Effects of whey protein supplementation prior to, and following, resistance exercise on body composition and training responses: A randomized double-blind placebo-controlled study

  • [Purpose] The composition of protein supplements, the consumption timing immediately before and after resistance exercise training (RET), and the quantity of protein supplementation may be important factors for the im-provement of muscle mass and function. Although these factors should be considered comprehensively for effective improvement of muscular function in protein supplementation, relatively few studies have focused on this area. Therefore, this study was designed to investigate whether a protein blend supplement before and after resistance exercise for 12 weeks would be effective in increasing muscular function. [Methods] In total, 18 participants were randomly assigned to a placebo (PLA) or protein blend supplement (PRO) group. All subjects followed the same training routine 3 times per week for 12 weeks, taking placebo or protein supplements immediately before and after each exercise session. The protein supplement consisted of 40 g of blend protein, including hydrolyzed whey protein. The RET consisted of lower body (barbell squat, dead lift, seated leg extension, and lying leg curl) and upper body (bench press, barbell rowing, preacher bench biceps curl, and dumbbell shoulder press) exercises. A repetition was defined as three sets of 10-12 times with 80% of one repetition maximum (1RM). [Results] Although the PRO group had a lower protein intake in terms of total food intake than the PLA group, the mean changes in muscle circumference, strength, and exercise volume increased, especially at week 12, compared to the PLA group. [Conclusion] These results suggest that the composition and timing of protein intake are more important than the total amount.

스마트폰의 가속도 센서를 이용한 정적균형능력 측정의 신뢰도 연구 (Reliability of static balance abilities measure using a smartphone's acceleration sensor)

  • 본 연구는 스마트폰의 가속도 센서를 이용하여 정적균형능력을 측정하는데 있어 신뢰도를 알아보고자 실시하였다. 본 연구의 연구대상자는 최근 3개월 이내에 골절, 수술, 염증성 관절염이 없고 정기적인 운동을 시작하지 않은 Y대학교의 학생 30명을 대상으로 하였다. 본 연구에 사용된 스마트폰은 갤럭시S5LTE (SM-G900F, Samsung, Korea, 2014)였고 어플리케이션은 Sensor Kinetics pro (Ver.2.1.2, INNOVENTIONS Inc, US, 2015)를 사용하였다. 정적균형능력은 1회당 3번씩 측정하였으며 하루 간격으로 검사-재검사로 시행되었다. 1차 측정과 2차 측정은 동일하게 진행되었다. 본 연구의 분석은 Wilcoxon 부호순위검정, 급내상관계수(ICC(2,1))를 사용하였다. 본 연구의 연구결과는 다음과 같았다. 눈을 뜬 상태에서 1차 측정과 2차 측정 간에 유의한 차이가 없었고(p>0.05) 강한 양의 상관관계를 보였으며(r>0.75, p<0.05) 매우 높은 일치도(ICC>0.80)를 보였다. 눈을 감은 상태에서 1차 측정과 2차 측정 간에 유의한 차이가 없었고(p>0.05) 강한 양의 상관관계를 보였으며(r>0.75, p<0.05) 매우 높은 일치도(ICC>0.80)를 보였다. 본 연구를 통해 스마트폰은 정적균형능력 측정에 있어 가능성이 있다는 사실을 알 수 있었으며 본 연구가 향후 균형능력측정 전용 어플리케이션 개발에 기초자료가 되기를 기대해본다.

뇌정위 방사선수술을 위한 컴퓨터 치료계획시스템의 개발 (Development of a Stereotactic Radiosurgery Planning System)

