Oncologists Experience with Second Primary Cancer Screening: Current Practices and Barriers and Potential Solutions

  • Objectives: Screening for second primary cancer (SPC) is one of the key components of cancer survivorship care. The aim of the present study was to explore oncologists' experience with promoting second primary cancer screening. Methods: Two focus group interviews were conducted with 12 oncologists of diverse backgrounds. Recurrent issues were identified and placed into thematic categories. Results: Most of the oncologists did not consider SPC screening promotion as their responsibility and did not cover it in routine care. All of the study participants had experience with unexpected SPC cases, and they were under emotional tress. There was no systematic manner of providing SPC screening. Oncologists usually prescribe SPC screening in response to patients' requests, and there was no active promotion of SPC screening. Short consultation time, limited knowledge about cancer screening, no established guideline for SPC screening, and disagreement with patients about oncologists' roles were major barriers to its promotion. An institution-based shared care model was suggested as a potential solution for promoting SPC screening given current oncology practices in Korea. Conclusion: Oncologists could not effectively deal with the occurrence of SPC, and they were not actively promoting SPC screening. Lack of knowledge, limited health care resources, and no established guidelines were major barriers for promoting SPC screening to cancer survivors. More active involvement of oncologists and a systematic approach such as shared-care models would be necessary for promoting SPC screening considering increasing number of cancer survivors who are vulnerable.

충북지역 노인들의 약물복용 및 영양상태 II. 도시와 농촌 노인들의 영양실태 (Drug Consumption and Nutritional Status of the Elderly in Chung-buk Area II. Nutritional Status of Urban and Rural Elderly)

  • The nutritional status of 362 elderly men and women in Chung-bud area was evaluated in terms of their nutrient intakes, biochemical and anthrophometric measurements by interviews with questionnaires from August to October 1996. Mean intake of all nutrients except ascorbic acid did not meet the RDA for this sample. Protein, vit A, reboflavin, calcium were the most likely to be deficient on the basis of propotions of elderly consuming less than 75% of the RDA. The subjects nutrient intake was significantly affected by gender, marital status, number of family, family composition, educational level, pocket money, and region. Men in rural areas and women over 75 in urban areas were the most vulnerable groups with nutritional deficiency. According to serum biochemical indices, mean level of cholesterol, triglycerides, LDL, total protein, albumin and iron belonged to normal range but mean level of HDL showed below the normal range. More elderly men and women in urban areas showed a higher percentage of abormal level of cholesterol, triglycerides, LDL and HDL than those in rural areas. More elderly men and women in rural areas had abnormal levels of RBC, Hematocrit and hemoglobin compared to those in urban areas. Mean height and weight of elderly men was 161.4cm and 56.2Kg, respectively and 149.1cm and 50.5kg for women. The elderly in rural areas were taller than those in urban areas but had less weight, MAC, TSF, MAMC. Mean BMI of this sample belonged to normal range. However, the elderly in rural areas had a higher rate of underweight and lower for overweight than those in urban areas. The elderly in urban areas had higher blood pressure than those in rural areas.

농촌지역 취약계층의 주거이용에 관한 연구 (A Study on the Residential using of Socially Vulnerable in Rural Areas)

  • Due to difficulty of marriage with women in Korea, unmarried men in countryside tend to have an international marriage. A ratio of international marriage was 3.7% in 2004, but increased by over 10% in 2004 due to the change of population composition in countryside. Without social or national preparation on a multicultural family and an increase of elder who lives alone made problems such as changes of population composition in countryside, a high rate of divorce due to differences on culture and society. These problems are becoming critical problems in countryside, not only for a family. This Study has a purpose as below, Firstly, this study will examine living place of elder who live alone and multicultural families. Secondly, characteristics of using each space will be analyzed and then surveyed efficiency and satisfaction. The result of survey will be provided to space planning for improvement of living standard. The results of this study can be summarized as follows: The characteristics of the surveyed households, the elderly ratio of 70 to 80 units high, and when the elderly become less than 10 years, 51.6%, 77.4% of women were overwhelmingly high. The characteristics of multicultural families in the in the middle or early 2000s when immigration is concentrated and national origin in Vietnam was 66.7 %. Housing characteristics of households with elderly housing type, the ratio improved to 61.3% is Majority. Type strain and 29% in traditional houses, traditional housing type is 9.7%. And house of multicultural families improve housing type is 87.5% Majority. The main space of the housing for the elderly, most are usually satisfied. But generally low level of self-determined housing survey confers real. These results have lived a long time because of space adaptation and can be viewed as attachment. For the elderly passive use of space and simplify the Act and heating cooling at the expense of the use of housing space due to the reduction, but multicultural families is the space required for the growth of their children because there are more modifications required.

