Cost-benefit Analysis of Mandatory Prescription in Korea (의약분업의 비용-편익 분석)

  • Objective : To evaluate the relative benefits and the costs associated with the introduction of the new pharmaceutical provision called 'Mandatory Prescription System' which separates the role of physicians from that of pharmacists with respect to the prescription and dispensation of from the perspective of consumers (i.e., patients). Methods : The costs of the system were measured by considering both direct and indirect costs. Direct costs included additional payments for ambulatory care and dispensing fees that occurred under the new system. indirect costs consisted of transportation expenses and costs related to time spent for physician consultation, waiting for the prescriptions to be filled, and extra traveling. Benefits identified in this study were the reduction of drug misuse and overuse, and the overall decrease in drug consumption among the Korean population. Sensitivity analysis was peformed for the inclusion of benefits for outpatients of hospitals, price elasticity, and increased fees for established patients. Results : The net benefit was estimated to be about minus 1,862 billion won and the benefit-cost ratio was 0.478. This indicates that the costs of 'Mandatory Prescription' outweigh its benefits, relative to the previous system. The sensitivity analysis results for all the variables considered in this study consistently showed the benefit-cost ratio to be less than 1. Conclusion : The results of this study suggest that implementing Mandatory Prescription System in Korea might be inefficient from the consumer's perspective. The results of this study do not coincide with the results of previous studies, presumably because of the differences in study design and in which items of costs and benefits were considered.

  • A flexible mechanism is proposed in this paper to improve the dynamic response performance of a traffic flow control system in an urban area. The roads, vehicles, and traffic control systems are all modeled as intelligent systems, wherein a wireless communication network is used as the medium of communication between the vehicles and the roads. The necessary sensor networks are installed in the roads and on the roadside upon which reinforcement learning is adopted as the core algorithm for this mechanism. A traffic policy can be planned online according to the updated situations on the roads, based on all the information from the vehicles and the roads. This improves the flexibility of traffic flow and offers a much more efficient use of the roads over a traditional traffic control system. The optimum intersection signals can be learned automatically online. An intersection control system is studied as an example of the mechanism using Q-learning based algorithm, and simulation results showed that the proposed mechanism can improve the traffic efficiency and the waiting time at the signal light by more than 30% in various conditions compare to the traditional signaling system.

  • As today's market share of Intelligent Network (IN) service and wireless tele-communication is growing rapidly, the increment in service requests of wireless IN service subscribers and users has to be taken into account for implementing advanced IN services. In this paper, we design Global Service Logic for IN Freephone service, which is one of the commercially most interesting IN services, applied call queuing service feature with mass call processing to enhance call completion rate by considering the mobility of wireless service subscribers with mobile terminals as well as existing wireline service subscribers. Due to the location management of each IN subscribers 'mobile terminals, we design the structure and operation mechanism of Queue Manager which adjusts dynamically service subscribers groups according to their mobility. We optimize the queue size according to the call attempt rate and drop rate, and present the appropriate waiting time in the queue.

  • VoD 전송 방식을 설계할 때 가장 중요한 요소들에는 정해진 대역폭에서 사용자 대기시간 (Viewer's Waiting Time)과 클라이언트에서 요구하는 최대 버퍼 요구량(Max Client Buffer Requirement)을 줄이는 문제를 꼽을 수 있다. 점점 더 복잡해지는 VoD 전송 기술의 개발로 인해 사용자 대기시간과 최대 버퍼 요구량의 관점에서 성능 향상과 반비례하는 시스템 복잡도를 줄이는 문제 또한 VoD 전송 방식을 설계 시 고려 해야 할 중요 사항 중의 하나이다. 이 논문에서는 시스템 복잡도를 낮추면서 사용자 대기시간과 최대 버퍼 요구량의 성능을 동시에 향상시키기 위한 방법으로 기존의 VoD 전송 방식들 가운데 가장 간단한 모델과 최대 버퍼 요구량의 성능을 가장 향상시킨 모델의 결합으로 구현 가능한 Hybrid 방식을 제안하고 있다. 이 논문은 결과적으로 사용자 대기시간은 기존 Staircase Broadcasting방식과 Fast Broadcasting 방식과 비교했을 때 동일한 성능을 나타내고 있고 최대 버퍼 요구량은 유사성능을 나타냄을 알 수 있다. 이 논문에서 제안한 방식은 사용자 대기시간이 짧아야 하고 저장공간이 많이 요구되지 않으며 사용자의 요청이 갑자기 증가될 수 있는 hot 비디오에 적용이 가능하다. 또한 다양한 환경에 따라 Staggered Broadcasting 방식과 Staircase Broadcasting 방식의 채널 수를 적절히 조정함으로써 데이터 송수신 효율을 극대화 시킬 수 있다. 이 논문의 구성은 제시한 방법에 적용된 방식들과 비교대상이 되는 방식들에 대해 살펴보고 제시한 방식의 성능 모델을 이용한 실험을 통해 각종 VoD 전송 방식들과 비교 분석하는 과정으로 기술하였다.

