Implantation of a permanent pacemaker is a widely accepted procedure for the patient with complete heart block.As a result of these device, the prognosis for patients with Adams-Stokes syndrome caused by complete A-V block and other cardiac arrhythmia have become much more optimistic. Permanent pacemaker implantation by means of a transvenous approach has made the operative risk much less and the procedure simpler. However, a number of complications have been reported in the literature regarding transvenous endocardial pacemaker implantation during the last a decade. The patient presented in this paper is a 26-year old girl who was implanted with a permanent pacemaker at 14 years of age because of a congenital A-V block. Following first exchange of pulse generator, the electrode (lead) was fractured, so that by the pulse generator, a change to the transvenous technique of implantation was made, After this, there were episodes of recurrent wound infection on three occasions, even though the site of pulse generator implantation was exchanged to the contralateral side of chest wall, massive doses of antibiotics were administered and sensitivity tests for coagulase positive staphylococcal infection were performed. Though there was no definite evidence of blood stream infection by blood culture, we decided not to use the transvenous technique and not to implant the pulse generator in the chest wall because the venous system and the entire anterior chest wall appeared to be diseased or contaminated by virulent pyogenic organisms. Finally this intractable systemic and local wound infection was successfully controlled by myocardial lead implantation via a subxiphoid approach and implantation of the pulse generator far down in the abdominal wall. The causes and routes of recurrent wound infection and possible blood born infection in this particular patient are still obscure. We strongly believe that myocardial pacemaker implantation is much safer than transvenous endocardial pacemaker implantation & myocardial pacemaker implantation is a definite method for controlling such an intractable wound infection. following transvenous pacemaker implantation.
Objective: To explore the value of porous titanium alloy plates for chest wall reconstruction after resection of chest wall tumors. Materials and Methods: A total of 8 patients with chest wall tumors admitted in our hospital from Jan. 2006 to Jan. 2009 were selected and underwent tumor resection, then chest wall repair and reconstruction with porous titanium alloy plates for massive chest wall defects. Results: All patients completed surgery successfully with tumor resection-induced chest wall defects being $6.5{\times}7cm{\sim}12{\times}15.5$ cm in size. Two weeks after chest wall reconstruction, only 1 patient had subcutaneous fluidify which healed itself after pressure bandaging following fluid drainage. Postoperative pathological reports showed 2 patients with costicartilage tumors, 1 with squamous cell carcinoma of lung, 1 with lung adeno-carcinoma, 1 with malignant lymphoma of chest wall, 2 with chest wall metastasis of breast cancers and 1 with chest wall neurofibrosarcoma. All patients had more than 2~5 years of follow-up, during which time 1 patient with breast cancer had surgical treatment due to local recurrence after 7 months and none had chest wall reconstruction associated complications. The mean survival time of patients with malignant tumors was ($37.3{\pm}5.67$) months. Conclusions: Porous titanium alloy plates are safe and effective in the chest wall reconstruction after resection of chest tumors.

Background The survival rate of newborns with severe congenital abdominal wall defects has increased. After successfully addressing life-threatening complications, it is necessary to focus on the cosmetic and functional outcomes of the abdominal wall. Methods We performed a chart review of five cases treated in our institution. Results Five patients, ranging from seven to 18 years of age, underwent the following surgical approaches: simple approximation of the rectus abdominis fascia, the rectus abdominis sheath turnover flap, the placement of submuscular tissue expanders, mesh repair, or a combination of these techniques depending on the characteristics of each individual case. Conclusions Patients with severe congenital abdominal wall defects require individualized surgical treatment to address both the aesthetic and functional issues related to the sequelae of their defects.
Experimental programs were accomplished to improve and evaluate the structural performance of test specimens, such as the hysteretic behavior, the maximum horizontal strength, crack propagation of and ductility etc. Test variables are restraining factors of frame, with or without masonry infilled wall, and masonry method Six reinforced concrete rigid frame and masonry infilled wall were tested and constructed in one-third scale size under vertical and cyclic loads simultaneously. Based on the test results, the following conclusions can be made. For masonry infilled wall with restraining factors of frame, maximum horizontal capacities were increased by 1.91~2.24 times in comparision with that of rigid frame. For masonry infilled wall with restraining factors of frame(IFWB-l~3), cumulated energy dissipation capacities wear increased by 1.35~l.60 times in comparision with that of masonry infilled wall(IFB-1) at final stage of testing.