성년후견과 의료 -개정 민법 제947조의 2를 중심으로- (The Adult Guardianship and Medical Issue According to the Amendments of Civil Code)

  • The adult guardianship system has been introduced through amendments of Korean Civil Code for the first time in the March 2011(Act No. 10429, 7. 1. 2013. enforcement). The adult guardianship system has the main purposes to provide a lot of help vulnerable adults and elderly, and protect them on the welfare related with property act, treatment, care, etc. There could be a controversy about whether the protection Legal Guardian's consent(formerly known as the Mental Health Act) or permission of the Family Court(revised Civil Code) are required to, or the Mental Health Act should be revised, when mental patient will be hospitalized forcibly. The author proposes that mental patient with Adult guardians should be determined by Legal Guardian's consent and approval of the Family Court, but mental patient without Adult guardians could be determined by Legal Guardian's consent. The issue of Withdrawing of life-sustaining treatment could be occurred due to the aging society and the development of modern medicine, and this has provided difficult, various problems to mankind in Legal, ethical, and social welfare aspects. The need of Death with dignity law or Natural death law has been reduced for a revision of the Civil Code. Therefore, on the issue of Withdrawing of life-sustaining treatment, in the future, intervention of the court is necessary in accordance with the revised Civil Code Section, and Organ Transplantation Act and the brain death criteria may serve as an important criterion.

Prenatal nursing intervention studies published in Korean nursing journals: a scoping review

  • The purpose of this study was to describe prenatal nursing intervention studies on pregnant women and their families published in Korean nursing journals to identify research trends and to analyze the characteristics of intervention studies. This scoping review was conducted using Arksey and O'Malley's framework. We identified a research question and searched six domestic electronic databases for relevant articles. Forty-five references that met the inclusion and exclusion criteria were finally selected. We extracted the data using an analytic framework, and then collated and summarized the characteristics of the intervention studies. The most frequently used research designs were non-randomized controlled trials (91.1%), and only a few studies applied a specific theoretical framework (24.4%). The participants were mainly pregnant women only (64.4%) during the third trimester (35.6%) of pregnancy. Prenatal education was the most common type of intervention (48.9%), followed by complementary therapy (37.8%) and psychosocial support programs (13.3%). The most commonly used outcome variables were drawn from the psychological domain (44.5%), although distinct types of outcome variables-especially from the psychological and physical domains-were used to measure the effectiveness of different types of prenatal interventions. This review suggests that further prenatal nursing intervention studies in Korea should expand the study participants to include pregnant women's family members, high-risk and vulnerable groups, and women throughout entire pregnancy. Furthermore, it is necessary to develop integrative prenatal nursing interventions that promote family support and participation by facilitating partnerships among women, families, and nurses before, during, and after pregnancy.

A comparative study of the psychological impacts of tasks related and unrelated to COVID-19 on nurses: a cross-sectional study

  • Background: This study assessed the psychological impact of the outbreak of coronavirus disease 2019 (COVID-19) on university hospital nurses. It provides an assessment of mental health issues, including depression, anxiety, distress, and burnout of nurses dealing directly and indirectly with COVID-19. Methods: In a web-based, cross-sectional study, 111 nurses from Daegu Catholic University Hospital in Korea were enrolled from August 4 to August 9, 2020. Patient Health Questionnaire-9, Generalized Anxiety Disorder-7, Impact of Event Scale-Revised, and Maslach Burnout Inventory were used to assess the psychological symptoms of depression, anxiety, psychological distress, and burnout among the study participants. Results: Of 111 nurses, 35 (31.5%), nine (8.1%), 26 (23.4%), and 49 (44.1%) experienced depression, anxiety, distress, and burnout, respectively. Nurses who performed COVID-19-related tasks were more likely to have moderate depression (related vs. unrelated, 52.0% vs. 25.6%; p=0.037). There were no differences in anxiety, distress, and burnout between nurses with and without COVID-19-related tasks. More than 50% of the participants showed receptive and positive attitudes toward caring for COVID-19 patients. Conclusion: Nurses who performed COVID-19-related tasks had a higher risk of depression. There were no significant differences in anxiety, distress, and burnout between the two groups. Since nurses who perform COVID-19-related tasks are more prone to psychological distress, continued psychiatric interventions are required for infectious disease outbreaks with a high mortality rate for healthcare workers who are emotionally vulnerable.