  • 선형가속기를 이용한 뇌정위 방사선수술을 시행하기 위하여 PC에서 실행 가능한 치료 계획시스템을 개발하였다. 128 MB RAM 용량의 Pentium Pro 200 MHz의 PC에서 Windows 95를 기반으로 하였으며, Research Systems사의 프로그램 제작 툴인 IDL을 이용하여 프로그래밍 하였다. Brown-Robert-Wells(BRW) 정위틀을 두개골에 고정한 후 전산화단층촬영을 시행하여 얻은 영상데이터를 광자기디스크를 이용하여 PC에서 입력받을 수 있도록 인터페이스를 구축하였다. 영상데이터를 입력시킨 후 각 단면영상에서의 정위틀의 위치를 자동으로 인식하고 정위좌표계를 설정하여 단변영상의 좌표, 기울어짐, 축소율 등을 정확히 계산하여 보정할 수 있다. 좌표계의 설정이 이루어지면 각 단면에서 표적 및 방사선 민감 조직들의 외곽선을 입력, 수정한다. 외곽선의 입력이 종료되면 표적의 중심이 결정되고 방사선 수술을 위한 빔의 설계한다. 빔 설계의 효율성을 위하여 Beam's eye view(BEV) 및 Room's eye view(REV)를 동시에 확인할 수 있도록 개발하였다. 빔의 설계가 끝나면 3 차원적인 선량계산을 시행한다 .5개의 arc로 설계하였을 때 선량계산에 소요되는 시간은 약 1-2 분이었다. 선량분포는 각 단면영상에서 확인할 수 있으며 표적 및 주변 방사선 민감 조직들의 Dose-Volume Histogram(DVH)을 평가하여 선량분포 및 치료계획의 합리성을 정량적으로 분석할 수 있도록 구현하였다. 기본적인 기능을 지원하는 PC 기반의 뇌정위 방사선수술을 위한 치료계획시스템으로 앞으로 임상적용이 가능할 것으로 생각된다.

  • 목 적 : 폐암의 정위 절제 방사선 치료(Stereotactic Ablative Radiation Therapy, SABR)시 복부압박을 이용한 치료계획과 양압지속유지기(The Continuous Positive Air Pressure, CPAP)를 이용한 치료계획을 비교, 분석하여 방사선 치료 효과 향상에 기여하고자 한다. 대상 및 방법 : 본원의 폐의 SABR 환자 중 2명을 대상으로 복부 압박 고정 장치(the Body Pro-Lok, BPL)와 CPAP를 이용한 치료 계획을 수립하여, RTOG 0813에서 제안한 parameter들과 균질도(Homogeneity Index, HI), 일치도(Conformity Index, CI)를 통해 치료계획을 분석하였다. 또한 모든 4D CT에서 각 Phase별 계획용 표적 체적(Planning Target Volume, PTV) 중심의 X, Y, Z축 움직임을 분석하고, PTV와 손상위험장기(Organ At Risk, OAR)의 체적과 평균 선량을 구하여 비교하였다. 그리고 4개의 원뿔형빔 전산화단층촬영(Cone Beam Computed Tomography, CBCT)을 이용하여, PTV 중심점과 0°, 90°, 180°, 270° 중 세 방향에서의 흉강내 접점까지의 직선거리를 측정하고, 각 방향에서 평균거리 값과의 차이를 비교하였다. 결 과 : BPL과 CPAP를 사용하여 얻은 치료계획은 모두 RTOG의 권고값을 따랐으며, 균질도와 일치도에서도 큰 차이가 없었다. PTV 중심의 X축, Y축, Z축 움직임은 A 환자의 경우 BPL 사용시 0.49 cm, 0.37 cm, 1.66 cm, CPAP 사용시 0.16 cm, 0.12 cm, 0.19 cm을 보였고, B 환자의 경우 BPL 사용시 0.22 cm, 0.18 cm, 1.03 cm, CPAP 사용시 0.14 cm, 0.11 cm, 0.4 cm을 보였다. CPAP 사용시 BPL 사용시보다 A 환자의 경우 ITV가 46.27% 감소하였고 좌측 폐의 체적이 41.94% 증가하였으며, 평균선량은 심장에서 52.81% 감소하였다. B 환자의 경우 좌측 폐 106.89%, 우측 폐 87.32% 체적이 증가하였고, 평균선량은 위에서 44.30% 감소하였다. 각 방향의 직선거리 값과 평균거리 값과의 최대 차이는 A 환자의 경우 a방향에서 0.05 cm, b방향에서 0.05 cm, c방향에서 0.41 cm 났고, B 환자의 경우 d방향에서 0.19 cm, e방향에서 0.49 cm, f방향에서 0.06 cm 차이가 났다. 결 론 : CPAP 사용시 폐용적의 증가를 통해 표적 근처 OAR의 선량을 BPL 사용시보다 더 효과적으로 감소시킬 수 있으며, 호흡에 따른 종양의 움직임 제한에도 더 효과적으로 기여할 수 있다는 것을 확인하였다. 추후 CPAP의 다양한 부위 적용과 다른 치료기와의 결합을 통해 방사선 치료 효과를 개선시킬 수 있을 것으로 사료된다.

