Effect of home-based exercise program on balance, activities of daily living, depression and quality of life in elderly with chronic diseases (방문재활운동 프로그램이 만성질환 노인의 균형, 일상생활활동과 우울 및 삶의 질에 미치는 영향)

  • Background: The purpose of this study was to identify the effect of home-based exercise program in elderly with chronic diseases on balance, activities of daily living, depression and quality of life. Design: One group pretest-posttest design Methods: The subjects consisted of 21 elderly people over the age of 65 years with chronic diseases lived in seo-gu, Gwangju. Home-based exercise program was performed twice a week, for an hour, during 6 weeks under supervision of an experienced instructor. The home-based exercise program was structured to last a total of 1 hour, consisting of 10 minutes of warm-up, 30 minutes of main exercises, and 20 minutes of concluding exercises. For the warm-up, we performed range of motion, stretching, and flexibility exercises. The main exercises comprised of strength, weight-bearing and balance exercises, and endurance exercises. The concluding exercises were tailored to the individual's home environment and needs, including task-oriented daily living training such as getting up from bed, moving to the restroom, and walking indoors. The subjects were assessed before and after the exercise program with berg balance scale(BBS), Korean-version modified barthel index(K-MBI), euro quality of life questionnaire 5-dimensional classification(EQ-5D), and geriatric depression scale short form-Korea(GDSSF-K). The collected data was analyzed using SPSS Win 18.0 program. Results: The results showed statistically significant difference in all domains of the BBS, K-MBI, EQ-5D, GDSSF-K after the exercise program. Conclusion: These results suggest that Home-based exercise program has resulted in a significant improvement on ADL, depression and quality of life in elderly with chronic diseases.

Effect of Physical Activity on Blood Sugar Control in Young and Middle-aged Diabetic Patients Using the 8th Korea National Health and Nutrition Examination Survey Data (국민건강영양조사 자료를 이용한 청년 및 중·장년층 당뇨병 환자의 신체활동량이 혈당조절에 미치는 영향 분석)

  • Purpose : The number of young patients with diabetes is on the rise, and they face challenges in managing their blood sugar levels. This study aimed to investigate the effect of physical activity on blood sugar control in young and middle-aged patients with diabetes aged 19-64 years using data from the 8th Korea National Health and Nutrition Examination Survey (2019-2021). Methods : Blood sugar control in patients with diabetes was determined based on FBG 110 mg/㎗, and physical activity was measured using physical activity categories that considered the sum of walking, moderate intensity, and high intensity. Statistical analysis was conducted using SAS 9.4, and the Rao-Scott chi-square test was conducted to determine differences in blood sugar control based on the sociodemographic characteristics and physical activity of patients with diabetes. Complex-sample multiple logistic regression analysis was conducted to assess the effect of physical activity on blood sugar control. Results : There were statistically significant differences in blood sugar control between young and middle-aged patients with diabetes, depending on sex, age, marital status, obesity, and amount of physical activity. As a result of the complex sample simple logistic regression analysis without adjusting for variables, the non-physical activity group was found to have a higher risk of blood sugar dysregulation than the health-promoting physical activity group (OR: 2.80). Complex-sample multiple logistic regression analysis with control variables showed that the non-physical activity group had a higher risk of blood sugar dysregulation than the health-promoting physical activity group (OR: 3.70). Conclusion : Multifaceted efforts are needed to develop health intervention programs that can increase physical activity, diabetes awareness, and treatment rates, including controlling blood sugar levels and preventing complications in young and middle-aged patients with diabetes.

Development of the Home-Based Pulmonary Rehabilitation Program for Patients with Chronic Lung Disease (만성 폐질환 환자에서 재택 호흡재활치료방법 개발 연구)

  • Background : Even though it is well known that pulmonary rehabilitation (PR) improves exercise capacity, and the quality of life, in patients with chronic lung diseases, not many patients can attend hospital based intensive PR in Korea. The purpose of this study was to develop a method for a home-based PR program, and study its effectiveness. Materials and Methods : Twenty patients with chronic lung diseases were randomly divided into two groups : a home PR group comprising of 10 male patients, with a mean age of 70 years, and a control group comprising of 10 male patients, with a mean age of 65 years. We developed exercise programs, depending on the exercise capacity of each patient, which were easy to do at home. The PR program consisted of a 12 week period of enforced aerobic (mostly walking) and muscle strengthening exercises, as prescribed by the exercise specialist, in accordance with the functional capacity of the patient. In addition to the education, nutritional and psychiatric consultation was undertaken, and respiratory muscle training arranged. Patients visited hospital every 2 weeks for evaluation and exercise prescription. Results : All patients finished the 12 week course of therapy. Following the home PR, the endurance times and work capacity of the upper and lower extremities were significantly increased in the treatment group in comparison to the controls. The six minute working (Eds note:should) 'working' read "walking"?) distance was increased from $465{\pm}60m$ to $508{\pm}37m$ and the maximal inspiratory pressure from $72.8{\pm}27.2cmH_2O$ to $91.4{\pm}30.9cmH_2O$. The quality of life, as assessed by St Georges Respiratory Questionnaire (SGRQ), was also improved following PR. Conclusion : The home PR program we developed seemed to be applicable, and effective, to most of the patients with chronic lung diseases in this study.

