3축 가속도 신호를 이용한 편마비 환자의 보행 분석에 대한 연구 (The Study of Gait Analysis for Hemiplegic Patient Using 3-axis Acceleration Signal)

  • In this study, we proposed an algorithm which can detect the walking event in hemiplegic patient using three axis acceleration signal. Twenty hemiplegic patients were participated in an experiment on a level corridor. To evaluate the accuracy, we compared the time difference between the detected event and signal from FSR-Sensor. Consequently, the mean difference of 46.1ms was obtained and it suggests that the proposed method is effective to detect the walking event in hemiplegic patient. In future, these results could be used to evaluate the walking ability in hemiplegic patient in clinical practice.

건강한 아동이 걸을 때에 생리학적 소비지수 (Physiological Cost Index of Walking in Healthy Children)

  • Physiological Cost Index (PCI) of walking has been widely used to predict oxygen consumption in healthy subjects or patients. The purpose of this study was to evaluate the predictability of physiological cost index of walking for the amount of exercise and cardiac function. Walking exercise was conducted in 67 healthy children (age 4-12) with a self-selected comfortable walking speed on the level surface. Walking speed was calculated, and heart rate was measured before and immediately after the walking. PCI was calculated for statistical analysis. The results were as follows; 1) The walking speed tends to increase and PCI of walking tends to decrease with age. There was significant difference in walking speed and PCI of walking among three age groups (p<.05). The change of walking heart rate tends to decrease with age, however, there was no significant difference among three age groups. 2) Linear regression equation between walking speed and age was 'Y (walking speed) = 2.124X (age) + 48.286' ($R^2$=.337), (p=.00). 3) The walking heart rate tends to decrease with age. Linear regression equation between walking heart rate and age was 'Y (walking heart rate) = 143.346 - 2.63X (age)' ($R^2$=.3425), (p=.00). 4) The walking heart rate decreased as body surface area (BSA) increased. Linear regression equation between walking heart rate and BSA was 'Y (walking heart rate) = 149.830 - 27.115X (BSA)' ($R^2$=.3066), (p=.00). In conclusion, these equations and PCI could be useful to quantify the variation of energy expenditure of children with pathological gait when compared with age-matched healthy children.

  • Stable and comfortable walking supports, which can reduce the body weight load partially, are needed for the recovering patients from neurologic disease and orthopedic procedures. In this paper, the development of a manipulator of rehabilitation robot for the patients with walking disabilities are studied. A force controller using pneumatic actuators is designed and implemented to the human friendly rehabilitation robot considering the safety of patients, reliability of the system, effectiveness of the unloading control and economic maintenance of the system. The mechanism of the unloading manipulator is devised to improve the sensibility for the movement of the patients such as direction and velocity. For the unloading force control, fuzzy control algorithm is adopted to reduce the partial body weight and suppress the unwanted fluctuation of the body weight load to the weak legs due to the unnatural working of the patients with walking disabilities. The effectiveness of the force control is experimentally demonstrated.

상하지 연동된 새로운 보행재활 로봇의 설계 (Design of a Novel Gait Rehabilitation Robot with Upper and Lower Limbs Connections)

  • This paper proposes a new rehabilitation robot with upper and lower limb connections for gait training. As humans change a walking speed, their nervous systems adapt muscle activation patterns to modify arm swing for the appropriate frequency. By analyzing this property, we can find a relation between arm swinging and lower limb motions. Thus, the lower limb motion can be controlled by the arm swing for walking speed adaptation according to a patent's intension. This paper deals with the design aspects of the suggested gait rehabilitation robot, including a trajectory planning and a control strategy. The suggested robot is mainly composed of upper limb and lower limb devices, a body support system. The lower limb device consists of a slider device and two 2-dof footpads to allow walking training at uneven and various terrains. The upper limb device consists of an arm swing handle and switches to use as a user input device for walking. The body support system will partially support a patient's weight to allow the upper limb motions. Finally, we showed simulation results for the designed trajectory and controller using a dynamic simulation tool.