일 농촌지역 여성결혼이주자 자녀의 신체성장과 발달 평가 (Evaluation of Physical Growth and Developmental Status of Infants and Children of Married Immigrant Women in Rural Areas)

  • Purpose: The purpose of this study was to evaluate the physical growth and developmental status of infants and children of married immigrant women. Methods: The participants were 92 infants and children aged from birth to 6 yr whose mothers were married immigrant women living in one rural area. Physical growth and developmental status were evaluated by using anthropometric assessment, and Korean Denver II developmental screening test. Results: Of the participants, 10.8% were below the 3rd percentile for weight, 13.0% for length, 5.4% for head circumference, and from 69.6% to 79.3% were distributed below the 50th percentile on growth charts. Further, 18.5% were classified as having questionable development. Factors related to low physical growth in infants and children were mother's weight and mother's level in Korean language classes. Conclusion: The physical growth and developmental status of children is vulnerable and serious. The study results suggest a need for regular growth and developmental evaluations. Political support is important for these mothers. In addition, there also is a need to develop early intervention strategies to promote growth and development of the infants and children of these married immigrant women.

경량 암호 알고리듬 CHAM에 대한 오류 주입 공격 (Fault Injection Attack on Lightweight Block Cipher CHAM)

  • 최근 가용 자원이 제한된 디바이스에서 사용할 수 있는 구현 성능이 효율적인 경량 블록 암호 알고리듬 CHAM이 제안되었다. CHAM은 키의 상태를 갱신하지 않는 스케줄링 기법을 사용함으로써 키 저장 공간을 획기적으로 감소시켰으며, ARX(Addition, Rotation, and XOR) 연산에 기반하여 설계함으로써 계산 성능을 크게 향상시켰다. 그럼에도 불구하고 본 논문에서는 CHAM은 오류 주입 공격에 의해 라운드 키가 노출될 가능성이 있으며 4개의 라운드 키로부터 마스터 비밀 키를 추출할 수 있음을 보이고자 한다. 제안된 오류 주입 기법을 사용하면 약 24개의 정상-오류 암호문 쌍을 이용하여 CHAM-128/128에 사용된 비밀 키를 찾을 수 있음을 컴퓨터 시뮬레이션을 통해 확인하였다.

Oxidative stress and the antioxidant enzyme system in the developing brain

  • Preterm infants are vulnerable to the oxidative stress due to the production of large amounts of free radicals, antioxidant system insufficiency, and immature oligodendroglial cells. Reactive oxygen species (ROS) play a pivotal role in the development of periventricular leukomalacia. The three most common ROS are superoxide ($O2^{\cdot-}$), hydroxyl radical ($OH^{\cdot}$), and hydrogen peroxide ($H_2O_2$). Under normal physiological conditions, a balance is maintained between the production of ROS and the capacity of the antioxidant enzyme system. However, if this balance breaks down, ROS can exert toxic effects. Superoxide dismutase, glutathione peroxidase, and catalase are considered the classical antioxidant enzymes. A recently discovered antioxidant enzyme family, peroxiredoxin (Prdx), is also an important scavenger of free radicals. Prdx1 expression is induced at birth, whereas Prdx2 is constitutively expressed, and Prdx6 expression is consistent with the classical antioxidant enzymes. Several antioxidant substances have been studied as potential therapeutic agents; however, further preclinical and clinical studies are required before allowing clinical application.

빈곤 가정 학령기 아동의 건강실태분석 (An Analysis Survey on Physical Development and Health Status of Elementary Children in Poverty in Incheon Area)

  • Purpose: This study was conducted to figure out the physical development and health status of elementary children in poverty. Method: 306 children in poverty registered in 3 regional children centers located in Incheon were selected. The results were compared with those of the physical status of students all over the country in 2008. Result: The children in poverty were relatively poor in the growth state such as height and weight, compared with those in non-disadvantage family children. In eyesight to be corrected, the children in poverty reached 26%. As a result of a blood laboratory test, the children with high T-cholesterol reached 5.84%; high SGOT/GPT 6.3%. In anemia, 41.4% of boys in poverty were suffering from it, compared to 7.5% of girls. In Internet addiction, children in poverty showed lower value than those in non-disadvantage children, but there are correlated between CDI and internet addiction test. Conclusions: These results implies the children in poverty have more physical and emotional problems, so they are in danger of related to health. These matters mean that the management by professional health care should be provided, and the plan and policy for promoting health for the children in vulnerable group should be established and provided.