Analysis of Waiting Time and its Associated Factors at School Lunch Room Using Ultrasonic and Noise Sensors (초음파와 소음 감지 센서를 이용한 학교 급식실 대기 시간과 연관 요소 분석)

  • 본 연구는 아두이노 보드와 다중 센서들을 사용하여 학교 급식실 대기 공간에서의 대기 상황을 분석한다. 실험에 사용한 초음파 및 소음 감지 센서들로부터 초음파 및 소음 데이터를 입력 받아 송신하는 아두이노 보드 기반 프로그램을 작성하고, 데이터를 수집, 저장, 관리하기 위하여 CoolTerm 프로그램을 사용한다. 또한, C 언어를 기반으로 정규화 프로그램과 필터링 프로그램을 구현하여 대기 인원 감지라고 인정할 수 있는 조건(일정 소음 이상 발생, 초당 5회 이상 감지 및 3미터 미만 거리에서 감지)에 맞지 않는 데이터를 걸러낸다. 예비 실험 이후 실시한 본 실험 범위는 8월 27일(화)부터 30일(금)까지 4일간, 점심 식사 시간 중 중간 시간대인 12시 20분부터 12시 39분까지이다. 분석 결과 식단 선호도에 따라 대기 시간에 확연한 차이가 발생하는 것을 확인하였으며, 배식 시간 역시 대기 시간에 미치는 영향이 있는 것을 알 수 있었다. 또한 초음파 센서로부터 분석한 결과와 소음 감지 센서로부터 분석한 결과, 상당한 유사성이 관찰되었다. 본 연구는 대기 시간만의 측정에 그치는 것이 아니라, 식단과 대기 시간과의 관계 분석을 통해 학생 식사 행태가 대기 시간에도 영향을 미친다는 추가적인 사실을 증명하였는데, 이는 대기 시간 문제 해결이 단순히 급식 대기 상황 개선에만 있는 것이 아니라 식단 및 배식 방식 등의 개선과 같이 이루어져야 함을 보여준다. 이는 기존 연구들이 확인하지 못했던 사실로, 본 연구의 주요한 기여로 볼 수 있다. 향후 본 연구를 확대하여 무선 인터넷 및 알림 시설을 갖춘다면, 현재의 학교 급식 환경을 획기적으로 개선할 수 있을 것으로 기대한다.

Determining Level-of-Service Criteria of Headway Adherence (버스 운행 정시성의 서비스수준 기준산정)

  • In case of public transit such as bus system, the probability concept is used to evaluate the Level-of-Service of the operations. And each levels could be classified according to the linear probability value. (TCQSM: Transit Capacity and Quality of Service Manual-2nd Edition, TRB, Washington DC., 2003) In this case, the drivers or passengers wouldn't think that the service level isn't equivalent to the linear probability value. Thus the linear probability value doesn't exactly reflect the service level. This study shows the problems of using the linear probability value in classifying the service level through the case of evaluation of bus operation's punctuality, presented in TCQSM. To make up for the problems of such case, two methodologies are presented in this study. The method of determining Level-of-Service criteria using probability density of headway variation's distribution, presented in this paper, adequately reflects passenger's expected waiting time. According to the application result to real bus operation data, it tis better than the method of TCQSM to evaluate the reliability of bus operations. However further research about the relations between utility difference and passenger feeling of service level in necessary to apply the method that uses the utility function. It remains as the limitation of this paper.

