Background and Objectives : Bowing of the vocal fold, a bowed edge of the vocal fold results from various disorder causes glottic incompetence resulting in voice disorders. The results of treatment were not satisfactory even though a variety of methods of treatment were applied. The goal of this study is to evaluate of efficacy of the non-surgical, voice therapy in patients with vocal fold bowing. Materials and Method : Twenty two patients of vocal fold bowing not resulting from vocal fold palsy and sulcus vocalis were analyzed. After voice therapy and medical treatment, parameters of perceptual, acoustic, aerodynamic analysis and stroboscopic findings were compared those of pretreatment results. Results and Conclusion : Jitter, shimmer and maximal phonation time were significantly improved and subjective satisfaction was improved in 73% of patients. In the videostroboscopic examination, vocal fold gap was decreased in 27% of patents. The result of this study indicates that voice therapy alone has effect in treatment of vocal fold bowing. Voice therapy is one of the mainstay of treatment of vocal fold bowing.

Mucosal bridges of vocal fold have been described as a parallel band to vocal folds with the presence of sulcus vocalis. However, the type of mucosal bridge crossing each vocal fold has not been well documented in the literature. Herein, we present two cases of mucosal bridge found in the vocal fold connecting the mid-portions of true vocal folds. Two patients who had no history of laryngeal trauma, surgery or oro-tracheal intubation visited our clinic due to voice change. Laryngoscopic examination revealed that they had a mid-portion mucosal band without any other mucosal lesions. Two patients underwent laryngomicrosurgery with pulsed dye laser. After the surgery, they showed significant improvement of voice quality.
False vocal fold hypertrophy caused by diverse pathologic lesion, such as laryngeal amyloidosis, laryngeal lipidosis, laryngocele, saccular cyst and sulcus vocalis. False vocal fold hypertrophy, however, is also caused laryngeal structure deformity, irrespective of pathologic lesions. In this article, we report some cases of false vocal fold hypertrophy caused by inward bowing of thyroid cartilage. At the clinic of the department of otorhinolaryngology in Gangnam Severance Hospital, with 3 male complained of hoarseness as subjects, and comfirmed of false vocal fold hypertrophy using the stroboscopy and larynx CT we checked vocal fold and laryngeal structure. Three patients with apparent hypertrophy of false vocal fold were investigated with computerized tomography (CT). In all patients, marked concavity of thyroid cartilage was revealed in CT scan at the level of the false vocal fold, and this deformity of the thyroid cartilage seemed to cause a protrusion of false vocal fold which taken as hypertrophy in stroboscopy. Careful palpation of the larynx and a CT scan taken at the level of the false vocal fold should be useful in determining whether hypertrophy of the false vocal fold is pathologic. For the next articles, It is necessary to discuss for the cause, diagnosis, treatment and prevention of inward bowing of thyroid cartilage.
Introduction: 585 nm Pulsed dye laser (PDL) laryngeal surgery is based on the photodynamic characteristics of selective photothermolysis and photoangiolysis and recently considered to be the treatment for a variety of benign laryngeal disease. Objective: To review the indications and outcome of office-based 585nm PDL surgery and summarize new developments. Method: Retrospective study involving 402 patients was performed, The PDL surgery could be applied to various laryngeal diseases such as laryngeal papilloma, vocal fold dysplasia, laryngeal granuloma, vocal polyp, capillarectasia, scarred vocal fold and sulcus vocalis. Results : The physiologic properties of the vascular specificity of PDL provide many advantages and appear to be effective for laryngeal treatment. The PDL resulted in precise, selective coagulation of the microvasculature without damage to the surrounding tissue. Therefore PDL surgery is safe and effective for office-based treatment of benign laryngeal disease and for all patients regardless of their overall medical condition. Conclusion: PDL surgery provides potential benefits and advantage for treating common benign laryngeal disease.