The Preference Analysis of Adults on the Forest Therapy Program with regard to Demographic Characteristics (인구통계학적 특성에 따른 성인 대상별 산림치유 프로그램 선호도 분석)

  • The purpose of this study was to obtain the basic data for the effective development and operation of forest healing by analysing the preference of adults on the forest therapy program. The survey of 516 normal adults on the forest therapy program was conducted. Using SPSS 21.0 Program, data analysis of descriptive statistics and correlation analysis of each program confirmed number of covariate, and so we conducted a factor analysis to short meaningful data. And then we looked at the forest healing program preference difference according to demographic characteristics(sex, age, residence, occupation) through reliability analysis, frequency analysis, T-test, analysis of variance(ANOVA). Facter analysis was performed by use of the VariMax orthogonal rotation factor analysis to abbreviate 37 forest therapy program. Through this, the program has been reduced such as 7 program group(1: psychology-based treatment, 2: teaching and counselling-based treatment, 3: camping and forest bath-based treatment, 4: diet-based treatment, 5: nature and plant-based treatment, 6: meditation-based treatment, 7: respiration and yoga-based treatment) and 7 independent programs(1: sleeping in the forest, 2: vision quest, 3: soaking in water, 4: walking wearing shoes, 5: viewing the forest, 6: mountaineering, 7: athletics in the forest). With this criterion, the forest therapy program difference in accordance with sex, age, residence, occupation has been investigated. First, it was verified that there was a significant difference between male and female in the group of such as psychology-based treatment, diet-based treatment, mountaineering. Second, there appeared a significant difference in groups such as teaching and counselling-based treatment, camping and forest bath-based treatment, nature and plant-based treatment, respiration and yoga-based treatment, athletics in the forest. Third, according to the residence, there appeared a significant difference in groups such as psychology-based treatment, teaching and counselling-based treatment, diet-based treatment, nature and plant-based treatment, meditation-based treatment, vision quest, mountaineering, athletics in the forest. Fourth, according to the occupation, there appeared a significant difference in groups such as nature and plant-based treatment, medication-based treatment, vision quest, walking wearing shoes, viewing the forest, mountaineering. As shown before, it seems to be necessary that we should be mindful of this investigation which shows variety of preference of adults on the forest therapy program according to demographic characteristics. We expect the results of this study to be utilized as basic data for the development of forest therapy program targeting on adults.

Activation plan of social safety network of the aged living alone - Focused on the aged using wireless paging system in Gwangyang-si - (독거노인의 사회안전망에 대한 활성화 방안 - 광양시 무선페이징시스템 대상자 중심으로 -)