An Implementation of $A^*$ Algorithm with Turn Heuristic for Enhancing the Straightness of a Path (경로의 직진성을 고려한 턴 휴리스틱 $A^*$ 알고리즘의 구현)

  • In driving a car, u-turn or left turn makes the speed of car decrease considerably or require more waiting time at the cross for the traffic signal to turn green. A more straight path, therefore, is probably fester to arrive at the destination than zig-zaged path with same distance. Previous works related to the path navigation do not consider the straightness of the path. In this paper, we have proposed the path navigation algorithm with turn heuristic for enhancing the straightness of a path. We have implement the proposed algorithm and compared it with a traditional $A^*$ algorithm. The experimental result shows that the degree of the straightness of a path is enhanced by 30% and the navigation distance of a path is deceased by 3.3%.

Simulation Analysis for Appointment Scheduling Patterns in a Private Plastic Surgery Clinic (시뮬레이션을 이용한 성형외과 예약패턴 분석)

  • Generally, solutions that can increase patients' satisfaction are in conflict with the solutions that can increase the satisfaction and profitability of the medical personnels. In this paper, we compare appointment patterns using multiple performance measures that take into account both hospital's position and customers' position, and propose the best appointment pattern especially for an efficient scheduling in a plastic surgery clinic. A simulation model using ARENA is designed based on real hospital data. Based on this model, we compare 5 appointment patterns. To check the robustness of the appointment patterns, we have experimented on various appointment situations by considering peak, extreme peak seasons, and peak, off-peak days. We found that Triangle-like pattern turned out to be most efficient regardless of appointment situations.

Study of the Performance of a Dry Cleaning Method for Polluted Ballast Gravel of Railroad Fields (철도부지 오염도상자갈의 건식 정화 기술 성능 연구)

  • Ballast gravel in a railroad field is often polluted by grease and heavy metals. In this paper, the performances of a dry cleaning method for polluted ballast gravel in which pollutants on the gravel surface can be physically removed was extensively studied. A polluted ballast cleaning device able to shoot emery blasting media onto the surface using compressed air was prepared. Polluted ballast gravel was put into this device for cleaning, with the treatment time varied from 1 to 10 min. The cleaning efficiency of the total petroleum hydrocarbons and heavy metals were studied. The total petroleum hydrocarbon removal efficiency was 70-80% for gravels sampled from a locomotive waiting line, while it was 40-60% for gravels sampled from a turnout area. The heavy metal removal efficiency exceeded 90% for copper and lead, while it was 65-80% for nickel and zinc. This system was found to be effective for the remediation of polluted ballast gravels.

A Study on the Mitigation of Taxi Supply and Demand Discrepancy by Adjusting Expected Revenues of Platform Taxi Calls (택시호출 간 기대수익 조정을 통한 택시 수급불일치 완화방안 연구)

  • As smartphones spread and ICT technologies develop, taxi services have changed from hovering to platform-based calls and reservations. This has improved the mobility and accessibility of taxi users but caused problems, such as digital observing (no-responses to calls) for either short-distance services or services during the peak-demand periods. Digital Observing means ignoring and not accepting calls when they occur, which require improvement. Therefore, this study aims to derive measures to mitigate discrepancies in taxi supply and demand by adjusting the expected revenue of each taxi service using reinforcement learning based on the Taxi operation data. The results confirmed that the average complete response rate to calls would increase from 50.29% to 54.24% when incentives are applied, and an improvement of 5.86% can be achieved in short-distance sections of less than 5,000 won incentives. It is expected that the improvement will increase profitability for drivers, reduce the waiting time for passengers, and improve satisfaction with taxi services overall.