순환식 장애물 집단 보행 훈련이 뇌졸중 환자의 보행 능력과 정서에 미치는 효과 (The Effects of Circuit Obstacle Group Gait Training on Gait and Emotion in Stroke Patients)

  • Purpose : This study is designed to demonstrate the effects of circuit obstacle group gait training on walking ability and emotion in stroke patients. Methods : Twenty one patients with stroke were participated in this study. The subjects were divided into control group(n=10) and experimental group(n=11). Circuit obstacle group gait training consisted of walking around obstacles, walking over obstacles, walking up and down slopes and walking up and down stairs. Circuit obstacle group gait training was conducted five times per week, 1 hour per session, for 6 consecutive weeks. At pre-test and post-test, subjects were tested with 10 m walking test, timed up and go test, up and down 4 stairs test, depression and self esteem. Results : After 6 weeks of research, the experimental group showed statistically significant difference in all items when comparing prior to training and after training (p<.05), but the control group showed statistically significant difference in items other than depression and self esteem(p<.05). In the comparison between the two groups, the experimental group showed higher improvement than the control group in the 10 m walking test, timed up and go test, and up and down 4 stairs test, and there was statistically significant difference in decrease of degree in depression between the experimental group and control group(p<.05). Conclusion : This study have shown that circuit obstacle group gait training improves walking ability and emotion in stroke patients.

보행보조 로봇의 운동학적 특성 (Kinematic Characteristics of Walking-Assistance Robot)

  • 보행 재활 훈련용 보행보조 로봇을 개발하고, 시제품의 운동학적 특성을 평가하였다. 이 보행보조 로봇은 고관절(hip), 슬관절(knee), 족관절(ankle) 등으로 구성되며, 각 관절은 감속기가 포함된 모터에 의해 구동된다. 인체 보행 운동을 이론적으로 해석하여, 보행 운동 중 각 관절의 각도 변위를 계산하는 식을 구하였고, 계산된 각도 변위를 로봇 구동기에 입력하였다. 트레드밀(treadmill) 위에서의 실험을 통해 다양한 보행 속도(walking speed) 및 보폭(stride)에서 각 관절의 출력 각도 변위를 측정하고 입력 값과 비교하였다. 입력 각도 변위와 출력 각도 변위의 차이가 고관절에서는 5.22%, 슬관절에서는 2.97% 이내로 일치함을 확인하여, 설계대로 보행보조 로봇이 작동함을 입증하였다.

어지럼증 재활을 위한 증강현실 기반 보행훈련 콘텐츠 (Walking training contents based on Augmented Reality for dizziness rehabilitation)

  • 일반적으로 어지럼증은 여러 가지 복합적인 원인에 의해서 발생하지만 그 중에서 신경계에 속하는 전정계통의 기능 장애에 의한 증상이 가장 환자에게 심한 증상으로 다가오며, 구역질과 구토를 동반하게 된다. 이러한 어지럼증에 대한 치료는 약물요법, 수술요법, 재활치료 등이 있으며, 약물요법이나 수술요법은 대체적으로 후유증의 위험 때문에 재활치료요법인 전정재활훈련을 많이 시행하고 있다. 전정재활훈련은 안구훈련, 자세안정훈련, 보행훈련 등이 있는데 그 중 보행훈련은 의사 또는 전문치료사의 감독하에 일정한 공간에서 진행되므로 시간적, 공간적 부담이 가중되어 환자들이 훈련을 진행기 불편한 단점을 가진다. 이를 해결하고자 본 논문에서는 증강현실 기술을 활용해 사용자가 직접 보행재활훈련을 진행 할 수 있는 보행훈련 콘텐츠를 구현하였다. 추후 어지럼증 환자를 대상으로 한 임상시험을 통해 의료 환경에서 사용 가능한 어지럼증 재활 콘텐츠로 활용 가능할 것으로 기대한다.