The voice of aged persons is known generally to be somewhat different from that of other adults, suggesting that laryngeal change occurs with advancing age. However, because knowledge of the voice characteristics of aged persons is limited, it is difficult to judge whether their voices arc normal. Chart review and laryngoscopic examination from ninety-one patients with hoarseness over the age of 60(1st group) and one hundred sixteen patients with hoarseness below the age of 50(2nd group) were done to define aging related voice disorders. The following results were obtained. 1) Associated diseases related to laryngeal disease were hypertension(12%), pulmonary disease(4.4%), thyroid disease(1.1%) in 1st group and hypertension(9.5%), thyroid disease(1.7%) in 2nd group. 2) The underlying diseases causing hoarseness in order of frequency were benign vocal fold lesion(37.7%), inflammatory disease(36.8%), functional dysphonia(17%) in 1st group and benign vocal fold lesion(43.6%), functional dysphonia(26.3%), inflammatory disease(16.5%) in 2nd group. 3) In stroboscopic findings, atrophy and sulcus of vocal cords are more prevalent in males than in females and edema of vocal cords is more common in females. Generally the voice characteristics of aged persons depend on the mass of the vocal folds which may be decreased through atrophy or be increased by edema. However, other factors such as systemic diseases, drug side effects and compensatory mechanism to presbylaryngis must be taken into account in diagnosing and treating voice disorders in aged persons.
Background and Objectives: Benign vocal fold lesions have shown various voice onset types on phonation, however, they have not been documented yet. We studied to know the relationships between benign vocal fold lesions and voice onset types. Materials and Method: 114 subjects were evaluated by using videokymographic examinations. The subjects were classified into three types: normal, contact, and open types according to the patterns of voice onset types on phonation. Benign vocal fold lesions were investigated and voice onset types were compared between normal and disease groups. Voice parameters were obtained from and compared in all subjects to assess acoustic and aerodynamic factors. Results: The normal type among onset types were more than contact type or open type in both normal and disease groups. Disease group showed many contact and open types when. compared with normal group. Vocal nodule and vocal polyp were showed many normal and contact types, however, sulcus vocalis was almost showed open type among voice onset groups. The values of mean flow rate (MFR) of contact type were significantly higher compared to normal type in disease group (p<0.05). Shimmer of contact type was higher than normal type in diseasegroup, but the difference was not significant (p=0.057). Conclusion: Benign vocal fold lesions were related to the various types of voice onset. The various types of voice onset should be considered when benign vocal fold lesions were examined.
Background and Objectives : Despite of numerous treatment modalities for glottic insufficiency, it still remains unresolved problem. Recently, autologous collagen injection was introduced as a new treatment for glottic insufficiency. This study was attempted to evaluate the efficacy of this treatment modality. Materials and Methods : Ten patients diagnosed as symptomatic sulcus vocalis with glottic insufficiency and agreed with this procedure were selected. Injectable collagen was obtained from the patient's dermis. It was injected into the pathologic area of the vocal fold. After injection, 4 of 10 patients were 1311owe4 up more than 5 months, and their voice were evaluated prior to injection and at 5 months after injection, using subjective, perceptual, aerodynamic, acoustic and videostroboscopic assessments. Results : In the aerodynamic assessment, 2 of 4(50%) patients was improved in MFR(mean flow rate), but no change was noted in MPT(maximum phonation time). In the acoustic assessment, no definite improvement was noted. In the subjective and perceptual assessments, 2 of 4(50%) patients was improved. In the videostroboscopic assessment, chink was reduced but still remained. Serious complication was not noted. Conclusion : Autologous collagen injection was reported as ideal treatment for glottic insufficiency, but our results were relatively unsatisfactory. But considering that our study is preliminary, it is too hasty to determine the efficacy of autologous collagen injection. We think that further study is required.
Background and Objectives Therapies have been reported to treat the glottal gap previously. However, these voice therapies showed the limits because many techniques focused only on one among breathing, resonance and phonation. In addition patients often have difficulties visiting hospital frequently. 'Gliding and humming' is vocal training technique that readjusts total vocal patterns such as breathing, resonance and phonation. This technique can be easily applied during short term sessions. The purpose of this study is to evaluate the efficiency of voice therapy with 'gliding and humming' for patients with glottic gap during short-term treatment sessions. Materials and Method Twenty-three patients with glottal gap were selected. Of all patients, 14 patients had sulcus vocalis and 12 patients had muscle tension dysphonia (MTD). Voice therapies were performed 1.9 sessions in average. GRBAS, jitter, shimmer, noise to harmonic ratio, semitone range, closed quotient_vowel and maximum phonation time were compared before and after the therapies. In addition, changes of glottal gap and MTD severity were evaluated. Results Statistically significant improvement was observed. MTD improvement was observed only among the patients with glottal gap improvement. Also sulcus vocalis group showed the statistically significant improvement. Conclusion 'Gliding and humming' was effective to the patients with glottic gap and sulcus vocalis. Also, among patients who have both glottic gap and MTD, the data suggests that voice therapy for glottic gap also makes improvement in MTD.