Recently, drywall's demand is increasing by interest about spread of remodeling house and separated wall structure. This research evaluated panel's SRI and found out panel properties using material of small size. Conclusion of this research is as following. First, we confirmed the effectiveness of small-scale material. Measuring results appeared equally about 400 ${\sim}$ 500 Hz that is fc frequency. Second,, it is no big difference in SRI that use CRC or magnesium board that is used for protection of panel surface. Third, it is compared SRI by used junction to make wall that become disjointing assembly. By the result, sealed wall secures resemblant SRI performance almost with normal wall. Therefore, using joint materials and sealing junction became wall that is detached with high SRI.
Wall wetting in diesel engines has been considered as a bad phenomenon because of fuel deposition which makes fuel/air mixing and evaporation worse. In order to avoid the problem, many research works have been carried out. One of the studies is on new combustion chamber systems which are using spray impacting on a wall. In this study a new type of chamber system is analysed using wall impaction model introduced and assessed in the coupled paper. The gas phase is modelled in terms of the Eulerian continuum conservation equations of mass, momentum, energy and fuel vapour fraction, The liquid phase is modelled following the discrete droplet model approach in Lagrangian form. With various conditions the spray distribution, vapor contour and gas flows are analyzed, and then design factors of those combustion systems are recommended.
The Hwasong City Wall was built for three years(1794-1796), but the real construction term was taken only 28 months. The wall and the buildings on the wall were built simultaneously, and the esttimated construction cost was highly restricted. These conditions were main factors for considering the high efficiency of construction productivity. This study examines these factors that are reflected in the operating system of the architectural standard scale of length, the architectural composition system in the Hwasong City Wall to reduce the construction cost. Through the study, following factors are found. 1.The rule of multiple proportion is found in the relationship between the partition distance, that is, span and the height of the girder which is related to the productivity of ready-made building members. 2.The ruling grade system of scale in the component members is found in the chang-bang(창방, penetration members)
In this paper, the wall follwing method of a motorized wheelchair is discussed. The wall fellowing problem is characterized by maintaining a constant distance to the wall, which should be possible using a distance measuring sensor only. Ultrasonic sensors are cheap are fairly simple to use in this case. The main problem is the calculation of the distance and orientation of the wheelchair with respect the wall from the sensor data. This is solved by the method that sensor data is obtained from 3 ultrasonic sensors arranged at a same perpendicular pivot. The results show that a new method is very effiecient for a motorized wheelchair.
강판파일로 지지되고 있는 미니 교대하부에, 압성토의 제거에 따른 교대의 안정성을 확보하기 위한 방법으로서 쏘일네일링 공법을 채택하였다. 네일벽체에 각종 계측기를 매설하여 벽체의 거동을 추적하였다. 또한 3차원 유한요소해석기법을 이용하여 쏘일네일 벽체와 기존 구조물사이의 상호영향과 관련된 벽체의 거동을 분석하였다. 압성토의 순차적 제거와 네일타설 등의 전 축조 시퀀스를 모델링 할 수 있는 기법을 개발하였으며 개발된 시뮬레이션 기법의 타당성을 검증하기 위해 현장에 축조된 실물 계측데이터를 이용하여 보정하였고, 압성토의 제거와 네일타설이 기존 파일의 축하중 및 휨모멘트에 미치는 영향과 네일 인장력의 변화와의 상관관계 등, 설계시 고려되어야 할 몇 가지 주요한 사항에 대한 해석결과를 구할 수 있었다.

All the specimens were constructed in one-third scale size. Based on the test results, the following conclusions can be made. For masonry infilled wall(IFB-1), maximum horizontal capacity was increased by 1.45 time in comparision with that of rigid frame(FB-0). For cut off masonry infilled wall (IFBC-1~3), maximum horizontal capacity was increased by 1.73~1.98 time in comparision with that of rigid frame(FB-0). For cut off masonry infilled wall(IFBG-1~3), ductility was increased by 1.48~2.08 time in comparision with that of masonry infilled wall (IFB-1).