Relationship between straight and curved walking abilities among inpatients in the subacute phase according to walking independence level

  • Objective: This study was performed to examine differences in the association between straight and curved walking abilities of inpatients in the subacute phase and walking independence level. Design: Cross-sectional study. Methods: Subjects were divided into an independent group and a supervised group (n=10 each) by walking independence level within the ward decided by physical therapists. Inclusion criteria comprised the ability to ambulate independently within the ward, regardless of the use of walking aids. Straight walking abilities (walking velocity, stride length, and cadence) were evaluated using the 5-meter walk test. Curved walking abilities were evaluated using the Figure-of-8 Walk Test (F8W) and the 3-meter zigzag walk test (3ZW). Differences in associations between straight and curved walking abilities of inpatients were examined by calculating correlation coefficients between straight and curved walking abilities. Results: Age, walking velocity, stride length, F8W and 3ZW varied markedly between independent and supervised groups. In the independent group, F8W and 3ZW correlated significantly with walking velocity and cadence (p<0.05). In the supervised group, F8W correlated significantly with walking velocity and stride length (p<0.05), but 3ZW did not correlate significantly with straight walking abilities. Conclusions: The association between straight and curved walking abilities varied between inpatients who could ambulate independently and inpatients requiring supervision for ambulating. These differences may depend on skill on straight and curved walking abilities. There is a possibility that curved walking exercise is necessary for supervised group.

The effects of dual-task training on ambulatory abilities of stroke patients: Review of the latest trend

  • Objective: Dual-task walking deficits impact functional daily life, which often requires walking while performing simultaneous tasks such as talking, thinking or carrying an object. This study is to find out the latest trend of dual-task training's influence on ambulatory abilities of a stroke patient. Design: Systematic review of randomized controlled trials. Methods: This literature review was conducted in Pubmed and Sciencedirect with the follwing key words:stroke, cerebro-vascular accident, hemiplegia, gait, rehabilitation, exercise. 7 studies were chosen in findings by search tool. 3 studies were case study, 3 studies were cross sectional observational study and 1 study was randomized controlled trial. Results: It was found that stroke patients have difficulties in doing 2 motor tasks simultaneously and when they do 2 tasks, one is done in a naturally preferred activity areas. Moreover, when simply applying dual-tasks, the walking speed decreased. Meanwhile, when applying them through training, the speed increased. This showed the improvement of effective task-implementation abilities after dual-task training using task-integration models. Conclusions: In the beginning of the 2000s, dual-tasks were implemented by simply combining walking and cognition or exercise task, and the results of this study suggest that subjects with stroke have difficulty performing dual task. However, the latest trend is to let patients do the dual-task training by combining it with virtual reality. Therefore, dual task training could be performed in a safe in the environment such as virtual reality or augment reality.

The effect of gait training with an elastic ankle-foot orthosis on balance and walking ability of persons with chronic stroke: a randomized controlled trial

  • Objective: The purpose of this study was to investigate the effect of treadmill exercise on the posture and walking speed of chronic stroke survivors with an ankle-foot orthosis. Design: Randomized controlled trial. Methods: Twenty-four chronic persons with chronic stroke admitted to Bobath Memorial Hospital in Seongnam city were divided into two groups by random blind method. Treadmill exercise with an elastic ankle-foot orthosis was performed in the experimental group and treadmill exercise was performed in the control group. The experiment was carried out for 6 weeks, and the experiment was carried out three times a week for 20 minutes per session. To measure the effect, static balance was measured using the MTD system before and after training, and the Berg Balance Scale (BBS) was used to measure functional balance. Results: There was a statistically significant difference between the 2 groups in the BBS measurement results for confirming the functional balance (p<0.05). Also, there was a significant difference between the 2 groups in single limb support time, step time and step length (p<0.05). Conclusions: In this study, it was found that treadmill exercise with an elastic ankle-foot orthosis in persons with chronic stroke was effective in maintaining functional balance, walking ability, step length, and step time. Therefore, it is necessary to use a flexible ankle-foot orthosis with proper treadmill exercise as a method of improving balance and walking speed of chronic stroke survivors.