Radiographic evaluation of the symphysis menti as a donor site for an autologous bone graft in pre-implant surgery

  • Purpose: This study was performed to obtain a quantitative evaluation of the cortical and cancellous bone graft harvestable from the mental and canine regions, and to evaluate the cortical vestibular thickness. Materials and Methods: This study collected cone-beam computed tomographic (CBCT) images of 100 Italian patients. The limits of the mental region were established: 5 mm in front of the medial margin of each mental foramen, 5 mm under the apex of each tooth present, and above the inferior mandibular cortex. Cortical and cancellous bone volumes were evaluated using SimPlant software (SimPlant 3-D Pro, Materialize, Leuven, Belgium) tools. In addition, the cortical vestibular thickness (minimal and maximal values) was evaluated in 3 cross-sections corresponding to the right canine tooth (3R), the median section (M), and the left canine tooth (3L). Results: The cortical volume was $0.71{\pm}0.23mL$ (0.27-1.96 mL) and the cancellous volume was $2.16{\pm}0.76mL$ (0.86-6.28 mL). The minimal cortical vestibular thickness was $1.54{\pm}0.41mm$ (0.61-3.25 mm), and the maximal cortical vestibular thickness was $3.14{\pm}0.75mm$ (1.01-5.83 mm). Conclusion: The use of the imaging software allowed a patient-specific assessment of mental and canine region bone availability. The proposed evaluation method might help the surgeon in the selection of the donor site by the comparison between bone availability in the donor site and the reconstructive exigency of the recipient site.

항복응력과 미끄럼현상을 고려한 분말사출성형 충전공정의 유한요소해석 (Finite Element Analysis of Powder Injection Molding Filling Process Including Yield Stress and Slip Phenomena)

  • 본 연구에서는 분말혼합체의 특성인 항복응력을 포함한 Generalized Newtonian Fluid의 구성 방정식을 도입하고 미끄럼현상을 고려한 신소재의 사출성형 충전과정 해석용 CAE(computer aided engineering)시스템을 개발하였다. 수치모사를 위한 수치해석방법으로는, 유한요소법(finite element method)과 유한차분법(finite difference method)을 함께 사용하였다. 유한요소법과 검사체적법(control volume technique) 을 병용하여 유동의 진행을 수치모사 하였으며, 유한차분법을 사용하여 온도분포를 계산하였다.

Combination of a new ultrasonic tip with rotary systems for the preparation of flattened root canals

  • Objectives: This study evaluated 2 nickel-titanium rotary systems and a complementary protocol with an ultrasonic tip and a small-diameter instrument in flattened root canals. Materials and Methods: Thirty-two human maxillary second premolars with flattened canals (buccolingual diameter ≥4 times larger than the mesiodistal diameter) at 9 mm from the radiographic apex were selected. The root canals were prepared by ProDesign Logic (PDL) 30/0.01 and 30/0.05 or Hyflex EDM (HEDM) 10/0.05 and 25/0.08 (n = 16), followed by application of the Flatsonic ultrasonic tip in the cervical and middle thirds and a PDL 25/0.03 file in the apical third (FPDL). The teeth were scanned using micro-computed tomography before and after the procedures. The percentage of volume increase, debris, and uninstrumented surface area were analyzed using the Kruskal-Wallis, Dunn, Wilcoxon, analysis of variance/Tukey, and paired and unpaired t-tests (α = 0.05). Results: No significant difference was found in the volume increase and uninstrumented surface area between PDL and HEDM (p > 0.05). PDL had a higher percentage of debris than HEDM in the middle and apical thirds (p < 0.05). The FPDL protocol resulted in less debris and uninstrumented surface area for PDL and HEDM (p < 0.05). This protocol, with HEDM, reduced debris in the middle and apical thirds and uninstrumented surface area in the apical third (p < 0.05). Conclusions: High percentages of debris and uninstrumented surface area were observed after preparation of flattened root canals. The HEDM, Flatsonic tip, and 25/0.03 instrument protocol enhanced cleaning in flattened root canals.