  • Objective : The purpose of this study is to identify actual operations and issues of wireless paging system operation for the aged living alone as the end users of wireless paging system primarily in Gwangyang city, and thereby to explore possible advanced and integrated ways to promote social safety network for the aged. Methods : The survey tool used in this study was a structured questionnaire form consisting of question items. The researcher hereof conducted this survey by means of direct visit and interview during two seasons, i.e. from February to August, 2008 and from December 2008 to March 2009, respectively. Results : 1) For general demographic characteristics, it was found that 90.9% of all respondents were women and 61.2% of all respondents were at age 75 to 84. for health conditions, it was found that more than 90% of all respondents often took medicines due to their unhealthy body, and most of respondents suffered from musculoskeletal diseases 79.3% and circulatory diseases 61.6%. for walking capacity, it was found that 45.5% of all respondents used walking aids, and disable respondents (11.5% of all respondents) were represented primarily by those with physical disability (52.6% of disable respondents). for actual use of medical institutions, it was found 47.3% of all respondents relied on local clinics, since they preferred neighborhood hospitals or clinics they can trust for medical care. for social activities and supports, it was found that 43.6% respondents had 'needs for assistance at times' and 33.9% respondents have 'no need for assistance'. And it was found that the major difficulties in living alone at old age were represented primarily by health problems 37.8% and economic difficulties 33.5%. 2) For characteristics related to wireless paging system, it was found that 90.3% respondents used wireless paging system recommended by firemen, and 28.5% respondents used this system. and it was found that 59.6% respondents used this system once, and 85.2% respondents used it because of acute or chronic diseases. more than 90% respondents thought that they knew about wireless paging system and considered themselves safe, but 83.6% respondents didn't attach a remote control on their upper clothes, and even 49.1% respondents turned off the power of wireless paging system due to their concern about electricity bill and noise. 3) It was found that 83.6% respondents felt it necessary to use wireless paging system, and wireless paging users felt more satisfied with using the system than non-users, and 50.7% showed high satisfaction at certain psychological benefits like 'confidence in coping with critical situations' and 'a sense of relief'. In addition, it was found that some respondents who answered that 'they didn't turn off the paging system as they knew how it works' and those who answered that they knew 'how to use it' showed relatively high satisfaction. And some respondents who kept it well and felt it necessary also showed high satisfaction. 4) It was found that the level of satisfaction our respondents felt with using wireless paging system varied significantly availability($x^{2}$ = 12.759, p = .002), psychological advantages($x^{2}$ = 12.174, p = .002), knowledge about how to use system($x^{2}$ = 7.021, p = .016), power on/off($x^{2}$ = 13.221, p = .001), level of knowledge about system($x^{2}$ = 21.002, p = .000), maintenance($x^{2}$ = 9.871, p = .007) and level of necessity($x^{2}$ = 34.939, p = .000) on the statistical basis.

Effects of Breed and Sex on Behavioral Characteristics and Economic Traits of Performance-tested Pigs (품종과 성이 검정돈의 행동특성과 경제형질에 미치는 영향)

  • This experiment was carried out to determine the effect of breed and sex on behavioral characteristics and economic traits of performance-tested pigs. Totally 32 tested pigs in 4 different breeds were assigned for behavioral observation. Behavior was recorded for 4 hours(each 2 hours, before and after none) of tested pigs in each tested pen. The average daily gain, age at 90 kg and feed efficiency on the basis of the performance data were collected from 8,477 performance-tested pigs in which pure breeds of Duroc, Yorkshire, Landrace and Berkshire at the Korea Swine Testing Station. The results obtained in this study are summarized as follows; The effect of breed was statistically significant for all traits studied. Average daily gain were the highest(P<0.05) in Duroc and ages at 90 kg were the highest(P<0.05) in Berkshire. In feed efficiency of male, Landrace and Yorkshire were the most efficient. The average daily gain and feed efficiency were superior to those of female, however, the age at 90 kg was not different. Overally, females were superior to males in various traits examined. Berkshire breed had more proportion of time spent in ventral tying and sitting whereas the proportion of time spent in walking, drinking and eating was less than that of other breeds. The male pigs showed more time spent in standing and social behavior.

Effect of HSEP on elderly Women's Life Function Fitness and Health-related QOL (재가운동 프로그램이 여성 고령자의 생활기능 체력과 건강관련 삶의 질에 미치는 영향)

  • The purpose of this study was try to check the change of a subjective evaluation about the life function fitness and health-related quality of life by executing the home support exercise program three times a week for total 16 weeks with elderly women that are order than 70 years old. The experiment group(total : 17 people, mean age : 79.4) and the control group(total : 17 people, mean age : 80.7) live Yeonje-gu in Busan city, and they don't have any physical injury and any inconvenience to move. and they are all composed of elderly women who agree to participate in this study. We carried out an analysis of covariance with SAS 9.2 statistics package. These are the results from it. First, the experiment group showed statistically significant increase(p<.001) in lifting dumbbells, repeatedly standing up and sitting down, 2.24m shuttle running, walking at the same place for 2 minutes. Second, there was no particular effect with the experiment group in flexibility(catching hands with their backs leaned against each other). This result point out that there is a need of supplementation like adding exercise item which can improve flexibility to home support exercise program. Third, there was no particular difference between the experiment group and the control group in 8 sub-factors with the subjective evaluation about health-related quality of life through SF-36. However, there the bodily pain and vitality sub-factors of experiment group was better than that of the control group with p<.10 level, so we considered this result showed us the positive effect slightly.

