Background and Objectives : The main purpose of this study is to clarify perceptual and acoustic analysis for the patients with hypothyroidism after thyroidectomy especially focused on the characteristics of speech articulation with special reference to the consonant production. Materials and Methods : The subjects of the research were 40 male and female adults (males : 5, females : 35). They were all received radioactive iodine treatment which after total thyroidectomy. Voice samples were collected during the three stages of after surgery, pre-radioisotope treatment (RIT), and post-RIT. The acoustic analysis was conducted by using Pratt (ver.5.2.21) after measuring voice onset time (VOT). The subjective evaluation of the voices used CAPE-V. Results : A significant decrease in overall severity was displayed in the CAPE-V following RIT. It may be conjectured that this is connected to the change in voice following RIT. The loudness of the sound displayed a significant decrease in the CAPE-V following RIT. It is conjectured that this is connected to the decrease in vocal intensity following RIT. No statistically significant results were revealed for the comparative analysis on the voice onset time (VOT) in all plosives during the three periods. Conclusion : Perceptually, the overall severity of the voice with hypothyroidism was changed significantly before and after RIT. Eventhough VOT were not significantly changed, it tended to decrease VOT in patients with hypothyroidism.
This paper is generally divided in 2 parts. One is the study on vowels about korean singer's lyric song in view of Daniel Jones' Cardinal Vowel. The other is acoustic study on vowels in my singing about korean lyric song. Analysis data are KBS concert video tape and CSL's. NSP file on my singing and Informants are famous singers i.e. 3 sopranos, 1 mezzo, 2 tenors, 1baritone, and me. Analysis aim is to find out Korean 8 vowels([equation omitted]) quality in singing. The methods of descrition are used in closed vowels, half closed vowels, half open vowels, open vowels and rounded vowels, unroundes vowels and formants. The study of the former is while watching the monitor screen to stop the scene that is to be analysixed. The study of the latter is to analysis the spectrogram converted by CSL's. SP file. Analysis results are an follows: Visual and auditory korean vowels quality in singing have the 3 tendency. One is the tendency of more rounded than is usual Korean vowels. Another is the tendency of centralized to center point in Cardinal Vowel and the other is the tendency of diversity in vowel quality. Acoustic analysis is studied by means of 4 formants. Fl and F2 show similiar step in spoken. In Fl there is the same formant values. This seems to vocal organization be perceived the singign situation. The width of F3 is the widest of all, so F3 may be the characteristics in singing. In conclude, the characteristics of vowels in Korean lyric songs are seems to have the tendencies of rounding, centralizing to center point in Cardinal Vowel, diversity in vowel quality and, F3'widest width in compared with usual Korean vowels.
Background; Post-intubation injury is known to be the most common cause of tracheal stenosis. Treatment strategy for tracheal stenosis varies accoring to the extent of pathologic lesion. Focal mucosal lesion can be treated with laser photoablation, but full thickness tracheal lesion should be treated with resection and anastomosis. Material and Method; From Aptil 1998 to May 1999, twelve patients suffering from tracheal stenosis as a complication of endotracheal intubation were managed by resection and end-to-end anastomosis in the Department of Thoracic and Cardiovascular Surgery, Gyeongsang National University Hospital. Result; There was no operative mortality. Five temporary vocal cord paralysis and one wound infection occurred as early complications. During 18 months of follow-up, re-stenosis was not found. Conclusion; Tracheal resection and anastomosis can be considered as an excellent surgical treatment for tracheal stenosis which developed as a complication of endotracheal intubation.

The present study aimed to investigate the changes of acoustic parameters of the aspirated voice in stroke patients. The eighty-eight subjects diagnosed with cerebro-vascular accident were divided into 32 penetration/aspiration (P/A) and 56 Non-P/A groups according to the videofluroscopic swallowing study (VFSS) results, and 26 control subjects participated. All subjects preformed VFSS and vowel /a/ was recorded three times pre- and post VFSS. Since the variation in the acoustic parameters within a single phonation has been observed, we proposed a delta formula for the acoustic parameters which can reflect the temporal changes of the each parameter in an utterance. We measured from the voice data eight acoustic parameters: fundamental frequency (F0), standard deviation of F0 (F0_SD), Jitter, relative average perturbation (RAP), Shimmer, amplitude perturbation quotient (APQ), harmonic to noise ration (HNR), noise to harmonic ratio (NHR). Then we found parameters which show the meaningful biggest temporal change in an utterance using the suggested delta parameter. Among them, the deltas of shimmer and APQ were significantly different pre- and post VFSS. These deltas of the P/A and the control group were increased after VFSS, while those of the Non-P/A group was descended. The variation patterns of the P/A and the control group were similar but the change width of the P/A group was larger. The large variations in an aspirated phonation of the P/A group are thought to be caused by irregular changes in air resistance due to residual food on the vocal cords.