Micro-CT evaluation of the removal of root fillings using rotary and reciprocating systems supplemented by XP-Endo Finisher, the Self-Adjusting File, or Er,Cr:YSGG laser

  • Objectives: This study aimed to compare the effectiveness of a single-file reciprocating system (WaveOne Gold, WOG) and a multi-file rotary system (ProTaper Universal Retreatment, PTUR) in removing canal filling from severely curved canals and to evaluate the possible adjunctive effects of XP-Endo Finisher (XPF), the Self-Adjusting File (SAF), and an erbium, chromium: yttrium, scandium, gallium garnet (Er,Cr:YSGG) laser using microcomputed tomography (µCT). Materials and Methods: Sixty-six curved mandibular molars were divided into 2 groups based on the retreatment technique and then into 3 based on the supplementary method. The residual filling volumes and root canals were evaluated with µCT before and after retreatment, and after the supplementary steps. The data were statistically analyzed with the t-test, Mann-Whitney U test, analysis of covariance, and factorial analysis of variance (p < 0.05). Results: PTUR and WOG showed no significant difference in removing filling materials (p > 0.05). The supplementary techniques were significantly more effective than reciprocating or rotary systems only (p < 0.01). The supplementary steps showed no significant differences in canal filling removal effectiveness (p > 0.05), but XPF showed less dentin reduction than the SAF and Er,Cr:YSGG laser (p < 0.01). Conclusions: The supplementary methods significantly decreased the volume of residual filling materials. XPF caused minimal changes in root canal volume and might be preferred for retreatment in curved root canals. Supplementary approaches after retreatment procedures may improve root canal cleanliness.

Generation of a skeletal mechanism of coal combustion based on the chemical pathway analysis

  • A skeletal mechanism of coal combustion was derived from a detailed coal combustion kinetic mechanism through an importance analysis of chemical pathways. The reduction process consists of roughly two parts. The first process is performed based on a connectivity analysis between species. In this process, DRGEPSA is chosen for reduction process. Strongly connected species and related reactions from the important species set as start species by the operator are sorted into the reduced mechanism. About 70% of species and reactions can be removed with a limited accuracy loss. Subsequently the second reduction process, CSP, is performed. This method focuses on an importance of each reaction and can reduce a volume of mechanism appropriately. Through these analyses, a skeletal mechanism is generated that is including 65 species and 150 reactions. The generated skeletal mechanism is verified through a comparison with the detailed mechanism in the homogeneous reactor model of CHEMKIN-PRO under wide range of conditions. The generated mechanism can give an advantage in the analysis of coal combustion characteristics in detail in large scale simulations such as LES and DNS.

NUMERICAL APPROACH TO MICROSTRUCTURAL CHARACTERIZATIONS FOR DENSE AND POROUS THERMAL BARRIER COATINGS

  • During spray coating, especially in an air plasma spray (APS), pores, cracks, and splat boundaries are developed and those factors exert influence on thermomechanical properties such as elastic modulus, thermal conductivity, and coefficient of thermal expansion. Moreover, the thermo mechanical properties are crucial elements to determine the thermoelastic characteristics, for instance, temperature distribution, displacements, and stresses. Two types of thermal barrier coating (TBC) model, the dense and porous microstructures, are taken into account for the analysis of microstructural characterizations. $TriplexPro^{TM}$-200 system was applied to prepare TBC samples, and the METECO 204 C-NS powder is adopted for the relatively porous microstructure and METECO 204 NS powder for the dense microstructure in the top coat of TBCs. Governing partial differential equations were derived based on the thermoelastic theory and approximate estimates for the thermoelastic characteristics were obtained using a finite volume method for the governing equations.