A Study on the Foot Plantar Pressure and Temperature changes of the Developed Combat boots with Functional Impact Absorption and Ventilation Insole (충격흡수 및 통기기능 인솔을 적용한 개발 전투화의 족저압력 및 온도변화 연구)

  • The purpose of this study was to compare the foot plantar pressure and temperature changes of the developed combat boots with functional impact absorption and ventilation insole. A total of 11 male subjects(age: $21.8{\pm}2.2yrs$, height: $174.3{\pm}3.6cm$, weight: $71.6{\pm}8.6kg$, foot length: $261.0{\pm}1.0mm$) were recruited to compare the foot plantar pressure and temperature changes of the three types of combat boots: Combat boots A (generalized combat boots), Combat boots B (developed combat boots with ventilation function), Combat boots C (Application of ventilation function and impact absorption insole to combat boots B). Pedar-X and a portable thermistor temperature sensor were used to measure the foot plantar pressure parameters and the internal temperature of the combat boots, respectively. One-way ANOVA was used to compare the results of plantar pressure and temperature changes. The results were as follows: First, in the foot plantar pressure parameters, combat boots C showed the significant lower maximum foot plantar pressure in the right/left rear foot compared with combat boots A and average foot plantar pressure in the left foot compared with combat boots B. Second, after 40 minutes from the start of walking, the developed combat boots B and C showed the significant lower temperature than the general combat boots A.

Changes of Center of Pressure and Cadence Analysis According to the Carrying Type and Weight of a Bag in College Students (일부 대학생들의 가방 휴대 방법과 무게에 따른 압력중심점 변화와 보행률 분석)

  • This study examined the changes in the center of pressure (COP) and cadence according to three types of bags carried and three different bag weights. The experimental period was from June 3, 2013 to January 20, 2014, and thirty eight healthy adults (mean age: $26.10{\pm}5.32$ years, age range: 22-30 years) volunteered under three conditions. In the first condition, the subjects wore a bag over one shoulder and they walked. In the second condition, they carried a bag across the shoulder and walked. With the third condition, they carried a backpack using both shoulders and walked. 3-Dimentional motion analysis system and a force plate were used to measure the kinematic parameters of gait and COP displacement during gait. Each subject walked 6 meters carrying a 3kg, 5kg and 7kg bag under all three conditions. The COP of the antero-posterior and medio-lateral, and cadence variable were significantly different according to the bag weight of 3kg, 5kg and 7kg. These findings suggest that walking while carrying a backpack using both shoulders is more effective on the changes in COP and gait of adults than the other conditions.

Establishment of Early Diagnosis and Surgical Operative Method in Puppies with Congenital Patellar Luxation (선천성 슬개골탈구를 지닌 자견의 조기 진단법 및 외과적 수술법 확립)

  • Medial patellar luxation in dogs is one of the most common patellar problems presented to the veterinary practitioner. It is observed in toy and miniature breed and the majority of cases is a congenital form. Because of extensor mechanism's instability, it causes deformity and disorder in the growth of the affected limb when the luxation is left without treatment As lameness is not easily detectable in puppies, early diagnosis and correction are essential for therapy. Up to now, there has not been any reports refering to the diagnostic methods and the optimal age for correction in young dogs. Thirteen 45-90 days old puppies, have grade I and/or II medial patellar luxation. Only by palpation, all 13 dogs were diagnosed of patelar luxation. Skyline radiographic view was useful to interpret patellar morphology and depth of trochlear groove only above 60 days old. However, it was difficult to make definite diagnosis patellar luxation. The caudocranial and lateral radiographic view as well as ultrasonographic skyline view were not showed of patellar luxation. 2 puppies had unilateral patellar luxation and 11 puppies had bilateral patellar luxation which more serious on the left than on the right. Only 3 puppies among 11 puppies with bilateral patellar luxation were observed of lameness degree 1. Regardless of grade of patellar luxation and lameness, we performed trochlear chondroplasty using a U-shape sculpture blade to minimize cartilage injury, transposition of tibia tuberosity with No. 1 Supramid to align extensor mechanism and lateral imbrication. After surgery, we examined the operated animal daily for 10 days and on 15, 30 and 60 days after surgery respectively. After operation, pain and fever became normal on 7 days, swelling on 10 days, respectively. On 10 days after surgery, dogs showed normal standing position, and normal walking was observed in 15 days after surgery. In force plate analysis, the operated legs were normal weight bearing at 30 days after operation. After surgery, not only patellar luxation and clinical signs have been gradually reduced but also bone growth have become normal without showing growth physeal plate injury. The survival rate of puppies over 62 days old was 100%, while 42-45 days old 37.5%. The above results suggest that optimal age for surgical correction of congenital medial patellar luxation is recommended over 60 days old. In conclusion, combination of trochlear chondroplasty, transposition of tibia tuberosity, and lateral retinacular imbrication is appropriate for over 60 days old puppies to efficiently correct patellar luxation.