In the B-ISDN using satellite between geo-satellites and earth stations, the laser having high security and broad band width has to be used as a carrier for transmitting massive information of visual, vocal, and high rate data. In this paper, by computer simulation we analyzed the number of optical detector array of optical satellite communication downlink in case of using channel coding and no channel coding for BISDN between geo-satelites and earth stations under clear weather condition. It was supposed that 1 watt semiconductor laser was used and as modulation method, the binary FSK was used. The data rate of 10Gbps was used for B-ISDN. Also, hardly affected by atmospheric absorption 1.55$\mu$m wave-length was used to reduce influence of dispersion and chirp generated at a high speed transmission. We analyzed the received power, SNR and BER. The number of optical detector array was determined to satisfy for the BER less than 10$^{-7}$. Also, we ananlyzed the possibility of reducting the number of optical detector array in case of using channel coding. the number of optical detector array is one in the region where the elevation nangle is between 38$^{\circ}$ and 90$^{\circ}$ and two where the elevation angle is between 33$^{\circ}$ and 37$^{\circ}$ and three where the elevation angle is between 30$^{\circ}$ and 32$^{\circ}$ and increases per one as the elevation angle decreases per 1.deg.. So in the region where the elevation angle is 25$^{\circ}$, the number of optical detector arrays is eight. In case of using channel coding, the number of optical detector arrays decreases to five in the region where the elevation angle is 25$^{\circ}$. Therefore, we remaark the advantage of the channelcoding to decrease the size of received antenna and the number ob optical detector arrays.
Purpose: The purpose of this study was to determine the effects of surgery information service on one-day surgery patients' anxiety and satisfaction with nursing care. Methods: The study used the nonequivalent control group time difference design. Sampling and measurement of the control group participants (n=30) was completed first. Later, participants in the experimental group (n=30) were sampled, intervened, and measured. The experimental group participants received the surgery information services twice: before and after the surgery, 20~30 minutes for each of the sessions. The anxiety was measured with the State-Trait Anxiety Inventory developed by Spielberger (1975), and modified by Kim & Shin (1978). The patient satisfaction with nursing care was measured with the Patient Satisfaction with Nursing Care Scale developed by La Monica and colleagues (1986), and modified by Shin (1999). The data was collected between February 1 and May 30, 2006. Results: The results were as follows: 1) The level of anxiety in the experimental group was not significantly different from that in the control group. 2) The level of patient satisfaction with nursing care in the experimental group was significantly higher than in the control group (t=-4.53, p=.00). Conclusion: These findings suggested that the one-day vocal cord surgery information service could be a useful nursing intervention to improve patient satisfaction with nursing care, but not for controlling the anxiety of one-day surgery patients.
A fundamental assumption in conventional linear predictive coding (LPC) analysis procedure is that the input to an all-pole vocal tract filter is white process. In the case of periodic inputs, however, a pitch bias error is introduced into the conventional LP coefficient. Multi-pulse (MP) LP analysis can reduce this bias, provided that an estimate of the excitation is available. Since the prediction error of conventional LP analysis can be modeled as the sum of an MP excitation sequence and a random noise sequence, we can view extracting MP sequences from the prediction error as a classical detection and estimation problem. In this paper, we propose an algorithm in which the locations and amplitudes of the MP sequences are first obtained by applying a likelihood ratio test (LRT) to the prediction error, and LP coefficients free of pitch bias are then obtained from the MP sequences. To verify the performance enhancement, we iterate the above procedure with adaptive threshold at each step.
