ for a reversed T-shaped abutment. The second analysis examined cases with the design of a geosynthehtic RAR. When the friction angles were $40^{\circ}$ or $50^{\circ}$ after improvement of the backfill material, the reduction in the construction cost of the abutment was not as large (2.0-3.9%), even though the horizontal forces on the abutment were decreased by 18-48%. However, in the case of applying the RAR, a maximum cost reduction of 30% was achieved by decreasing the horizontal force to zero. The cost reduction results from the decreased wall thickness, base slab size, and the number of pile foundations for the abutment, as well as changing the material.
Purpose: The purpose of this study was to compare the fracture strength of traditional metal-ceramic crowns and full zirconia crowns according to the occlusal thickness. Materials and methods: A mandibular first molar resin tooth was prepared with 1.5 mm occlusal reduction, 1.0 mm rounded shoulder margin and $6^{\circ}$ taperness in the axial wall. Duplicating the resin tooth, 64 metal dies were fabricated. 48 full zirconia crowns were fabricated using Prettau zirconia blanks by ZIRKONZAHN CAD/CAM and classified into six groups according to the occlusal thickness (0.5 mm, 0.6 mm, 0.7 mm, 0.8 mm, 0.9 mm, 1.0 mm). 16 metal-ceramic crowns were fabricated and classified into two groups according to the occlusal porcelain thickness (1.0 mm, 1.5 mm). All crowns were cemented on each metal die and mounted in a universal testing machine. The load was directed at the functional cusp of each specimen until catastrophic failure occurred. One-way ANOVA, Tukey multiple comparison test (${\alpha}=.05$) and t-test (${\alpha}=.05$) were used. Results: The results were as follows. 1. The test 1 group (646.48 N) showed the lowest fracture strength (P<.05), and the value of the test 2.3.4.5 groups (866.40 N, 978.82 N, 1196.82 N, 1222.41 N) increased as thickness increased, but no significant difference were found with the groups (P>.05). The value of test 6 group (1781.24 N) was significantly higher than those of the other groups (P<.05). 2. There were no significant differences of the fracture strength of metal ceramic crowns according to occlusal porcelain thickness 1.0 mm (2515.71 N) and 1.5 mm (3473.31 N) (P<.05). Conclusion: Full zirconia crown needs to be 1.0 mm or over in occlusal thickness for the posterior area to have higher fracture strength than maximum bite force.

Purpose : To reduce the Irradiation dose to the lungs and heart in the case of chest wail irradiation using an oppositional electron beam, we used an Individualized custom bolus, which was precisely designed to compensate for the differences In chest wall thickness. The benefits were evaluated by comparing the normal tissue complication probablilties (NTCPS) and dose statistics both with and without boluses. Materials and Methods : Boluses were made, and their effects evaluated in ten patients treated using the reverse hockey-stick technique. The electron beam energy was determined so as to administer 80% of the irradiation prescription dose to the deepest lung-chest wall border, which was usually located at the internal mammary lymph node chain. An individualized custom bolus was prepared to compensate for a chest wall thinner than the prescription depth by meticulously measuring the chest wall thickness at 1 emf intervals on the planning CT Images. A second planning CT was obtained overlying the individuailzed custom bolus for each patient's chest wall. 3-D treatment planning was peformed using ADAC-Pinnacle$^{3}$ for all patients with and without bolus. NTCPS based on 'the Lyman-Kutcher' model were analyzed and the mean, maximum, minimum doses, V$_{50}$ and V$_{95}$ for 4he heari and lungs were computed. Results .The average NTCPS in the ipsliateral lung showed a statistically significant reduction (p<0.01), from 80.2${\pm}$3.43% to 47.7${\pm}$4.61%, with the use of the individualized custom boluses. The mean lung irradiation dose to the ipsilateral iung was also significantly reduced by about 430 cGy, Trom 2757 cGy to 2,327 cGy (p<0.01). The V$_{50}$ and V$_{95}$ in the ipsilateral lung markedly decreased from the averages of 54.5 and 17.4% to 45.3 and 11.0%, respectively. The V$_{50}$ and V$_{95}$ In the heart also decreased from the averages of 16.8 and 6.1% to 9.8% and 2.2%, respectively. The NTCP In the contralateral lung and the heart were 0%, even for the cases with no bolus because of the small effective mean radiation volume values of 4.4 and 7.1%, respectively Conclusion : The use of an Individualized custom bolus in the radiotherapy of postrnastectorny chest wall reduced the NTCP of the ipsilateral lung by about 24.5 to 40.5%, which can improve the complication free cure probability of breast cancer patients.
This study examined the effects of growth retardants and midsummer drainage on lodging-related characters in direct seeding rice culture on flooded paddy surfaces. Treatments included in this study were continuous flooding, two time midsummer drainages (TTD), and inabenfide or trinexapac-ethyl application after TTD. Culm length was reduced most by 11.4cm in trinexapac-ethyl treatment after TTD, followed by 4.1cm in inabenfide treatment after TTD compared to TTD treatment. A significant reduction in culm length occurred at the 3rd internode in inabenfide treatment applied at 40 days before heading (DBH), while trinexapac-ethyl application at 8 DBH reduced the 3rd>2nd>1st internode. The culm diameter of the 4th internode was increased by the application of trinexapac-ethyl following TTD compared to TTD treatment. Thickness of culm wall was significantly increased by trinexapac-ethyl but not by inabenfide applications. Lodging index was the highest in continuous flooding and the lowest in trinexapac-ethyl treatment after TTD. Subsequently, field lodging did not occur in plots treated by trinexapac-ethyl. Rice plants subjected to midsummer drainage or growth retardants showed higher ripened grain ratio and 1000-grain weight, resulting in higher rice yield. These results indicate that trinexapac-ethyl, which can be applied at the relatively later growth stage after proper judgement of lodging occurrence, could be a useful tool for reducing lodging in direct seeding rice culture on flooded paddy fields.

Nowadays, three-dimensional computer added design(3D CAD) tool are widely and actively used for design of mechanical machine. Because using the tool is more effective to understand design concept and to collaborate with other operation than using two-dimensional design tool. In this study, the 3D CAD tool which is called I-DEAS was applied for three-dimensional modeling of main parts and assembling of modeled parts for identification the entire shape of a injection molding machine. In addition, a study was also performed regarding reduction for the weight of main plates for saving production cost and energy in the machine. A finite element method(FEM) program in I-DEAS tool was used for the improvement study. First, the current main plates were structural analysed and then the plate deformations, weak regions and stress distributions were graped. By the FEM results, the 2nd improved designing of the plates was conducted such as reinforcement or slimming of the plate wall thickness. The 2nd structural FEM was performed for verification of the redesigned plates and then the FEM results were compared with the 1st FEM's result. The weight of the main plates were averagely reduced approximately 3 - 7%. By these results, it was seemed that the improved plates have a useful availability.

In this study, we evaluated the effect of fibrin, a natural material, on the local inflammatory reaction of PLGA in vivo. PLGA degradation products can decrease the pH in the surrounding tissue, causing local inflammatory reaction. To solve this problem, fibrin/PLGA scaffolds were implanted in 5-week-old Wister rats. To evaluate the influence of fibrin content on inflammatory cytokine expression induced by PLGA, RT-PCR analysis was used. Fibrous wall thickness and macrophage infiltration were evaluated by H&E and ED-1 immunohistochemical staining, respectively. In this study, we showed that fibrin/PLGA scaffolds reduced inflammatory reaction as compared to PLGA scaffold. We concluded that fibrin could reduce inflammatory response of PLGA.