성대구증은 불완전 성대폐쇄부전으로 평균호기류율(mean airflow rate, MFR)이 높은 것이 특징이다. MFR은 phonatory aerodynamic system(PAS)의 두 공기역학적 검사 방법(MXPH, VOEF)으로 측정하며, 두 검사 방법에 따라 결과가 다르게 나타날 수 있다. 본 연구는 성대구증 환자 30명을 대상으로 PAS의 MXPH, VOEF 프로토콜에 따른 MFR의 치료 전과 후 차이, 치료 전후 변화량 간의 차이가 있는지 비교하였다. 그리고 치료 전과 후의 Voice handicap index(VHI), GRBS 및 기본주파수(F0)를 비교하였다. 연구결과, 성대구증 환자의 치료 전과 후 모두에서 검사 방법 간에 유의한 차이가 나타났으나 치료 전후의 변화량은 유의한 차이가 없었다. VHI와 GRBS는 치료 전에 비해 치료 후에 유의하게 감소하였으나 F0는 치료 전과 후에 유의한 차이가 없었다. 본 연구를 통해 성대구증 환자의 치료에 따른 MFR 변화를 평가할 때, 두 공기역학적 검사 방법(MXPH, VOEF) 중 어떠한 검사 방법을 사용해도 된다는 것을 알 수 있다.
최근의 후두미세수술 (Laryngo-microsurgery)의 보편화와 이에 따른 음성외과학(phonosurgery)의 발달로 종전까지 성대결절이나 성대폴립등의 양성종양 절제나 반회신경 마비등예에서 진단된 성문간격 (glottic chink)을 $Teflon^{{\circledR}}$ injection등으로 치료해오던 후두경술(Laryngoscopy)하에서의 수술적 조작이 그 한계와 범위를 넘어서서 이제는 우리 나라에 있어서도 후두의 기능외과적인 측면에서 음성을 개선하려는 시도가 고조되고 있으며 특히 성대마비나 변성기발성장애 (Mutational dysphonia), 성대위축 (Vocal cord atrophy), 운동기능 항진성발성장애 (Hyperkinetic dysphonia), 성대구증(Sulcus vocalis)등을 위한 음성개선수술이 실시되고 있음은 우리나라의 음성외과학의 발전이라는 견지에서도 매우 기꺼운 일이다. 이러한 뜻에서 내시경술을 통해서 성대에 직접 수술적 조작을 가하지않고서도 성문외적으로 후두구조를 수술하여 성대의 위치와 물리적 성질을 변화시킴으로서 음성을 개선하려는 갑상연골성형술 (thyroplasty)이 성행되고 있음을 강조하고싶다. Isshiki(1974)는 이러한 thyroplasty 를 4종류로 분류하였으며 편측성 성대마비의 경우 I 형 thyroplasty (lateral compression of vocal cord) 와 IV형 thyroplasty (Lengthening of vocal cord) 가 효과적인 수술방법인데 이들 수술의 장점은 국소마취하에서 환자의 목소리를 들어가면서 lateral compression의 정도를 조정해서 시술할 수 있다는것과 갑상연골내측 연골막 밖에서 의 조작으로서 후두내 출혈이나 호흡곤란등의 위험성이 전혀 없다는 것이다. 본 교실에서는 1981 년 9월부터 1982년 3월까지 7개월간에 경험한 편측성 성대마비 7예에 대하여 국소마취하에서 thyroplasty를 시행하여 약간의 지견을 얻었기에 보고하는 바이다. 수술에 앞서서 모든 환자에게 미리 공기역학검사, 청각심리적검사, 스트로보스콥검사(stroboscopy) 및 음향분석(Sound spectrographic analysis) 을 실시하였으며 thyroplasty 시행 2 개월후에 상기한 검사를 다시 시행해서 수술전후의 음성을 비교관찰하여 다음과 같은 성적을 얻었다. 1) 공기역학검사상 최장발성지속시간 (Maximum phonation time)은 58 % 증가되었으며 이에따른 발성시호기유율 (Phonation quotient)과 평균호기유율(Mean flow rate)은 각각 58 %, 54 %로 감소되었다. 2) 청각심리적검사에서 애성의 정도가 호전되었으며 스트로보스콥검사에서도 발성시 성문간격의 개선을 보았다. 3) 음향분석도상에서 성대 진동의 주기성 (Periodicity)이 회복되었으며 특히 고주파역에서의 잡음분포가 감소되었다.