It's not easy to produce very high tones during singing for not only untrained ordinary people but also even trained singers. To get high singing tones from the low tones, some trained singers used to use a distinguished singing technique, Passaggio (vocal register transition). The purpose of this study is to compare several voice parameters variation between when to sing with using the passaggio technique and to sing without using it. We selected 18 male singers (tenor 8, baritone 10), who had more than 7 years of experience and were well trained in passaggio technique. Simultaneous measurements of fundamental frequency (F0), mean flow rate (MFR), intensity (I), and subglottal pressure (Psub) were performed using the phonatory function analyzer (Nagashima). For the tenor, target tones /a/ were presented: 1) easy phonation: $B_{2}$, 2) high tone without passaggio: F$#_{3}$ 3) high tone with passaggio: F$#_{3}$. For the baritone, target tones /a/ were presented: 1) easy phonation: G$#_{3}$, 2) high tone without passaggio: D$#_{3}$, 3) high tone with passaggio: D$#_{3}$. F0 of the target tones between non-passaggio group and passaggio group was almost the same in both tenor and baritone groups. Intensity of the non-passaggio and passaggio vocalization was much louder than that of easy phonation and pasaggio was louder than non-passaggio vocalization (especially statistically significant in baritone singers). MFR of the passaggio vocalization was greater than non-passaggio vocalization in both tenor and baritone group, but statistically significant only in baritone. Psub of the passaggio vocalization was greater than that of the non-passaggio vocalization in both tenor and baritone group, but statistically not significant in tenor.
Background: Video-assisted thoracic surgery(VATS) is being used as a therapeutic modality in many diseases in which thoracotomy has been used. We studied that the VATS can substitute the thoracotomy in benign esophageal disease. Material and Method: Group I (n=18) underwent video-assisted thoracic surgery, and group II(n=19) thoracotomy. Group I includes 14 leiomyomas and 4 achalasias. Group II includes 16 leiomyomas and 3 achalasias. Operative technique is enucleation in the leiomyoma and modified Heller's myotomy in the achalasia. Analyzing factors of operation-efficacy are anesthetic time, operation time, hospital stay, chest tube drainage amount and chest tube removal day. The degree of the postoperative pain is assessed by the frequency of opioid analgesics injection. Result: There was no death in both groups. There were 5 complications in the group I and 2 in the group II. Prolonged pleural effusion and restenosis of achalasia occurred to 1 patient in each group. In the group I, there were 1 temporaty vocal cord palsy and 2 mucosal tear leading to thoracotomy. There were no differences in anesthesia time, operation time, hospital stay, total chest tube drainage amount, chest tube removal day and frequency of opioid analgesics injection. The amount of the chest tube drainage at POD 1 day was significantly lower in group I(155.6$\pm$77.8cc) than in group II(572.8$\pm$280.1cc)(p<0.05). Conclusion: The results of our data showed that video-assisted thoracic surgery for benign esophageal disease is as effective as thoracotomy and in addition, cosmetic effect is much better. We concluded VATS may be a substitute for thoracotomy in benign esophageal disease.
Even in the hands of the expert endoscopists, an occasional instrumental perforation of the esophagus occurs. But instrumental perforation of the esophagus should not be difficult to diagnose if the possibility is borne in mind. Occasionally patient with esophageal perforations show little reaction at first, but usually they develop systemic manifestation if surgical management is delayed. Early surgical drainage of esophageal perforation is very important & effective therapeutic method. The delayed surgical treatment of esophageal perforation would have increased the morbidity & mortality by allowing mediastinitis & empyema thoracis. We have experienced 6 cases of delayed surgical management of instrumental perforation of esophagus from May 1974 to April 1986 in the department of thoracic and cardiovascular surgery, Yonsei University, college of the medicine. The ages ranged from 4 years to 57 years. The underlying esophageal diseases consisted of esophageal stricture in 3 cases, foreign bodies in the esophagus in 2 cases and esophageal ca. in one case. Most clinical manifestations on admission were high fever, chest discomfort, chest pain, dysphagia and subcutaneous emphysema. Most complications due to esophageal rupture were acute mediastinitis with or without empyema thoracis. Failure to diagnose promptly and failure to promptly institute adequate treatment undoubtedly were largely responsible for this patients death. All 6 patients had been taken delayed surgical drainage more than 24 hours following esophageal perforation. One patient had been in the open drainage state for long time and the another patient has been in the tracheostomy with postintubation vocal cord ulceration. The third patient died due to respiratory failure and sepsis due to fulminant mediastinitis & empyema thoracis. Even if the patients with esophageal perforation have been taken delayed surgical management, the patients should be survived with aggressive & effective surgical drainage with intensive post-operative care.
